Non-Hodgkin lymphoma (NHL) occurs with increased frequency in HIV seropositive individuals. The increase in NHL has been more marked in the last decade, with HIV being the major contributor to this increase. More than 70% of the adult NHL patients at Chris Hani Baragwanath Academic Hospital (CHBAH), Soweto, Johannesburg, are HIV seropositive. In addition, HIV has impacted on the clinical presentation—being more aggressive and atypical. Histologically, HIV-NHL typically manifests as B-cell, high grade lymphomas, including diffuse large B-cell lymphoma (DLBCL);Burkitt lymphoma (BL);B-cell lymphoma, unclassifiable, with features intermediate between DLBCL and BL and plasmablastic lymphoma. The latter two entities, which were previously rare or unknown, have gained prominence in the last decade, occurring primarily in HIV seropositive individuals. HIV-NHL, being associated with all these adverse prognostic factors results in a poorer overall survival. 展开更多

The envelope protein (Env) of lentiviruses such as HIV, SIV, FIV and EIAV is larger than that of other retroviruses. The Chinese EIAV attenuated vaccine is based on Env and has helped to successfully control this virus, demonstrating that envelope is crucial for vaccine. We compared Env variation of the four kinds of lentiviruses. Phylogenetic analysis showed that the evolutionary relationship of Env between HIV and SIV was the closest and they appeared to descend from a common ancestor, and the relationship of HIV and EIAV was the furthest. EIAV had the shortest Env length and the least number of potential N-linked glyeosylation sites (PNGS) as well as glyeosylation density compared to various immunodefieiency viruses. However, HIV had the longest Env length and the most PNGS. Moreover, the alignment of HIV and SIV showed that PNGS were primarily distributed within extraeellular membrane protein gp120 rather than transmembrane gp41. It implies that the size difference among these viruses is associated with a lentivirus specific function and also the diversity of env. There arc low levels of modification of glycosylation sites of Env and selection of optimal protective epitopes might be useful for development of an effective vaccine against HIV/AIDS. 展开更多

Objective To characterize the human immunodeficiency virus (HIV) -specific T lymphocyte responses and identify the immunodominant regions in Chinese HIV-1 recombinant subtype B/C chronic infectors at complete genome level. Methods Twenty-five HIV-I B/C recombinant chronic infectors were screened for their specific T lymphocyte responses to a panel of peptides corresponding to the complete HIV-1 subtype B genome by gamma interferon ELISPOT assay. Kruskal-Wallis nonparametric analysis of variance was used to test significant differences across gene regions, and Tukey pairwise analysis was used to identify differences between gene regions. Spearman rank correlation was used to assess the relation between responses. Results The order of recognized frequencies of specific T lymphocyte responses to HIV proteins was Nef〉Vpr〉Gag〉Pol〉Vpu〉Env〉Rev〉Vif〉Tat. When adjusted for protein length, Nef, Vpr, Gag, and Pol were the most intensely targeted proteins and the central region of Nef, Gag p24, Pol RT, and Vpr was most frequently recognized. No significant correlation was observed between the magnitude of IFN-γ production of HIV-l-specific T lymphocyte responses and plasma viremia, breadth of response and CD4 counts. Conclusion The central region of Nef, Gag p24, Pol RT, and Vpr is most frequently targeted in HIV-1 B/C recombinants chronic infectors. HIV-1-specific T lymphocyte responses and plasma viremia or CD4 counts play no protective role at complete genome level in these infectors. 展开更多

