BACKGROUND A giant juvenile fibroadenoma(GJF)is a rare,benign breast tumor that affects females<18 years of age.GJFs are generally suspected based on a palpable mass.GJFs influence breast shape and mammary gland de...BACKGROUND A giant juvenile fibroadenoma(GJF)is a rare,benign breast tumor that affects females<18 years of age.GJFs are generally suspected based on a palpable mass.GJFs influence breast shape and mammary gland development via the pressure effect from their enormous size.CASE SUMMARY Herein we report a case involving a 14-year-old Chinese female with a GJF in the left breast.GJF is a rare,benign breast tumor that usually occurs between 9 and 18 years of age and accounts for 0.5%-4.0%of all fibroadenomas.In severe cases,breast deformation may occur.This disease is rarely reported in Chinese people and has a high clinical misdiagnosis rate due to the absence of specific imaging features.On July 25,2022,a patient with a GJF was admitted to the First Affiliated Hospital of Dali University.The preoperative clinical examination and conventional ultrasound diagnosis needed further clarification.The mass was shown to be an atypical lobulated mass during the operation and confirmed to be a GJF based on pathologic examination.CONCLUSION GJF is also a rare,benign breast tumor in Chinese women.Evaluation of such masses consists of a physical examination,radiography,ultrasonography,computer tomography,and magnetic resonance imaging.GJFs are confirmed by histopathologic examination.Mastectomy is not selected when the patient benefits from a complete resection of the mass with breast reconstruction and an uneventful recovery.展开更多

However, our case is a rare one that reached 18 cm in a 23-year-old female referred post-partum for evaluation of a huge left breast mass. On the other hand, there is a persistent debate regarding the best modality of the surgical approach to giant tumors;most of the experts recommend the enulceation of the tumor after delivery while others argue that extirpation during pregnancy is the best since we have to exclude malignancy. In our patient, the whole tumor was excised at the postpartum period and the pathology did not show any sign of malignancy. Even if our case does not clearly reveal the long term result of the postpartum excision of a giant fibroadenoma, the short term result we have had after this approach was satisfying. Further studies are important to be published in order to confirm pros and cons of the postpartum approach.展开更多

It has been appro...Background: Ultrasound-guided, vacuum-assisted excision [UGVAE] of fibroadenomas is an emerging minimally invasive procedure. UGVAE is being increasingly employed by breast radiologists and surgeons. It has been approved for this use since 2006 by the National Institute for Health and Clinical Excellence in the UK. The aim of this systematic review is to evaluate the efficacy and safety of this procedure based on the up to date available evidence. Methods: A comprehensive literature search of the Cochrane library, Medline, Embase, PubMed, Google scholar, Trip-database, Internet, and the reference lists of relevant articles was performed. We searched for literature with sizeable cases of therapeutic vacuum-assisted excision of fibroadenomas. All prospective studies with more than 20 study participants were screened. The study quality was assessed using the Grading of Recommendations Assessment, Development, and Evaluation [GRADE] approach. All were in English. Results: Of the four case series that fulfilled the inclusion criteria there were 516 patients in which 520 procedures were performed. Overall complete excision rate was 89.4%. In two of the studies, this was 100% when the lesions were less than 1.5 cm but reduced to 61% when between 1.5 and 2 cm. Most common significant complication was bleeding with 2 cases of pneumothorax. None of the studies can be regarded as high quality going by GRADE approach. Conclusion: Within the limit of the current published case series, UGVAE appears to be an effective and safe procedure for excision of small fibroadenomas of less than 15 mm in size. A well designed case control study or randomized controlled trials with a sufficient sample size is needed to further assess its safety, effectiveness and more importantly, patients’ satisfaction especially in larger sized fibroadenomas.展开更多

It is a source of concern and psychological problems in females...Background: Fibroadenoma is the commonest benign tumour of the breast affecting the majority of young females below the age of thirty in our environment. It is a source of concern and psychological problems in females requiring medical attention in our hospitals. Aim: This study describes the clinicopathologic characteristics of fibroadenoma in Eastern Nigerian Igbo females. Methodology: The archives of the Department of Pathology Federal Medical Centre, Owerri was used for the data collection and analyzed using simple statistics. Results: 504 fibroadenomas were analyzed. The average size was 1.99 cm with a range of 0.5 cm to 18 cm. The average age of the patients was 22.6 ± 6.7 years with a range of 9 to 60 years. The peak age of incidence was in the 11 - 20 (44.64%) age group followed closely by the 21 - 30 (43.45%) age group. The ratio of involvement of the left to the right breast was 1:1.2. Fibroadenoma was 18% multiple and 10.7% bilateral. 5.3% (27), 0.6% (3), and 0.2% (1) of the cases were giant, complex fibroadenoma and malignant breast disease in fibroadenoma respectively. Conclusion: This study has provided insights into the clinicopathologic characteristics of fibroadenoma among Nigerian Igbos and calls for more research in this area in other parts Nigeria and elsewhere.展开更多

