(Pro)renin receptor [(P)RR], a receptor for renin and prorenin, was first cloned in 2002. (Pro)renin receptor [(P)RR], a receptor for renin and prorenin, was first cloned in 2002. Since then, the pathophysiological roles of(P)RR have been growing concerns.(P)RR binds renin and prorenin, with two important consequences, nonproteolytic activation of prorenin, leading to the tissue renin-angiotensin system activation and the intracellular signalings. It is now also known to play an important role as vacuolar H+-ATPase associated protein, involving in Wnt signaling, main component of embryonic development. Extracellular domain of full-length(P)RR is cleaved in golgi-complex forming soluble(P)RR [s(P)RR]. The s(P)RR is now possible to be measured in human blood and urine. It is now measured in different pathophysiological states, and recent study showed that elevated plasma s(P)RR levels in the early stage of pregnancies are associated with higher incidence of gestational diabetes mellitus later in the pregnancies. Plasma s(P)RR levels of neonates are known to be higher than that of adults. It was also shown that, increased s(P)RR concentrations in cord blood, associated with a lower small for gestational age birth likelihood. These data suggests the involvement of(P)RR in embryo's growth. In thisreview article, we attempt to figure out the possible pathophysiological roles of the(P)RR in maternal glucose intolerance and embryo's growth, through reviewing previous studies. 展开更多

The role of (pro)rennin receptor (PRR) in cardiomyocytes of a heart failure (HF) rat model was studied. Spontaneously hypertensive rats (SHR) with HF (SHR-HF) or not were identified by two-dimensional (2-D) ultrasound. Age-matched Wistar Kyoto normotensive (WKY) rats were used as controls. PRR short hair RNA (sh-RNA) was injected into the heart of SHR-HF. Simultaneously SHR and controls received the same shRNA injection into the heart. Scramble shRNA was injected into the heart as controls. The expression of PRR mRNA and protein in cardiomyocytes was detected by using real-time PCR and Western blotting respectively. The heart function was evaluated by 2-D ultrasound, including eject fraction (EF%), fractional shortening (FS%), left ventricle thickness (LV), and in- ter-ventricular septal thickness (IVS). The number of apoptotie cardiomyocytes was counted by using flow cytometry. The results showed that the mRNA and protein expression levels of PRR were signifi- cantly higher in cardiomyocytes of SHR-HF group than in those of SHR group or control group. The apoptosis of myocytes in SHR-HF group was increased as compared with SHR group or control group. After knock-down of PRR with shRNA in SHR-HF group, the apoptosis of myocytes was reduced, re- sulting in the improved heart function. It was suggested that down-regulation of PRR might protect the heart from development of HF in SHR-HF by inhibiting the apoptosis of cardiomyocytes. 展开更多

目的探讨脐血中可溶性髓样细胞触发受体(sTREM-1)、肾上腺髓质素前体(pro-ADM)水平对未足月胎膜早破(pPROM)早产儿早发型细菌感染的预测作用。方法选取2019年3月至2020年8月本院新生儿科收治的80例pPROM早产儿作为观察组,根据是否发生... 目的探讨脐血中可溶性髓样细胞触发受体(sTREM-1)、肾上腺髓质素前体(pro-ADM)水平对未足月胎膜早破(pPROM)早产儿早发型细菌感染的预测作用。方法选取2019年3月至2020年8月本院新生儿科收治的80例pPROM早产儿作为观察组,根据是否发生早发型细菌感染将其分为感染组和非感染组;另选同期收治的30例健康新生儿为对照组。比较观察组与对照组、感染组与非感染组的脐带血sTREM-1、pro-ADM水平,以及感染组与非感染组患儿的白细胞计数(WBC)、C反应蛋白(CRP)、白细胞介素6(IL-6)水平。应用受试者工作特征(ROC)曲线,评估各指标对pPROM早产儿早发型细菌感染的预测价值。结果观察组pPROM早产儿的sTREM-1、pro-ADM水平高于对照组(t值分别是2.137、1.999,P<0.05);感染组患儿的sTREM-1、pro-ADM、IL-6水平高于非感染组患儿(t值分别是7.307、6.179、3.179,P<0.05);血清sTREM-1、pro-ADM、IL-6水平对pPROM患儿早发型细菌感染具有较高的诊断价值(AUC值分别是0.813、0.792、0.769)。结论脐血中sTREM-1、pro-ADM、IL-6对pPROM早产儿早发型细菌感染具有较高的预测价值,可为临床鉴别pPROM早产儿早发型细菌感染提供有效依据。 展开更多

