Synchronization and firing mode transition of two neurons in a bilateral auditory system driven by a high–low frequency signal

The FitzHugh–Nagumo neuron circuit integrates a piezoelectric ceramic to form a piezoelectric sensing neuron,which can capture external sound signals and simulate the auditory neuron system.Two piezoelectric sensing neurons are coupled by a parallel circuit consisting of a Josephson junction and a linear resistor,and a binaural auditory system is established.Considering the non-singleness of external sound sources,the high–low frequency signal is used as the input signal to study the firing mode transition and synchronization of this system.It is found that the angular frequency of the high–low frequency signal is a key factor in determining whether the dynamic behaviors of two coupled neurons are synchronous.When they are in synchronization at a specific angular frequency,the changes in physical parameters of the input signal and the coupling strength between them will not destroy their synchronization.In addition,the firing mode of two coupled auditory neurons in synchronization is affected by the characteristic parameters of the high–low frequency signal rather than the coupling strength.The asynchronous dynamic behavior and variations in firing modes will harm the auditory system.These findings could help determine the causes of hearing loss and devise functional assistive devices for patients. 展开更多

Objectives:The objective of the study was to explore the impact of auditory stimulation protocol on auditory response in cerebrovascular stroke(CVS)patients.Materials and Methods:A pre-posttest nonequivalent control group quasi-experimental study was conducted at Al Mainal Hospital at Al Kars Einy Hospital,Cairo University,among 60 CVS patients,and the sample was randomly assigned through a coin to allocate subjects to study or control group(30 for each group).Auditory stimulation techniques were applied three times per week for the study group for two consecutive weeks.Patients in the control group received only routine hospital care during the study period.The Glasgow Coma Scale(GCS)score for assessing the neurological function and level of arousal and the auditory subscale score of Western Neuro Sensory Stimulation Profile(WNSSP)for assessing auditory response were recorded and compared between the two groups before and after the intervention.Results:There were statistically significant differences between the study group and the control group regarding third reading of GCS score(11.5±0.3 vs.10.6±0.9,t=4.676,P=0.000).The scores of all auditory subscales of WNSSP in the study groups increased after implementing the auditory stimulation protocol(P<0.05).Conclusion:Utilizing the auditory stimulation protocol by nurse proves to be an effective method for enhancing hearing responses in individuals with CVS. 展开更多

This paper aims to explore how a veteran teacher organizes online teaching initiated by the pandemic and how she deals with the problems in online teacher-student verbal interaction.By analyzing a corpus of 20 audio-recorded online lessons between a math teacher and her students during the COVID-19 pandemic from April 11 to May 10,2022,four interactional segments are selected as the focus of the study.The results of the conversation analysis of the segments showed that students’modesty,lack of confidence,lack of ability,and network delay are the main factors affecting online teacher-student interaction.By encouraging students to answer questions,enlightening students to give answers,enriching students’answers,and entertaining the teaching atmosphere(“4Es”strategies),the teacher solved the problems successfully.The findings from this study can provide pedagogical experience and implications for practical teaching. 展开更多

