CAD is a shared burden disease and the leading ...Background: Diabetes mellitus (DM) is independently associated with an increased risk of cardiovascular mortality and morbidity, including coronary artery disease (CAD). CAD is a shared burden disease and the leading cause of death in developed and developing countries. We aimed to assess the angiographic patterns of coronary arteries in patients with DM in a developing country (Yemen) as the first study. Methods: This study is a cross-sectional, prospective, observational study that includes a total of 250 patients who were admitted for elective diagnostic coronary angiography. Results: 96 (38.4%) patients were diabetics;68% were male;mean age was 57 ± 11 years. The incidence of three-vessel disease was 31.2% of patients. Considering the severity of lumen occlusion, (11.2%) of patients had non-significant lesions, (37.6%) of patients had significant lesions, and (32%) had total occlusive lesions. Lesions were of LAD in 76%, RCA in 60%, and LCX in 52% of the population. Among diabetics, two and 3-vessel diseases (33.3% vs. 20.8% & 50% vs. 19.5%, P = 0.001), left main lesion (10.4% vs. 2.6%, P = 0.012), significant stenosis (41.7% vs. 35.1%, P = 0.032), total occlusion of coronary arteries (43.8% vs. 19.5%, P = 0.032) and type C lesion (66.7% vs. 35.1%, P = 0.010) were more frequent than non-DM patients. Conclusion: The burden of significant and severe coronary lesions is more common among DM, which may be the major cause of morbidity and mortality of DM in developing countries.展开更多

Rapid improvements in multislic...Multislice CT angiography represents one of the most exciting technological revolutions in cardiac imaging and it has been increasingly used in the diagnosis of coronary artery disease. Rapid improvements in multislice CT scanners over the last decade have allowed this technique to become a potentially effective alternative to invasive coronary angiography in patients with suspected coronary artery disease. High diagnostic value has been achieved with multislice CT angiography with use of 64- and more slice CT scanners. In addition, multislice CT angiography shows accurate detection and analysis of coronary calcium, characterization of coronary plaques, as well as prediction of the disease progression and major cardiac events. Thus, patients can benefit from multislice CT angiography that provides a rapid and accurate diagnosis while avoiding unnecessary invasive coronary angiography procedures. The aim of this article is present an overview of the clinical applications of multislice CT angiography in coronary artery disease with a focus on the diagnostic accuracy of coronary artery disease; prognostic value of coronary artery disease with regard to the prediction of major cardiac events; detection and quantification of coronary calcium and characterization of coronary plaques. Limitations of multislice CT angiography in coronary artery disease are also briefly discussed, and future directions are highlighted.展开更多
Objective:We aimed to evaluate the diagnostic performance of three-dimensional whole-heart magnetic resonance coronary angiography(MRCA)in detecting coronary artery disease(CAD)with invasive coronary angiography as th...Objective:We aimed to evaluate the diagnostic performance of three-dimensional whole-heart magnetic resonance coronary angiography(MRCA)in detecting coronary artery disease(CAD)with invasive coronary angiography as the reference standard.Methods:We searched PubMed and Embase for studies evaluating the diagnostic performance of three-dimensional whole-heart MRCA for the diagnosis of CAD with invasive coronary angiography as the reference standard.The bivariate mixed-effects regression model was applied to synthesize available data.The clinical utility of whole-heart MRCA was calculated by the posttest probability based on Bayes’s theorem.Results:Eighteen studies were included,of which 16 provided data at the artery level.Patient-based analysis revealed a pooled sensitivity of 0.90(95%confi dence interval[CI]0.87–0.93)and specifi city of 0.79(95%CI 0.73–0.84),while the pooled estimates were 0.86(95%CI 0.82–0.89)and 0.89(95%CI 0.84–0.92),respectively,at the artery level.The areas under the summary receiver operating characteristic curve were 0.93(95%CI 0.90–0.95)and 0.92(95%CI 0.90–0.94)at the patient and artery levels,respectively.With a pretest probability of 50%,the patients’posttest probabilities of CAD were 81%for positive results and 11%for negative results.Conclusions:Whole-heart MRCA can be an alternative noninvasive method for diagnosis and assessment of CAD.展开更多

The cardiovascular professional may be involved in their angiographic diagnosis, fimctional assessment and eventual endovascular treatment. A complete angiographic definition is mandatory in order to understand the functional effects and plan any intervention in CAAs: computed tomography and magnetic resonance imaging are useful non-invasive tools to detect three-dimensional morphology of the anomalies and its relationships with contiguous cardiac structures, whereas coronary arteriography remains the gold standard for a definitive anatomic picture. A practical idea of the possible functional sig- nificance is mandatory for deciding how to manage CAAs: non-invasive stress tests and in particular the invasive pharmacological stress tests with or without intravascular ultrasound monitoring can assess correctly the functional significance of the most CAAs. Finally, the knowledge of the particular endovascular techniques and material is of paramount importance for achieving technical and clinical success. CAAs represent a complex issue, which rarely involve the cardiovascular professional at different levels. BACKGROUND Portal vein gas(PVG)is an abnormal accumulation of gas within the portal and intrahepatic portal veins.It is associated with various abdominal diseases,ranging from benign conditions to life-threatening one...BACKGROUND Portal vein gas(PVG)is an abnormal accumulation of gas within the portal and intrahepatic portal veins.It is associated with various abdominal diseases,ranging from benign conditions to life-threatening ones that require immediate surgical intervention.Coronary angiography is the standard diagnostic procedure for coronary artery disease.There were no prior reports are available of PVG as a complication of coronary angiography.CASE SUMMARY In the specific case described here,the patient did not show signs of peritoneal irritation;however,computed tomography scans findings revealed pneumatosis in the wall of the small intestine,hepatic portal vein,and mesenteric vein,along with acute enteritis(etiology pending classification).A cesarean section was not performed,and the patient received treatment with fasting,rehydration,and antiinfection therapy.Subsequently,the patient's symptoms of abdominal distension and pain improved,and follow-up computed tomography scans indicated resolution of the portal system pneumatosis and intestinal wall edema,resulting in a favorable clinical outcome.CONCLUSION Portal venous gas complication following coronary angiography was a complication of coronary angiography.展开更多

