Muslim cancer patients and health professionals are faced with a dilemma between the necessity of fasting and its possible adverse effects during Ramadan on the health status of cancer patients. We believe that addressing the issue of fasting in cancer patients is of global interest. Objectives: Prevalence of fasting in cancer patients during Ramadan;Predictive factors of Ramadan fasting in cancer patients;The effect of fasting during Ramadan on the quality of life of cancer patients. Materials and Methods: A structured questionnaire was used to interview adult cancer patients who received chemotherapy IV—or oral treatment at the medical oncology department at the Hassan II University Hospital in Fez during the month of Ramadan 1443 Hijri (April to May 2022). The quality of cancer patients’ lives during their fasting practice in Ramadan was evaluated via the QLQ-C30 questionnaires. Results: 48% of patients did not fast during Ramadan, 22% and 30% were partial and complete fasters respectively. Patients, with OMS 0 to 1, who receive outpatient treatment, non-metastatic patients and patients on oral treatment were more frequent in the completely fasted group. All these factors showed statistically significant values (p < 0.05). 44% of patients sought advice from the treating oncologist if they could fast, while only 15% of patients during Ramadan asked an imam if they could be excused from fasting. A comparison of the QLQ-C30 data showed a significant improvement in emotional functioning with fasting, however, patients had clinically more but not significantly fatigue after fasting. Conclusion: Female gender, good WHO, non-metastatic disease, and oral or outpatient treatment were more likely to be fasting. This study argues that fasting did not influence the quality of life, and appears to significantly improve emotional functioning in cancer patients during Ramadan.展开更多

Background:Hypertension is one of the leading causes of death in developed and developing countries that need urgent strategies to implement interventions.Appropriate lifestyle modification practices are the corner st...Background:Hypertension is one of the leading causes of death in developed and developing countries that need urgent strategies to implement interventions.Appropriate lifestyle modification practices are the corner stone of the prevention and control.Objective:To assess lifestyle modification practices and associated factors among hypertensive patients.Methods:A facility-based cross-sectional study was conducted among hypertensive patients from 1 April to 30 April 2019.Simple random sampling was used to select the study subjects.Data were entered to EpiData 3.1 and exported to Statistical Package for Social Sciences,version 25.0,for analysis.A binary logistic regression analysis was performed to determine predictors of lifestyle modifications among hypertensive patients.Results:The findings of this study revealed that the level of lifestyle modification practice was 79(39.5%).This finding is very low,and it has a significant effect on the management of hypertensive disorders and hypertension-related acute and chronic complications.Ethnicity,family history of hypertension,and knowledge about hypertension were identified as predictors of lifestyle modification practice.Conclusions:In this study,lifestyle modification practice is low(about 40%),but scientifically,almost all hypertensive patients on follow-up should practice lifestyle modification modalities.展开更多
The loudspeakers played a crucial role in rural community during the COVID-19 pandemic,and had returned to the public with the help of short video platforms.However,with the continued popularity of digital media,is th...The loudspeakers played a crucial role in rural community during the COVID-19 pandemic,and had returned to the public with the help of short video platforms.However,with the continued popularity of digital media,is the revival of the loudspeakers which belong to traditional media just a flash in the pan?What the historic process of return has the loudspeakers undergone?Based on these questions,the paper applies Bourdieu’s sense of practice theory,focuses on“habit”throughout,adopts the research methods of online ethnography and fieldwork,and conducts participant observation and targeted in-depth interviews in a village in North China for about nine months.The study found that:(1)There is the lag of habit in the state’s handling of social relations in villages.Based on rational habit,village cadres used the loudspeakers to manifest their physical presence and restructure their authority;the villagers,operating under perceptual habit,rationalized the presence of the loudspeakers through using the key words such as elders,nostalgia,and homesickness;(2)the loudspeakers was maintained under the habit of village cadres and ordinary villagers,and the outbreak of the epidemic made the loudspeakers as an habit truly“appearing”;(3)the young village cadres,on the other hand,acquired this habit through imitation,transposed the loudspeakers into the short video platform,and carried out different forms of experimentation and innovation,thus surpassing the previous forms of loudspeakers practice.The return of the loudspeakers is the result of the game of different habits.As a long-lasting affection medium,the loudspeakers,on the one hand,has become an important media scene and memory field indispensable to rural community,and on the other hand,the people living on this land have a deep attachment to the loudspeakers,which is indirectly transformed into the bearer of homesickness.This brings new inspiration for us to rethink the relationship between the traditional media and the new media,the iteration of cadres and the change of rural community.展开更多