Gene therapy is one of several approaches that are being tested in the search for an effective anti HIV treatment. In this strategy, a “resistant” gene would be introduced into target cells, rendering them resistance to the infection of HIV. The HIV 1 Tat protein transactivate HIV 1 gene expression at the transcriptional level by interacting with its response element(TAR) in the long terminal repeat(LTR). Previously, we have shown that antisense polyTAR Core RNAs can inhibit the transactivation of HIV 1 Tat protein in transiently transfected Jurkat cells. To determine whether this antisense polyTAR Core RNAs could inhibit HIV 1 replication in CD 4+ T cells, we transfected the antisense polyTAR Core gene to MT4 cells and challenged them with HIV 1 SF33 strain. Levels of HIV 1 p24gag antigen were reduced more than 4 fold in cultures of the transduced MT4/LR cells infected with HIV 1SF33 strain. In contrast, cultures of nontransduced MT4 cells and control LX vector transduced MT4/LX cells infected with the same viruses had high levels of HIV 1 p24gag. Our work showed that antisense polyTAR Core RNAs were able to inhibit HIV 1 replication in CD 4+ T cells, and could be used as resistance gene in further studying for gene therapy against HIV 1. 展开更多

The infection due to human immunodeficiency virus(HIV) is characterized by the progressive reduction of CD4+ T lymphocytes and the compromise of other cell lines of the immune system, resulting in immunosuppression. In this context, autoimmune diseases could be considered contradictory, however, cases of autoimmune diseases during this infection have been described, including autoimmune hepatitis(AIH), which is uncommon and has few case reports within medical literature, none of them from Latin America. In this case report where a patient with an HIV infection on combined antiretroviral treatment developed acute elevation of transaminases, hyperbilirubinemia, and deterioration in hepatic synthetic function. Although initially an antiretroviral drug-induced liver injury was suspected, during the study a diagnosis of autoimmune hepatitis was proven, which required treatment with corticosteroid and azathioprine, obtaining a satisfactory response and managing to continue the antiretroviral therapy. Autoimmune diseases in HIV infection must be taken into account. In the case of hepatitis in patients with HIV on antiretroviral treatment, the differentiation between viral hepatitis caused by autoimmune diseases or medications is essential to establish an adequate treatment, and avoid the suspension of the antiretroviral therapy. 展开更多

HIV/AIDS患者并发EBV和HCMV感染临床免疫学特征及影响因素分析

目的调查人类免疫缺陷病毒/获得性免疫缺陷综合征(human immunodeficiency virus/acquired immune deficiency syndrome,HIV/AIDS)患者感染EB病毒(Epstein-Barr virus,EBV)和人类巨细胞病毒(human Cytomegalovirus,HCMV)的情况,检测相... 目的调查人类免疫缺陷病毒/获得性免疫缺陷综合征(human immunodeficiency virus/acquired immune deficiency syndrome,HIV/AIDS)患者感染EB病毒(Epstein-Barr virus,EBV)和人类巨细胞病毒(human Cytomegalovirus,HCMV)的情况,检测相关临床免疫学指标,分析其影响因素。方法选取2022年1~12月在长沙市第一医院住院并接受EBV和HCMV筛查的1093例HIV/AIDS患者。流式细胞术检测CD4^(+)T淋巴细胞数量;荧光定量PCR检测HIVRNA载量、EBV-DNA载量和HCMV-DNA载量。采用SPSS 27.0统计学软件进行统计分析,并通过Logistic回归分析HIV/AIDS患者并发病毒感染的危险因素。结果1093例HIV/AIDS患者中,EBV-DNA阳性率为48.22%(527/1093),HCMV-DNA阳性率为19.03%(208/1093)。随着CD4^(+)T淋巴细胞数量增加,EBV-DNA和HCMV-DNA的阳性率下降(χ^(2)=39.50,143.0,均P<0.001);随着HIV-RNA载量增加,EBV-DNA和HCMV-DNA的阳性率增加,差异具有统计学意义(χ^(2)=46.18,124.3,均P<0.001)。另外,患者接受抗逆转录病毒治疗(antiretroviral therapy,ART)也可明显降低EBV-DNA和HCMV-DNA的阳性率,差异具有统计学意义(χ^(2)=30.60,96.59,均P<0.001)。CD4^(+)T淋巴细胞数量和HIV-RNA载量有显著的负相关关系(r=-0.49,P<0.001)。Logistic回归分析显示CD4^(+)T淋巴细胞数量<200个/μl(OR=1.46,95%CI:1.02~2.08,P=0.037),HIV-RNA载量>200 copies/ml(OR=1.70,95%CI:1.18~2.44,P=0.004),年龄>30岁(OR=2.15,95%CI:1.44~3.19,P<0.001)是HIV/AIDS患者并发EB病毒感染的危险因素;未持续接受ART(OR=1.83,95%CI:1.10~3.02,P=0.019),HIV-RNA载量>200 copies/ml(OR=2.56,95%CI:1.50~4.35,P<0.001),CD4^(+)T淋巴细胞数量<200个/μl(OR=4.61,95%CI:2.57~8.28,P<0.001)是HIV/AIDS患者并发HCMV感染的危险因素。结论在艾滋病的治疗与管理中,当CD4^(+)T淋巴细胞数量下降(<200个/μl),HIV-RNA载量升高(>200 copies/ml)或者年龄>30岁时,应加强对病毒的监测和ART,减少HIV/AIDS患者机会性感染的可能。 展开更多