Patients with depression who have a myocardial infarction are more likely to die and patients who have depressive symptoms during hospitalization may have increased cardiovascular events. This study aimed to determine the relationship between anxiety, depression and coronary artery disease among patients undergoing angiography in Ghaem Hospital, Mashhad. This was a case-control study conducted between September 2011 and August 2012 among patients undergoing coronary angiography in Ghaem Hospital, Mashhad, Iran. There were 486 cases that were found to have one or more coronary stenoses, with a stenosis of equal or more than 50% of the diameter of at least one major coronary artery. The other patient group consisted of the patients in whom the coronary artery stenosis was less than 50% in diameter which was classified as angiography negative, and a control group that consisted of 440 healthy adults aged 18 years old and above who were selected among people who attended for routine medical checkup and medical examination of employment. The dependent variables were Beck Anxiety and Depression Inventory scores and the independent factors were coronary artery disease, and socioeconomic profiles. Validated and reliability-tested questionnaires were used for data collection. The mean age of patients was 55.75 ± 10.64 years and in the healthy group was 55.83 ± 8.55 years;there was no significant difference in age between subject groups (p = 0.897) nor a significant difference in the gender frequency distribution of subjects (p = 0.610). Sixty-five patients disease, without percutaneous transluminal coronary angioplasty or coronary bypass-grafting, were investigated using 16-slice CT. Eight patients with pre-scan heart rate of more than 80 beats/min were given β-blockers. After the retrospectively ECG-gated axial imaging reconstruction, volume redering (VR), multi-planar reconstruction (MPR), curved MPR and maximum intensity projection (MIP) were used to reconstruct. Every segment of coronary artery with a diameter ≥1.5 mm was assessed, and the presence on CT with a stenosis exceeding 50 % diameter reduction was compared with that on SCA. The reasons which lead to some segments unevaluable were analysed. Compared with SCA, 93 % coronary segments and 94 % main branches were evaluable. Residual cardiac motion artifacts, severe calcification and poor opacification made 58 %, 28 % and 14 % of the remaining 60 segments unevaluable respectively. Without routine administration of β-blockers, good coronary imaging quality can be acquired using 16-slice spiral CT. It is a reliable noninvasive method for detection of obstructive coronary artery disease.展开更多

Growing evidence suggests that the ratios of Apo B/Apo A-I and Lp(a) are better indexes for risk assessment of CAD. Elevated plasma levels of lipoprotein(a) in humans represent a major in-herited risk factor for atherosclerosis. Thus, a study was performed to determine the association betwwen serum Apo B, Apo A-I, and lipoprotein(a) levels, and severity of CAD in patients with CAD confirmed on coronary angiography findings. An analytical case control study was carried out with 85 patients (58 males and 27 females) 40 - 60 years of age confirmed as having CAD on coronary angiography and 85 age and sex matched healthy volunteers as controls. Serum samples were an-alyzed for Apo A-1 LDL, Apo B, Apo A-I, and lipoprotein(a) concentration and the severity of CAD was assessed using coronary angiography scoring method. Patients with CAD had significantly high serum LDL-C, Apo B and Lp(a) levels compared to control subjects. However, serum Apo A-I level did not show a significant difference between two groups. Subjects with a positive family history of CAD with increased serum Lp(a) ≥ 17.3 mg/dL have high risk for development of CAD. Present study suggests that serum Lp(a) cut-off value of 17.3 mg/dL may be an important predictor in ruling out major vessel disease and luminal narrowing by atheroma.展开更多
Objective:To compare the feasibility and safety between right distal radial artery access and right radial artery access in patients undergoing coronary angiography(CAG)or percutaneous coronary intervention(PCI).Metho...Objective:To compare the feasibility and safety between right distal radial artery access and right radial artery access in patients undergoing coronary angiography(CAG)or percutaneous coronary intervention(PCI).Methods:On the basis of arterial access,113 patients who underwent CAG or PCI in Kunshan Hospital of Traditional Chinese Medicine between January and October 2018 were divided into two groups:a right distal radial artery group(52 patients)and a right radial artery group(61 patients).We collected general information,the number of puncture attempts,access times,postoperative compression time,and complications.Results:The general characteristics,rate of successful radial artery puncture,and rate of successful catheter placement in the two groups were not different.The right radial artery group had fewer puncture attempts(1.26±0.44 times vs.2.19±0.53 times,P=0.001)and a shorter access time(3.23±0.86 min vs.4.77±1.49 min,P=0.001)than the right distal radial artery group.However,the postoperative compression time in the right distal radial artery group was shorter(3.44±0.9 h vs.7.16±1.21 h,P=0.001).Two cases of bleeding,four cases of hematoma,and one case of artery occlusion in the right radial artery group and one case of hematoma in the right distal artery group occurred before discharge.The rate of total complications in the right distal radial artery group was lower than in the right radial artery group(1.93%vs.11.48%,P=0.048).Conclusion:CAG or PCI through the right distal radial artery is feasible and safe.展开更多

