Methods: A total of 80 patients with acute coronary syndrome in our hospital from July 2014 to July 2017 were enrolled in this study. The subjects were divided into the control group (n=40) and the treatment group (n=40) randomly. The control group was treated with xuesaitong injection combined with nadroparin calcium;the treatment group was treated with danhong injection combined with nadroparin calcium. The two groups were treated for 2 weeks. The serum CyS-C, CRP, IL-6, vWF, ET and VEGF levels of the two groups before and after treatment were compared. Results: There were no significantly differences of the serum CyS-C, CRP, IL-6, vWF, ET and VEGF levels of the two groups before treatment. The serum CyS-C, CRP, IL-6, vWF, ET and VEGF levels of the two groups after treatment were significantly lower than before treatment, and that of the treatment group were significantly better than that of the control group. Conclusion: Danhong injection combined with nadroparin calcium can significantly reduce the serum CyS-C, CRP, IL-6, vWF, ET and VEGF levels of patients with acute coronary syndrome, and it was worthy clinical application.展开更多
AIM: To investigate whether continuous veno-venous hemofiltration (CVVH) in different filtration rate to eliminate cytokines would result in different efficiency in acute pancreatitis, whether the saturation time o...AIM: To investigate whether continuous veno-venous hemofiltration (CVVH) in different filtration rate to eliminate cytokines would result in different efficiency in acute pancreatitis, whether the saturation time of filter membrane was related to different filtration rate, and whether the onset time of CWH could influence the survival of acute pancreatitis. METHODS: Thirty-seven patients were classified into four groups randomly. Group 1 underwent low-volume CVVH within 48 h of the onset of abdominal pain (early CVVH, n = 9). Group 2 received low-volume CVVH after 96 h of the onset of abdominal pain (late CVVH, n= 10). Group 3 underwent high-volume CVVH within 48 h of the onset of abdominal pain (early CVVH, n = 9). Group 4 received high-volume CVVH after 96 h of theonset of abdominal pain (late CVVH, n = 9). CVVH was sustained for at least 72 h. Blood was taken before hemofiltration, and ultrafiltrate was collected at the start of CVVH and every 12 h during CVVH period for the purpose of measuring the concentrations of TNF-α, IL-1β and IL-6. The concentrations of TNF-α, IL-1β and IL-6 were measured by swine-specific ELISA. The Solartron 1 255 B frequency response analyzer (British) was used to observe the resistance of filter membrane. RESULTS: The survival rate had a significant difference (94.44% vs68.42%, P〈0.01) high-volume and low-volume CVVH patients. The survival rate had also a significant difference (88.89% vs 73.68%, P〈0.05) between early and late CVVH patients. The hemodynamic deterioration (MAP, HR, CVP) was less severe in groups 4 and 1 bhan that in group 2, and in group 3 than in group 4. The adsorptive saturation time of filters membranes was 120-180 min if the filtration rate was 1 000-4 000 mL/h. After the first, second and third new hemofilters were changed, serum TNF-α concentrations had a negative correlationwith resistance (r: -0.91, -0.89, and -0.86, respectively in group 1; -0.89, -0.85, and -0.76, respectively in group 2; -0.88, -0.92, and -0.82, respectively in group 3; -0.84, -0.87, and -0.79, respectively in group 4). The decreasing extent of TNF-α, IL-1β and IL-6 was significantly different between group 3 and group I (TNF-α P〈0.05, IL-1β P〈0.05, IL-6 P〈0.01), between group 4 and group 2 (TNF-α P〈0.05, IL-1β P〈0.05, IL-6 P〈0.01), between group 1 and group 2 (TNF-α P〈0.05, IL-1β P〈0.05, IL-6 P〈0.05), and between group 3 and group 4 (TNF-α P〈0.01, IL-1β P〈0.01, IL-6 P〈0.05), respectively during CVVH period. The decreasing extent of TNF-α and IL-1β was also significantly different between survival patients and dead patients (TNF-α P〈0.05, IL-1β P〈0.05). In survival patients, serum concentration of TNF-α and IL-1β decreased more significantly than that in dead patients. CONCLUSION: High-volume and early CWH improve hemodynamic deterioration and survival in acute pancreatitis patients. High-volume CVVH can eliminate cytokines more effidently than low-volume CVVH. The survival rate is related to the decrease extent of TNF-α and IL-1β. The adsorptive saturation time of filter membranes are different under different filtration rate condition. The filter should be changed timely once filter membrane adsorption is saturated.展开更多

METHODS: The SAP rat models were randomly divided into the model control group, Baicalin treatment group, octreotide treatment group and sham operation group. All groups were randomly subdivided into 3 h, 6 h and 12 h groups with 15 rats in each group. The survival, ascites volume and pathological changes of pancreas in all rats were observed at different time points after operation. The plasma amylase content and serum TNF-α, IL-6, malonaldehyde (MDA) and PLA2 contents were also determined. RESULTS: The survival was not obviously different between the treated groups, and was significantly higher in treated groups at 12 h compared to the model control group (P 〈 0.05, 15 vs 10). The ascites/body weight ratio at 3 h and 6 h was significantly lower in Baicalin treatment group compared to the model control group and octreotide treatment group (P 〈 0.05, 1.00 vs 2.02 and 1.43 and P 〈 0.001, 2.29 (1.21) vs 2.70 (0.80) and 2.08 (2.21), respectively). The contents of amylase, TNF-α, IL-6, MDA and PLA2 were significantly lower in the treated groups than in the model control group (P 〈 0.05, 4342 vs 5303, 5058 vs 6272 in amylase, P 〈 0.01, 21.90 vs 36.30, 23.80 vs 39.70, 36 vs 54.35 in MDA and 56.25 vs 76.10 in PIA2, or P 〈 0.001, 65.10 and 47.60 vs 92.15 in TNF-α, 3.03 vs 5.44, 2.88 vs 6.82, 2.83 vs 5.36 in IL-6, respectively). The pathological scores of pancreas in the treated groups were significantly lower than that in the model control group (P 〈 0.05, 9.00 vs 10.05, 6.00 vs 9.00, 8.00 vs 10.05), but no marked difference was found between the treated groups. CONCLUSION: The Baicalin injection has significant therapeutic effects on SAP rats, its effects are similar to those of octreotide. Objective:To explore the protective effect and mechanism of Tetramethyl Pyrazine(TMP)on the pancreas function of acute pancreatitis rats.Methods:A total of 75 SD rats were randomly divided into three groups(A,B,C) wit...Objective:To explore the protective effect and mechanism of Tetramethyl Pyrazine(TMP)on the pancreas function of acute pancreatitis rats.Methods:A total of 75 SD rats were randomly divided into three groups(A,B,C) with 25 rats in each group.Group A served as sham operation group.In the groups B and C,AP model was prepared as by injecting taurocholic acid sodium.Group B was model group.After modeling,rats were administrated by intraperitoneal injection of normal saline.Group C was TMP treatment group,which was administrated by intraperitoneal injection of 0.6% TMP after modeling.The rat blood specimens in each group were collected with 1 mL/100 g solution after modeling of 2,6.12 and 24 h.Levels of amylase(AMS).blood urea nitrogen(BUN),creatinine(CR).TNF-α and IL-6 were detected,and 5 rats were sacrificed.Histopathological examination was performed in he pancreatic tissue specimens of each group to observe pancreatic tissue damage.Results:After modeling in each time point.AMS,BUN.CR.TNF-α and IL-6 in groups B and C were significantly higher than that of in group A(P<0.05).After modeling of 2 h.AMS.BUN and CR in group B increased significantly and reached the peak value at6 h.After modeling of 12 h.scrum level of TNF-α and IL-6 were significantly lower than that of in control group,while after 24 h of modeling,serum level of AMS.BUN,CR,TNF-α and IL-6 were significantly lower than that of in control group(P<0.05).The histological observation showed that pancreatic tissue in rats of group A was normal without damage lesions.Massive bleeding,necrosis and serious injury were visible in pancreatic tissue of group B.The rut pancreatic tissue was bleeding in group C with small pieces of necrotic lesions.The degree of inflammatory cell infiltration was lower than group B.and the degree of injury was significantly lower than group B.Conclusions:TMP can significantly decrease the serum level of TNF- α and IL-6 in AP rats,inhibits inflammatory response of AP.and has significant protective effect on pancreatic tissue and function in AP rats.展开更多
Systemic lupus erythematosus-related acute pancreatitis(SLEAP) has a poor prognosis with a high mortality. We described the clinical features of SLEAP, and discussed the feasibility of plasma exchange(PE) combined...Systemic lupus erythematosus-related acute pancreatitis(SLEAP) has a poor prognosis with a high mortality. We described the clinical features of SLEAP, and discussed the feasibility of plasma exchange(PE) combined with glucocorticosteroids(GC) in short-term prognosis and possible mechanism in reducing serum inflammatory cytokine IL-6 and removing serum lipids. A retrospective study was performed by an independent rheumatologist. Medical records of SLEAP from March 2010 to December 2014 were retrieved from Tongji Hospital information system, and patients were divided into two groups according to whether PE therapy was adopted. Sixteen patients treated with PE in combination with GC were classified as group A, and the other 10 patients who were treated with merely GC were classified as group B. Patients' clinical remission rate and average daily GC dosage after two-week therapy were compared between the two groups. Patients' serum inflammatory cytokines and lipid concentration were compared between baseline and after two-week treatment in both groups. Pearson correlation test was performed to determine association between serum cytokines and Ranson score. SLEDAI score in group A patients at baseline(14.8±3.1) showed no statistical difference from that in group B(14.1±3.3). At baseline serum IL-6 levels had no significant difference between group A [13.14(11.12, 16.57) mg/L] and group B [14.63(11.37, 16.37) mg/L]; after two-week therapy IL-6 decreased significantly in group A [9.16(7.93, 10.75)mg/L] while it did not show decreasing trend in group B [13.62(9.29,17.63) mg/L]. Serum lipid concentration after two-week therapy in group A [(TC=5.02±0.53, TG=1.46±0.44) mmol/L] decreased significantly compared to baseline [(TC=6.11±0.50, TG=2.14±1.03) mmol/L], while similar tendency was not observed in group B. The remission rate after two-week therapy was higher in group A(70.0%) than in group B(25.0%). Acute pancreatitis(AP) was one of the clinical manifestations of active SLE. PE combined with GC could reduce serum IL-6 level, and remove serum lipid to improve short-term prognosis. Therefore, it might be a safe and effective way in treating SLEAP and was worth continuing to explore its feasibility.展开更多
