Measurements of some anthropometric and physiological parameters are carried out as ba...This study investigated the effect of fixed height standing-workstation on different people with diverse anthropometry dimensions. Measurements of some anthropometric and physiological parameters are carried out as bases for the calculation of body mass index (BMI) and the determination of the maximum heart rate (HRmax) and aerobic power (VO2max) of individual subject while performing manual cutting operation with hacksaw on fixed vice height of 940 mm. Twenty subjects (S1 to S20) parted 2 mm thick square-pipe of 25 mm × 25 mm. Each subject carried out cutting operation in 5 replicates and their physiological parameters during activities are measured to determine their expended energy (EE) and oxygen consumption rate (VO2). The results showed that subject S4 with BMI of 20.76 kg/m2 has maximum cutting rate of 5.33 stroke/s, while subject S8 with BMI of 23.39 kg/m2 has minimum cutting rate of 0.92 stroke/s. There was a statistically significant effect on the interaction between BMI, EE and Cutting rate, with F = 827.54, P = 0.000, R2 = 0.967 and S = 1.749 units. Subject S11 was discovered to have VO2 (28.54 l/min) and VO2max (24.36 ml/min/kg), with highest value of EE (2.94 kcal/min). Wear rates of 1.86 teeth/s and 9.55 teeth/s have the same energy cost (EE = 0.87 kcal/min) but different cutting time of 36.65 s (S18) and 10.89 s (S20) respectively. This could explain in-part that excess 25.76 s utilized in operation time by subject S18 is responsible for keeping approximately 7.7 teeth intact as regards tool management. EE and Tool Wear Rate in one-way analysis of variance, were statistically significant (F = 45.87, P = 0.000, R2 = 54.69% and S = 1.617 units) at 0.05 level.展开更多

Methods: From July 1965 to January 2005. we collected patients (n = 49) of...Objective:To evaluate the clinical effects of the abdominal aorta block in controlling haemorrhage during operations of the gynecologic tumor. Methods: From July 1965 to January 2005. we collected patients (n = 49) of gynecologic tumor complicated with haemorrhage during operations, who were divided into 3 groups: preventive blocking group (PG, n=12), treatment blocking group (TG, n = 20) used abdominal aorta block technique with sterilized cotton band and silica gel tube, and control group (CG, n = 17) which were used the regular haemostatic methods, such as ligature, suture and ribbon gauze packing. During operations, the vital signs including the amount of bleeding and transfusion were measured. Results: Compared with the CG, the amount of bleeding and transfusion in the PG and TG decreased significantly (P<0. 01). After using the technique, 32 cases of haemorrhage were controlled completely. All patients finished operation smoothly in the end and the vital signs were stable. The vision field of operation was clear and the operating time was shortened dramatically (3. 0 h vs 5. 7 h and 3. 8 h vs 5. 7 h, P< 0. 01). No complications caused by the block occurred in the post-operation. Conclusion: Lower abdominal aorta block is safe and effective in controlling haemorrhage during operations of the gynecologic tumor.展开更多

Introduction: Post-operative (post-op) complete atrio-ventricular heart block (CAVB) occurs after 1% - 4% of pediatric cardiac operations. Current practice dictates implantation of permanent pacemaker (PPM) when post-...Introduction: Post-operative (post-op) complete atrio-ventricular heart block (CAVB) occurs after 1% - 4% of pediatric cardiac operations. Current practice dictates implantation of permanent pacemaker (PPM) when post-op CAVB persists >9 days. We propose that earlier PPM implantation may be the most cost-effective methodology since patient costs increase with extended length of stay (LOS). Methods: Data on the probabilities of persistent post-op CAVB were extracted from published reports. No individual patient data were utilized during this study. This was utilized to create a decision-making model and a total cost analysis on post-op day 0 - 10 to determine the most cost-efficient day to implant a PPM. Cost variables included estimates of daily cardiac ICU care, cost of PPM implantation, LOS, cost related to possible superficial or deep infection based on published prevalence rates (2.3% and 4.9%, respectively) and need for explant due to deep infection or recovery of native conduction. The model assumes 5-day minimum LOS and 1 day increase in LOS with PPM implantation. Cost data were obtained from relevant billing codes and manufacturer list prices for PPM and leads. A secondary analysis evaluated probability of unnecessary PPMs implanted and excess costs. Results: Post-op day (POD) 4 is the lowest total cost of PPM implantation for post-op CAVB, even when accounting for possible risk of either superficial or deep infection. A one-way sensitivity analysis accounting for variability of cardiac ICU care costs between centers ranging from $3000 - $9000 per day consistently replicates POD 4 as the most cost-effective day for PPM implantation. Implant on POD 4 results in a 26% chance of unnecessary implantation. Conclusions: The most cost-efficient day for PPM implantation for post-op CAVB is post-op day 4, which results in a minimum total cost savings of $17,422 per patient. Added costs due to risk of superficial or deep infection are marginal due to low prevalence of post-operative infection in this population.展开更多