Coronary obstructions are generally described by location and severity of stenosis. Manual tracing and measurement of the stenosis vessel in comparison to reference location of stenosis can be helpful to classify whether a lesion is significant or not. The aim of this study is to identify the dominant type of circulation carried out in patients presented in heart institute and associated with degree of stenosis in RCA and LAD artery. Materials and Methods: This was a retrospective study carried out on patients reporting to Heart Institute from June 2013 to August 2013 (311 patients records as a total coverage were collected as sample) whose coronary artery angiography revealed pathology in coronary arteries. Result: The findings were 71.7% right dominant, 23.5% left dominant and 4.8% co-dominant;LAD tends to be stricture with right dominant circulation type. LAD artery tends to be stenosis at proximal and mid segment while in the RCA tends to be stenosis in mid and distal segments. The right dominant artery is popular type of circulation in Sudanese people reported in Sudan Heart Institute;there is no significance association between gender and arterial type of circulation and also degree stenosis in LAD degree of stenosis in RCA. I observed that the site of stenosis in LAD artery was proximal and mid (P value < 0.05 significance association). Conclusion: Patients present at the heart institute are right dominant mostly coming with proximal and middle LAD stenosis unlike RCA which commonly presents with distal and middle stenosis, so the middle segment of coronary artery is stenosis in both situations. There are wild variations in dominance artery in relation to reduction of diameter in deferent ethic group. There are no significant associations between the gender and dominant artery.展开更多

The current study investigate the relation between obstructive coronary versus normal coronary angiographies and the prevalence of complications during pregnancy including preeclampsia, pregnancy induced-hypertension, low birth weight, and preterm birth. Methods: All consecutive women aged <55 year, who had coronary angiography were included. Based on angiography outcome, patients were classified as normal (controls: no stenosis, wall irregularity without stenosis, or minimal calcification present) or abnormal (subjects). A standard questionnaire was sent to all participating women. The questionnaires inquired into risk factors for coronary vascular disease, in particular history of hypertensive diseases, and specific obstetric history. The results were analyzed using the unpaired t-test, the Chi-squared test, and Pearson’s correlation coefficients as appropriate. A significance level of P < 0.05 was used. Results: Of the 211 consecutive women aged <55 year, 62% (n = 131) had normal coronary angiography and 38% (n = 80) abnormal coronary angiography. We found no differences of reported pregnancy complications including preeclampsia, hypertension, low birth weight, and preterm birth (combined RR = 0.70, 95% C.I. = 0.38 - 1.3) in case of abnormal coronary angiography compared to women with normal coronary angiography. The traditional risk factors hypercholesterolemia, tobacco use, and diabetes were identified as the major risk factors for developing cardiovascular disease, with a relative risk ranging from 4.2 - 1.8. Conclusion: These data suggest that pregnancy complications are not an important denominator for cardiovascular disease.展开更多

The serum levels of material metabolism indexes, cytokines as well as collagen synthesis and degradation indexes of the four groups were measured. Results: Serum UA, Hcy, PCSK9, LDL-C, GDF-15, CXCL5, hs-CRP, RANTES, IL-6, MMP1, ST2, CysC and I-CTP levels of soft plaque group, mixed plaque group and calcified plaque group were significantly higher than those of control group, serum UA, Hcy, PCSK9, LDL-C, GDF-15, CXCL5, hs-CRP, RANTES, IL-6, MMP1, ST2, CysC and I-CTP levels of soft plaque group and mixed plaque group were significantly higher than those of calcified plaque group, and serum UA, Hcy, PCSK9, LDL-C, GDF-15, CXCL5, hs-CRP, RANTES, IL-6, MMP1, ST2, CysC and I-CTP levels of soft plaque group were significantly higher than those of mixed plaque group. Background The computational fluid dynamics(CFD)approach has been frequently applied to compute the fractional flow reserve(FFR)using computed tomography angiography(CTA).This technique is efficient.We developed the D...Background The computational fluid dynamics(CFD)approach has been frequently applied to compute the fractional flow reserve(FFR)using computed tomography angiography(CTA).This technique is efficient.We developed the DEEPVESSEL-FFR platform using the emerging deep learning technique to calculate the FFR value out of CTA images in five minutes.This study is to evaluate the DEEPVESSEL-FFR platform using the emerging deep learning technique to calculate the FFR value from CTA images as an efficient method.Methods A single-center,prospective study was conducted and 63 patients were enrolled for the evaluation of the diagnostic performance of DEEPVESSEL-FFR.Automatic quantification method for the three-dimensional coronary arterial geometry and the deep learning based prediction of FFR were developed to assess the ischemic risk of the stenotic coronary arteries.Diagnostic performance of the DEEPVESSEL-FFR was assessed by using wire-based FFR as reference standard.The primary evaluation factor was defined by using the area under receiver-operation characteristics curve(AUC)analysis.Results For per-patient level,taking the cut-off value<0.8 referring to the FFR measurement,DEEPVESSEL-FFR presented higher diagnostic performance in determining ischemia-related lesions with area under the curve of 0.928 compare to CTA stenotic severity 0.664.DEEPVESSEL-FFR correlated with FFR(R=0.686,P<0.001),with a mean di&ference of-0.006士0.0091(P=0.619).The secondary evaluation factors,indicating per vessel accuracy,sensitivity,specificity,positive predictive value,and negative predictive value were 87.3%,97.14%,75%,82.93%,and 95.45%,respectively.Conclusion DEEPVESSEL-FFR is a novel method that allows efficient assessment of the functional significance of coronary stenosis.展开更多