We report a case of right breast cancer in a 15-year-old female student diagnosed in 2020. This is the youngest reported case of breast cancer arising within fibroadenomas in the literature. The teenager has no personal or family history of cancer or known hereditary disease. Currently, there is no consensus on the management of breast cancer arising from adenofibromas in adolescents. Clinically, the tumor was inflammatory with axillary lymph node involvement (T4dN1M0). The fine-needle aspiration cytologic was suspicious of malignancy. The patient received four courses of neoadjuvant chemotherapy with 5-fluorouracil<sup>®</sup>, doxorubicin<sup>®</sup> and cyclosphosphamide<sup>®</sup>, followed by radical right mastectomy. The histological result of the surgical specimen is a non-specific carcinoma with a therapeutic effect greater than fifty percent. The patient received three courses of adjuvant chemotherapy with paclitaxel<sup>®</sup>. The adolescent underwent radiation therapy at her chest wall of 50 Gy and regional lymph node of 46 Gy. After radiotherapy, hormone therapy with Tamoxifene<sup>®</sup> at 20 mg per day was started. At 16 months from radiotherapy, there is no recurrence. The reconstruction of the right breast is planned in a few months.展开更多
Objective To analyze the characteristics of the intestinal microflora in patients with breast fibroadenoma using 16S ribosomal RNA(rRNA)high-throughput sequencing.Methods Fecal samples from 20 patients with breast fib...Objective To analyze the characteristics of the intestinal microflora in patients with breast fibroadenoma using 16S ribosomal RNA(rRNA)high-throughput sequencing.Methods Fecal samples from 20 patients with breast fibroadenoma and 36 healthy subjects were randomly collected and analyzed using high-throughput sequencing technology for 16S rRNA V4 region sequencing,and the alpha diversity(Chao index,Shannon index)was calculated using Mothur(v.1.39.5)software.Beta diversity was analyzed using QIIME(v1.80).SPSS software(version 23.0)and the t-test of two independent samples were used to analyze differences in the abundance of bacteria between the two groups.Results Compared with that in the healthy control group,theαdiversity of the intestinal microflora in breast fibroadenoma patients increased,but the difference was not statistically significant(P>0.05).At the phylum level,significant differences were observed between the two groups.The abundance of Firmicutes was higher in the breast fibroadenoma group(P<0.05),whereas the abundance of Synergistetes was higher in the healthy control group(P<0.005).A total of five bacterial genera showed significant differences between the two groups:the breast fibroadenoma group showed higher levels of Bautia(P<0.005),Coprococcus(P<0.005),Roseburia(P<0.05),and Ruminococcus(P<0.005),whereas Sutterella was more abundant in the healthy control group than in the breast fibroadenoma group(P<0.05).Conclusion The diversity and abundance of the intestinal flora in patients with breast fibroadenoma are significantly different from those in healthy subjects,suggesting that an imbalance in the intestinal flora is correlated with the occurrence of breast fibroadenoma.展开更多

文摘Objective To analyze the characteristics of the intestinal microflora in patients with breast fibroadenoma using 16S ribosomal RNA(rRNA)high-throughput sequencing.Methods Fecal samples from 20 patients with breast fibroadenoma and 36 healthy subjects were randomly collected and analyzed using high-throughput sequencing technology for 16S rRNA V4 region sequencing,and the alpha diversity(Chao index,Shannon index)was calculated using Mothur(v.1.39.5)software.Beta diversity was analyzed using QIIME(v1.80).SPSS software(version 23.0)and the t-test of two independent samples were used to analyze differences in the abundance of bacteria between the two groups.Results Compared with that in the healthy control group,theαdiversity of the intestinal microflora in breast fibroadenoma patients increased,but the difference was not statistically significant(P>0.05).At the phylum level,significant differences were observed between the two groups.The abundance of Firmicutes was higher in the breast fibroadenoma group(P<0.05),whereas the abundance of Synergistetes was higher in the healthy control group(P<0.005).A total of five bacterial genera showed significant differences between the two groups:the breast fibroadenoma group showed higher levels of Bautia(P<0.005),Coprococcus(P<0.005),Roseburia(P<0.05),and Ruminococcus(P<0.005),whereas Sutterella was more abundant in the healthy control group than in the breast fibroadenoma group(P<0.05).Conclusion The diversity and abundance of the intestinal flora in patients with breast fibroadenoma are significantly different from those in healthy subjects,suggesting that an imbalance in the intestinal flora is correlated with the occurrence of breast fibroadenoma.
文摘Fibroadenomas are the most common lumps found in adolescence girls. They usually occur under the influence of hormonal changes that the girls undergo at the time of puberty and they happen due to overgrowth of glandular tissue in the breast. Ultrasonography is the best diagnosis for fibroadenoma. There is no medical treatment for fibroadenoma except surgical excision and a new method recently comes out—vacuum assisted fibroadenomas resection.
文摘Breast fibroadenoma is the most common benign tumor of the breast in women,and can occur at all ages.However,these tumors are more commonly seen in women aged 15 to 35 years.[1]Most fibroadenomas often undergo self-limited growth and usually stabilize after several years.The clinical diagnosis is mainly based on clinical palpation and ultrasonographic examination,while the golden standard diagnosis is pathological examination.To standardize the clinical diagnosis and treatment of breast fibroadenoma,the Chinese Society of Breast Surgery(CSBrS)conducted a literature review of experts’opinions,and determined the key clinical questions for the clinical practice guideline of breast fibroadenoma.The group evaluated the relevant evidences using the grading of recommendations assessment,development,and evaluation system,and developed the clinical practice guideline for breast fibroadenoma:CSBrS practice guideline 2021,with the aim of providing clinical practice guidance to breast surgeons in China.