Although the distribution of cytomegalovirus (CMV) infection has a wide regional variation, it is more common in Africa and Asia with high prevalence approaching 100% in some communities. CMV is a frequent opportunistic infection and major cause of morbidity and mortality among HIV patients with severe immunosuppression. It can be contracted from body fluid, including saliva, urine, blood, cervical secretions, and semen. CMV seropositivity is considered the best laboratory measure of past infection. We determined the prevalence of CMV and the risk factors associated with its acquisition in a population of HIV positive patients. Anti-CMV IgG seroprevalence was 100% among HIV infected subjects and 98.6% among controls. Having multiple sexual partners and traditional practices were identified as risk factors associated with risk of contracting CMV infection. 展开更多

中医药治疗HIV/AIDS相关腹泻疗效和安全性的Meta分析与GRADE证据评价

目的:系统评价中医药治疗人类免疫缺陷病毒感染或获得性免疫缺陷综合征(HIV/AIDS)相关腹泻(HRD)的临床疗效和安全性,并进行GRADE证据质量评价。方法:检索中国知网(CNKI)、PubMed等中英文数据库和临床试验注册平台,检索中医药治疗HRD的... 目的:系统评价中医药治疗人类免疫缺陷病毒感染或获得性免疫缺陷综合征(HIV/AIDS)相关腹泻(HRD)的临床疗效和安全性,并进行GRADE证据质量评价。方法:检索中国知网(CNKI)、PubMed等中英文数据库和临床试验注册平台,检索中医药治疗HRD的随机对照试验(检索时间从各个数据库建库至2023年3月31日),由2名研究人员独立进行数据检索、数据提取、偏倚风险评价和Meta分析。结果:最终纳入31篇文献和1项临床研究,共计3474例患者,其中试验组1830例,对照组1644例。Meta分析结果显示,中医药治疗能显著提高HRD患者的临床疗效有效率,降低大便次数积分、中医症状积分、腹泻量表积分、腹痛积分,加快腹泻消失时间,增加体质量。临床疗效有效率和中医症状积分均未发现发表偏倚。GRADE证据质量评价结果显示:中医症状积分、大便次数积分、CD4+T细胞绝对数量为高级质量证据;临床疗效有效率、腹泻恢复时间、腹泻量表积分、腹痛积分、不良反应发生率和体质量为中级质量证据;中医证候临床疗效有效率为低级质量证据。结论:中医药能提高HRD患者的临床疗效有效率,改善腹泻临床症状,且安全性较好,推荐使用于HRD的治疗。但由于所纳入的研究方法质量较低和部分结局指标证据质量较低,尚需更多设计严谨的多中心随机对照试验增加证据强度。 展开更多