METHODS:A PubMed search was performed for articles between 2000 ...AIM:To describe the characteristics of coronary artery fistulas(CAFs) in adults,including donor vessels and whether termination was cameral or vascular. METHODS:A PubMed search was performed for articles between 2000 and 2010 to describe the current characteristics of congenital CAFs in adults.A group of 304 adults was collected.Clinical data,presentations,diagnostic modalities,angiographic fistula findings and treatment strategies were gathered and analyzed.With regard to CAF origin,the subjects were tabulated into unilateral,bilateral or multilateral fistulas and compared.The group was stratified into two major subsets according to the mode of termination;coronary-cameral fistulas(CCFs) and coronary-vascular fistulas(CVFs) . A comparison was made between the two subsets. Fistula-related major complications[aneurysm formation,infective endocarditis(IE) ,myocardial infarction(MI) ,rupture,pericardial effusion(PE) and tamponade] were described.Coronary artery-ventricular multiple micro-fistulas and acquired CAFs were excluded as well as anomalous origin of the coronary arteries from the pulmonary artery(PA) . RESULTS:A total of 304 adult subjects(47%male) with congenital CAFs were included.The mean age was51.4 years(range,18-86 years) ,with 20%older than 65 years of age.Dyspnea(31%) ,chest pain(23%) and angina pectoris(21%) were the prevalent clinical presentations.Continuous cardiac murmur was heard in 82%of the subjects.Of the applied diagnostic modalities,chest X-ray showed an abnormal shadow in 4%of the subjects.The cornerstone in establishing the diagnosis was echocardiography(68%) ,and conventional contrast coronary angiography(97%) .However,multislice detector computed tomography was performed in 16%.The unilateral fistula originated from the left in 69%and from the right coronary artery in 31%of the subjects.Most patients(80%) had unilateral fistulas,18%presented with bilateral fistulas and 2%with multilateral fistulas.Termination into the PA was reported in unilateral(44%) ,bilateral(73%) and multilateral(75%) fistulas.Fistulas with multiple origins(bilateral and multilateral) terminated more frequently into the PA(29%) than into other sites(10.6%)(P=0.000) . Aneurysmal formation was found in 14%of all subjects.Spontaneous rupture,PE and tamponade were reported in 2%of all subjects.In CCFs,the mean age was 46.2 years whereas in CVFs mean age was 55.6 years(P=0.003) .IE(4%) was exclusively associated with CCFs,while MI(2%) was only found in subjects with CVFs.Surgical ligation was frequently chosen for unilateral(57%) ,bilateral(51%) and multilateral fistulas(66%) ,but percutaneous therapeutic embolization(PTE) was increasingly reported(23%,17%and 17%,respectively) . CONCLUSION:Congenital CAFs are currently detected in elderly patients.Bilateral fistulas are more frequently reported and PTE is more frequently applied as a therapeutic strategy in adults.展开更多
文摘Background: Diabetes mellitus (DM) is independently associated with an increased risk of cardiovascular mortality and morbidity, including coronary artery disease (CAD). CAD is a shared burden disease and the leading cause of death in developed and developing countries. We aimed to assess the angiographic patterns of coronary arteries in patients with DM in a developing country (Yemen) as the first study. Methods: This study is a cross-sectional, prospective, observational study that includes a total of 250 patients who were admitted for elective diagnostic coronary angiography. Results: 96 (38.4%) patients were diabetics;68% were male;mean age was 57 ± 11 years. The incidence of three-vessel disease was 31.2% of patients. Considering the severity of lumen occlusion, (11.2%) of patients had non-significant lesions, (37.6%) of patients had significant lesions, and (32%) had total occlusive lesions. Lesions were of LAD in 76%, RCA in 60%, and LCX in 52% of the population. Among diabetics, two and 3-vessel diseases (33.3% vs. 20.8% & 50% vs. 19.5%, P = 0.001), left main lesion (10.4% vs. 2.6%, P = 0.012), significant stenosis (41.7% vs. 35.1%, P = 0.032), total occlusion of coronary arteries (43.8% vs. 19.5%, P = 0.032) and type C lesion (66.7% vs. 35.1%, P = 0.010) were more frequent than non-DM patients. Conclusion: The burden of significant and severe coronary lesions is more common among DM, which may be the major cause of morbidity and mortality of DM in developing countries.
文摘Multislice CT angiography represents one of the most exciting technological revolutions in cardiac imaging and it has been increasingly used in the diagnosis of coronary artery disease. Rapid improvements in multislice CT scanners over the last decade have allowed this technique to become a potentially effective alternative to invasive coronary angiography in patients with suspected coronary artery disease. High diagnostic value has been achieved with multislice CT angiography with use of 64- and more slice CT scanners. In addition, multislice CT angiography shows accurate detection and analysis of coronary calcium, characterization of coronary plaques, as well as prediction of the disease progression and major cardiac events. Thus, patients can benefit from multislice CT angiography that provides a rapid and accurate diagnosis while avoiding unnecessary invasive coronary angiography procedures. The aim of this article is present an overview of the clinical applications of multislice CT angiography in coronary artery disease with a focus on the diagnostic accuracy of coronary artery disease; prognostic value of coronary artery disease with regard to the prediction of major cardiac events; detection and quantification of coronary calcium and characterization of coronary plaques. Limitations of multislice CT angiography in coronary artery disease are also briefly discussed, and future directions are highlighted.

