The “Rubber Hand Illusion” (RHI) is one of the most popular experimental paradigms to investigate this phenomenon. During this illusion, ownership of a rubber hand is temporarily induced. It was shown that external and continuous cooling of the palm enhanced the RHI, suggesting an association between altered the autonomic nervous system regulation and altered the sense of ownership of a specific limb. Purpose: To investigate whether artificially cooling the entire hand for a short period affects the magnitude of the illusion. Methods: Participants immersed their entire hand in cool, cold, or warm water for 1 min before the RHI procedure. Results: We found that cooling the entire hand enhanced the proprioceptive drift during the RHI but not the subjective feeling of ownership. In contrast, warming and intense cooling of the entire hand did not affect the RHI strength. Conclusion: Our results suggest that transient and moderate cooling of the entire hand was sufficient in enhancing the illusory disembodiment of one’s own hand.展开更多

Purpose: To assess whether a group of people with hand osteoarthritis (hOA) who received different types of treatment improved their function after two years of follow-up. Method: The entire sample (n = 97) underwent three follow-up assessments regarding anthropometric parameters of the upper limbs and ability to perform functional activities. Subsequently, the sample was divided into two groups for the intervention periods, called the First Period (n = 73) and the Second Period (n = 24);the First Period kept the same protocol with orientations, and the Second Period went to an intervention with orientation strength exercises and use of orthosis. Findings: In the separate analysis of the three questions of the DASH pain module, no differences were found between the assessment moments for groups of guidelines, treatment, or symptoms. Significant effects were observed for F(2, 162) = 3.5, p = 0.033, η2 = 0.04, and interaction for moments and intervention F(2, 162) = 4.3, p = 0.016, η2 = 0.05. Implications: It can be concluded that only guidance treatment does not benefit patients with hand osteoarthritis. In contrast, guidance, exercise, and orthosis treatment can significantly improve the disease.展开更多

Results: One patient was cured within 7 days, the cure rate was 10% in 7 days, 8 patients were cured within 10 days, the cure rate was 80% in 10 days, 10 patients were cured within 15 days, the cure rate was 100% in 15 days. Conclusion: The surgical nursing method is effective for patients with grade III HFS caused by Doxorubicin hydrochloride liposome chemotherapy after breast cancer surgery, shortening the treatment time of HFS, and is worthy of clinical promotion.展开更多

The functionality and outcome of hand injuries can...Background: Hand injuries are very common and responsible for a significant number of emergency department (ED) visits, most of which are not to hand specialists [1]. The functionality and outcome of hand injuries can vary significantly depending on the mechanism and pattern of injuries, which is why it is imperative for emergency physicians to recognize the complexities, and the potential repercussion of missed injuries in such cases. Objective: The aim of this study is to provide epidemiological information on hand injuries and their patterns. The objective is 1) to assess whether most hand injuries are superficial (simple), or involve underlying deeper structures (complex) and 2) to assess whether most hand injuries presented to the emergency department were managed by the emergency physician or plastic/orthopaedic surgeon. Methods: This retrospective single-centre observational study conducted at an emergency department in a tertiary care hospital in Mumbai, India collected data from hand trauma patients using a standardized documentation form. Demographic data, trauma-related data, and disposition plans were analysed. Results: A total of 489 cases sustained hand injuries over a period of one year. The patients were predominantly males in the 20 - 30 year age group and injuries were mainly sustained over the right hand. Most of the injuries were sustained at home (42%). The most common mechanism (34%) was sharp object injury (including needle-stick and other sharps in hospital), followed by blunt injury (30%). Among grievous hand injuries, door jamb was a mechanism noted in 11% of patients, accounting for 50% of all crush injuries. Lacerations were the most common pattern (24.7%) noticed, followed closely by fractures (23.3%). Digits II - IV were injured most commonly (54%), followed by carpals (14%) and the thumb (10%). Nearly 80% of the hand injuries were managed by emergency physicians alone, with 61% of cases involving superficial structures. Though 14% of the cases required plastic surgery intervention, the initial evaluation of all these patients was performed by the emergency physician. Conclusions: Our study highlights the burden of hand injuries on the emergency physician, as well as the odds of missed injuries, directly indicating the necessity of a thorough anatomical knowledge of the structures of the hand, and in turn, a proper physical examination. A dedicated registry for hand trauma would help quantify the mechanism and pattern of injuries, and formulate preventive strategies.展开更多