目的分析艾滋病病毒(HIV)阳性肺结核患者抗结核疗效影响因素,探索其影响因素预测指标。方法收集2019年1月至2022年2月在石家庄市第五医院诊治的64例HIV阳性肺结核患者病例资料。根据抗结核治疗6个月疗效分为有效组和无效组。比较2组研... 目的分析艾滋病病毒(HIV)阳性肺结核患者抗结核疗效影响因素,探索其影响因素预测指标。方法收集2019年1月至2022年2月在石家庄市第五医院诊治的64例HIV阳性肺结核患者病例资料。根据抗结核治疗6个月疗效分为有效组和无效组。比较2组研究对象一般资料、实验室指标的差异。通过二元Logistics回归分析HIV阳性肺结核患者抗结核疗效的影响指标。通过ROC曲线分析HIV阳性肺结核抗结核疗效的预测指标及诊断效能。结果单因素分析结果显示,抗结核治疗有效组和无效组血清白蛋白(ALB)、血红蛋白(HGB)、淋巴细胞计数(LYM%)、中性粒细胞(NEUT)、天冬氨酸氨基转移酶(AST)、C-反应蛋白(CRP)、CD4计数差异有统计学意义(均P<0.05)。Logistics回归分析结果显示,HGB和CD4细胞越低HIV阳性肺结核抗结核无效的风险越高。ROC曲线结果显示,CD4细胞计数、HGB、两者联合均可用于预测HIV阳性肺结核抗结核疗效,其AUC分别为0.838、0.843和0.911。结论CD4细胞、HGB是HIV阳性肺结核患者抗结核疗效的重要影响因素,二者联合应用可有效预测HIV阳性肺结核患者抗结核疗效。 展开更多

目的:分析同伴支持联合健康宣教对HIV感染患者心理状态及睡眠质量的影响。方法:选取2022年1月至2023年1月厦门大学附属第一医院感染科收治的HIV感染患者60例作为研究对象,按照随机表法分为对照组和观察组,每组30例。对照组患者给予一般... 目的:分析同伴支持联合健康宣教对HIV感染患者心理状态及睡眠质量的影响。方法:选取2022年1月至2023年1月厦门大学附属第一医院感染科收治的HIV感染患者60例作为研究对象,按照随机表法分为对照组和观察组,每组30例。对照组患者给予一般性护理,观察组患者在对照组的基础上给予同伴支持联合健康宣教护理干预。采用行为检查量表(BCL)比较2组患者焦虑、退缩、抑郁3项指标,采用匹兹堡睡眠质量指数(PSQI)比较2组患者干预前后睡眠质量的变化。结果:护理干预后,观察组焦虑、退缩、抑郁指标改善幅度优于对照组,观察组PSQI评分显著低于对照组,差异均有统计学意义(均P<0.05)。结论:同伴支持联合健康宣教能有效促进HIV感染患者心理状态及睡眠质量改善,值得推广使用。 展开更多

目的了解杭州市HIV患者新冠疫苗接种现状及其相关影响因素。方法采用中国疾病预防控制信息系统艾滋病防治基本信息系统及自制一般情况调查问卷于2021年11月-12月调查我院接受规律高效抗逆转录病毒疗法(HAART)的HIV患者,用卡方检验、二分... 目的了解杭州市HIV患者新冠疫苗接种现状及其相关影响因素。方法采用中国疾病预防控制信息系统艾滋病防治基本信息系统及自制一般情况调查问卷于2021年11月-12月调查我院接受规律高效抗逆转录病毒疗法(HAART)的HIV患者,用卡方检验、二分类logistic回归做统计分析。结果本研究共调查1687例HIV感染者新冠疫苗接种情况。男性(95.60%)患者较多,抗病毒治疗平均时间为(4.71±2.90)年。疫苗未接种组、接种组占比分别为9.37%、90.63%。接种灭活疫苗、重组亚单位疫苗、腺病毒载体疫苗例数构成比分别为97.91%、1.57%、0.52%。二分类logistic回归分析结果显示,年龄≥50岁[OR=2.465,95%CI:1.038~5.856]、合并皮疹[OR=1.885,95%CI:1.043~3.404]、合并其他疾病[OR=2.050,95%CI:1.297~3.240]是HIV患者拒绝接种新冠疫苗的独立危险因素。结论为进一步提高HIV感染者新冠疫苗接种覆盖率,建议临床医务工作者关注年龄≥50岁、合并皮疹及其他疾病的HIV感染者,充分评估其疫苗接种安全性并制定相关策略。 展开更多