文摘Objective:We aimed to evaluate the diagnostic performance of three-dimensional whole-heart magnetic resonance coronary angiography(MRCA)in detecting coronary artery disease(CAD)with invasive coronary angiography as the reference standard.Methods:We searched PubMed and Embase for studies evaluating the diagnostic performance of three-dimensional whole-heart MRCA for the diagnosis of CAD with invasive coronary angiography as the reference standard.The bivariate mixed-effects regression model was applied to synthesize available data.The clinical utility of whole-heart MRCA was calculated by the posttest probability based on Bayes’s theorem.Results:Eighteen studies were included,of which 16 provided data at the artery level.Patient-based analysis revealed a pooled sensitivity of 0.90(95%confi dence interval[CI]0.87–0.93)and specifi city of 0.79(95%CI 0.73–0.84),while the pooled estimates were 0.86(95%CI 0.82–0.89)and 0.89(95%CI 0.84–0.92),respectively,at the artery level.The areas under the summary receiver operating characteristic curve were 0.93(95%CI 0.90–0.95)and 0.92(95%CI 0.90–0.94)at the patient and artery levels,respectively.With a pretest probability of 50%,the patients’posttest probabilities of CAD were 81%for positive results and 11%for negative results.Conclusions:Whole-heart MRCA can be an alternative noninvasive method for diagnosis and assessment of CAD.
文摘Coronary artery anomalies (CAAs) may be discovered more often as incidental findings during the normal diagnostic process for other cardiac diseases or less frequently on the basis of manifestations of myocardial ischemia. The cardiovascular professional may be involved in their angiographic diagnosis, fimctional assessment and eventual endovascular treatment. A complete angiographic definition is mandatory in order to understand the functional effects and plan any intervention in CAAs: computed tomography and magnetic resonance imaging are useful non-invasive tools to detect three-dimensional morphology of the anomalies and its relationships with contiguous cardiac structures, whereas coronary arteriography remains the gold standard for a definitive anatomic picture. A practical idea of the possible functional sig- nificance is mandatory for deciding how to manage CAAs: non-invasive stress tests and in particular the invasive pharmacological stress tests with or without intravascular ultrasound monitoring can assess correctly the functional significance of the most CAAs. Finally, the knowledge of the particular endovascular techniques and material is of paramount importance for achieving technical and clinical success. CAAs represent a complex issue, which rarely involve the cardiovascular professional at different levels. A timely practical knowledge of the main issues regarding CAAs is important in the management of such entities.
文摘Objective:Coronary artery anatomical variations and anomalies are an important topic due to their potential clinical manifestations.This study aims to investigate the prevalence of coronary artery anatomical variations and anomalies in symptomatic patients with coronary computed tomography angiography(CCTA).Methods:This is a retrospective study that included all symptomatic patients who had CCTA in a tertiary care hospital in Saudi Arabia during a period of seven years.Results:The total number of included patients was 507(60%males)with a mean age of 57.4 years.Approximately 41%had luminal stenoses,averaging 49.7%.The total num-ber of patients with coronary anatomical variations(CAV)and coronary artery anomalies(CAA)was 217(43%).CAV prevalence was 26%,which included 14%non-right coronary dominance,5%short left main coronary artery(LMCA),and 7%division variations(trifurcation and quadrifurcarion)of the LMCA.The prevalence of CAA was 29%,which included 5%origin anomalies,22%myocardial bridge,and 2%course anomalies.Conclusions:A high prevalence of coronary artery anatomic variations and anomalies in symptomatic patients is reported in this study.Systematic reviews,meta-analyses,reporting guidelines,and unified definitions and classifications of cor-onary variations and anomalies are lacking in the literature,presenting potential opportunities for future research and publications.

All patients were subjected to detailed medical history, physical examination, full electrocardiography study including tissue Doppler imaging on mitral annulus, MSCT coronary angiography and different laboratory investigation as CBC, creatinine, lipid profile. Results:?From 80 patients who have done both echocardiography examination and MSCT coronary angiography, the CT scan results were 38 patients with non-obstructive coronary artery disease and 42 patients with obstructive CAD, higher number of patients with diastolic dysfunction with higher values of E/E’, medial e’, lateral e’, LAVI (left atrial volume index) and TRV (tricuspid regurge velocity) were found in the group with obstructive coronary artery disease.?Conclusion:?Our study showed that diastolic dysfunction is more prevalent in patients with obstructive coronary artery disease with higher grade of diastolic dysfunction.
文摘BACKGROUND Portal vein gas(PVG)is an abnormal accumulation of gas within the portal and intrahepatic portal veins.It is associated with various abdominal diseases,ranging from benign conditions to life-threatening ones that require immediate surgical intervention.Coronary angiography is the standard diagnostic procedure for coronary artery disease.There were no prior reports are available of PVG as a complication of coronary angiography.CASE SUMMARY In the specific case described here,the patient did not show signs of peritoneal irritation;however,computed tomography scans findings revealed pneumatosis in the wall of the small intestine,hepatic portal vein,and mesenteric vein,along with acute enteritis(etiology pending classification).A cesarean section was not performed,and the patient received treatment with fasting,rehydration,and antiinfection therapy.Subsequently,the patient's symptoms of abdominal distension and pain improved,and follow-up computed tomography scans indicated resolution of the portal system pneumatosis and intestinal wall edema,resulting in a favorable clinical outcome.CONCLUSION Portal venous gas complication following coronary angiography was a complication of coronary angiography.
