Effects of Anthropometry Variations on Physiological Parameters of Heart Rate, Oxygen Consumption, Aerobic Power While Performing Manual Operation at Fixed Vice Height

This study investigated the effect of fixed height standing-workstation on different people with diverse anthropometry dimensions. Measurements of some anthropometric and physiological parameters are carried out as bases for the calculation of body mass index (BMI) and the determination of the maximum heart rate (HRmax) and aerobic power (VO2max) of individual subject while performing manual cutting operation with hacksaw on fixed vice height of 940 mm. Twenty subjects (S1 to S20) parted 2 mm thick square-pipe of 25 mm × 25 mm. Each subject carried out cutting operation in 5 replicates and their physiological parameters during activities are measured to determine their expended energy (EE) and oxygen consumption rate (VO2). The results showed that subject S4 with BMI of 20.76 kg/m2 has maximum cutting rate of 5.33 stroke/s, while subject S8 with BMI of 23.39 kg/m2 has minimum cutting rate of 0.92 stroke/s. There was a statistically significant effect on the interaction between BMI, EE and Cutting rate, with F = 827.54, P = 0.000, R2 = 0.967 and S = 1.749 units. Subject S11 was discovered to have VO2 (28.54 l/min) and VO2max (24.36 ml/min/kg), with highest value of EE (2.94 kcal/min). Wear rates of 1.86 teeth/s and 9.55 teeth/s have the same energy cost (EE = 0.87 kcal/min) but different cutting time of 36.65 s (S18) and 10.89 s (S20) respectively. This could explain in-part that excess 25.76 s utilized in operation time by subject S18 is responsible for keeping approximately 7.7 teeth intact as regards tool management. EE and Tool Wear Rate in one-way analysis of variance, were statistically significant (F = 45.87, P = 0.000, R2 = 54.69% and S = 1.617 units) at 0.05 level.

Postoperative Analgesia for Abdominal Laparoscopic Surgery: Tap Block vs Peri-Orificial Infiltrations

Introduction/Purpose: The “transverse abdominal plane block” or TAP block was described by Rafi in 2001. It describes an approach to the neurofascial plane of the transverse abdominal muscle via the Jean-Louis Petit triangle and provides analgesia of the entire homolateral hemi abdomen. The aim of our study was to compare post-operative analgesia and post-operative morphine requirements between transverse abdominal plane block (TAP) and peri-orificial infiltration during laparoscopic abdominal surgery. Material and method: Prospective, randomized study conducted over a 2-year period. The study was conducted in the operating theatre of the Saint Louis Regional Hospital in Senegal. All adult patients undergoing laparoscopic abdominal surgery were included. Clinical aspects, pain scales and morphine consumption were analyzed. Results: A total of 60 patients were enrolled: 30 patients in the TAP group and 30 patients in the infiltration group. The average age was 32.9 years. The indications for laparoscopy were acute appendicitis in 50% of cases, gallbladder stones in 16% and inguinal hernia in 8%. For the TAP group, the mean numerical pain scale was 3.9 at 6 hours post-operatively and 2.1 at 24 hours post-operatively. For the infiltration group, the mean numerical pain scale was 4.3 at 6 hours post-op and 3 at 24 hours post-op. Morphine consumption at 6 hours post-op was on average 0.4 mg/patient for the TAP group and 0.9 mg/patient for the infiltration group. Discussion/conclusion: Analgesia provided by ultrasound-guided TAP block for laparoscopic abdominal surgery appears to be identical to periorificial infiltration. However, the simplicity and reproducibility of ultrasound-guided TAP block gives it a definite advantage.

Clinical analysis of abdominal aorta block in operation of gynecologic tumor

Objective:To evaluate the clinical effects of the abdominal aorta block in controlling haemorrhage during operations of the gynecologic tumor. Methods: From July 1965 to January 2005. we collected patients (n = 49) of gynecologic tumor complicated with haemorrhage during operations, who were divided into 3 groups: preventive blocking group (PG, n=12), treatment blocking group (TG, n = 20) used abdominal aorta block technique with sterilized cotton band and silica gel tube, and control group (CG, n = 17) which were used the regular haemostatic methods, such as ligature, suture and ribbon gauze packing. During operations, the vital signs including the amount of bleeding and transfusion were measured. Results: Compared with the CG, the amount of bleeding and transfusion in the PG and TG decreased significantly (P<0. 01). After using the technique, 32 cases of haemorrhage were controlled completely. All patients finished operation smoothly in the end and the vital signs were stable. The vision field of operation was clear and the operating time was shortened dramatically (3. 0 h vs 5. 7 h and 3. 8 h vs 5. 7 h, P< 0. 01). No complications caused by the block occurred in the post-operation. Conclusion: Lower abdominal aorta block is safe and effective in controlling haemorrhage during operations of the gynecologic tumor.

FIXED POINT THEOREMS FOR MEIR-KEELER CONDENSING OPERATORS VIA MEASURE OF NONCOMPACTNESS

In this article, we introduce the notion of Meir-Keleer condensing operator in a Banach space, a characterization using L-functions and provide a few generalization of Darbo fixed point theorem. Also, we introduce the concept of a bivariate Meir-Keleer condensing operator and prove some coupled fixed point theorems. As an application, we prove the existence of solutions for a large class of functional integral equations of Volterra type in two variables.

HYPONORMALITY OF BLOCK TOEPLITZ OPERATORS ON THE WEIGHTED BERGMAN SPACES

In this paper we consider the block Toeplitz operators TФ on the weighted Bergman space A2α（D, Cn） and we give a necessary and sufficient condition for the hyponor-mality of block Toeplitz operators with symbol in the class of functions Ф=F ＋ G＊ withmatrix-valued polynomial functions F and G with degree 2.