目的探讨芒果叶提取物的体外抗人类免疫缺陷病毒1(HIV-1)活性,为研究与开发传统中医药资源治疗获得性免疫缺陷综合征(AIDS)提供参考。方法通过三磷酸腺苷(ATP)法评估干预药物对TZM-bl细胞和MT-2细胞活性的影响。构建TZM-bl-HIV-1_(ⅢB)... 目的探讨芒果叶提取物的体外抗人类免疫缺陷病毒1(HIV-1)活性,为研究与开发传统中医药资源治疗获得性免疫缺陷综合征(AIDS)提供参考。方法通过三磷酸腺苷(ATP)法评估干预药物对TZM-bl细胞和MT-2细胞活性的影响。构建TZM-bl-HIV-1_(ⅢB)、MT-2-HIV-1_(ⅢB)两种细胞-病毒感染模型,通过荧光素酶活性检测试剂检测病毒活性,评估干预药物的抗HIV-1活性。观察MT-2-HIV-1_(ⅢB)细胞系统中芒果叶提取物对细胞病变效应(CPE)的抑制情况。计算芒果叶提取物的半数毒性浓度(CC_(50))、半数抑制浓度(IC_(50))和选择指数(SI)。结果在TZM-bl-HIV-1_(ⅢB)和MT-2-HIV-1_(ⅢB)两种细胞-病毒感染模型中,芒果叶提取物均显示出抗HIV-1活性,且呈现剂量依赖性。在TZM-bl-HIV-1_(ⅢB)模型中,芒果叶提取物的CC_(50)为(320.00±29.44)μg/ml,IC_(50)为(8.86±0.26)μg/ml,SI为36.14。在MT-2-HIV-1_(ⅢB)模型中,芒果叶提取物CC_(50)为(174.13±22.36)μg/ml,IC_(50)为(15.23±9.99)μg/ml,SI为11.44。结论芒果叶提取物的体外细胞毒性小,抗HIV-1活性显著,具有潜在的开发利用价值。 展开更多

目的:探讨人类免疫缺陷病毒感染者和艾滋病病人(以下简称HIV/AIDS病人)重复住院的危险因素,以期为临床诊治和护理提供参考依据。方法:纳入医院2020年1月—6月住院的HIV/AIDS病人为研究对象,将随访2年后重复住院(住院次数≥2次)病人设为... 目的:探讨人类免疫缺陷病毒感染者和艾滋病病人(以下简称HIV/AIDS病人)重复住院的危险因素,以期为临床诊治和护理提供参考依据。方法:纳入医院2020年1月—6月住院的HIV/AIDS病人为研究对象,将随访2年后重复住院(住院次数≥2次)病人设为重复入院组,根据年龄、性别等一般资料匹配原则选取1∶1病例数为单次入院组,比较分析两组病人一般资料、住院诊断信息、CD 4+T淋巴细胞计数、营养风险筛查2002(NRS 2002)和综合性医院焦虑/抑郁情绪测定表(Hospital Anxiety and Depression Scale,HADS)等评估结果,采用二元Logistic回归分析艾滋病病人重复住院的危险因素。结果:共纳入重复住院的HIV/AIDS病人157例,单次住院的HIV/AIDS病人154例。157例重复住院HIV/AIDS病人中,男143例(91.08%),年龄为(36.49±10.91)岁;63.06%是通过同性感染的,33.76%的病人无工作,最低CD 4+T淋巴细胞计数为(126.44±54.22)/μL,首次住院体质指数(BMI)为(19.13±1.30)kg/m 2,首次抗病毒治疗以来年份中<1年的病人占71.34%;重复住院病人前3位的主要疾病诊断为艾滋病相关疾病(65.11%)、非艾滋病病毒相关感染(13.83%)、非艾滋病相关肿瘤(6.38%)。二元Logistic回归分析结果显示,重复住院HIV/AIDS病人职业状况的OR值为2.380(P<0.05),而最低CD 4+T淋巴细胞计数和首次住院BMI的OR值分别为0.673和0.720(P<0.05)。结论:最低CD 4+T淋巴细胞计数和首次住院BMI较低是HIV/AIDS病人重复住院的危险因素,而有工作是其保护因素。 展开更多