文摘The prevalence of coronary artery disease (CAD) is increasing in Iran. Patients with depression who have a myocardial infarction are more likely to die and patients who have depressive symptoms during hospitalization may have increased cardiovascular events. This study aimed to determine the relationship between anxiety, depression and coronary artery disease among patients undergoing angiography in Ghaem Hospital, Mashhad. This was a case-control study conducted between September 2011 and August 2012 among patients undergoing coronary angiography in Ghaem Hospital, Mashhad, Iran. There were 486 cases that were found to have one or more coronary stenoses, with a stenosis of equal or more than 50% of the diameter of at least one major coronary artery. The other patient group consisted of the patients in whom the coronary artery stenosis was less than 50% in diameter which was classified as angiography negative, and a control group that consisted of 440 healthy adults aged 18 years old and above who were selected among people who attended for routine medical checkup and medical examination of employment. The dependent variables were Beck Anxiety and Depression Inventory scores and the independent factors were coronary artery disease, and socioeconomic profiles. Validated and reliability-tested questionnaires were used for data collection. The mean age of patients was 55.75 ± 10.64 years and in the healthy group was 55.83 ± 8.55 years;there was no significant difference in age between subject groups (p = 0.897) nor a significant difference in the gender frequency distribution of subjects (p = 0.610). There was a significant difference in anxiety score between the Angio positive and Angio negative patients and healthy control subjects
文摘The role of 16-slice spiral CT was selective X-ray coronary angiography (SCA) who were suspected of having coronary heart evaluated in the diagnosis of coronary stenosis, with serving as the reference standard. Sixty-five patients disease, without percutaneous transluminal coronary angioplasty or coronary bypass-grafting, were investigated using 16-slice CT. Eight patients with pre-scan heart rate of more than 80 beats/min were given β-blockers. After the retrospectively ECG-gated axial imaging reconstruction, volume redering (VR), multi-planar reconstruction (MPR), curved MPR and maximum intensity projection (MIP) were used to reconstruct. Every segment of coronary artery with a diameter ≥1.5 mm was assessed, and the presence on CT with a stenosis exceeding 50 % diameter reduction was compared with that on SCA. The reasons which lead to some segments unevaluable were analysed. Compared with SCA, 93 % coronary segments and 94 % main branches were evaluable. Residual cardiac motion artifacts, severe calcification and poor opacification made 58 %, 28 % and 14 % of the remaining 60 segments unevaluable respectively. Without routine administration of β-blockers, good coronary imaging quality can be acquired using 16-slice spiral CT. It is a reliable noninvasive method for detection of obstructive coronary artery disease.

文摘Atherosclerosis is the most important contributor to increasing burden of coronary artery disease (CAD). Growing evidence suggests that the ratios of Apo B/Apo A-I and Lp(a) are better indexes for risk assessment of CAD. Elevated plasma levels of lipoprotein(a) in humans represent a major in-herited risk factor for atherosclerosis. Thus, a study was performed to determine the association betwwen serum Apo B, Apo A-I, and lipoprotein(a) levels, and severity of CAD in patients with CAD confirmed on coronary angiography findings. An analytical case control study was carried out with 85 patients (58 males and 27 females) 40 - 60 years of age confirmed as having CAD on coronary angiography and 85 age and sex matched healthy volunteers as controls. Serum samples were an-alyzed for Apo A-1 LDL, Apo B, Apo A-I, and lipoprotein(a) concentration and the severity of CAD was assessed using coronary angiography scoring method. Patients with CAD had significantly high serum LDL-C, Apo B and Lp(a) levels compared to control subjects. However, serum Apo A-I level did not show a significant difference between two groups. Subjects with a positive family history of CAD with increased serum Lp(a) ≥ 17.3 mg/dL have high risk for development of CAD. Present study suggests that serum Lp(a) cut-off value of 17.3 mg/dL may be an important predictor in ruling out major vessel disease and luminal narrowing by atheroma.
文摘Objective:To compare the feasibility and safety between right distal radial artery access and right radial artery access in patients undergoing coronary angiography(CAG)or percutaneous coronary intervention(PCI).Methods:On the basis of arterial access,113 patients who underwent CAG or PCI in Kunshan Hospital of Traditional Chinese Medicine between January and October 2018 were divided into two groups:a right distal radial artery group(52 patients)and a right radial artery group(61 patients).We collected general information,the number of puncture attempts,access times,postoperative compression time,and complications.Results:The general characteristics,rate of successful radial artery puncture,and rate of successful catheter placement in the two groups were not different.The right radial artery group had fewer puncture attempts(1.26±0.44 times vs.2.19±0.53 times,P=0.001)and a shorter access time(3.23±0.86 min vs.4.77±1.49 min,P=0.001)than the right distal radial artery group.However,the postoperative compression time in the right distal radial artery group was shorter(3.44±0.9 h vs.7.16±1.21 h,P=0.001).Two cases of bleeding,four cases of hematoma,and one case of artery occlusion in the right radial artery group and one case of hematoma in the right distal artery group occurred before discharge.The rate of total complications in the right distal radial artery group was lower than in the right radial artery group(1.93%vs.11.48%,P=0.048).Conclusion:CAG or PCI through the right distal radial artery is feasible and safe.
