Effect of electroacupuncture combined with rehabilitation techniques on shoulder function in patients with rotator cuff injuries

BACKGROUND The rotator cuff is located below the acromion and deltoid muscles and comprises multiple tendons that wrap around the humeral head,maintaining shoulder joint stability.AIM To explore the effect of electroacupuncture combined with rehabilitation techniques on shoulder function in patients with rotator cuff injuries.METHODS We selected 97 patients with rotator cuff injuries treated in the People's Hospital of Yuhuan from February 2020 to May 2023.Patients were grouped using the envelope method.RESULTS After treatment,the study group’s treatment effective rate was 94.90%(46/49 patients),significantly higher than that in the control group(79.17%,38/48 cases;P<0.05).Before treatment,there was no difference in Constant Murley Score(CMS)scores,shoulder mobility,or 36-Item Short Form Health Survey(SF-36)scale scores(P>0.05).Compared with those before treatment,the CMS scores(including pain,daily living ability,shoulder mobility,and muscle strength),all aspects of shoulder mobility(forward flexion,posterior extension,external rotation,internal rotation),and SF-36 scale scores(including physiological,psychological,emotional,physical,vitality,and health status)were higher in both groups after treatment and significantly higher in the study group(P<0.05).There was no difference in the occurrence of complications between the two treatment groups(P>0.05).CONCLUSION Electroacupuncture combined with rehabilitation techniques has a good treatment effect on patients with rotator cuff injuries,helps accelerate the recovery of shoulder function,improves the quality of life,and is highly safe. 展开更多

The effective evaluation and management of orthopaedic conditions including shoulder disorders relies upon understanding the level of disability created by the disease process. Validated outcome measures are critical to the evaluation process. Traditionally, outcome measures have been physician derived objective evaluations including range of motion and radiologic evaluations. However, these measures can marginalize a patient's perception of their disability or outcome. As a result of these limitations, patient self-reported outcomes measures have become popular over the last quarter century and are currently primary tools to evaluate outcomes of treatment. Patient reported outcomes measures can be general health related quality of life measures, health utility measures, region specific health related quality of life measures or condition specific measures. Several patients self-reported outcomes measures have been developed and validated for evaluating patients with shoulder disorders. Computer adaptive testing will likely play an important role in the arsenal of measures used to evaluate shoulder patients in the future. The purpose of this article is to review the general health related quality-of-life measures as well as the jointspecific and condition specific measures utilized in evaluating patients with shoulder conditions. Advancesin computer adaptive testing as it relates to assessing dysfunction in shoulder conditions will also be reviewed. 展开更多

Cortical activation pattern during shoulder simple versus vibration exercises: a functional near infrared spectroscopy study

To date, the cortical effect of exercise has not been fully elucidated. Using the functional near infrared spectroscopy, we attempted to compare the cortical effect between shoulder vibration exercise and shoulder simple exercise. Eight healthy subjects were recruited for this study. Two different exercise tasks(shoulder vibration exercise using the flexible pole and shoulder simple exercise) were performed using a block paradigm. We measured the values of oxygenated hemoglobin in the four regions of interest: the primary sensory-motor cortex(SM1 total, arm somatotopy, and leg and trunk somatotopy), the premotor cortex, the supplementary motor area, and the prefrontal cortex. During shoulder vibration exercise and shoulder simple exercise, cortical activation was observed in SM1(total, arm somatotopy, and leg and trunk somatotopy), premotor cortex, supplementary motor area, and prefrontal cortex. Higher oxygenated hemoglobin values were also observed in the areas of arm somatotopy of SM1 compared with those of other regions of interest. However, no significant difference in the arm somatotopy of SM1 was observed between the two exercises. By contrast, in the leg and trunk somatotopy of SM1, shoulder vibration exercise led to a significantly higher oxy-hemoglobin value than shoulder simple exercise. These two exercises may result in cortical activation effects for the motor areas relevant to the shoulder exercise, especially in the arm somatotopy of SM1. However, shoulder vibration exercise has an additional cortical activation effect for the leg and trunk somatotopy of SM1. 展开更多

Objective:To investigate the clinical effects of electroacupuncture combined with extracorporeal shock wave in the treatment of periarthritis of shoulder.Methods:136 patients with periarthritis of shoulder admitted to our hospital from September 2018 to September 2019 were randomly divided into two groups by double-blind method.68 patients in the control group were treated with extracorporeal shock wave;68 patients in the observation group were treated with electroacupuncture combined with extracorporeal shock wave.The shoulder joint pain,joint function and activities of daily living were compared between the two groups.Results:The VAS score of shoulder joint pain in the observation group after treatment(1.92±0.24)was lower than that in the control group(3.51±0.32),and the UCLA score of shoulder joint function(31.28±1.96)was higher than that of the control group(27.42±2.36),the differences were statistically significant(P<0.05).After treatment,the active degree of forward flexion and extension of shoulder joint in the observation group was higher than those of the control group,and the activities of daily living was higher than that of the control group,with statistically significant differences(P<0.05)Conclusion:Electroacupuncture combined with extracorporeal shock wave treatment for periarthritis of shoulder has achieved ideal effects,safety and non-invasiveness,and can quickly restore the functions of shoulder joints. 展开更多

