Functional imaging of skeletal muscle glucose metabolism by <sup>18</sup>FDG PET to characterize insulin resistance in patients at high risk for coronary artery disease

Insulin resistance is associated with several coronary risk factors and is thought to play a critical role for the development of coronary artery disease. Insulin resistance has several causes, including an impaired skeletal muscle glucose utilization rate (SMGU), reduced peripheral blood flow, and altered fatty tissue metabolism, with SMGU being considered the most important. Nonetheless, insulin resistance has only been estimated by the glucose disposal rate (GDR) in previous studies. Methods: Skeletal muscle metabolic imaging with 18FDG and positron emission tomography (PET) was undertaken to measure SMGU during hyperinsulinemiceuglycemic clamping in 22 normotensive type-2 diabetics under no medications (T2- DM), 17 normotensive non-diabetic hypertriglyceridemics, 22 patients with hypertension, and 12 agematched controls. Whole body insulin resistance was assessed by the GDR during hyperinsulinemiceuglycemic insulin clamping. Results: The SMGU and GDR were significantly reduced in T2DM (32.1 ± 16.6 μmol/min/kg and 24.3 ± 13.0 μmol/min/kg, respectively), hypertriglyceridemics (36.5 ± 13.5 μmol/min/ kg and 22.7 ± 8.07 μmol/min/kg respectively) and patients with hypertension (35.4 ± 26.6 μmol/min/kg and 29.0 ± 9.90 μmol/min/kg, respectively) compared with controls (72.2 ± 44.1 μmol/min/kg and 43.0 ± 22.9 μmol/min/kg, p < 0.01, respectively). In all groups studied, SMGU was significantly correlated with GDR (r = 0.76, p < 0.01) and GDR (F = 13.9) was independently related to SMGU (r = 0.81, p < 0.01). Conclusion: Insulin resistance is significantly associated with SMGU to a similar degree among patients with T2DM, essential hypertension and hypertriglyceridemia. 18FDG PET functional imaging allows insulin resistance to be assessed. 展开更多

Currently,18F-FDG coincidence SPECT(Co-SPECT)/CT scan still serves as an important tool for diagnosis,staging,and evaluation of cancer treatment in developing countries.We implemented full physical corrections(FPC) to Co-SPECT(quantitative Co-SPECT) to improve the image resolution and contrast along with the capability for image quantitation.FPC included attenuation,scatter,resolution recovery,and noise reduction.A standard NEMA phantom filled with 10:1 F-18 activity concentration ratio in spheres and background was utilized to evaluate image performance.Subsequently,15 patients with histologically confirmed thoracic carcinomas were included to undergo a 18 F-FDG Co-SPECT/CT scan followed by a 18 F-FDG PET/CT scan.Functional parameters as SUVmax,SUVmean,SULpeak,and MTV from both quantitative Co-SPECT and PET were analyzed.Image resolution of Co-SPECT for NEMA phantom was improved to reveal the smallest sphere from a diameter of 28 mm to 22 mm(17 mm for PET).The image contrast was enhanced from 1.7 to 6.32(6.69 for PET) with slightly degraded uniformity in background(3.1% vs.6.7%)(5.6% for PET).Patients’ SUVmax,SUVmean,SULpeak,and MTV measured from quantitative Co-SPECT were overall highly correlated with those from PET(r=0.82-0.88).Adjustment of the threshold of SUVmax and SUV to determine SUVmean and MTV did not further change the correlations with PET(r=0.81-0.88).Adding full physical corrections to Co-SPECT images can significantly improve image resolution and contrast to reveal smaller tumor lesions along with the capability to quantify functional parameters like PET/CT. 展开更多