目的探讨血清N端脑利钠肽前体(NT-proBNP)、NOD样受体热蛋白结构域相关蛋白3(NLRP3)水平与急性缺血性卒中(AIS)患者恶性脑水肿(MBE)的关系。方法选取302例AIS患者,根据是否发生MBE分为MBE组和非MBE组。收集患者基线资料,采用酶联免疫吸... 目的探讨血清N端脑利钠肽前体(NT-proBNP)、NOD样受体热蛋白结构域相关蛋白3(NLRP3)水平与急性缺血性卒中(AIS)患者恶性脑水肿(MBE)的关系。方法选取302例AIS患者,根据是否发生MBE分为MBE组和非MBE组。收集患者基线资料,采用酶联免疫吸附法检测血清NT-proBNP、NLRP3。多因素Logistic回归分析AIS患者MBE的影响因素,采用受试者工作特征(ROC)曲线分析血清NT-proBNP、NLRP3水平对AIS患者MBE的预测价值。结果302例AIS患者共发生MBE 47例(15.56%)。与非MBE组比较,MBE组血清NT-proBNP、NLRP3水平更高(P均<0.05)。多因素Logistic回归分析显示,美国国立卫生研究院卒中量表评分(OR=1.175,95%CI:1.089~1.267)、NT-proBNP(OR=1.002,95%CI:1.001~1.003)、NLRP3(OR=2.527,95%CI:1.745~3.661)为AIS患者MBE的独立危险因素(P均<0.05)。ROC曲线分析结果显示,血清NT-proBNP、NLRP3水平联合预测AIS患者MBE的曲线下面积大于二者单独预测(P均<0.05)。结论血清NT-proBNP、NLRP3水平升高是AIS患者MBE独立危险因素,二者可作为AIS后MBE的辅助预测指标。 展开更多

目的探讨三种新型肾上腺髓质素前体(ProADM)、可溶性糖基化终末产物受体(sRAGE)、高迁移率族蛋白B-1(HMGB-1)炎症标志物水平对重症监护室(ICU)感染患者病情严重程度及预后的评估价值。方法选取2018年1月至2019年12月本院ICU收治的86例... 目的探讨三种新型肾上腺髓质素前体(ProADM)、可溶性糖基化终末产物受体(sRAGE)、高迁移率族蛋白B-1(HMGB-1)炎症标志物水平对重症监护室(ICU)感染患者病情严重程度及预后的评估价值。方法选取2018年1月至2019年12月本院ICU收治的86例感染患者为研究对象,根据脓毒症定义的标准将其分为脓毒症组55例、脓毒性休克组31例;另选取40例同期本院健康体检者作为对照组。收集三组临床一般资料,于入院24 h内采用酶联免疫吸附法检测ProADM、sRAGE、HMGB-1水平。评估研究组患者急性生理学及慢性健康状况评分系统(APACHE Ⅱ)评分、序贯器官功能障碍评分(SOFA),并随访1个月,记录随访结果(死亡或存活)。采用Pearson相关性分析ProADM、sRAGE、HMGB-1与APACHE Ⅱ、SOFA评分及预后的相关性。结果三组的ProADM、sRAGE、HMGB-1指标比较差异具有统计学意义(P<0.05);经两两比较发现,脓毒症组及脓毒性休克组的ProADM、sRAGE、HMGB-1指标显著高于对照组,差异具有统计学意义(P<0.05);且脓毒性休克组的各指标显著高于脓毒症组,差异具有统计学意义(P<0.05)。脓毒性休克组的APACHE Ⅱ、SOFA评分均显著高于脓毒症组,差异具有统计学意义(P<0.05)。随访结果显示,有36例患者死亡,50例患者存活,死亡率为41.86%,死亡组患者的ProADM、sRAGE、HMGB-1指标显著高于存活组,差异具有统计学意义(P<0.05)。Pearson相关性分析结果显示,ProADM、sRAGE、HMGB-1指标与APACHE Ⅱ、SOFA评分及预后均呈正相关关系(P<0.05)。结论 ICU脓毒症患者ProADM、sRAGE、HMGB-1水平高于健康人,且与患者感染严重程度及预后情况密切相关。 展开更多