This paper presents a novel method for dynamic hand gesture detection using Hidden Markov Models (HMMs) where we detect different English alphabet letters by tracing hand movements. The process involves skin color-based segmentation for hand isolation in video frames, followed by morphological operations to enhance image trajectories. Our system employs hand tracking and trajectory smoothing techniques, such as the Kalman filter, to monitor hand movements and refine gesture paths. Quantized sequences are then analyzed using the Baum-Welch Re-estimation Algorithm, an HMM-based approach. A maximum likelihood classifier is used to identify the most probable letter from the test sequences. Our method demonstrates significant improvements over traditional recognition techniques in real-time, automatic hand gesture recognition, particularly in its ability to distinguish complex gestures. The experimental results confirm the effectiveness of our approach in enhancing gesture-based sign language detection to alleviate the barrier between the deaf and hard-of-hearing community and general people.展开更多

The Ponseti technique is the goal standard for clubfoot treatment. During the correction phase, the provider’s right hand manipulates the right foot and the left hand, left foot. Often, one foot is ready for Achilles tenotomy before the other in bilateral clubfoot. Objective: To determine the effect of the provider’s hand dominance would have on bilateral clubfoot treated with the Ponseti technique. Method: This was a prospective cross-sectional study that analyzed idiopathic bilateral clubfoot patients aged 0 - 5 years and treated using the Ponseti technique at FMC Umuahia from October 2019 to September 2020. Informed consent and ethical clearance were obtained. The Pirani scores were obtained and compared at presentation and at each clinic visit. All trained manipulators were right-handed. Two-tailed t-test was used and a p-value less than 0.05 was deemed significant. Results: Forty-seven patients participated in the study with an M:F of 2.6:1 and mean age of 13.79 ± 13.39 months. Thirty-six patients (76.6%) had the same Pirani score on both feet at presentation, while the right and left feet were more severely affected in 8 and 3 cases respectively. The mean number of casts before readiness for tenotomy was 4.95 on the right and 5.28 on the left with p-value of 0.042. Conclusion: Though the right foot had a worse mean Pirani score on presentation, however, it required fewer casts before readiness for tenotomy than the left.展开更多

Introduction: Hand hygiene (HH) is an effective way to fight infections in healthcare settings. The general purpose of our study was to explore the knowledge, attitudes and practices of health care providers on HH at ...Introduction: Hand hygiene (HH) is an effective way to fight infections in healthcare settings. The general purpose of our study was to explore the knowledge, attitudes and practices of health care providers on HH at Dapaong regional hospital (DRH). Methodology: This was a prospective, descriptive cross-sectional study conducted from March to June 2022 in the DRH wards. Data were collected using a questionnaire and observation grid. Results: 90 care providers were surveyed. Males and non-physician personnel predominated with 57.8%, and 94.4% respectively. The survey on staff’s knowledge reported: 31.1% of practitioners did not wash their hands on arrival and departure in services. 24% did not know the difference between simple hand washing (SHW) and hygienic hand washing (HHW). 23.3% did not know the type of soap to use for HHW. The caregivers did not know the type of hand washing (HW) required after a septic and non-septic procedure in respectively 41.6%, and 37.8%. They did not know that there are two types of hand antiseptics (45.4%), nor the amount of antiseptic for HW (78.9%). The survey on staff’s attitude regarding HW found that: 70% did not remove all jewels prior HW, and 51.1% did not know that wearing gloves cannot replace the HW. For HW Staff Practice: 62.2% did not wash their hands before treatment. 91.1% did not spread the soap on their hands and forearms after wetting them. 65.55% did not rinse hands from nails to elbows. Conclusion: The HH was poorly known, the attitude of the staff was dangerous in relation to the HH and the practice of HH was very inadequate at the RHC-Dapaong. As a result, there is a need to retrain staff to increase their capacity to prevent care-related infections and enhance patient safety in the hospital.展开更多