Mucosa-associated lymphoid tissue (MALT) lymphoma usually originates from the stomach and presents with low ^(18)F-fluorodeoxyglucose (FDG) avidity with average maximum standard uptake value of 3.6. Colorectal MALT lymphoma is a rare entity that contributes to 1.6% of all MALT lymphomas and < 0.2% of large intestinal malignancies. The case reported herein firstly revealed stage Ⅱ MALT lymphoma with unexpected higher ^(18)F-FDG avidity of 18.9 arising at the colorectal anastomosis in a patient with a surgical history for sigmoid adenocarcinoma, which was strongly suspected as local recurrence before histopathological and immunohistochemical examinations. After accurate diagnosis, the patient received four cycles of standard R-CVP regimen (rituximab, cyclophosphamide, vincristine and prednisone), combined target therapy and chemotherapy, instead of radiotherapy recommended by National Comprehensive Cancer Network guidelines. 目的通过18F-FDG PET/CT脑葡萄糖代谢显像在感音性聋大鼠模型中探讨持续强噪声对脑功能代谢的影响。方法 SD大鼠给予120 dB SPL噪声刺激前24 h和刺激后72 h分别检测每只大鼠的听性脑干反应。继而对噪声刺激前、后大鼠分别行18F-FDG PET... 目的通过18F-FDG PET/CT脑葡萄糖代谢显像在感音性聋大鼠模型中探讨持续强噪声对脑功能代谢的影响。方法 SD大鼠给予120 dB SPL噪声刺激前24 h和刺激后72 h分别检测每只大鼠的听性脑干反应。继而对噪声刺激前、后大鼠分别行18F-FDG PET/CT脑葡萄糖代谢显像,采用脑代谢定量分析软件计算标准摄取值的平均值以及全脑/小脑靶本底比值,SPM软件评估噪声刺激前、后不同脑功能区脑葡萄糖代谢水平的差异。结果给予噪声刺激后,大鼠双侧颞叶脑葡萄糖代谢均显著高于未给予噪声刺激时双侧颞叶葡萄糖代谢水平(P<0.01)。SPM分析发现噪声刺激导致双侧听觉功能区脑葡萄糖代谢显著增加。结论长时间持续噪声刺激不但导致听觉功能障碍,还引起大脑听觉功能区脑葡萄糖代谢明显增加,具体机制还有待于进一步研究。 展开更多

目的评价^(18)F-氟脱氧葡萄糖(^(18)F-flurodeoxyglucose,^(18)F-FDG)双探头符合线路显像(dual head coincidence imaging,DHCI)结合血清肿瘤标志物分析在肺癌诊断中的价值。方法选取77例经病理证实的肺癌患者同时行^(18)F-FDG DHCI检查... 目的评价^(18)F-氟脱氧葡萄糖(^(18)F-flurodeoxyglucose,^(18)F-FDG)双探头符合线路显像(dual head coincidence imaging,DHCI)结合血清肿瘤标志物分析在肺癌诊断中的价值。方法选取77例经病理证实的肺癌患者同时行^(18)F-FDG DHCI检查和4项肿瘤标志物癌胚抗原(carcinoembryonic antigen,CEA)、糖类抗原199(carbohydrate antigen,CA199)、CA125和CA153的测定。选取45例肺良性病变患者和49例健康对照者同时行肿瘤标志物检测。结果肺癌组与肺良性病变组、对照组比较,4种肿瘤标志物水平差异均具有统计学意义(均P<0.05)。^(18)F-FDG DHCI对肺癌诊断的阳性率为85.7%。血清肿瘤标志物CEA、CA125、CA153和CA199的阳性率分别为39.0%、24.7%、9.1%和16.9%,4种肿瘤标志物任意1项阳性且DHCI阳性的为52.0%,4种肿瘤标志物任意1项阳性或DHCI阳性的为92.2%。结论单一^(18)F-FDG DHCI检测与肿瘤标志物联合DHCI检测阳性率无差异,但可减少显像阴性、肿瘤标志物阳性肺癌患者的漏诊率。 展开更多

