目的 探讨经过多重钻孔并复合自体骨髓的冷冻异体犬足趾关节移植的疗效。方法 24只健康成年杂交犬,制备双侧后肢第2足趾近侧趾间关节1.5cm缺损模型,共48侧,随机分成A、B、C3组(n=16)。分别采用新鲜自体趾间关节(A组)、经钻孔并... 目的 探讨经过多重钻孔并复合自体骨髓的冷冻异体犬足趾关节移植的疗效。方法 24只健康成年杂交犬,制备双侧后肢第2足趾近侧趾间关节1.5cm缺损模型,共48侧,随机分成A、B、C3组(n=16)。分别采用新鲜自体趾间关节(A组)、经钻孔并复合自体新鲜骨髓的冷冻异体趾间关节(B组)及单纯冷冻异体趾间关节(C组)修复缺损。于术后1、3、6和12个月分别行X线摄片和组织病理学检查,了解移植骨关节的成活情况。结果 根据移植关节术后X线片和组织学改变,犬趾骨关节移植后的病变可分为轻度、中度和重度变性3级。A组移植骨关节3~12个月始终表现为轻度变性;B组移植骨关节1~6个月为轻度变性,骨孔中央软骨内成骨现象明显,12个月部分移植骨关节为中度变性;C组从1个月出现移植骨关节中度变性,3个月移植骨关节重度变性。结论 冷冻异体犬足趾关节内多重钻孔并复合自体新鲜骨髓.能有效延缓异体骨关节的早、中期变性,单纯冷冻异体骨关节移植后早期可能发生严重变性。 展开更多

With patients undergoing first time 1st metatarsophalangeal joint arthrodesis using graft material when it was required to fill cystic bone voids, we retrospectively compared the time to fusion (clinical and radiographic), and non-union rate between the patient’s own bone autograft (n = 62) versus a mesenchymal stem cell impregnated allograft group (n = 51). A third control group (n = 52) was included in which an end-to-end arthrodesis was performed and no graft interposition was used or necessary. The non-union rate was 4% (n = 2) in the control group, 5.9% (n = 4) in the autograft group, and 9.5% (n = 5) in the mesenchymal stem cell allograft group. The time for radiographic fusion was 6.46 weeks for the control group, 6.52 weeks for the autograft group, and 6.53 weeks for the mesenchymal stem cell allograft group. The difference in time to clinical and radiographic union and the non-union rate were not found to be statistically significant among all 3 groups. Patient comorbidities and their possible effects on union rates were also analyzed within the populations. Some comorbidities had statistically significantly non-unions within the population, notably smoking (p = 0.024) and Rheumatoid arthritis (p = 0.001), however the populations were fairly small. The use of allogeneic bone graft impregnated with mesenchymal stem cells yields a similar fusion rate as with the use of autologous bone graft harvested from the surrounding area. The allograft impregnated with mesenchymal stem cells is a viable alternative yielding similar results when local autogenous bone graft is not available, not obtainable, or conditions warrant its use. 展开更多

The authors report toward a monocentric retrospective and descriptive study on a 08 year period, 33 cases of knee osteoarticular tuberculosis (OAT) of adult, certified by bacteriologic and/or histolological evidence proof of the sample (synovial biopsy) after a knee arthrotomy. There were 07 cases of arthritis and 26 cases of osteoarthritis. The treatment of knee OAT was medical (anti-tubercular poly-chemotherapy) on one year duration. The surgery was useful in front of some clinical presentations. After a minimum of 18 months, a functional evaluation was done following the Lequesne algo-functionnal index. None had signs of local recurrence testifying the adequacy of medical treatment, even if it is long and binding. The main of our case series is to establish an epidemiological, clinical and biological profile of the knee tuberculosis of adult and to research elements of prognosis through an analysis of results. 展开更多