However, direct re... Direct coaptation of contralateral C7 to the upper trunk could avoid the interposition of nerve grafts. We have successfully shortened the gap and graft lengths, and even achieved direct coaptation. However, direct repair can only be performed in some selected cases, and partial procedures still require autografts, which are the gold standard for repairing neurologic defects. As symptoms often occur after autografting, human acellular nerve allografts have been used to avoid concomitant symptoms. This study investigated the quality of shoulder abduction and elbow flexion following direct repair and acellular allografting to evaluate issues requiring attention for brachial plexus injury repair. Fifty-one brachial plexus injury patients in the surgical database were eligible for this retrospective study. Patients were divided into two groups according to different surgical methods. Direct repair was performed in 27 patients, while acellular nerve allografts were used to bridge the gap between the contralateral C7 nerve root and upper trunk in 24 patients. The length of the harvested contralateral C7 nerve root was measured intraoperatively. Deltoid and biceps muscle strength, and degrees of shoulder abduction and elbow flexion were examined according to the British Medical Research Council scoring system;meaningful recovery was defined as M3–M5. Lengths of anterior and posterior divisions of the contralateral C7 in the direct repair group were 7.64 ± 0.69 mm and 7.55 ± 0.69 mm, respectively, and in the acellular nerve allografts group were 6.46 ± 0.58 mm and 6.43 ± 0.59 mm, respectively. After a minimum of 4-year follow-up, meaningful recoveries of deltoid and biceps muscles in the direct repair group were 88.89% and 85.19%, respectively, while they were 70.83% and 66.67% in the acellular nerve allografts group. Time to C5/C6 reinnervation was shorter in the direct repair group compared with the acellular nerve allografts group. Direct repair facilitated the restoration of shoulder abduction and elbow flexion. Thus, if direct coaptation is not possible, use of acellular nerve allografts is a suitable option. Triangle tilt and humeral surgery:Meta-analysis of efficacy and functional outcome

AIM: To systematically review and analyze the overall impact and effectiveness of bony surgical procedures, the triangle tilt and humeral surgery in a comparative manner in permanent obstetric brachial plexus injury(OBPI) patients.METHODS: We conducted a literature search and identified original full research articles of OBPI patients treated with a secondary bony surgery, particularly addressing the limitation of shoulder abduction and functions. Further, we analyzed and compared the efficacy and the surgical outcomes of 9 humeral surgerypapers with 179 patients, and 4 of our secondary bony procedure, the triangle tilt surgical papers with 86 patients. RESULTS: Seven hundred and thirty-one articles were identified, using the search term "brachial plexus" and obstetric or pediatric(246 articles) or neonatal(219 articles) or congenital(188 articles) or "birth palsy"(121 articles). Further, only a few articles were identified using the bony surgery search, osteotomy "brachial plexus" obstetric(35), "humeral osteotomy" and "brachial plexus"(17), and triangle tilt "brachial plexus"(14). Of all, 12 studies reporting pre- and postoperative or improvement in total Mallet functional score were included in this study. Among these, 9 studies reported the humeral surgery and 4 were triangle tilt surgery. We used modified total Mallet functional score in this analysis. Various studies with humeral surgery showed improvement of 1.4, 2.3, 5.0 and 5.6 total Mallet score, whereas the triangle tilt surgery showed improvement of 5.0, 5.5, 6.0 and 6.2.CONCLUSION: The triangle tilt surgery improves on what was achieved by humeral osteotomy in the management of shoulder function in OBPI patients. 展开更多

目的研究针刺治疗后肩周炎患者的临床疗效和生存质量改善情况。方法严格按照纳入排除标准选择2022年3月—2023年3月收治的80例肩周炎患者进行研究,按照等比例随机数字表法分为两组,对照组40例采用口服药物+功能锻炼治疗,研究组40例采用... 目的研究针刺治疗后肩周炎患者的临床疗效和生存质量改善情况。方法严格按照纳入排除标准选择2022年3月—2023年3月收治的80例肩周炎患者进行研究,按照等比例随机数字表法分为两组,对照组40例采用口服药物+功能锻炼治疗,研究组40例采用经络别通法针刺治疗,治疗1个疗程后观察比较两组临床疗效、肩关节功能、日常生活能力、炎症因子水平以及疼痛恢复情况。结果研究组治疗总有效率(97.50%,39/40)显著高于对照组(77.50%,31/40),差异有统计学意义(P<0.05);均未发生严重安全事件,仅少数患者发生不良反应,差异无统计学意义(P>0.05)。治疗前,两组肩关节评分量表(CMS)分值、疼痛程度、简化生活质量量表(ADL)评分和炎症因子水平比较差异均无统计学意义(P>0.05);治疗后,研究组CMS分值、ADL评分高于对照组,疼痛状况(PPI)分值、视觉模拟疼痛(VAS)分值、疼痛分级指数(PRI)分值、C反应蛋白(CRP)、白介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)水平低于对照组,差异有统计学意义(P<0.05)。结论经络别通法针刺治疗肩周炎疗效显著,可显著改善疼痛,从而改善患者生存质量,相关作用机制可能与降低炎症因子水平,恢复肩关节功能相关。 展开更多