目的:比较新旧两种糖负荷联合静脉注射胰岛素方法在冠心病(CHD)患者氟代脱氧葡萄糖(^(18)F-FDG)正电子发射型计算机断层扫描(PET/CT)心肌代谢显像中的价值。方法:回顾性分析广东省人民医院2018年4月16日至2021年4月2日收治的160例CHD患... 目的:比较新旧两种糖负荷联合静脉注射胰岛素方法在冠心病(CHD)患者氟代脱氧葡萄糖(^(18)F-FDG)正电子发射型计算机断层扫描(PET/CT)心肌代谢显像中的价值。方法:回顾性分析广东省人民医院2018年4月16日至2021年4月2日收治的160例CHD患者的心肌代谢显像临床资料,比较新旧两种调糖方案的图像质量以及两种调糖方案的不良反应情况。结果:(1)38例旧方案有糖尿病(DM)或糖耐量异常的患者(A1)中,29例患者图像质量较好,9例图像质量差,其中有8例通过延迟1 h后图像可以诊断;29例新方案有DM或糖耐量异常患者的(B1)中,23例患者图像质量较好,6例图像质量差,通过延迟1 h后图像均可以诊断。A1方案中图像质量较好占比为76.3%(29/38),B1方案则为79.3%(23/29),两种方案比较,差异无统计学意义(P>0.05)。(2)旧方案无DM或糖耐量异常患者(A2)中,图像质量较好占比67.9%(38/56),新方案无DM或糖耐量异常患者(B2)中,图像质量较好占比86.5%(32/37),且所有图像均可诊断。(3)旧方案出现3例低血糖患者,新方案无患者出现低血糖不良反应,两种方案不良反应发生率比较,差异无统计学意义(P>0.05)。结论:在CHD患者^(18)F-FDG PET/CT心肌代谢显像中采用新调糖方案是可行的,且图像质量与旧方案无差异,并且不出现低血糖等不良反应。 展开更多

AIM To investigate the relationship between glucose metabolism and glypican-3(GPc3)expression in hepatocellular carcinoma(Hcc).METHODSImmunohistochemical staining of pathological samples for GPc3 and glucose transporter 1(GLUT1),and whole-body ^(18)F-FDG PET/c T for measuring tumour glucose uptake were performed in 55 newly diagnosed Hcc patients.The maximum standard uptake value(s UVmax)and tumour-to-non-tumourous liver uptake(T/NT)ratio were used to quantify ^(18)F-FDG uptake.In vitro ^(18)F-FDG uptake assay of GPc3-expressing Hep G2 and non-GPc3-expressing RH7777 cel ls was used to examine the effect of GPc3 in cellular glucose metabolism.The relationships between GPc3 expression and ^(18)F-FDG uptake,GLUT1 expression,tumour differentiation,and other clinical indicators were analysed using spearman rank correlation,univariateand multiple logistic regression analyses.RESULTSPositive GPc3 expression was observed in 67.3%of Hcc patients,including 75.0%of those with well or moderately differentiated Hcc and 36.4%of those with poorly differentiated Hcc.There was an inverse relationship between GPc3 expression and s UVmax(Spearman correlation coefficient=-0.281,P=0.038)and a positive relationship between GLUT1 expression and sU Vmax(Spearman correlation coefficient=0.681,P<0.001)in patients with Hcc.Univariate analysis showed that two glucose metabolic parameters(sU Vmax and T/NT ratio),tumour differentiation,lymph node metastasis,and TNM stage were all significantly associated with GPc3 expression(P<0.05),whereas GLUT1 expression,sex,age,tumour size,intrahepatic lesion number,and distant metastasis showed no statistical association(P>0.05).Further multivariate analysis revealed that only the T/N ratio was significantly correlated with GPC3 expression in patients with Hcc(P<0.05).In vitro assay revealed that the uptake of ^(18)F-FDG in GPc3-expressing HepG2 cells was significantly lower than that of non-GPc3-expressing RH7777 cells(t=-20.352,P<0.001).CONCLUSIONThe present study demonstrated that GPc3 expression is inversely associated with glucose metabolism,suggesting that GPc3 may play a role in regulating glucose metabolism in Hcc. 展开更多

A 18FDG PET/CT-based volume parameter is a predictor of overall survival in patients with local advanced gastric cancer 被引量:2