It mainly affects the development of the limbs, its clin...Split hand/foot malformation (SHFM), formerly known as ectrodactyly is a rare congénital anomaly, its incidence varies from 1/8.500 to 1/25.000 live birth. It mainly affects the development of the limbs, its clinical variability is standard, can present as an isolated feature or as a syndrome associated with other congenital anomalies. Our objective was to present the two cases of SHFM, and to review the literature on the clinical aspects and discuss a probable origin. The father went to school and is a driver because the malformations concerned only the fingers, were less severe, and did not prevent the realization of certain simple gestures of the daily life. On the other hand, the malformations of the fingers of the newborn were severe and the absence of the thumbs compromised the later prehension function. Also the association of a microglossia and a cleft palate contributed to a weight loss that justified hospitalization. The clinical presentation of split hands and feet is variable and the prognosis depends on the type of anomaly. Familial cases suggest a probable genetic origin. Genetic testing is necessary to establish genetic counseling.展开更多

Their consequences, sometimes irreversible, are the cause of major handicaps. The aim of our work was to describe the epidemiological and lesional aspects of hand trauma in the Orthopedics-Traumatology department of the CHU Aristide Le DANTEC. Material and Method: This was a continuous retrospective study over a period of 2 years. From June 1, 2015, to December 31, 2016, concerning all patients who came for a trauma recent hand, followed and whose file was found and completed. Results: Of 351 patients, we retained 291, 247 men for 44 women, aged in average age of 34 and carrying out different activities with a predominance of workers manuals (78.01%). Most of them came from the Dakar region (75.26%). THE work accidents predominated with 47.08%. The involvement of the right side was 58.76% of cases and concerned the dominant side in 56.35%. On the lesion level, there were 36.4% osteo-articular lesions, 30.58% lesions of the parts soft, 23% nail lesions and 9.9% complex lesions associating at least 3 lesions elementary. Conclusion: Recent hand injuries account for 13.49% of injuries in our service;they are severe, affecting the different structures of the hand, either singly or jointly. They involve the functional prognosis of the hand with socio-professional repercussions important.展开更多

The Wide Awake Local Anesthesia No Tourniquet (WALANT) is...Background: In sub-Saharan Africa, surgery for hand injuries is usually performed under axillary or general anesthesia. This is often not without consequences. The Wide Awake Local Anesthesia No Tourniquet (WALANT) is, therefore, an anesthetic asset in the surgical treatment of hand injuries. This study aims to share an experience on this technique’s effectiveness and to spark interest among African authors and practitioners. Method: This prospective study focuses on 27 surgeries of hand injuries carried out with WALANT. The study spanned over nine months (November 2021-August 2022) and included 19 men and eight women with a mean age of 35.4 years (extremes: 19 and 54). There were five (18%) flexor tendon ruptures, three of which were in zone 2, eleven (41%) metacarpal fractures, four of which were open, seven (26%) phalangeal fractures, two of which were open, and four (15%) finger springs. The operated lesions were assessed at a minimum of three months. The pain was assessed using the Visual Analog Scale. The QuickDASH score was evaluated for each patient at the end of the follow-up period. Results: Intra-operatively, the mean value of the Visual Analog Scale was 1/10, with extreme values of 1/10 and 3/10. Of the five cases of flexor tendon ruptures, three were located in zone 2 and two in zone 3. The metacarpal fractures were divided into seven closed diaphyseal fractures and four extra-articular metaphyseal fractures. Four of the seven phalangeal fractures were oblique diaphyseal, and three were transverse diaphyseal. The treatment of the protruding fingers followed the conventional technique. The assessment of the QuickDASH score in the months following surgery showed a score of 11.8 in the first month, 10.3 in the second month, and 5.0 in the third month. Conclusion: WALANT is an easy-to-use, unexpensive anesthetic technique that enables shorter operating times. It rarely presents complications. It is recommended for hand surgeries performed with limited technical resources.展开更多