目的:探讨铍针结合三七散外敷治疗肱二头肌长头腱鞘炎效果及对患者肌骨超声参数、血清炎症因子、肩关节功能的影响。方法:80例肱二头肌长头腱鞘炎患者随机分为对照组与观察组,各40例。对照组采用放射式冲击波治疗,观察组则实施铍针结合... 目的:探讨铍针结合三七散外敷治疗肱二头肌长头腱鞘炎效果及对患者肌骨超声参数、血清炎症因子、肩关节功能的影响。方法:80例肱二头肌长头腱鞘炎患者随机分为对照组与观察组,各40例。对照组采用放射式冲击波治疗,观察组则实施铍针结合三七散外敷治疗,均连续治疗4周。比较两组临床疗效,治疗前后两组患者疼痛情况[视觉模拟疼痛评分(VAS)、压痛阈值(PPTs)]、肩关节功能[Constant-Murley肩关节功能活动评分量表(CMS)]、肌骨超声参数(肌腱厚度、腱鞘厚度、积液最大径)、血清炎症因子[肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、白细胞介素-6(IL-6)]的变化。结果:观察组患者临床疗效87.50%,明显高于对照组的67.50%(P<0.05);治疗后,观察组VAS评分低于对照组,观察组PPTs值高于对照组(P<0.05);观察组肌力、疼痛、肩关节活动度及功能评分均高于对照组(P<0.05);观察组肌腱厚度、腱鞘厚度、积液水平均低于对照组(P<0.05);观察组血清TNF-α、CRP、IL-6水平均低于对照组(P<0.05)。结论:铍针结合三七散外敷治疗肱二头肌长头腱鞘炎可提高临床效果,减轻患者疼痛,提高疼痛阈值,同时可降低患者血清炎症因子水平,改善肌骨超声参数和肩关节功能。 展开更多

目的:观察筋结点微针刀松解术治疗冻结期肩周炎的临床疗效。方法:将60例冻结期肩周炎患者纳入研究,按随机数字表法分为治疗组与对照组各30例。治疗组采取筋结点微针刀松解术治疗,对照组给予小针刀治疗,比较2组治疗后临床疗效、肩关节功... 目的:观察筋结点微针刀松解术治疗冻结期肩周炎的临床疗效。方法:将60例冻结期肩周炎患者纳入研究,按随机数字表法分为治疗组与对照组各30例。治疗组采取筋结点微针刀松解术治疗,对照组给予小针刀治疗,比较2组治疗后临床疗效、肩关节功能、疼痛程度[疼痛视觉模拟评分法(VAS)评分]及安全性。结果:治疗组总有效率为96.67%,对照组为80.00%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组摸耳、摸背、内旋、外旋等肩关节功能评分均较治疗前升高(P<0.05),且治疗组上述各项评分均高于对照组(P<0.05)。治疗后,2组VAS评分较治疗前降低(P<0.05),且治疗组VAS评分低于对照组(P<0.05)。治疗组不良反应发生率为6.66%,对照组为23.33%,2组比较,差异有统计学意义(P<0.05)。结论:筋结点微针刀松解术治疗冻结期肩周炎能显著改善患者肩关节功能,减轻疼痛,疗效优于小针刀治疗,且安全性更高。 展开更多

目的 探讨Bobath康复训练改善脑卒中后肩手综合征(SHS)患者上肢运动功能效果。方法 采用便利抽样方法选取2022年4月至2023年1月南通市第三人民医院(南通大学附属南通第三医院)康复科收治的110例脑卒中后SHS患者作为研究对象,根据随机数... 目的 探讨Bobath康复训练改善脑卒中后肩手综合征(SHS)患者上肢运动功能效果。方法 采用便利抽样方法选取2022年4月至2023年1月南通市第三人民医院(南通大学附属南通第三医院)康复科收治的110例脑卒中后SHS患者作为研究对象,根据随机数字表法分为研究组和对照组,各55例。2组患者均实施基础护理,对照组实施常规康复锻炼,研究组同时联合Bobath康复训练。干预前后,对比2组患者Fugl-Meyer量表(FMA)评分、肩关节活动度评分、肩手综合征评定量表(SHSS)评分及患侧上肢Ashworth分级。结果 干预后,2组患者FMA评分及肩关节前屈、后伸、外展角度均大于干预前,且研究组大于对照组(P<0.05)。干预后,2组患者SHSS评分均低于干预前,且研究组低于对照组(P<0.05)。干预前,2组患者中不同Ashworth分级患者占比差异无统计学意义(P>0.05);干预后,研究组Ashworth分级0~Ⅰ+级患者占比高于对照组(P<0.05)。结论 对脑卒中后SHS患者实施Bobath康复训练可提高上肢功能及肩关节活动度,可改善患者患侧上肢痉挛情况。 展开更多