Transcranial direct current stimulation(tDCS)has been reportedly beneficial for different neurodegenerative disorders.tDCS has been reported as a potential adjunctive or alternative treatment for auditory verbal hallucination(AVH).This study aims to review the effects of tDCS on AVH in patients with schizophrenia through combining the evidence from randomized clinical trials(RCTs).The databases of PsycINFO(2000–2019),PubMed(2000–2019),EMBASE(2000–2019),CINAHL(2000–2019),Web of Science(2000–2019),and Scopus(2000–2019)were systematically searched.The clinical trials with RCT design were selected for final analysis.A total of nine RCTs were eligible and included in the review.Nine RCTs were included in the final analysis.Among them,six RCTs reported a significant reduction of AVH after repeated sessions of tDCS,whereas three RCTs did not show any advantage of active tDCS over sham tDCS.The current studies showed an overall decrease of approximately 28%of AVH after active tDCS and 10%after sham tDCS.The tDCS protocols targeting the sensorimotor frontal-parietal network showed greater treatment effects compared with the protocols targeting other regions.In this regard,cathodal tDCS over the left temporoparietal area showed inhibitory effects on AVHs.The most effective tDCS protocol on AVHs was twice-daily sessions(2 mA,20-minute duration)over 5 consecutive days(10 sessions)with the anode over the left dorsolateral prefrontal cortex and the cathode over the left temporal area.Some patient-specific and diseasespecific factors such as young age,nonsmoking status,and higher frequencies of AVHs seemed to be the predictors of treatment response.Taken together,the results of tDCS as an alternative treatment option for AVH show controversy among current literatures,since not all studies were positive.However,the studies targeting the same site of the brain showed that the tDCS could be a promising treatment option to reduce AVH.Further RCTs,with larger sample sizes,should be conducted to reach a conclusion on the efficacy of tDCS for AVH and to develop an effective therapeutic protocol for clinical setting. 展开更多

Auditory verbal hallucination(AVH)is emphasized as a pathological hallmark of schizophrenia.Neuroimaging studies provide evidence linking AVH to overlapping functional abnormalities in distributed networks.However,no clear conclusion has still been reached.This study aimed to further explore the brain activity of patients with schizophrenia having AVH from both local activity(LA)and functional connectivity(FC)insights,while excluding confounding factors from other positive symptoms.A total of 42 patients with AVH(AVH patients group,APG),26 without AVH(non-AVH patients group,NPG),and 82 normal controls(NC)underwent resting-state functional magnetic resonance imaging(fMRI).LA measures,including regional homogeneity(ReHo)and fractional amplitude of low-frequency fluctuations(fALFF),and FC measures were evaluated to understand the neuroimaging mechanism of AVH.APG showed increased ReHo and fALFF in the bilateral putamen(Put)compared with NPG and NC.FC analysis(using bilateral putamen as seeds)revealed that all patients showed abnormal FC of multiple resting state network regions,including the anterior and post cingulate cortex,middle frontal gyrus,inferior parietal gyrus,and left angular gyrus.Interestingly,APG showed significantly decreased FC of insula extending to the superior temporal gyrus and inferior frontal gyrus compared with NPG and NC.The present findings suggested a significant correlation of abnormal LA and dysfunctional putamen-auditory cortical connectivity with the neuropathological mechanism of AVH,providing evidence for the functional disconnection hypothesis of schizophrenia. 展开更多

The deployment of vehicle micro-motors has witnessed an expansion owing to the progression in electrification and intelligent technologies.However,some micro-motors may exhibit design deficiencies,component wear,assembly errors,and other imperfections that may arise during the design or manufacturing phases.Conse-quently,these micro-motors might generate anomalous noises during their operation,consequently exerting a substantial adverse influence on the overall comfort of drivers and passengers.Automobile micro-motors exhibit a diverse array of structural variations,consequently leading to the manifestation of a multitude of distinctive auditory irregularities.To address the identification of diverse forms of abnormal noise,this research presents a novel approach rooted in the utilization of vibro-acoustic fusion-convolutional neural network(VAF-CNN).This method entails the deployment of distinct network branches,each serving to capture disparate features from the multi-sensor data,all the while considering the auditory perception traits inherent in the human auditory sys-tem.The intermediary layer integrates the concept of adaptive weighting of multi-sensor features,thus affording a calibration mechanism for the features hailing from multiple sensors,thereby enabling a further refinement of features within the branch network.For optimal model efficacy,a feature fusion mechanism is implemented in the concluding layer.To substantiate the efficacy of the proposed approach,this paper initially employs an augmented data methodology inspired by modified SpecAugment,applied to the dataset of abnormal noise sam-ples,encompassing scenarios both with and without in-vehicle interior noise.This serves to mitigate the issue of limited sample availability.Subsequent comparative evaluations are executed,contrasting the performance of the model founded upon single-sensor data against other feature fusion models reliant on multi-sensor data.The experimental results substantiate that the suggested methodology yields heightened recognition accuracy and greater resilience against interference.Moreover,it holds notable practical significance in the engineering domain,as it furnishes valuable support for the targeted management of noise emanating from vehicle micro-motors. 展开更多