文摘The coronary arteries diseases are one of the most causes of mortality and morbidity worldwide, particularly in the developing countries. Coronary obstructions are generally described by location and severity of stenosis. Manual tracing and measurement of the stenosis vessel in comparison to reference location of stenosis can be helpful to classify whether a lesion is significant or not. The aim of this study is to identify the dominant type of circulation carried out in patients presented in heart institute and associated with degree of stenosis in RCA and LAD artery. Materials and Methods: This was a retrospective study carried out on patients reporting to Heart Institute from June 2013 to August 2013 (311 patients records as a total coverage were collected as sample) whose coronary artery angiography revealed pathology in coronary arteries. Result: The findings were 71.7% right dominant, 23.5% left dominant and 4.8% co-dominant;LAD tends to be stricture with right dominant circulation type. LAD artery tends to be stenosis at proximal and mid segment while in the RCA tends to be stenosis in mid and distal segments. The right dominant artery is popular type of circulation in Sudanese people reported in Sudan Heart Institute;there is no significance association between gender and arterial type of circulation and also degree stenosis in LAD degree of stenosis in RCA. I observed that the site of stenosis in LAD artery was proximal and mid (P value < 0.05 significance association). Conclusion: Patients present at the heart institute are right dominant mostly coming with proximal and middle LAD stenosis unlike RCA which commonly presents with distal and middle stenosis, so the middle segment of coronary artery is stenosis in both situations. There are wild variations in dominance artery in relation to reduction of diameter in deferent ethic group. There are no significant associations between the gender and dominant artery.
文摘Background: Recently, epidemiologic studies have suggested an association between pregnancy complications and the development of coronary artery disease later in life. The current study investigate the relation between obstructive coronary versus normal coronary angiographies and the prevalence of complications during pregnancy including preeclampsia, pregnancy induced-hypertension, low birth weight, and preterm birth. Methods: All consecutive women aged <55 year, who had coronary angiography were included. Based on angiography outcome, patients were classified as normal (controls: no stenosis, wall irregularity without stenosis, or minimal calcification present) or abnormal (subjects). A standard questionnaire was sent to all participating women. The questionnaires inquired into risk factors for coronary vascular disease, in particular history of hypertensive diseases, and specific obstetric history. The results were analyzed using the unpaired t-test, the Chi-squared test, and Pearson’s correlation coefficients as appropriate. A significance level of P < 0.05 was used. Results: Of the 211 consecutive women aged <55 year, 62% (n = 131) had normal coronary angiography and 38% (n = 80) abnormal coronary angiography. We found no differences of reported pregnancy complications including preeclampsia, hypertension, low birth weight, and preterm birth (combined RR = 0.70, 95% C.I. = 0.38 - 1.3) in case of abnormal coronary angiography compared to women with normal coronary angiography. The traditional risk factors hypercholesterolemia, tobacco use, and diabetes were identified as the major risk factors for developing cardiovascular disease, with a relative risk ranging from 4.2 - 1.8. Conclusion: These data suggest that pregnancy complications are not an important denominator for cardiovascular disease.
文摘Objective To compare coronary angiography(CAG) and optical coherence tomography(OCT) findings in stenosis and culprit lesion morphology assessment in coronary artery disease patients with moderate stenosis(50%~70%).Methods 23 CAD patients with moderate stenosis(50%~70%) were enrolled in the study.These patients then underwent OCT analysis.CAG and OCT stenosis results were compared for conformity.OCT images of the culprit lesions were analyzed for more details about the morphological characteristics of the plaques in relation with the clinical diagnosis.Results Evaluation of stenosis of coronary lesions by Quantitative Coronary Angiography(QCA) corresponds approximately with OCT analysis estimation:CAG stenosis was 57.26% ±5.05,whereas the same OCT stenosis analysis reached a value of 61.17%±11.36,(P=0.032).From OCT image analysis,the vulnerable plaque was detected more in the culprit lesions of patients with a clinical diagnosis of unstable angina,than in stable angina pectoris patients(61.5%vs10%;P=0.012).
文摘Objective: To study the clinical characteristics of different computed tomography coronary angiography (CTCA) plaque properties in patients with coronary artery disease. Methods:Patients who were diagnosed with coronary artery disease in Ankang Central Hospital between February 2015 and March 2017 were selected and divided into soft plaque group, mixed plaque group and calcified plaque group according to the plaque properties judged by CTCA examination, and healthy volunteers who received physical examination in Ankang Central Hospital during the same period were selected as control group. The serum levels of material metabolism indexes, cytokines as well as collagen synthesis and degradation indexes of the four groups were measured. Results: Serum UA, Hcy, PCSK9, LDL-C, GDF-15, CXCL5, hs-CRP, RANTES, IL-6, MMP1, ST2, CysC and I-CTP levels of soft plaque group, mixed plaque group and calcified plaque group were significantly higher than those of control group, serum UA, Hcy, PCSK9, LDL-C, GDF-15, CXCL5, hs-CRP, RANTES, IL-6, MMP1, ST2, CysC and I-CTP levels of soft plaque group and mixed plaque group were significantly higher than those of calcified plaque group, and serum UA, Hcy, PCSK9, LDL-C, GDF-15, CXCL5, hs-CRP, RANTES, IL-6, MMP1, ST2, CysC and I-CTP levels of soft plaque group were significantly higher than those of mixed plaque group. Conclusion: CTCA can accurately determine the plaque properties and evaluate the disorder of material metabolism, cytokine secretion as well as collagen synthesis and degradation during the change of plaque properties in patients with coronary artery disease.