A TETRACYCLINE LABELING STUDY OF SUBCHONDRAL BONE TISSUES IN OSTEOARTHRITIC HIP JOINT

During operation specimen... Seven female patients (mean age of 56 years) with advanced hip joint osteoarthritis underwent total hip replacement. Four days before operation they were given oral tetracycline for two days. During operation specimens were taken from the white articular cartilage, the yellowish articular cartilage and the ivory bone together with their subchondral bone tissues. The undecalcified specimens were cut into 10 μm sections and observed under the fluorescence microscope. In all the specimens the following findings could be observed. 1. The osteoarthritic articular cartilage became thinner, with uneven surface and fissures. 2. The superficial and deep surfaces and the central part of the subchondral bone plate showed bright golden yellow fluorescence. It reflected extensive new bone formation. 3. The subchondral bone trabeculae also revealed bright golden yellow fluorescence on their peripheral borders, so trabeculae turned thicker obviously. 4. The marrow tissues between the bone trabeculae exhibited particulate or spherical bright golden yellow fluorescence, reflecting new bone formation in the marrow. The particulate and spherical bright golden yellow fluorescent materials might aggregate, enlarge and merge into large piece of new bone and they also fused with the neighbouring bone trabeculae. The aforementioned changes in the structure of the subchondral bone tissues increased greatly the mass in the osteoarthritic femoral head. 展开更多

背景:Anybody骨肌建模系统,使用数学建模技术模拟人体骨骼、肌肉和环境的关系,可对人体的逆向动力学进行研究,得出下肢关节力等指标。目的:分析练习太极拳动作时下肢骨主要关节的应力分布规律,为其科学训练和锻炼价值提供依据。方法:在... 背景:Anybody骨肌建模系统,使用数学建模技术模拟人体骨骼、肌肉和环境的关系,可对人体的逆向动力学进行研究,得出下肢关节力等指标。目的:分析练习太极拳动作时下肢骨主要关节的应力分布规律,为其科学训练和锻炼价值提供依据。方法:在北京体育大学武术学院选取8名太极拳健将级运动员进行7组步法动作和右腿股骨CT的数据采集。使用BTS红外捕捉系统、Kistler三维测力台采集太极(八法五步)7组步法动作的运动和力学数据,利用Anybody 7.2骨肌模型的多体动力学仿真技术计算下肢关节动力学参数,结合Workbench 19.2对股骨进行应力计算分析。结果与结论:①利用Workbench软件分析得出了7组步法动作的股骨应力结果,7组动作的应力峰值由大至小顺序是:退步捋势(22.00 MPa)、退步採势(19.379 MPa)、左右移步挤按(9.35 MPa)、左右移步肘靠(6.30 MPa)、进步掤势(4.68 MPa)、进步挒势(2.57 MPa)、中定独立势(0.31 MPa)。②在7组步法动作中2种向后退步动作股骨应力最大(P<0.05),且在7组动作运动过程中的股骨最大应力位置均不同。③上述结果证实,在太极(八法五步)7种步法动作练习时,股骨应力阈值和最大应力位置会随着5种方向(7组动作)运动不同而变化,通过连续训练能够全面地刺激股骨体,进步动作对于股骨体正面和外侧上端影响较大,退步动作对股骨体后面和内侧影响较大,左右横向步法动作主要是股骨体两侧对称受力。④初学者要根据不同步法动作的受力特点来进行针对训练,进步动作和退步动作训练时要注重太极拳的旋转用力以及左右横移步法动作训练时的内侧对抗用力,根据自身薄弱问题,对太极拳训练步法有所侧重,进而达到更好的锻炼效果。 展开更多