Objective: The present study investigated the prognosis value of preoperative fluorodeoxyglucose(FDG)positron emission tomography/computed tomography(PET/CT) in patients with local advanced gastric cancer(LAGC).Methods: In total, 144 patients [median age 63(range: 48-80) years old] with LAGC underwent 18 F-FDG PET/CT prior to any treatment. The maximum standardized uptake values(SUVmax), mean standardized uptake values(SUVmean), metabolic tumor volume(MTV) and total lesion glycolysis(TLG) of the primary lesion were measured on PET/CT and correlated with clinicopathological features and survival.Results: Significant differences in SUVmean, SUVmax, MTV and TLG were found according to Lauren’s classification, histologic grade and T category(P<0.05). During the 26.5-month follow-up, 51(35.4%) patients died and 70(48.6%) exhibited disease progression. The optimal thresholds of MTV and TLG were 15.1 cm^3 and 47.3 cm^3, respectively. The 3-year progression-free survival(PFS) and overall survival(OS) for patients with high TLG values were 30% and 38% compared to 38% and 47% for low TLG values, respectively(P<0.05). Univariate and multifactor analyses demonstrated that lymph node metastasis and T stage were independent prognostic factors for PFS;T stage, histologic grade and TLG were independent prognostic factors for OS(P<0.05). Molecular markers had no relationship with patient’s outcomes.Conclusions: Metabolic activity of primary gastric tumors from 18 F-FDG PET/CT is a prognostic factor in patients with LAGC. 展开更多

目的 探讨SPECT/PET^18F-FDG检查对淋巴瘤患者化疗早期及治疗后评价疗效的应用价值.方法 回顾性分析我院1998年至2008年病理活检和免疫组化确诊的70例淋巴瘤患者的83例次SPECT/PET检查结果,预后的单因素分析采用生存分析法.结果 40例患... 目的 探讨SPECT/PET^18F-FDG检查对淋巴瘤患者化疗早期及治疗后评价疗效的应用价值.方法 回顾性分析我院1998年至2008年病理活检和免疫组化确诊的70例淋巴瘤患者的83例次SPECT/PET检查结果,预后的单因素分析采用生存分析法.结果 40例患者化疗2~4个疗程后进行SPECT/PET检查,结果阳性者和阴性者中位无进展生存(PFS)期分别为5.5个月和15.5个月,2年的PFS率分别为12.5%和66.8%,两组之间差异有统计学意义(P<0.01);中位总体生存(OS)期分别为12.5个月和17.0个月,1年OS率分别为28.8%和94.1%,两组之间差异有统计学意义(P=0.003).43例患者在完成全部疗程后行SPECT/PET检查,结果阳性者和阴性者中位PFS期分别为10个月和23个月,2年的PFS率分别为23.3%和83.2%,两组之间差异有统计学意义(P<0.001);中位OS期分别为17个月和27个月,2年OS率分别为60.0%和100%,两组之间差异有统计学意义(P=0.001).结论 SPECT/PET检查结果对淋巴瘤患者监测疗效和判断预后有重要意义. 展开更多

Background: Skeletal muscle glucose utilization (SMGU) can be accessed by positron emission tomography (PET) and18F-FDG to characterize insulin resistance. The quantity of skeletal muscle in the lumbar is sufficient to indicate that SMGU in the lumbar (SMGU- lumbar) can be measured with18F-FDG PET of the chest instead of obtaining thigh muscle SMGU (SMGU-thigh). This would reduce PET scan time to avoid thigh muscle PET scan. This study was aimed to compare SMGU-lumbar and thigh muscle SMGU under insulin clamping to identify the validity of measurements of SMGU in the lumbar for studies of insulin resistance. Methods: Thirty-three patients underwent sequential dynamic18F-FDG PET of both the thoracic (37 min) and thigh region (22 min) during hyperinsulinemic euglycemic insulin clamping. Both SMGU-lumbar and SMGU-thigh were calculated by Patlak graphical analysis. Whole body insulin resistance was assessed by a whole body glucose disposal rate during hyperinsulinemic euglycemic insulin clamping. Input function was obtained from the time activity curve of the descending aorta and venous blood sampling as previously validated. Results: SMGU-thigh (0.0506 ± 0.0334 μmol/min/g) was comparable to SMGU-lumbar (0.0497 ± 0.0255 μmol/min/g). The Bland-Altman method of difference plot analysis showed a significant correlationship between SMGU- thigh and SMGU-lumbar (r = 0.506, p = 0.0028). There were seen very good significant correlationship between whole body glucose utilization rate in both thigh (r = 0.737, p = 0.0001) and lumbar (r = 0.772, p = 0.0001). Conclusion: These results support the validity of measuring SMGU-lumbar to estimate insulin resistance during PET imaging of the chest. 展开更多