Introduction: While approaching the aspect of learning disorders, particular attention is paid to verbal dyspraxia, a phenomenon that runs its course regularly over the last years. Verbal dyspraxia is inherent in the person without mental disorders and accompanies them throughout the whole spectrum of life. Comorbidity is an added issue. Although dyspraxia is met in homogeneous groups, some common elements such as intelligence, difficulty regarding linguistic skills, low learning performance and low self-esteem are present. Purpose: The object is to research how dyspraxia is manifested and how it affects a 6-year-old boy as well as the possibility of promptly interfering and simplifying his everyday life. Method: In the current case study, Achenbach’s questionnaire was used, combined with the use of expressive vocabulary. Results: The results of the research were unveiled through experts’ references in coordinance with the conferences conducted. Conclusion: Winding up, dyspraxia is a learning disorder that exists within the person through their lifespan. Immediate diagnosis, combined with experts’ personalized intervention programs (and perhaps, a differentiated curriculum, where applicable) can guide the person to live up to the educational needs. Family’s role is to be supportive, intending to eliminate possible emotional strains. 展开更多

Effect of Specific Verbal Instructions on the Identification of Pain Location during a Passive Straight Leg Raise Test

Because pain is subjective, objectively describing it is crucial. However, pain assessment may cause distress in patients;therefore, physical therapists (PTs) should conduct these tests quickly and accurately. Simple and clear instructions are recommended for pain assessment. However, few studies have provided evidence to support this hypothesis. Correspondingly, this study aimed to confirm the effectiveness of specific verbal instructions for pain location during five consecutive Passive Straight Leg Raise (PSLR) tests. The 28 asymptomatic participants (age 27.4 ± 9.6 years) who provided informed consent received five consecutive PSLR tests: three without and two with specific verbal instructions to ascertain pain intensity, quality, and location. The participants drew pain locations on a body chart and described the pain intensity and quality after each test. All participants were interviewed regarding the differences they noted in the presence and absence of specific verbal instructions. Each pain location was classified into one of ten areas for statistical analysis. The proportion of participants who changed the pain location was compared between the tests using McNemar’s test, and the kappa coefficient was confirmed for consistency of pain location. There was a significant difference in the proportion of participants who changed their pain location between the second and third tests and from the third to the fourth test (McNemar’s test: p = 0.003). Kappa coefficients had low consistency (κ = 0.28) just after receiving the specific verbal instructions in the fourth test compared to the third test. Consistency improved in the fifth test (κ = 0.57);93% of the participants answered that the pain location had become clearer. This study revealed the effects of specific verbal instructions in identifying pain locations. This detailed information may help PTs provide appropriate treatment and contribute to reducing pain in clinical settings. 展开更多

BACKGROUND Stapes ankylosis is a rare cause of conductive hearing loss,and stapes suprastructure fixation is extremely rare with fewer than 30 reported cases.Patients usually visit the clinic with non-progressive conductive hearing loss that typically began in the early years of life.CASE SUMMARY Herein,we report a case of a 37-year-old female with an isolated stapedial suprastructure fixation.The patient presented with unusual fluctuating auditory symptoms of tinnitus,ear fullness and mixed hearing loss.Pre-operative temporal bone computed tomography findings and operative findings revealed an isolated stapedial suprastructure fixation with monopod stapes caused by elongated pyramidal eminence.The hearing threshold recovered completely,and fluctuating auditory symptoms disappeared after the surgery.CONCLUSION This is the first report of stapedial suprastructure fixation with fluctuating auditory symptoms.Successful results are expected with surgical treatment. 展开更多

