Efficacy of invisible advancement correction for mandibular retraction in adolescents based on Pancherz analysis

BACKGROUND Mandibular retraction is the main etiological mechanism of class II malocclusion in China and the subsequent distal molar relationship can cause functional discomfort in mastication,breathing and the temporomandibular joint.The use of mandibular advancement(MA)devices has recently emerged as an adolescent mandibular retraction treatment;however,current studies regarding the effect thereof are relatively few,and there is lack of sufficient clinical support.AIM To investigate the clinical effect of invisalign MA on the treatment of mandibular retraction in adolescents.METHODS This study included 30 adolescent patients who underwent treatment with the MA appliances from December 2017 to June 2021.The lateral cephalometric data before and after treatment were collected and imported into Dolphin Imaging software.The changes were measured by linear measurement superimposed with lateral cephalometric trajectory based on the Pancherz technology.RESULTS There was no significant difference in the length and position of maxilla before and after the treatment.The position of the mandible moved 3.13 mm,the length increased 4.14 mm,the mandibular ramus length increased 4.09 mm,the body length increased 4.25 mm,and the position of the condyle moved 1.03 mm forward after treatment.Additionally,changes in the incisor sagittal position and labial inclination were observed.The position of the upper incisor point moved back 1.33 mm,without statistical difference,the inclination and tooth angle decreased by 3.44°and 4.06°,respectively;the position of the lower incisor point was moved 2.98 mm,and the inclination and tooth angle increased by 2.62°and 1.23°,respectively.Furthermore,changes in the incisor overjet and molar relationship were seen.Overjet decreased by 4.31 mm,of which 1.78 mm was due to dental factors,accounting for 41.3%of the effect as opposed to 58.7%due to skeletal factors.Molar relationship improved 3.87 mm,with 1.34 mm due to dental factors,and dental and skeletal factors were accounted for 34.6%and 65.4%of the effect,respectively.CONCLUSION For adolescent patients with mandible retraction,invisalign MA can effectively promote the mandible growth,and it was proven to be mainly due to skeletal effects.

Wave Energy Study in China: Advancements and Perspectives

The history and current status of research and development of wave energy in the world is briefly introduced. The main problems existing in these studies are pointed out. The description is focused on the current status and the advancements achieved in China. After analysis of the wave energy resources and practical situations in China, it is pointed out that the studies on wave energy should be not only concentrated on the conversion efficiency and costs of wave energy devices, but also focused on the technology of independent operation and stable output of electricity. Finally, the perspectives of application of wave energy in China are discussed.

Facial microcystic adnexal carcinoma-treatment with a “jigsaw puzzle” advancement flap and immediate esthetic reconstruction: A case report

BACKGROUND Microcystic adnexal carcinoma(MAC)is a rare malignant tumor of the skin that is commonly found on the face.It grows slowly and has a low mortality rate.However,for various reasons,including strong histological invasiveness,clinical inexperience and inadequate procedure design,immediate or permanent facial deformity may occur after surgical operations.CASE SUMMARY This article describes a middle-aged female artist who was diagnosed with MAC on the left upper lip.She declined the recommended treatment plan,which included two-stage reconstruction,skin grafting,or surgery that could have resulted in obvious facial dysfunction or esthetic deformity.We accurately designed a personalized procedure involving a“jigsaw puzzle advancement flap”for the patient based on the lesion location and the estimated area of skin loss.The procedure was successful;both pathological R0 resection and immediate and long-term esthetic reconstruction effects were achieved.CONCLUSION This study suggests that when treating facial MAC or other skin malignancies,a surgical team should have sufficient plastic surgery-related knowledge and skills.An optimal surgical plan for an individual is needed to achieve good facial esthetics and functional recovery and shorten the treatment course.

This review article aims to analyze and evaluate the advantages, shortcomings, technological advancements and hereafter prospects in welding and processing of magnesium and its alloys. Studies have accounted that replacing other metals with Mg alloys will bring about in monumental weight reduction with significant addition in strength that could lead to dramatic transformation of metallic era of 21st century and onwards. However,wide application of Mg and its alloys depends on the development of advanced welding &joining technologies. The current paradigm and progress in welding,joining and processing of magnesium and its alloys are reviewed,and research trend in this field is briefly discussed.

Novel advancements in threedimensional neural tissue engineering and regenerative medicine

Neurological diseases and injuries present some of the great- est challenges in modern medicine, often causing irrevers- ible and lifelong burdens in the people whom they afflict. Conditions of stroke, traumatic brain injury, spinal cord injury, and neurodegenerative diseases have devastating con- sequences on millions of people each year, and yet there are currently no therapies or interventions that can repair the structure of neural circuits and restore neural tissue function in the brain and spinal cord. Despite the challenges of over- coming these limitations, there are many new approaches under development that hold much promise. Neural tissue engineering aims to restore and influence the function of damaged or diseased neural tissue generally through the use of stem cells and biomaterials. Many types of biomaterials may be implemented in various designs to influence the survival, differentiation, and function of developing stem cells, as well as to guide neurite extension and morphological architecture of cell cultures. Such designs may aim to reca- pitulate the cellular interactions, extracellular matrix char- acteristics, biochemical factors, and sequences of events that occur in neurodevelopment, in addition to supporting cell survival, differentiation, and integration into innate neural tissue.