Background: Existence of myocardial insulin resistance (IR) has been reported in type II diabetics (T2- DM) and coronary artery disease (CAD). Improvement in heart and skeletal muscle IR after thiazolidinedione’s therapy was reported in T2DM and CAD. However effects of troglitazone therapy (TRO) on myocardial IR remain uncertain. To clarify heart and skeletal muscle and whole body IR in T2DM without CAD by TRO to clarify whether TRO would provide different results. Methods: We analyzed data on 15 T2DM patients who underwent dynamic PET with 18F-FDG under insulin clamping before and during TRO (200 mg/day) and 17 controls. Results: Whole body glucose disposal rate (WBGR mg/min/kg) in T2DM before TRO (3.41 ± 1.72) was significantly lower than in controls (9.76 ± 2.97, p < 0.01) as was the skeletal muscle glucose utilization rate (SMGU mg/min/kg);T2DM (0.367 ± 0.217) vs. controls (1.34 ± 0.613, p < 0.01) and myocardial glucose utilization rate (MGU mg/min/kg;T2DM 5.86 ± 2.03 vs. controls 7.34 ± 1.80, p < 0.05). WBGR in T2DM during TRO (5.17 ± 2.75, p < 0.05) was significantly higher than that before TRO, as was the SMGU (0.782 ± 0.20, p < 0.05). The MGU in T2DM during TRO (6.59 ± 0.72) was comparable with that before TRO. Conclusion: Myocardial IR response to TRO differed from that in skeletal muscle and the whole body in T2DM without CAD. 展开更多

目的利用18氟-氟脱氧葡萄糖(18F-FDG)小动物正电子发射计算机断层显像(micro PET)观察糖尿病小鼠心肌葡萄糖代谢的改变。方法取正常野生型BKS小鼠5只、db/db糖尿病小鼠5只,正常进食,控制氧流量400ml/min,分别在1.5%~1.8%异氟烷麻醉下尾... 目的利用18氟-氟脱氧葡萄糖(18F-FDG)小动物正电子发射计算机断层显像(micro PET)观察糖尿病小鼠心肌葡萄糖代谢的改变。方法取正常野生型BKS小鼠5只、db/db糖尿病小鼠5只,正常进食,控制氧流量400ml/min,分别在1.5%~1.8%异氟烷麻醉下尾静脉弹丸式注射50~75μl18F-FDG 200~300uci(7.4~11.1MBq)进行micro PET动态显像60min,图像重建后勾画心肌感兴趣区(ROI)获取心肌时间-放射性曲线(TAC),计算心肌葡萄糖标准摄取值(SUV),同时经小鼠尾静脉多点采血获取尾静脉血TAC,利用输入函数数学模型显像分析系统(KIS),计算两组心肌的葡萄糖代谢率(MRGlu),并行t检验。结果与BKS小鼠比较,db/db小鼠显像45~55min SUV、Ki值[(12.92±1.68)vs(5.70±1.19)g/ml,P<0.01],[(0.20±0.05)vs(0.09±0.02)ml/(min·g),P<0.01]降低;平均血糖、葡萄糖代谢率[(6.10±1.31)vs(24.39±3.36)mmol/L,(21.01±2.98)vs(40.14±4.32)mg/(kg·min),P<0.01]升高。结论 18F-FDG micro PET显像可提示db/db糖尿病小鼠心肌葡萄糖摄取及代谢率的改变。 展开更多