目的探讨团体认知行为干预治疗精神分裂症幻听的临床疗效。方法选取115例有幻听症状的精神分裂症患者,采用随机数字表法分为对照组58例和研究组57例。对照组患者给予常规护理,研究组患者在对照组基础上给予团体认知行为干预。在干预前... 目的探讨团体认知行为干预治疗精神分裂症幻听的临床疗效。方法选取115例有幻听症状的精神分裂症患者,采用随机数字表法分为对照组58例和研究组57例。对照组患者给予常规护理,研究组患者在对照组基础上给予团体认知行为干预。在干预前后应用精神症状评定量表-幻听分量表(PSYRATS-AH)、焦虑自评量表(SAS)和一般自我效能量表(GSES)进行效果评价。结果完成研究的患者共106例,对照组53例、研究组53例。干预后,研究组患者PSYRATS-AH总分及对声音起源的信念、令人痛苦的声音数量、痛苦的程度、对生活的影响、对声音的控制评分及SAS评分均低于对照组(P<0.05或0.01),GSES评分高于对照组(P<0.05)。结论团体认知行为干预能减轻幻听对患者的负面影响,降低患者的焦虑水平,提升患者应对幻听的自我效能感。 展开更多

目的探讨采用卡尔曼加权平均(Kalman-weighted averaging,KWA)的听性脑干反应(ABR)测试中,受试者不同测试状态对残余噪声及ABR波V振幅的影响。方法选取21例(42耳)18~34岁听力正常成年人,分别在安静诊室内三种不同的状态下(安静平卧、坐... 目的探讨采用卡尔曼加权平均(Kalman-weighted averaging,KWA)的听性脑干反应(ABR)测试中,受试者不同测试状态对残余噪声及ABR波V振幅的影响。方法选取21例(42耳)18~34岁听力正常成年人,分别在安静诊室内三种不同的状态下(安静平卧、坐位、写字)使用卡尔曼加权平均的新型ABR系统(vivosonic integrity system)进行ABR测试,同时使用传统ABR测试系统(interacoustics,IA)在隔声屏蔽室内进行ABR测试,记录各状态下受试者ABR反应的残余噪声水平和波V幅值并进行比较。结果①传统ABR和卡尔曼加权平均ABR三种不同测试状态(平卧、坐位、写字)下波V振幅没有显著差异(P>0.05);②卡尔曼加权平均ABR安静平卧和坐位两种状态下的残余噪声水平较传统ABR低,但无统计学差异(P>0.05);卡尔曼加权平均ABR在写字状态下的残余噪声水平显著高于其他三种状态(P<0.05);③写字状态下卡尔曼加权平均ABR的残余噪声、波V振幅在左右耳之间没有显著差异(P>0.05)。结论与传统ABR相比,卡尔曼加权平均ABR在写字状态下的残余噪声显著升高,但安静平卧和坐位状态下的残余噪声水平与传统ABR无显著差异,受试者的不同测试状态对残余噪声水平有一定影响。 展开更多