Roux-en-Y augmented gastric advancement: An alternative technique for concurrent esophageal and pyloric stenosis secondary to corrosive intake

Select group of patients with concurrent esophagealand gastric stricturing secondary to corrosive intake requires colonic or free jejunal transfer. These technically demanding reconstructions are associated with significant complications and have up to 18% ischemic conduit necrosis. Following corrosive intake, up to 30% of such patients have stricturing at the pyloro-duodenal canal area only and rest of the stomach is available for rather less complex and better perfused gastrointestinal reconstruction. Here we describe an alternative technique where we utilize stomach following distal gastric resection along with Roux-en-Y reconstruction instead of colonic or jejunal interposition. This neo-conduit is potentially superior in terms of perfusion, lower risk of gastro-esophageal anastomotic leakage and technical ease as opposed to colonic and jejunal counterparts. We have utilized the said technique in three patients with acceptable postoperative outcome. In addition this technique offers a feasible reconstruction plan in patients where colon is not available for reconstruction due to concomitant pathology. Utility of this technique may also merit consideration for gastroesophageal junction tumors.

Mandibular advancement devices(MADs)are one of the treatments used for Obstructive sleep apnea(OSA).MADs try to maintain the mandible in an advanced position to keep the upper airways open when sleeping.To achieve this goal,most current MADs limit the mouth opening to a few millimetres.The study of the kinematic behaviour of the patient’s jaw is essential in order to design devices that allow greater aperture ranges.For this purpose,a 3D multibody model that reproduces jaw movement has been developed in this work.To this end,the movement of the lower incisor has been determined by means of a vision system and reflective markers.In addition,the kinematics of the temporomandibular joint has been modelled.Next,the device is designed and printed using a cam-follower mechanism.This way,the cam profiles and the followers are optimally designed and positioned for each patient depending on the physiognomy of the jaw and the opening and advancing movement prescribed by the specialist.

Examining the Relationship of Organizational Intelligence with Innovation Management and Career Advancement in an Organization

The present study aimed to examine the relationship of organizational intelligence with innovation management and career advancement in an organization. Considering its objective, this was a descriptive-correlational study. The statistical population included all high school managers in Zahedan at the year 2015. The sample contained 52 male managers and 51 female managers whom were selected through applying stratified random sampling method. To collect data, Organizational Intelligence Questionnaire, Innovation Management Inventory and Questionnaire of Career Advancement were used, the validity of which was assessed through applying the standpoint of a number of university professors and their reliability was obtained 0.74, 0.76, 0.84, respectively, using Cronbach’s alpha coefficient. Results indicated that considering managers’ perspectives, the status of organizational intelligence and career advancement were desirable;however, the status of innovation management was moderate. Moreover, organizational intelligence and its components were significantly and positively correlated with innovation management (r = 0.512, P < 0.01), and there was a significant and positive relationship between organizational intelligence and career advancement (r = 0.334, P < 0.01). Findings demonstrated that components of desire to change, spirit, unity and agreement, application of knowledge, performance pressure were significantly correlated to career advancement. However, components of strategic perspective and common fate were not significantly correlated with career advancement. Additionally, among the components of organizational intelligence, in the first step, application of knowledge and in the second step, desire to change were the best predicators of innovation management (which together could predict 28%). Application of knowledge was the only variable which entered the predictive model and predicted 16% of career advancement. Results of independent t-test indicated that no significant difference was found between male and female managers, considering their organizational intelligence, innovation management and career advancement.

Osteoarthritis(OA)is a common chronic degenerative disease of the bone and joint,which occurs frequently in the middle-aged and old people.The main clinical symptoms are joint pain,stiffness,swelling,and dyskinesia.In recent years,there have been many researches on traditional Chinese medicine treatment of bone and joint,and the therapeutic effect is significant.This paper analyzes and summarizes the domestic and foreign literature in recent years,based on the cellular signaling pathway of OA,discusses the mechanism of action of traditional Chinese medicine monomer,compound,acupuncture and so on for OA,in order to provide the corresponding reference for the research and treatment of OA.