文摘Coronary artery disease(CAD)has become a major illness endangering human health.It mainly manifests as atherosclerotic plaques,especially vulnerable plaques without obvious symptoms in the early stage.Once a rupture occurs,it will lead to severe coronary stenosis,which in turn may trigger a major adverse cardiovascular event.Computed tomography angiography(CTA)has become a standard diagnostic tool for early screening of coronary plaque and stenosis due to its advantages in high resolution,noninvasiveness,and three-dimensional imaging.However,manual examination of CTA images by radiologists has been proven to be tedious and time-consuming,which might also lead to intra-and interobserver errors.Nowadays,many machine learning algorithms have enabled the(semi-)automatic diagnosis of CAD by extracting quantitative features from CTA images.This paper provides a survey of these machine learning algorithms for the diagnosis of CAD in CTA images,including coronary artery extraction,coronary plaque detection,vulnerable plaque identification,and coronary stenosis assessment.Most included articles were published within this decade and are found in the Web of Science.We wish to give readers a glimpse of the current status,challenges,and perspectives of these machine learning-based analysis methods for automatic CAD diagnosis.
文摘Background The computational fluid dynamics(CFD)approach has been frequently applied to compute the fractional flow reserve(FFR)using computed tomography angiography(CTA).This technique is efficient.We developed the DEEPVESSEL-FFR platform using the emerging deep learning technique to calculate the FFR value out of CTA images in five minutes.This study is to evaluate the DEEPVESSEL-FFR platform using the emerging deep learning technique to calculate the FFR value from CTA images as an efficient method.Methods A single-center,prospective study was conducted and 63 patients were enrolled for the evaluation of the diagnostic performance of DEEPVESSEL-FFR.Automatic quantification method for the three-dimensional coronary arterial geometry and the deep learning based prediction of FFR were developed to assess the ischemic risk of the stenotic coronary arteries.Diagnostic performance of the DEEPVESSEL-FFR was assessed by using wire-based FFR as reference standard.The primary evaluation factor was defined by using the area under receiver-operation characteristics curve(AUC)analysis.Results For per-patient level,taking the cut-off value<0.8 referring to the FFR measurement,DEEPVESSEL-FFR presented higher diagnostic performance in determining ischemia-related lesions with area under the curve of 0.928 compare to CTA stenotic severity 0.664.DEEPVESSEL-FFR correlated with FFR(R=0.686,P<0.001),with a mean di&ference of-0.006士0.0091(P=0.619).The secondary evaluation factors,indicating per vessel accuracy,sensitivity,specificity,positive predictive value,and negative predictive value were 87.3%,97.14%,75%,82.93%,and 95.45%,respectively.Conclusion DEEPVESSEL-FFR is a novel method that allows efficient assessment of the functional significance of coronary stenosis.
文摘AIM:To describe the characteristics of coronary artery fistulas(CAFs) in adults,including donor vessels and whether termination was cameral or vascular. METHODS:A PubMed search was performed for articles between 2000 and 2010 to describe the current characteristics of congenital CAFs in adults.A group of 304 adults was collected.Clinical data,presentations,diagnostic modalities,angiographic fistula findings and treatment strategies were gathered and analyzed.With regard to CAF origin,the subjects were tabulated into unilateral,bilateral or multilateral fistulas and compared.The group was stratified into two major subsets according to the mode of termination;coronary-cameral fistulas(CCFs) and coronary-vascular fistulas(CVFs) . A comparison was made between the two subsets. Fistula-related major complications[aneurysm formation,infective endocarditis(IE) ,myocardial infarction(MI) ,rupture,pericardial effusion(PE) and tamponade] were described.Coronary artery-ventricular multiple micro-fistulas and acquired CAFs were excluded as well as anomalous origin of the coronary arteries from the pulmonary artery(PA) . RESULTS:A total of 304 adult subjects(47%male) with congenital CAFs were included.The mean age was51.4 years(range,18-86 years) ,with 20%older than 65 years of age.Dyspnea(31%) ,chest pain(23%) and angina pectoris(21%) were the prevalent clinical presentations.Continuous cardiac murmur was heard in 82%of the subjects.Of the applied diagnostic modalities,chest X-ray showed an abnormal shadow in 4%of the subjects.The cornerstone in establishing the diagnosis was echocardiography(68%) ,and conventional contrast coronary angiography(97%) .However,multislice detector computed tomography was performed in 16%.The unilateral fistula originated from the left in 69%and from the right coronary artery in 31%of the subjects.Most patients(80%) had unilateral fistulas,18%presented with bilateral fistulas and 2%with multilateral fistulas.Termination into the PA was reported in unilateral(44%) ,bilateral(73%) and multilateral(75%) fistulas.Fistulas with multiple origins(bilateral and multilateral) terminated more frequently into the PA(29%) than into other sites(10.6%)(P=0.000) . Aneurysmal formation was found in 14%of all subjects.Spontaneous rupture,PE and tamponade were reported in 2%of all subjects.In CCFs,the mean age was 46.2 years whereas in CVFs mean age was 55.6 years(P=0.003) .IE(4%) was exclusively associated with CCFs,while MI(2%) was only found in subjects with CVFs.Surgical ligation was frequently chosen for unilateral(57%) ,bilateral(51%) and multilateral fistulas(66%) ,but percutaneous therapeutic embolization(PTE) was increasingly reported(23%,17%and 17%,respectively) . CONCLUSION:Congenital CAFs are currently detected in elderly patients.Bilateral fistulas are more frequently reported and PTE is more frequently applied as a therapeutic strategy in adults.