目的 分析儿童人工耳蜗植入术后听觉言语康复情况的影响因素。方法 回顾性收集2015年1月至2020年1月南阳市中心医院收治的120例人工耳蜗植入术后耳聋儿童临床资料,术后均随访24个月,比较所有儿童开机前及术后1、3、6、9、12、24个月听... 目的 分析儿童人工耳蜗植入术后听觉言语康复情况的影响因素。方法 回顾性收集2015年1月至2020年1月南阳市中心医院收治的120例人工耳蜗植入术后耳聋儿童临床资料,术后均随访24个月,比较所有儿童开机前及术后1、3、6、9、12、24个月听觉行为分级、语言可懂度分级、婴幼儿有意义听觉整合量表评分,以多重线性回归分析耳聋儿童人工耳蜗植入术后24个月听觉言语康复情况的影响因素。结果 开机前及术后1、3、6、9、12、24个月,耳聋儿童人工耳蜗植入术后听觉行为分级、语言可懂度分级、婴幼儿有意义听觉整合量表评分均呈逐渐升高趋势,不同时间点比较,差异有统计学意义(P<0.05)。多重线性回归分析结果显示,术前有残余听力且佩戴助听器、植入时年龄≤3岁、人工耳蜗植入术电极类型为弯电极、监护人受教育程度为中及以上、康复模式为康复机构联合家庭、基因突变类型为GJB2、监护人为父母均为影响耳聋儿童人工耳蜗植入术后24个月听觉行为分级评分、婴幼儿有意义听觉整合量表评分的保护因素(P<0.05);术前有残余听力且佩戴助听器、植入时年龄≤3岁、人工耳蜗植入术电极类型为弯电极、康复模式为康复机构联合家庭、基因突变类型为GJB2、监护人受教育程度为高中及以上均为影响耳聋儿童人工耳蜗植入术后24个月语言可懂度分级评分的保护因素(P<0.05)。结论 影响耳聋儿童人工耳蜗植入术后24个月婴幼儿听觉言语康复的相关因素包括植入时年龄、术前有残余听力且佩戴助听器情况、人工耳蜗植入术电极类型情况、监护人情况、康复模式、基因突变类型,临床可针对以上因素对耳聋儿童进行干预,促进其听觉言语康复。 展开更多

目的 对影响人工耳蜗植入后电诱发中潜伏期反应(electrically evoked auditory middle latency response,EMLR)的时间因素,包括植入时年龄、植入后时间及现年龄等进行分析研究,探究听觉重塑过程中不同时间因素的影响。方法 对46例2008年... 目的 对影响人工耳蜗植入后电诱发中潜伏期反应(electrically evoked auditory middle latency response,EMLR)的时间因素,包括植入时年龄、植入后时间及现年龄等进行分析研究,探究听觉重塑过程中不同时间因素的影响。方法 对46例2008年~2014年在北京大学第三医院耳鼻咽喉科进行人工耳蜗植入的患者在术后进行EMLR测试。比较语前聋儿童与语后聋成人EMLR的差异,以及语前聋儿童中现年龄、植入时年龄及植入后时间等因素对EMLR的影响。结果 语前聋儿童EMLR各波潜伏期及波间潜伏期普遍较成人组长,各波间幅值普遍较成人组大,其中Po、Na、Pa、Nb及Pb波潜伏期,PoNa、NaPa及PaNb波间潜伏期,PoNa、NaPa、PaNb及NbPb波间幅值均存在统计学差异(P<0.05)。语前聋儿童中,Pa、Nb波潜伏期及NaPa波间期与现年龄及植入后时间存在显著负相关、与植入时年龄存在显著正相关(P<0.05)。结论 EMLR在语前聋儿童与语后聋成人存在较显著差异,对于语前聋儿童,较大的年龄、较长的植入时间、较小的植入年龄对应更为成熟的EMLR波形,提示上述因素在听觉重塑过程中的重要性。 展开更多