Val-Glu-Pro(VEP)是从钝顶螺旋藻(Spirulina platensis)中发现的一种血管紧张素转化酶(ACE)抑制肽.以原发性高血压大鼠(SHR)为模型,检测单次口服和一周间口服VEP的降压效果,并通过Real-timePCR和酶联免疫吸附法(ELISA)探索其对肾素-血管紧张素系统(RAS)主要成分在SHR大鼠肾脏和血清中的表达调控作用.结果表明:口服VEP的最低有效降压剂量为5mg/kg,加大剂量后表现出剂量效应,最低加权收缩压(WSBP)出现在口服后6h,最低加权舒张压(WDBP)出现在口服后4 h.在一周间口服试验中,10 mg/kg VEP处理组的WSBP在第5日显著低于负对照组.此外,口服VEP显著下调了SHR大鼠肾脏中肾素(renin)、ACE、血管紧张素Ⅱ(AngⅡ)类型1受体(AT1)的mRNA表达,并上调AngⅡ类型2受体(AT2)的mRNA表达,说明VEP的降压效果可能与对RAS系统的抑制作用相关,在高血压的预防和治疗中具有潜在的应用前景. 展开更多

Hepatic fibrosis is considered a common response to many chronic hepatic injuries. It is a multifunctional process that involves several cell types, cytokines, chemokines and growth factors leading to a disruption of homeostatic mechanisms that maintain the liver ecosystem. In spite of many studies regarding the development of fibrosis, the understanding of the pathogenesis remains obscure. The hepatic tissue remodeling process is highly complex, resulting from the balance between collagen degradation and synthesis. Among the many mediators that take part in this process, the components of the Renin angiotensin system (RAS) have progressively assumed an important role. Angiotensin (Ang) II acts as a profibrotic mediator and Ang-(1-7), the newly recognized RAS component, appears to exert a counter-regulatory role in liver tissue. We briefly review the liver fibrosis process and current aspects of the RAS. This review also aims to discuss some experimental evidence regarding the participation of RAS mediators in the pathogenesis of liver fibrosis, focusing on the putative role of the ACE2-Ang-(1-7)- Mas receptor axis. 展开更多

Portal hypertension is responsible for the bulk of the morbidity and mortality in patients with cirrhosis.Drug therapy to reduce portal pressure involves targeting two vascular beds.The first approach is to reduce intra hepatic vascular tone induced by the activity of powerful vasocontrictors such as angiotensin Ⅱ,endothelin-1 and the sympathetic system and mediated via contraction of perisinusoidal myofibroblasts and pervascular smooth muscle cells.The second approach is to reduce mesenteric and portal blood flow.Non-selective b-blockers are widely used and have been shown to prolong patient survival and reduce oesophageal variceal bleeding in advanced cirrhosis.However many patients are unable to tolerate these drugs and they are ineffective in a significant proportion of patients.Unfortunately there are no other drug therapies that have proven efficacy in the treatment of portal hypertension and prevention of variceal bleeding.This review briefly outlines current therapeutic approaches to themanagement of portal hypertension,and the evidence supporting the role of the renin angiotensin system(RAS) and the use of RAS blockers in this condition.It will also outline recent advances in RAS research that could lead to the development of new treatments focusing in particular on the recently discovered "alternate axis" of the RAS. 展开更多

AIM To investigate effect of losartan,an AT1receptor antagonist,on hepatic fibrosis induced byCCl<sub>;</sub>and to determine whether or not AT1receptors are expressed on hepatic stellate cells,METHODS AND RESULTS Fifty male Sprague-Dawley rats,weighing(180±20)g,wererandomized into five groups(control group,modelgroup,and three losartan treated groups),inwhich all rats were given the subcutaneousinjection of 40% CCl<sub>4</sub>(every 3 days for 6 weeks)except for rats of control group.Rats of losartan-treated groups were treated with losartan(20 mg/kg,10 mg/kg,5 mg/kg,daily gavage),After 6weeks liver tissue and serum samples of all ratswere examined.Serum hyaluronic acid(HA),procollagen typeⅢ(PCⅢ)were detected byradioimmunoassays,van Giesion collagen stainingwas used to evaluate the extracellular matrix of ratswith liver fibrosis.The expression of AT1receptors,transforming growth factor-beta(TGF-β),and alpha-smooth muscle actin(a-SMA)inliver tissue were determined byimmunohistochemical techniques.Compared withmodel group,serum ALT and AST of losartan-treated groups were significantly reduced(t=4.20,P【0.01 and t=4.57,P【0.01).Serum HAand PCⅢalso had significant differences(t=3.53,P【0.01 and t=2.20,P【0.05).Thedegree of fibrosis was improved by losartan and correlated with the expressions of AT1 receptors,TGF-β,and α-SMA in liver tissue.CONCLUSION AT1 receptor antagonist,losartan,could limit the progression of the hepatic fibrosisinduced by CCl<sub>4</sub>.The mechanism may be related tothe decrease in the expression of AT1 receptorsand TGF-β,ameliorating the injury of hepatocytes;activation of local renin-angiotensin system mightrelate to hepatic fibrosis;and during progressionof fibrosis,activated hepatic stellate cells mightexpress AT1 receptors. 展开更多

