AIM To assess the long-term prognostic value of vascular endothelial growth factor receptor 1(VEGFR1)and classⅢβ-tubulin(TUBB3)mRNA expression in nonmetastatic rectal cancer.METHODS A total of 75 consecutive patient...AIM To assess the long-term prognostic value of vascular endothelial growth factor receptor 1(VEGFR1)and classⅢβ-tubulin(TUBB3)mRNA expression in nonmetastatic rectal cancer.METHODS A total of 75 consecutive patients with non-metastatic rectal cancer from March 2004 to November 2008 were analyzed retrospectively at our institute.The mRNA expressions of VEGFR1 and TUBB3 were detected by multiplex branched DNA liquid-chip technology.The Cutoff Finder application was applied to determine cutoff point of mRNA expression.SPSS software version 22.0was used for analysis.RESULTS The median follow-up was 102.7 mo(range,6-153.6).Theχ~2 and Fisher’s exact tests showed that VEGFR1expression was related to lymph node metastasis(P=0.013),while no relationships between TUBB3 and clinicopathological features were observed.Univariate analysis showed that T stage,lymph node metastasis,tumor differentiation,VEGFR1 and TUBB3 mRNA expression were correlated to overall survival(OS)(P=0.048,P=0.003,P=0.052,P=0.003 and P=0.015,respectively).Also,lymph node metastasis and VEGFR1expression independently influenced OS by multivariate analysis(P=0.027 and P=0.033).VEGFR1 expression was positively correlated with TUBB3(P=0.024).The patients with low expression of both TUBB3 and VEGFR1 presented a better OS(P=0.003).In addition,the receiver operating characteristic analysis suggested that the combination of lymph node metastasis and VEGFR1 had a more favorable prognostic value(P<0.001).CONCLUSION VEGFR1 expression and lymph node metastasis independently and jointly affect survival.Moreover,low expression of VEGFR1 and TUBB3 presented a better OS in patients with non-metastatic rectal cancer,which might serve as a potential prognostic factor.展开更多
On November 26,2019 a strong earthquake of magnitude M6.4 occurred close to the City of Durrës,Albania(15.6 km WSW(west southwest)of Mamurras and 22 km SSW(south southwest)of Durrës),causing fatalities and c...On November 26,2019 a strong earthquake of magnitude M6.4 occurred close to the City of Durrës,Albania(15.6 km WSW(west southwest)of Mamurras and 22 km SSW(south southwest)of Durrës),causing fatalities and considerable damages in many buildings.In this article we present and analyze,by means of observational data and numerical simulation,the behavior under this earthquake of an 8-floor RC(reinforcement concrete)building,by using design spectra referring to KTP-N.2-89 and Eurocode 8.The main purpose of the authors is to better understand and evaluate the seismic performance of high-rise buildings under the design spectra with a period of soil oscillation close to the fundamental period of the structure.展开更多

Cons...A novel method for analysing the performance of power saving class of Type III in IEEE 802.16e is proposed, which is applicable to design, maintenance and management for mobile wireless metropolitan area network. Considering the memoryless nature of user initiated packet arrival, a Geom/G/1 queue model with multiple vacations and setup period is built to capture the principle for the power saving class of Type III. By using an embedded Markov chain method and the boundary state variable theory, we obtain the queueing measures such as queueing length, waiting time and busy cycle in steady state. Correspondingly, we derive explicitly the performance measures for the power saving class of Type III in terms of handover ratio, energy saving ratio, and average packet response time. Based on numerical results, we develop a cost function to determine numerically the optimal length of sleep window and the minimal cost with different offered loads.展开更多
Background: Surgical site infections (SSIs) are considered as result of the healthcare quality in hospitals. Objective: to study SSI at Saint Jean de Dieu Hospital Tanguieta (SJDHT), prior to the implementation of a p...Background: Surgical site infections (SSIs) are considered as result of the healthcare quality in hospitals. Objective: to study SSI at Saint Jean de Dieu Hospital Tanguieta (SJDHT), prior to the implementation of a permanent monitoring system. Method: transversal, and descriptive study with prospective data collection was performed from 1 July to 31 janvier 2017 in the department of general surgery of SJDHT. The hospital lacks in a microbiology unit. All patients who underwent surgery during this period were included and the monitoring lasted one month. SSIs diagnostic was carried out according to WHO criteria as described in the Practical Guide for the Prevention of Nosocomial Infections published in 2002. Statistical tests (χ-square and Student’s t-test) were applied and p 0.05 were statistically significant. Results: Of 343 patients recorded, 105 (30.6%) had SSI. Their age averaged 40.3 years and the sex-ratio (men/women) was 2.8. The emergency surgery resulted in a 50.0% rate of SSI (p = 0.00). The SSI rate for clean and clean-contaminated surgery was 6.3% against 94.6% for infected surgery (p = 0.00). The SSI rates were 100% and 66.7% for NNISS = 2 and NNISS = 1 (p = 0.00), respectively. Superficial SSI rate was 13.3%, while deep SSI and organ/space SSI were 46.7% and 40%, respectively. The hospital stay of patients with SSI was three times longer than the length of patients without SSI (p = 0.00). Conclusion: SSIs are real burden at SJDHT. Appropriate measures must be adopted to reduce its prevalence.展开更多