目的探讨献血者血液人类免疫缺陷病毒(HIV)实验室检测策略,为献血者HIV的检测、早期诊断与传播阻断提供参考依据。方法对117987份献血者血液样本使用三代及四代ELISA夹心法HIV检测试剂联合筛查,对单纯三代试剂检测阳性或者三代与四代试... 目的探讨献血者血液人类免疫缺陷病毒(HIV)实验室检测策略,为献血者HIV的检测、早期诊断与传播阻断提供参考依据。方法对117987份献血者血液样本使用三代及四代ELISA夹心法HIV检测试剂联合筛查,对单纯三代试剂检测阳性或者三代与四代试剂同时阳性者用Western blot法进行确证检测,对三代、四代试剂检测结果同时阴性者进行HIV核酸检测,仅四代试剂阳性者进行核酸检测,并追踪随访,核酸检测阳性者进行Western blot法确证试验。结果117987份献血者血液样本经不同试剂检测,其中三、四代HIV检测试剂同时检出双阳标本55例,占0.047%,Western blot法确证54例HIV抗体阳性,1例不确定,随访追踪后转为阳性。单纯三代试剂检测阳性者26例,经Western blot法检测HIV抗体阴性24例,抗体不确定样本2例,Western blot法检测带型分别为p24和糖蛋白160(gp160),经追踪随访检测确证为HIV阴性。31例单纯四代HIV检测试剂检测阳性者,经核酸检测发现29例阴性,其中2例核酸阳性,Western blot法确证实验均阴性,2~4周后追踪随访重新抽血复检,转为阳性。三代、四代试剂检测结果同时阴性者进行HIV核酸检测未能检出不合格标本。结论对献血者血液用三、四代ELISA HIV检测试剂联合筛查检测可以起到互补作用,并结合核酸检测及Western blot法检测等补充试验提升了血液供应的安全性,同时,也有助于HIV潜在感染献血者艾滋病的早期诊断预防传播及治疗。 展开更多

Background:Previous studies have examined the bulk transcriptome of peripheral blood immune cells in acquired immunodeficiency syndrome patients experiencing immunological non-responsiveness.This study aimed to investigate the characteristics of specific immune cell subtypes in acquired immunodeficiency syndrome patients who exhibit immunological non-responsiveness.Methods:A single-cell transcriptome sequencing of peripheral blood mononuclear cells obtained from both immunological responders(IRs)(CD4^(+)T-cell count>500)and immunological non-responders(INRs)(CD4^(+)T-cell count<300)was conducted.The transcriptomic profiles were used to identify distinct cell subpopulations,marker genes,and differentially expressed genes aiming to uncover potential genetic factors associated with immunological non-responsiveness.Results:Among the cellular subpopulations analyzed,the ratios of monocytes,CD16^(+)monocytes,and exhausted B cells demonstrated the most substantial differences between INRs and IRs,with fold changes of 39.79,11.08,and 2.71,respectively.In contrast,the CD4^(+)T cell ratio was significantly decreased(0.39-fold change)in INRs compared with that in IRs.Similarly,the ratios of natural killer cells and terminal effector CD8^(+)T cells were also lower(0.37-fold and 0.27-fold,respectively)in the INRs group.In addition to several well-characterized immune cell-specific markers,we identified a set of 181 marker genes that were enriched in biological pathways associated with human immunodeficiency virus(HIV)replication.Notably,ISG15,IFITM3,PLSCR1,HLA-DQB1,CCL3L1,and DDX5,which have been demonstrated to influence HIV replication through their interaction with viral proteins,emerged as significant monocyte marker genes.Furthermore,the differentially expressed genes in natural killer cells were also enriched in biological pathways associated with HIV replication.Conclusions:We generated an atlas of immune cell transcriptomes in HIV-infected IRs and INRs.Host genes associated with HIV replication were identified as markers of,and were found to be differentially expressed in,different types of immune cells. 展开更多