Two-tailed t-test was used and a p-value less than 0.05 was deemed significant. Results: Forty-seven patients participated in the study with an M:F of 2.6:1 and mean age of 13.79 ± 13.39 months. Thirty-six patients (76.6%) had the same Pirani score on both feet at presentation, while the right and left feet were more severely affected in 8 and 3 cases respectively. The mean number of casts before readiness for tenotomy was 4.95 on the right and 5.28 on the left with p-value of 0.042. Conclusion: Though the right foot had a worse mean Pirani score on presentation, however, it required fewer casts before readiness for tenotomy than the left.
文摘针对采摘机器人自主行走导航过程中,难以准确定位其与果树之间的相对位置,难以准确估计果树树干姿态的问题,提出基于双目eye in hand系统的多角度树干位姿估计方法。利用YOLOv5深度学习方法与半全局块匹配算法识别树干并生成局部点云;利用半径滤波和体素滤波减少树干点云数据;利用闭环式手眼标定方法对双目eye in hand系统进行标定,并对同一树干多角度相机位置的点云数据进行拼接;利用随机抽样一致(RANSAC)算法与无约束最小二乘法估计并优化树干的位置和姿态,获取树干的圆柱体参数。通过对30幅标定板图像进行实验,闭环式手眼标定方法的平均欧式误差为3.7177 mm;采用半径滤波和体素滤波可减少98.470%的点云数据;采用RANSAC算法、圆柱体估计算法拟合树干点云数据,得到圆柱体的半径r=41.2771 mm,R_(MAE)=2.57156 mm,R_(RMSE)=2.98936 mm;无约束最小二乘法优化后r=39.4028 mm,R_(MAE)=1.98955 mm,R_(RMSE)=2.46588 mm。该文通过对双目eye in hand系统进行标定,建立坐标系转换关系,多角度采集环境信息,准确定位机器人与果树之间的相对位置,估计果树树干的姿态。
文摘Introduction: Hand hygiene (HH) is an effective way to fight infections in healthcare settings. The general purpose of our study was to explore the knowledge, attitudes and practices of health care providers on HH at Dapaong regional hospital (DRH). Methodology: This was a prospective, descriptive cross-sectional study conducted from March to June 2022 in the DRH wards. Data were collected using a questionnaire and observation grid. Results: 90 care providers were surveyed. Males and non-physician personnel predominated with 57.8%, and 94.4% respectively. The survey on staff’s knowledge reported: 31.1% of practitioners did not wash their hands on arrival and departure in services. 24% did not know the difference between simple hand washing (SHW) and hygienic hand washing (HHW). 23.3% did not know the type of soap to use for HHW. The caregivers did not know the type of hand washing (HW) required after a septic and non-septic procedure in respectively 41.6%, and 37.8%. They did not know that there are two types of hand antiseptics (45.4%), nor the amount of antiseptic for HW (78.9%). The survey on staff’s attitude regarding HW found that: 70% did not remove all jewels prior HW, and 51.1% did not know that wearing gloves cannot replace the HW. For HW Staff Practice: 62.2% did not wash their hands before treatment. 91.1% did not spread the soap on their hands and forearms after wetting them. 65.55% did not rinse hands from nails to elbows. Conclusion: The HH was poorly known, the attitude of the staff was dangerous in relation to the HH and the practice of HH was very inadequate at the RHC-Dapaong. As a result, there is a need to retrain staff to increase their capacity to prevent care-related infections and enhance patient safety in the hospital.
文摘目的探究握力与认知功能的关联是否存在性别差异。方法利用2011—2014年美国国家健康与营养调查(National Health and Nutrition Examination Survey,NHANES)数据库开展横断面研究,评估即时记忆、延迟记忆、言语流畅性和注意力4个认知领域。采用加权线性回归、加权Logistic回归、相乘模型进行分析。结果回归分析显示,在男性,与握力最低组的研究对象相比,握力最高组研究对象有更低的整体认知功能降低(OR=0.377,95%CI:0.182~0.781)和即时记忆降低(OR=0.409,95%CI:0.221~0.759)的风险。在女性,握力与整体认知功能降低(OR=0.405,95%CI:0.224~0.733)、言语流畅性降低(OR=0.567,95%CI:0.356~0.902)以及注意力降低(OR=0.357,95%CI:0.211~0.607)的风险呈显著负相关关系。同时,性别和握力在整体认知功能、言语流畅性和注意力均存在显著交互作用,但是即时记忆、延迟回忆2个认知领域未检测到显著的交互作用。结论握力对整体认知功能和不同认知领域的影响存在性别差异。因此,提高握力水平,特别是女性,有利于预防认知功能下降。
