However, it required scientific and clinic knowledge of diagnostic area of the disorders temporomandibulares. This study was performed with the patient CN, 18 years, white, female, who went to the orofacial pain ambulatory with complaints of extreme pain on the right temporomandibular joint (TMJ) and limitation of maximum opening of 29 mm with shift to right, which she has been suffering for at least three years. After a criterion anamnese the diagnosis hypothesis found was a displacement of the non-reduction disk with limitation of opening. The treatment was based on infiltration in the right TMJ with anesthesic followed by mandibular manipulation. After the therapeutic conduct the patient presented clinical signals of normality with absence of trismus and mandibular movement without shift to right. The patient was followed up during five years not presenting return of the pathology.展开更多

The author has treated 66 cases by massotherapy, who were confirmatively diagnosed as having protrusion of the lumbar intervertebral disc by X-ray film and computer-aided tomography, with satisfactory results as reported in the following.Clinical DataAmong the 66 cases in this series, 49 were male and 17 female, ranging in age from 26 to 59 years, averaging 36.2 years. All the patients had got lumbago and unilateral ischialgia, with the left side affected in 37 cases and the right side in 29 cases. The leg pain radiating to lateral malleolus was found in 28 cases, to dorsum of the foot in 24 cases, and to the toes in 14 cases. Intermittent claudication was present in 22 cases, numbness of the affected foot in 16 cases, pain exacerbated when coughing in 35 cases, scoliosis in 21 cases, and positive Lasegue's sign in 48 cases.展开更多

Objective:To investigate the inhibition effect of siRNA interference on NGF induced by inflammatory factor IL-6,and JUL—1 so as to provide novel targets for clinical treatment of discogenic low back pain.Methods:The ...Objective:To investigate the inhibition effect of siRNA interference on NGF induced by inflammatory factor IL-6,and JUL—1 so as to provide novel targets for clinical treatment of discogenic low back pain.Methods:The intervertebral disc nucleus and annulus fibrosus cells of rats were separated-The cells were co-cultured with different concentrations(10 nmol/L,20nmol/L,50 nmol/L,100 nmol/L)of IL-6 and IL-1β.The NGF-siRNA was leaded into the cocultured cells with its import ability assessed by flow cytometry instrument tests,hefore and after which the NCF mRNA expression was detected by real-time Q-PCR and the NGF content was detected by ELISA.Results:Flow cytometry instrument test results showed that the NGFsiRNA cell conversion rate was 99.8%.Real-time Q-PCR detection results showed that compared with negative control group,the NGF mRNA expression of co-cultured cells treated by 10 nmol/L,20 nmol/L,50 nmol/L,100 nmol/L IL-6 and IL-1βwere respectively raised 3.4,3.7,4.7,3.7 times which were all significantly down-regulated after the import of NGF-siRNA.EILSA detection results showed that compared with negative control group,the NGF content of cocultured medium treated by 10 nmol/L,20 nmol/L,50 nmol/L,100 nmol/L I-L6 and IL-1βwere respectively raised 2.9,3.3,4.5,7.4 times which were all significantly decreased after the import of NGF-siRNA.Conclusions:These molecular biological results suggest that inflammatory factor IL-6 and IL-1βcould stimulate NCF on intervertebral disc cells in vitro culture model and its efficiency is concentration dependent,while siRNA interference can inhibit the stimulation effect of IL-6 and IL-1βon intervertebral disc cell,which provides a new targets for the clinical treatment of discogenic low back pain.展开更多

BACKGROUND The spontaneous resorption of lumbar disk herniations(LDHs)has been widely reported.However,the majority of these reports analyze the resorption of LDHs that were displaced backwards or downwards.There have...BACKGROUND The spontaneous resorption of lumbar disk herniations(LDHs)has been widely reported.However,the majority of these reports analyze the resorption of LDHs that were displaced backwards or downwards.There have been few reports on the spontaneous resorption of upwardly displaced L4/5 LDH that has caused femoral nerve symptoms.CASE SUMMARY A 55-year-old woman presented to our hospital with acute pain in her left leg.She had been suffering from recurrent lower back pain for approximately 1 year and began to feel pain accompanied with numbness at the anterior aspect of her left leg 7 d previously.On examination,a typical L4 nerve stimulation was noted.An upwardly displaced LDH at the L4/5 level was revealed by magnetic resonance imaging.The patient attained complete relief of her symptoms after 10 wk of nonsurgical treatment and the upwardly displaced herniation almost entirely disappeared.There was no recurrence during a follow-up of 2 years.CONCLUSION Clinicians should be aware that the nerves disturbed by LDHs vary according to the direction of the herniations and the probable resorption of upwardly displaced LDHs should be considered before making a decision on surgery.展开更多
This study compared the pre-sacral intervertebral joints of the American alligator(Alligator mississippiensis)with those from specimens of Varanus.These two taxa were chosen because they have similar number of pre-sac...This study compared the pre-sacral intervertebral joints of the American alligator(Alligator mississippiensis)with those from specimens of Varanus.These two taxa were chosen because they have similar number of pre-sacral vertebrae and similar body weights;however,Varanus can move bipedally and has diarthrotic intervertebral joints,whereas Alligator has intervertebral discs and cannot move bipedally.This study consisted of three objectives:(1)to document the anatomy of the intervertebral joint,(2)to quantify the compressive biomechanics of the intervertebral joints and explore which features contributed to compression resistance,and(3)to quantify the impact of compression on the intervertebral foramen and spinal nerves in these two taxa.The experimental results revealed that the diarthrotic intervertebral joints of Varanus were significantly(4x)stiffer than the intervertebral disc of Alligator,and that a significant component of this increased stiffness arose from the facet joints.Compressing the intervertebral joints of the two taxa caused a reduction in foraminal area,but the magnitude of this reduction was not significantly different.We hypothesize that the main factor preventing spinal nerve impingement in Varanus during gravitational compression is the relatively small size of the spinal ganglion/nerve relative to the foraminal area.展开更多