多种听力学检测方法在未引出听性脑干反应患儿诊断中的价值

目的探讨多种听力学检测方法在听性脑干反应(ABR)最大输出未引出患儿的听力学诊断中的应用价值。方法回顾性分析69例(138耳)ABR最大强度未引出患儿的临床资料,年龄42天到5岁,平均1岁6个月,鼓室导抗图均为A型或正向单峰,声反射均未引出,... 目的探讨多种听力学检测方法在听性脑干反应(ABR)最大输出未引出患儿的听力学诊断中的应用价值。方法回顾性分析69例(138耳)ABR最大强度未引出患儿的临床资料,年龄42天到5岁,平均1岁6个月,鼓室导抗图均为A型或正向单峰,声反射均未引出,影像学检查内耳无畸形。69例患儿均进行ABR、耳蜗微音电位(CM)、畸变产物耳声发射(DPOAE)和听性稳态反应(ASSR)测试。结果69例138耳中,8例16耳(11.59%)记录到CM,其中10耳(7.25%)记录到DPOAE,0.5、1、2、4 kHz ASSR反应阈值分别为83.2±13.1、82.9±13.0、75.3±12.4、63.1±9.1 dB nHL,结合其他检查结果诊断为听神经病。余61例(122耳)CM和DPOAE均未引出,0.5、1、2、4 kHz的ASSR引出率分别为82.3%、81.9%、76.9%、60.2%,其中20耳ASSR各频率均未引出,102耳至少一个频率引出,0.5、1、2、4 kHz ASSR反应阈分别为93.2±6.1、99.8±7.0、105.4±5.4、108.2±9.8 dB nHL,诊断为极重度感音神经性聋。结论对于ABR最大输出强度未引出的患儿,CM和/或DPOAE引出且ASSR各频率反应阈低于感音神经性聋患儿,有助于听神经病的诊断;CM和DPOAE均未引出有助于极重度感音神经性聋的诊断,ASSR测试有助于评估其残余听力。 展开更多

目的目前对意识障碍(DOC)患者的分级评估仍是相关领域的重点和难点。因效性网络可以通过时间序列间的因果关系直观地反映信息传递方向,帮助人们更好地理解患者大脑不同区域之间的信息交互作用。本文结合脑电图和因效性网络探讨听觉刺激... 目的目前对意识障碍(DOC)患者的分级评估仍是相关领域的重点和难点。因效性网络可以通过时间序列间的因果关系直观地反映信息传递方向,帮助人们更好地理解患者大脑不同区域之间的信息交互作用。本文结合脑电图和因效性网络探讨听觉刺激下无反应觉醒综合征(VS)患者与最低意识状态(MCS)患者的脑功能连通性差异。方法共纳入23例DOC患者,采集并分析唤名刺激下的脑电信号,通过多元格兰杰因果方法构建脑功能网络,利用脑网络节点度、聚类系数、全局效率以及因果流向性等参量从脑区之间协同工作的角度对比研究听觉刺激下不同意识水平患者的网络特征。结果唤名刺激下MCS患者的脑功能连通性强于VS患者,且呈现出因果流向差异,MCS与VS患者四个脑区的信息传递方向均不相同。结论唤名听觉刺激下MCS患者的信息传递能力强于VS患者;与VS患者相比MCS患者为因果源的电极通道数增多,对其他脑区的信息输出增多。本研究可为DOC患者意识水平的分级评估提供一定的理论依据。 展开更多

采用生理响度感知机制的车内噪声声品质预测

现有车内噪声声品质预测的响度计算中没有考虑真实人耳生理解剖结构的传声特性,因此提出了一种基于生理响度感知模型的车内噪声声品质预估方法。首先,采集两款轿车的车内噪声样本,并通过主观评价试验得到车内噪声的主观评价烦恼度。之后... 现有车内噪声声品质预测的响度计算中没有考虑真实人耳生理解剖结构的传声特性,因此提出了一种基于生理响度感知模型的车内噪声声品质预估方法。首先,采集两款轿车的车内噪声样本,并通过主观评价试验得到车内噪声的主观评价烦恼度。之后,整合中耳集总参数模型与耳蜗传输线模型,构建生理响度模型。然后,以生理响度模型的响度计算值为主要参数,结合尖锐度、粗糙度与车内噪声的主观评价值,通过TabNet模型构建了车辆声品质预测模型。最终,对比分析了所构建声品质模型与基于现有标准响度模型所构建的声品质模型的预测效果。结果表明,采用生理响度模型的声品质预测平均误差百分比仅有4.73%,优于采用Moore响度(6.13%)与Zwicker响度(6.94%)的声品质预测结果。此外,所构建的TabNet声品质预测模型的平均误差百分比也低于基于BP神经网络模型的平均误差百分比(7.60%)。采用生理响度模型的TabNet声品质预测能够提高车内噪声声品质客观评价的准确率。 展开更多