Analysis of Referrals for Mandibular Advancement Appliances for Sleep Apnoea

Aim: This article investigates the increasing awareness of Obstructive Sleep Apnoea (OSA) as a potentially serious health risk and highlights the role of the dentist in managing mild to moderate OSA cases which can be improved with an intra-oral mandibular advancement appliance. Methods: Data from referrals to a Prosthodontic Unit from 01-01-2019 to 18-8-2022 were analysed. Results: A total of 240 consecutive referrals to an NHS hospital where a patient was fitted with a mandibular advancement appliance (MAA) were analysed and 160 (67%) were male and 80 (33%) were female. The mean age in years was 52 (2019), 50 (2020), 52 (2021) and 51 (2022). Mean age of male referrals was 51 and female referrals was 53. Conclusion: There are a large number of patients with OSA seeking a dental device and practitioners should be aware of this treatment modality to help the medical team manage OSA. Clinical relevance: Dentists may need to have a greater understanding of OSA and be able to provide MAAs.

A simplified two-piece mandibular advancement appliance for obstructive sleep apnea

Nowadays mandibular advancement appliances (MA- As) are available and distributed widely in the field of dentistry for the treatment of snoring and ob- structtive sleep apnea (OSA). However, a few stud- ies discussed temporomandibular joint (TMJ) discom- fort and masticatory muscle stiffness during wearing MAAs are found. A new appliance that we introduced in this pilot study has shown significant jaw move- ment and could diminish TMJ and masticatory side effects.

Medial Rectus Muscle Advancement in Consecutive Exotropia

Purpose: Consecutive exotropia is a frequent consequence of surgery for convergent strabismus that may develop at differing rates postoperatively. Several surgical options on horizontal recti have been proposed, but none report a clearly standardized amount of surgery. The present study provides further results of the medial rectus muscle advancement. Methods: Twenty-eight patients, age range 6 - 55 years, who had undergone unilateral or bilateral medial rectus advancement to the physiological insertion, with or without contemporary unilateral lateral rectus muscle recession, were included in the study. Factors leading to the onset of consecutive exotropia were analyzed. Results: After 2 - 3 years, the overall mean angle reduction was 21.3 prismatic diopters (PD) for distance and 22.8 PD for near, with an effectiveness on near vision lower than expected. Only in the 7 cases with a preoperatively larger exotropia at near (of at least 10 PD), the reduction was 17.0 PD for distance and 24.6 PD for near. In unilateral medial rectus muscle advancement, the mean reduction was 14.3 PD for distance and 16.3 PD for near;in bilateral advancement it was 25.8 PD and 25.2 PD respectively. Conclusion: Medial rectus advancement is preferable to other options, especially in cases with convergence deficit, if no excess of divergence or limitation of bulb rotation is present, for which a lateral rectus muscle recession is indicated. Unilateral and bilateral surgeries are useful for exotropia of about 15 PD and 25 PD respectively. The effectiveness depends weakly or not at all by the amount of the previously executed recession.

Facilitators and Constraints:Perceptions of Gender Differences on Women’s Career Advancement

The underrepresentation in top management positions has made women’s status in the hospitality industry a great concern.The study examined hospitality students’,educators’,and industry recruiters’perceptions of facilitators and constraints on women’s career advancement and identified if there were any significant differences among them.Data were collected through an online survey.Perceptions of gender differences of facilitators and constraints in women’s career advancement and gender issues were analyzed within the three groups(students,educators,and recruiters).Results revealed significant gender differences between male and female educators and recruiters.The findings suggested that hospitality education should make a contribution and reveal barriers and gender issues in the industry.

The Impact of Rate of Feeding Advancement after Early Initiation of Enteral Nutrition in Critically Ill, Underweight Patients: A Single-Center Retrospective Chart Review

Background: The optimal rate of feeding advancement after initiation of early enteral nutrition (EEN) for underweight, critically ill patients is unknown. Methods: We conducted a retrospective chart review in intensive care unit (ICU) patients with a body mass index (BMI) < 20.0 kg/m2. Patients were categorized into Group R, which reached the energy target within 3 days of EEN initiation, and Group S, which reached the energy target 4 or more days after EEN initiation. Results: A total of 65 patients with a median age of 73 years were included in the study. No significant differences were observed between the two groups for all-cause mortality, ICU-free days, or length of hospital stay. Ventilator-free days (VFDs) were significantly fewer in Group R than in Group S (18.0 [0.0 - 22.0] vs. 21.0 [16.3 - 24.8] days;P = 0.046). A significantly higher number of patients requiring mechanical ventilation (MV) at hospital discharge were observed in Group R than in Group S (29% vs. 8%;P = 0.030). Multivariable analyses with adjustment for confounders found that days required to reach target energy intake after EEN initiation were significantly and independently associated with the requirement for MV at hospital discharge, but not with VFDs. Conclusion: A slow rate of feeding advancement after initiation of EEN in critically ill patients having a BMI of <20.0 kg/m2 might be associated with a reduced requirement for MV at hospital discharge. These results require confirmation in a large multicenter trial of underweight, critically ill patients.