For improving the theory of gradient microstructure of cartilage/bone interface, human distal femurs were studied. Scanning Electron Microscope (SEM), histological sections and MicroCT were used to observe, measure and model the micro- structure of cartilage/bone interface. The results showed that the cartilage/bone interface is in a hierarchical structure which is composed of four different tissue layers. The interlocking of hyaline cartilage and calcified cartilage and that of calcified car- tilage and subchondral bone are in the manner of"protrusion-pore" with average diameter of 17.0 gm and 34.1 lam respectively. In addition, the cancellous bone under the cartilage is also formed by four layer hierarchical structure, and the adjacent layers are connected by bone trabecula in the shape of H, I and Y, forming a complex interwoven network structure. Finally, the simplified structure model of the cartilage/bone interface was proposed according to the natural articular cartilage/bone interface. The simplified model is a 4-layer gradient biomimetic structure, which corresponds to four different tissues of natural cartilage/bone interface. The results of this work would be beneficial to the design of bionic scaffold for the tissue engineering of articular cartilage/bone. 展开更多

For the osseous tissue, its role has not been clearly defined. This study was aimed at determining chromium content in hip joint tissues. Methods A total of 91 hip joint samples were taken in this study, including 66 from females and 25 from males. The sample tissues were separated according to their anatomical parts. The chromium content was determined by the AAS method. The statistical analysis was performed with U Mann-Whitney's non-parametric test, P〈0.05. Results The overall chromium content in tissues of the hip joint in the study subjects was as follows: 5.73 μg/g in the articular cartilage, 5.33 μ/g in the cortical bone, 27.86 μ/g in the cancellous bone, 5.95 μg/8 in the fragment of the cancellous bone from the intertrochanteric region, and 2.28 μ/g in the joint capsule. The chromium contents were observed in 2 group patients, it was 7.04 μ/g in people with osteoarthritis and 22.59 μ/g in people with fractures. Conclusion The observed chromium content was highest in the cancellous bone and the lowest in the joint capsule. Chromium content was significantly different between the people with hip joint osteoarthritis and the people with femoral neck fractures. 展开更多

Brachydactyly usually occurs either as an isolated malformation or as a part of a complex malformation syndrome. Brachydactyly is classified as types A, B, C, D, or E;brachymetatarsus IV;Sugarman brachydactyly;or Kirner deformity. Various types of isolated brachydactyly are rare, except for types A3 and D. We describe a 15-year-old girl with isolated brachyphalangism of the basal finger bones of the little finger with symptoms of tenosynovitis. Tenosynovitis might be caused by growth deviation between the flexor digitorum superficialis and the flexor digitorum profundus. The patient responded very well to surgical treatment. 展开更多

A biomimetic hip joint simulator that can be used to evaluate the outcome of the cemented total hip replacement has been designed, manufactured and evaluated. The simulator produces motion in the extension/flexion plane, with a socket to rotate internal/externally. At the same time a dynamic loading cycle is applied. A validation test was performed on a cemented femoral stem within a novel composite femur. The hone quality has a strong effect on the stem migration and on the integrity of the interfaces. The migration of the stem is a combination of 3-D translation and rotation of the stem. Under the same loading conditions, weak bone allows more stem migration than strong bone. There is a great decrease in the strength of the stem-cement interface after the dynamic test, and the weak bone composite exhibited a greater reduction in interfacial strength than the strong bone composite. The decrease of the interfacial strength indicates that the primary bonding between the stem and the cement mantle had deteriorated and the integrity of stem-cement interface was damaged. The study demonstrates the value of using a hip joint simulator to investigate stem migration and interface integrity within the cemented hip replacement, suggesting that method can be used for in vitro evaluation of the biomaterials used in the cemented hip replacements. 展开更多