文摘Split hand/foot malformation (SHFM), formerly known as ectrodactyly is a rare congénital anomaly, its incidence varies from 1/8.500 to 1/25.000 live birth. It mainly affects the development of the limbs, its clinical variability is standard, can present as an isolated feature or as a syndrome associated with other congenital anomalies. Our objective was to present the two cases of SHFM, and to review the literature on the clinical aspects and discuss a probable origin. The father went to school and is a driver because the malformations concerned only the fingers, were less severe, and did not prevent the realization of certain simple gestures of the daily life. On the other hand, the malformations of the fingers of the newborn were severe and the absence of the thumbs compromised the later prehension function. Also the association of a microglossia and a cleft palate contributed to a weight loss that justified hospitalization. The clinical presentation of split hands and feet is variable and the prognosis depends on the type of anomaly. Familial cases suggest a probable genetic origin. Genetic testing is necessary to establish genetic counseling.
文摘Introduction: Hand injuries are frequent and serious. Their consequences, sometimes irreversible, are the cause of major handicaps. The aim of our work was to describe the epidemiological and lesional aspects of hand trauma in the Orthopedics-Traumatology department of the CHU Aristide Le DANTEC. Material and Method: This was a continuous retrospective study over a period of 2 years. From June 1, 2015, to December 31, 2016, concerning all patients who came for a trauma recent hand, followed and whose file was found and completed. Results: Of 351 patients, we retained 291, 247 men for 44 women, aged in average age of 34 and carrying out different activities with a predominance of workers manuals (78.01%). Most of them came from the Dakar region (75.26%). THE work accidents predominated with 47.08%. The involvement of the right side was 58.76% of cases and concerned the dominant side in 56.35%. On the lesion level, there were 36.4% osteo-articular lesions, 30.58% lesions of the parts soft, 23% nail lesions and 9.9% complex lesions associating at least 3 lesions elementary. Conclusion: Recent hand injuries account for 13.49% of injuries in our service;they are severe, affecting the different structures of the hand, either singly or jointly. They involve the functional prognosis of the hand with socio-professional repercussions important.
文摘Background: In sub-Saharan Africa, surgery for hand injuries is usually performed under axillary or general anesthesia. This is often not without consequences. The Wide Awake Local Anesthesia No Tourniquet (WALANT) is, therefore, an anesthetic asset in the surgical treatment of hand injuries. This study aims to share an experience on this technique’s effectiveness and to spark interest among African authors and practitioners. Method: This prospective study focuses on 27 surgeries of hand injuries carried out with WALANT. The study spanned over nine months (November 2021-August 2022) and included 19 men and eight women with a mean age of 35.4 years (extremes: 19 and 54). There were five (18%) flexor tendon ruptures, three of which were in zone 2, eleven (41%) metacarpal fractures, four of which were open, seven (26%) phalangeal fractures, two of which were open, and four (15%) finger springs. The operated lesions were assessed at a minimum of three months. The pain was assessed using the Visual Analog Scale. The QuickDASH score was evaluated for each patient at the end of the follow-up period. Results: Intra-operatively, the mean value of the Visual Analog Scale was 1/10, with extreme values of 1/10 and 3/10. Of the five cases of flexor tendon ruptures, three were located in zone 2 and two in zone 3. The metacarpal fractures were divided into seven closed diaphyseal fractures and four extra-articular metaphyseal fractures. Four of the seven phalangeal fractures were oblique diaphyseal, and three were transverse diaphyseal. The treatment of the protruding fingers followed the conventional technique. The assessment of the QuickDASH score in the months following surgery showed a score of 11.8 in the first month, 10.3 in the second month, and 5.0 in the third month. Conclusion: WALANT is an easy-to-use, unexpensive anesthetic technique that enables shorter operating times. It rarely presents complications. It is recommended for hand surgeries performed with limited technical resources.