FIXED POINTS OF INCREASING OPERATORS IN ORDERED SPACE WITH APPLICATIONS

The minimal-maximal fixed points theorems of increasing operators are proved in ordered space and some well-known results of increasing operators and monotone operators are improved and generalized. The obtained results are then applied to singular nonlinear boundary problem in ordinary differential equation without any assumption of continuity, compactness, convexity and concavity.

VISCOSITY APPROXIMATION METHODS FOR THE SPLIT EQUALITY COMMON FIXED POINT PROBLEM OF QUASI-NONEXPANSIVE OPERATORS

The split equality common fixed point problem（SECFP） in the infinite-dimensional Hilbert spaces introduced by Moudafi（Alternating CQ-algorithm for convex feasibility and split fixed-point problems. Journal of Nonlinear and Convex Analysis）is to find x ∈ F（U）, y ∈ F（T） such that Ax = By,（1）where U : H;→ H;and T : H;→ H;are two nonlinear operators with nonempty fixed point sets F（U） = {x ∈ H;: Ux = x} and F（T） = {x ∈ H;: Tx = x}. Note that,by taking B = I and H;= H;in（1）, we recover the split fixed point problem originally introduced in Censor and Segal. Recently, Moudafi introduced alternating CQ-algorithms and simultaneous iterative algorithms with weak convergence for the SECFP（1） of firmly quasi-nonexpansive operators. In this paper, we introduce two viscosity iterative algorithms for the SECFP（1） governed by the general class of quasi-nonexpansive operators. We prove the strong convergence of algorithms. Our results improve and extend previously discussed related problems and algorithms.

Furi-Martelli-Vignoli spectrum and Feng spectrum of nonlinear block operator matrices

We investigate the Furi-Martelli-Vignoli spectrum and the Feng spectrum of continuous nonlinear block operator matrices,and mainly describe the relationship between the Furi-Martelli-Vignoli spectrum(compared to the Feng spectrum)of the whole operator matrix and that of its entries.In addition,the connection between the Furi-Martelli-Vignoli spectrum of the whole operator matrix and that of its Schur complement is presented by means of Schur decomposition.

Block basis property of a class of 2×2 operator matrices and its application to elasticity

A necessary and sufficient condition is obtained for the generalized eigenfunction systems of 2 ×2 operator matrices to be a block Schauder basis of some Hilbert space, which offers a mathematical foundation of solving symplectic elasticity problems by using the method of separation of variables. Moreover, the theoretical result is applied to two plane elasticity problems via the separable Hamiltonian systems.

A fixed point theorem for a class of β-constrictive operators and its application to the integral equation in L^1(0,∞)

Fixed Point Theory for 1-Set Weakly Contractive Operators in Banach Spaces

In this work, using an analogue of Sadovskii＇s fixed point result and several important inequalities we investigate and give new existence theorems for the nonlinear operator equation F（x） =μx, （μ≥1） for some weakly sequentially continuous, weakly condensing and weakly 1-set weakly contractive operators with different boundary conditions. Correspondingly, we can obtain some applicable fixed point theorems of Leray-Schauder, Altman and Furi-Pera types in the weak topology setting which generalize and improve the corresponding results of [3,15,16].

Existence and Uniqueness of Fixed Points for Mixed Monotone Operators with Application

In this paper, a class of mixed monotone operators is studied. Some new fixed point theorems are presented by means of partial order theory, and the uniqueness and existence of fixed points are obtained without assuming the operator to be compact or continuous. Our conclusions extend the relevant results. Moreover,as an application of our result, the existence and uniqueness of positive solution for a class of fractional differential equation boundary value problem are proved.

Pediatric Post-Operative Atrio-Ventricular Block Meets the Affordable Care Act: A New Strategy for Management

Introduction: Post-operative (post-op) complete atrio-ventricular heart block (CAVB) occurs after 1% - 4% of pediatric cardiac operations. Current practice dictates implantation of permanent pacemaker (PPM) when post-op CAVB persists >9 days. We propose that earlier PPM implantation may be the most cost-effective methodology since patient costs increase with extended length of stay (LOS). Methods: Data on the probabilities of persistent post-op CAVB were extracted from published reports. No individual patient data were utilized during this study. This was utilized to create a decision-making model and a total cost analysis on post-op day 0 - 10 to determine the most cost-efficient day to implant a PPM. Cost variables included estimates of daily cardiac ICU care, cost of PPM implantation, LOS, cost related to possible superficial or deep infection based on published prevalence rates (2.3% and 4.9%, respectively) and need for explant due to deep infection or recovery of native conduction. The model assumes 5-day minimum LOS and 1 day increase in LOS with PPM implantation. Cost data were obtained from relevant billing codes and manufacturer list prices for PPM and leads. A secondary analysis evaluated probability of unnecessary PPMs implanted and excess costs. Results: Post-op day (POD) 4 is the lowest total cost of PPM implantation for post-op CAVB, even when accounting for possible risk of either superficial or deep infection. A one-way sensitivity analysis accounting for variability of cardiac ICU care costs between centers ranging from $3000 - $9000 per day consistently replicates POD 4 as the most cost-effective day for PPM implantation. Implant on POD 4 results in a 26% chance of unnecessary implantation. Conclusions: The most cost-efficient day for PPM implantation for post-op CAVB is post-op day 4, which results in a minimum total cost savings of $17,422 per patient. Added costs due to risk of superficial or deep infection are marginal due to low prevalence of post-operative infection in this population.