目的:分析磁共振脂肪抑制质子密度加权(PDWI-FS)序列诊断骨关节外伤性骨髓水肿(BME)的价值。方法:选取150例骨关节外伤患者为研究对象,均行矢状位PDWI-FS序列扫描及MRI常规序列扫描,比较两种方法对BME的检出情况、信号强度、图像质量、... 目的:分析磁共振脂肪抑制质子密度加权(PDWI-FS)序列诊断骨关节外伤性骨髓水肿(BME)的价值。方法:选取150例骨关节外伤患者为研究对象,均行矢状位PDWI-FS序列扫描及MRI常规序列扫描,比较两种方法对BME的检出情况、信号强度、图像质量、病灶信噪比及对比噪声比。结果:两种检查方法均显示150例骨关节外伤患者中134例存在225处BME征象,以膝关节外伤患者中多见。PDWI-FS序列显示病灶信号强度主要为3级,占比97.78%(220/225),MRI常规序列显示主要为2级,占比43.11%(97/225),且两种检查方法显示不同强度存在统计学差异(Z=15.919,P<0.05)。PDWIFS序列显示病灶清晰程度、空间分辨力、图像变形及诊断信心评分[(4.09±0.45)分、(3.65±0.42)分、(3.25±0.37)分、(4.21±0.38)分]均高于常规序列[(3.88±0.39)分、(3.41±0.36)分、(3.14±0.35)分、(3.97±0.34)分],且差异均有统计学意义(t=4.319,t=5.314,t=2.645,t=5.765;P<0.05)。PDWI-FS序列显示病灶信噪比及周围组织对比噪声比[(2.07±0.23)、(5.52±0.64)]均大于常规序列[(2.01±0.22)、(5.17±0.59)],且差异有统计学意义(t=2.309,t=4.925;P<0.05)。结论:较MRI常规序列而言,磁共振PDWI-FS序列能有效提高图像质量和病变显示效果,可为骨关节外伤性BME的诊断提供更准确的诊疗信息。 展开更多

目的:探讨早期PsA发生骨侵蚀患者的临床特征及相关因素。方法:选取2018年1月至2023年1月住院治疗的早期PsA(关节炎病程≤1年)患者50例,采用彩色多普勒超声仪对患者外周关节进行超声检查,分析发生骨侵蚀的部位及特征,根据超声下有无骨侵... 目的:探讨早期PsA发生骨侵蚀患者的临床特征及相关因素。方法:选取2018年1月至2023年1月住院治疗的早期PsA(关节炎病程≤1年)患者50例,采用彩色多普勒超声仪对患者外周关节进行超声检查,分析发生骨侵蚀的部位及特征,根据超声下有无骨侵蚀分为骨侵蚀组及无骨侵蚀组,分析两组间年龄、性别、病程、血沉、C反应蛋白等临床和实验室参数以及超声影像等的差异。结果:50例PsA患者中23例(46.00%)存在骨侵蚀,27例(54.00%)无骨侵蚀,骨侵蚀组较无骨侵蚀组受累外周关节数更多(P<0.05)。骨侵蚀组手、足关节受累及附着点炎发生率均明显高于无骨侵蚀组(P<0.05)。两组间在年龄、性别、病程、皮疹是否先发、中轴关节是否受累、HLA-B27的阳性率及炎性指标等方面均无统计学差异(P>0.05)。手、足关节受累、附着点炎、受累外周关节数与骨侵蚀呈正相关。结论:骨侵蚀在早期PsA患者较为多见,最常发生于手、足关节,并与附着点炎及受累外周关节数密切相关。 展开更多