目的:脓毒症是宿主对感染的反应失调引起的危及生命的器官功能障碍。由于缺乏有效的治疗手段,脓毒症一直是临床治疗的难点和挑战。研究表明脑源性神经营养因子前体(pro-brain-derived neurotrophic factor,proBDNF)可通过结合其高亲和... 目的:脓毒症是宿主对感染的反应失调引起的危及生命的器官功能障碍。由于缺乏有效的治疗手段,脓毒症一直是临床治疗的难点和挑战。研究表明脑源性神经营养因子前体(pro-brain-derived neurotrophic factor,proBDNF)可通过结合其高亲和力受体p75神经营养因子受体(p75 neurotrophin receptor,p75^(NTR))激活下游信号通路,扰乱免疫炎症微环境,在脓毒症病程的进展中发挥重要作用。本研究主要探讨淋巴细胞来源的proBDNF/p75^(NTR)在脓毒症患者中的表达变化及其对淋巴细胞分化的影响。方法:收集健康志愿者(对照组,n=40)和初次入院的脓毒症患者(脓毒症组,n=40)外周血样本,进行血常规临床指标检测,采用流式细胞术检测淋巴细胞亚群及其proBDNF/p75^(NTR)的表达变化;体外分选对照组外周血淋巴细胞并采用脂多糖(lipopolysaccharide,LPS)刺激培养,流式细胞分析技术检测LPS刺激对淋巴细胞亚群proBDNF/p75^(NTR)的表达影响;随后采用p75^(NTR)的拮抗剂(p75^(ECD)-Fc)抑制p75^(NTR)观察对淋巴细胞分化的影响。结果:与对照组相比,脓毒症组患者入院时白细胞计数、中性粒细胞计数和中性粒细胞百分比均显著升高,淋巴细胞计数与淋巴细胞百分比均显著降低(均P<0.001),中性粒细胞与淋巴细胞比值、单核细胞与淋巴细胞比值均显著升高(均P<0.05)。与对照组相比,proBDNF在脓毒症组外周血中CD19^(+)B细胞的表达上调(P<0.05),而p75^(NTR)在CD19^(+)B细胞、CD4^(+)T细胞和CD8^(+)T细胞中的表达均上调(均P<0.05)。体外采用LPS刺激可诱导对照组外周血淋巴细胞的proBDNF/p75^(NTR)表达上调(均P<0.05),趋势与在脓毒症组外周淋巴细胞的表达变化基本一致。抑制p75^(NTR)可增加CD4^(+)T细胞和CD19^(+)B细胞的百分比,促进细胞因子IL-4和IL-10的表达,并降低IL-1β和IL-6的产生(均P<0.05)。结论:脓毒症患者入院时即处于以淋巴细胞数量减少为特征的免疫抑制阶段,同时伴有中性粒细胞占比增加。ProBDNF/p75^(NTR)在脓毒症患者外周血淋巴细胞表达增加,抑制p75^(NTR)可能通过调节淋巴细胞的分化,参与脓毒症进展。 展开更多

Nonalcoholic fatty liver disease(NAFLD) is the commonest liver disease in Western countries.Treatment of NAFLD is currently based on lifestyle measures and no effective pharmacologic treatment is available so far.Emerging evidence,mainly from animal studies,suggests that the renin-angiotensin-aldosterone system may be of major importance in the pathogenesis of NAFLD and indicates that angiotensin-converting enzyme inhibitors(ACE-I) and angiotensin receptor blockers(ARBs) as a potentially useful therapeutic approach.However,data from human studies are limited and contradictory.In addition,there are few randomized controlled trials(RCTs) on the effects of ACE-I or ARB in patients with NAFLD and most data are from retrospective studies,pilot prospective studies and post hoc analyses of clinical trials.Accordingly,more and larger RCTs are needed to directly assess the effectiveness of ACE-I and ARBs in NAFLD. 展开更多