Although treatment with sorafenib und...Sorafenib is a new treatment indicated for patients with advanced renal cell carcinoma who have failed prior cytokine-based therapy or are considered unsuitable for such therapy. Although treatment with sorafenib under ‘ideal trial conditions’ has been extensively studied, registration and reimbursement authorities are also interested in the behavior of sorafenib in real-life practice. This study aims to conduct a literature review of the dosage and treatment duration;safety, tolerability and effectiveness;costs and cost-effectiveness of sorafenib in routine clinical care. Studies were identified by searching PubMed, Embase, Centre for Reviews and Dissemination databases, Cochrane Database of Systematic Reviews, and EconLit up to November 2010. The literature search included articles published in peer-reviewed journals, congress abstracts, and internal studies of Bayer Schering Pharma. Eight studies were included. An open-label study observed stable disease for at least eight weeks in 80% of patients. The most common drug-related adverse events were hand-foot skin reaction, rash, hypertension, and fatigue. Although treatment with sorafenib led to fewer dose reductions, it was also associated with a shorter treatment duration, less time to pro-gression and a shorter survival time as compared to sunitinib. Monthly health care costs were lower with sorafenib as compared to sunitinib. A post-marketing surveillance study showed that patients rated the tolerability and effectiveness of sorafenib as very good, good or sufficient. In conclusion, the current evidence is too limited to derive conclusions and existing studies suffer from methodological shortcomings.展开更多

There are several studies with both drugs performed in clinical practice (also called “real life studies”). Despite the pros and cons, studies performed in real life conditions represent everyday practice and add important information about long term treatment effectiveness and safety in this clinical setting. This review shows that patients treated with first line nucleos(t)ide analogs at referral centres, with good clinical follow-up and adherence to international guidelines, can achieve high treatment response rates with a very low rate of adverse events.展开更多

Background: Typical treatment of breast cancer is surgery, often followed by chemotherapy. Associated aesthetic ramifications, including hair loss, frequently cause patient distress. Methods: We conducted face-to-face semi-structured hour-long recorded interviews with three nurses. We then analyzed the developmental process according to the core competencies for interprofessional collaborative practice using the thematic content analysis. Results: Nurses worked with interdisciplinary team members with mutual respect and shared values such as concern for patients’ quality of life. Nurses used knowledge of appearance-related side effects combined with beautician-provided coping skills. Intervention involved responsible communication with other health professionals and patients in a team approach. Nurses promoted the developmental process and became program facilitators. Conclusion: This group-intervention program, facilitated by nurses, was integrated into daily practice. Implications for nursing management: Research partnerships between academics, clinical nurses, nurse managers, and beauticians can improve the integration of interventions in routine practice and increase awareness of patients’ needs.展开更多

Objective:The objective of the study is to describe the work-related quality of life(WRQOL)among nurses and explore its association with performance evaluation ratings.Methods:A cross-sectional design was utilized mea...Objective:The objective of the study is to describe the work-related quality of life(WRQOL)among nurses and explore its association with performance evaluation ratings.Methods:A cross-sectional design was utilized measuring the WRQOL scale and the previous performance appraisal rating.Multistage sampling approach was utilized with nurses stratified based on their position and then systematically random sampled based on their unit assignment.Results:One hundred and eighty-two nurses were included.About half reported a high quality of work life(101;55.5%).Low scores were noted on the subscale working conditions(100;54.9%),low to average responses for home-work interface(109;59.9%),control at work(100;54.9%),and stress at work(90;49.5%).A higher proportion of nurses reported positive responses toward the areas of general well-being(113;62.1%)and job-career satisfaction(112;61.5%),than in the other subscales.There were notable differences between WRQOL ratings:(1)Head nurses had the highest perceived quality of work life,followed by nurse supervisors and charge nurses(F=6.1,P<0.01)and(2)Nurses in the pay-patient services reported lower quality of working life,while those in office and outpatient services had more positive scores(F=4.6,P<0.01).Conclusion:Only more than half of the nurses reported a high quality of work life,some of its dimensions,particularly job and career satisfaction and working conditions,appeared to vary in the perceived degree across years in service,work hours,and position.The assessment of the quality of work life may serve as an important tool to address staff burnout,absenteeism and other issues that affect job performance among health-care professionals.展开更多