目的:观察骶髂关节错位患者在整复手法复位后腰背肌肌张力和对称性,骨盆、下肢生物力学的变化。方法:选取2022年5月至2023年3月期间就诊于福建中医药大学附属第二人民医院的骶髂关节错位患者120例,先理筋手法,再行整复手法,前错位60例,... 目的:观察骶髂关节错位患者在整复手法复位后腰背肌肌张力和对称性,骨盆、下肢生物力学的变化。方法:选取2022年5月至2023年3月期间就诊于福建中医药大学附属第二人民医院的骶髂关节错位患者120例,先理筋手法,再行整复手法,前错位60例,采用屈髋屈膝复位法,后错位60例,采用后伸扳法,隔日治疗1次,共治疗7次。对比治疗前后VAS评分、后表链肌张力K值、后表链肌张力对称值、骨盆倾斜、扭转、水平旋转情况,以及下肢支撑相、摆动相、足偏角的变化。结果:治疗后,骶髂关节前、后错位患者的VAS评分,后表链肌张力系数K值、肌张力对称性差值均较治疗前降低(P<0.05);治疗后,骶髂关节前、后错位患者的骨盆的倾斜、扭转、水平旋转角度均较治疗前减小(P<0.05);治疗后,骶髂关节前、后错位患者的步态周期中的支撑相较治疗前延长(P<0.05),摆动相较治疗前缩短(P<0.05),骶髂关节前错位患者较治疗前足偏角变大(P<0.05),后错位患者足偏角较治疗前变小(P<0.05)。结论:整复手法可以降低骶髂关节错位患者腰背肌肌张力,使两侧腰背肌趋于平衡,且能纠正骨盆的偏歪,改善下肢步态。 展开更多

The associations of TMJ pain and bone characteristics on the activities of daily living

The aim of this investigation was to analyze the associations of TMJ pain/discomfort and bone characteristics on the activities of daily living in a population based study. Methods: The study included 95 randomly selected participants. The impact of temporomandibular joint pain/discomfort on the daily activeties was assessed by using the daily living scale by List and Helkimo (1995). The levels of serum type I collagen telopeptide fragments (P1NP), C-telopeptide crosslaps of type I collagen (CTX-1), and vitamin D (25 (OH) D) were analysed. The Mann-Whitney U-test, Spearman rank correlation coefficent (rs) were used for statistical analyses. Results: Activities of daily living were influenced by temporomandibular joint pain/discomfort to varying degrees. The degree of pain/discomfort during eating was correlated with the level of C-telopeptide crosslaps of type I collagen (p = 0.02) and type 1 collagen telopeptide fragments (p = 0.04). The females in the study had significantly lower vitamin D levels compared to males (p = 0.04). Conclusions: This population based study indicates that TMJ pain/discomfort exerts a significant negative influence on activities of daily living and is related to the biochemical markers of bone turnover and 25 (OH) D level. Several functions of daily activities such as social life, exercising and eating are the most disturbed. 展开更多

目的 研讨胫骨骨髓炎伴长段骨缺损患者采用骨搬移技术治疗的效果。方法 回顾性选取2020年6月—2023年6月南宁市第一人民医院收治的80例胫骨骨髓炎伴长段骨缺损患者的临床资料,依据治疗方法不同分为两组,每组40例,对照组采用骨水泥占位+... 目的 研讨胫骨骨髓炎伴长段骨缺损患者采用骨搬移技术治疗的效果。方法 回顾性选取2020年6月—2023年6月南宁市第一人民医院收治的80例胫骨骨髓炎伴长段骨缺损患者的临床资料,依据治疗方法不同分为两组,每组40例,对照组采用骨水泥占位+二期植骨治疗,观察组采用骨搬移技术治疗。比较两组愈合优良率、恢复指标、踝关节功能评分、膝关节评分、踝肱指数、经皮氧分压。结果 观察组骨愈合优良率较对照组高,差异有统计学意义(P<0.05);观察组各恢复指标水平均优于对照组,差异有统计学意义(P均<0.05);观察组治疗后踝关节功能评分(96.98±4.32)分、膝关节功能评分(98.68±4.15)分较对照组的(89.45±4.23)分和(88.32±4.02)分高,差异有统计学意义(t=5.569、4.922,P均<0.05)。观察组治疗后的踝肱指数、经皮氧分压均高于对照组,差异有统计学意义(P均<0.05)。结论 胫骨骨髓炎伴长段骨缺损患者采用骨搬移技术治疗效果明显,患者骨折愈合效果明显,缩短其愈合时间,患者的膝关节、踝关节功能均有所提升。 展开更多