Background: Inflammation is often linked with the progress and poor outcome of lung cancer. The understanding of the relationship between tumor-associated macrophages (TAMs) and lung cancer cells involves in the underlying mechanism of inflammatory cytokine production. Toll-like receptors (TLRs) are engaged in promoting the production of pro-inflammatory cytokines and play an important role in tumor immunology. Methods: To investigate the mechanisms by which TAMs influence the production of pro-inflammatory cytoldnes in lung cancer cells, we established an in vitro coculture system using TAMs and human non- small cell lung cancer (NSCLC) cell line SPC-A1. Levels of interleukin (IL)-113, IL-6 and IL-8 in SPC-A1 were evaluated by RT-PCR and cytometric bead array assay after being cocultured with TAMs. Expression changes of TLRs and TLRs signaling pathway proteins in SPC-Al were further confirmed by RT-PCR and western blot. The level changes of IL-1β, IL-6 and IL-8 in SPC-Al were also detected after the stimulation of TLRs agonists. Results: We found that the phenotype markers of TAMs were highly expressed after stimulating human monocyte cell line THP-1 by phorbol-12-myristate-β-acetate (PMA). Higher mRNA and supernate secretion levels of IL-1β, IL-6 and IL-8 were detected in SPC-A1 after being eocultured with TAMs. We also found that TLR1, TLR6 and TLR7 were up-regulated in SPC-A1 in the coculture system with TAMs. Meanwhile, TLRs signaling pathway proteins were also significantly activated. Moreover, pre-treatment with agonist ligands for TLR1, TLR6 and TLR7 could dramatically promote inductions of IL-1β, IL-6 and IL-8. Conclusions: These findings demonstrated that TAMs may enhance IL-1β, IL-6 and IL-8 expressions via TLRs signaling pathway. We conclude that TAMs contribute to maintain the inflammation microenvironment and ultimately promote the development and progression of lung cancer. 展开更多

However, the association of RAS components to cancer still causes suspicion. To try to clarify this, here we aimed to show this association for three important components: Angiotensin Converting Enzyme 1 (ACE1), Angiotensin Type 1 Receptor (AGTR1) and Angiotensin Type 2 Receptor (AGTR2). The first articles show that association of RAS components with cancer dates back to the 70’s. ECA1 and AGTR1 have close association with cancer and ACE1 inhibitors or AGTR1 blockers are candidates to treatment of some tumors. Moreover, the action of AGTR2 is still controversial, but most studies show that the increased expression of AGTR2 can attack the cancer cells. In breast cancer, these components have also been widely studied and many works have shown that the correlation exists. Therefore specific target using these RAS components could be a beneficial, novel therapy to various tumors. 展开更多

目的探讨一氧化氮吸入(inhalation of nitric oxide,iNO)联合高频振荡通气(High frequency oscillatory ventilation,HFOV)治疗新生儿肺动脉高压的价值及对中性粒细胞明胶酶相关载脂蛋白(Neutrophil gelatinase-associated apolipoprote... 目的探讨一氧化氮吸入(inhalation of nitric oxide,iNO)联合高频振荡通气(High frequency oscillatory ventilation,HFOV)治疗新生儿肺动脉高压的价值及对中性粒细胞明胶酶相关载脂蛋白(Neutrophil gelatinase-associated apolipoprotein,NGAL)、氨基末端B型脑钠肽前体(N-terminal B-type pro-brain natriuretic peptide,NT-proBNP)、芳香烃受体核转位蛋白(Aryl hydrocarbon receptor nuclear translocator,ARNT)表达水平的影响。方法选取我院于2018年1月-2021年1月收治的164例肺动脉高压新生儿为研究对象,按照随机数字表法分为研究组和对照组,各82例。对照组给予米力农+HFOV治疗,研究组给予iNO+HFOV治疗。比较两组患儿治疗效果,治疗前、后新生儿动脉血气、氧合指数、通气指标以及NGAL、NT-proBNP和ARNT水平变化情况。结果研究组治疗有效率明显高于对照组(92.68%vs 81.71%),有创和无创通气时间以及氧暴露时间均短于对照组,差异有统计学意义(P<0.05)。主要并发症为上呼吸道感染、肺出血、肺炎、气漏、颅内出血以及低血压,其中研究组上呼吸道感染、肺炎发生率低于对照组(P<0.05)。治疗后两组患儿PaO_(2)、OI均明显增加,而PaCO_(2)明显降低,其中研究组动脉血气指标水平均优于对照组(P<0.05)。治疗后两组患儿血浆NGAL、NT-proBNP和ARNT水平均明显降低,其中研究组血浆NGAL、NT-proBNP和ARNT水平明显低于对照组(P<0.05)。结论iNO联合HFOV治疗新生儿肺动脉高压效果良好,可有效改善新生儿动脉血气状态、氧合状况,降低NGAL、NT-proBNP和ARNT水平,以及缩短有创和无创通气时间,且安全性较好。 展开更多
