Buffon’s needle experiment was originally devised to get the value of π. With the advent of computers, Buffon’s needle algorithm has been used pedagogically as an example of Monte Carlo methods in introduction classes, and there are many Buffon’s needle algorithm implementations available on the internet. However, for the calculation of π, the exact value of π is used in the programs for Buffon’s needle angle sampling, and hence the example is not demonstrated correctly. This brief note presents a random angle sampling algorithm for the Buffon’s needle. We then compare the Buffon’s needle and Hit-and-Miss integration algorithms using Monte Carlo laboriousness comparison, and find that the Hit-and-Miss algorithm is superior. 展开更多

目的:探讨“S”形固定法在行静脉留置针患者中的应用效果。方法:选取2023年1—12月于锦州医科大学附属第一医院行静脉留置针的患者80例作为研究对象,按照随机数字表法分为对照组和观察组,各40例。对照组采用常规固定方式,观察组采用“S... 目的:探讨“S”形固定法在行静脉留置针患者中的应用效果。方法:选取2023年1—12月于锦州医科大学附属第一医院行静脉留置针的患者80例作为研究对象,按照随机数字表法分为对照组和观察组,各40例。对照组采用常规固定方式,观察组采用“S”形固定法。比较两组疼痛程度、手臂活动自由度、皮肤红肿发生率、皮肤红肿严重度。结果:固定后即刻,两组视觉模拟评分法(VAS)评分比较,差异无统计学意义(P>0.05);固定后6 h、12 h、24 h,两组VAS评分低于固定后即刻,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组手臂最大弯曲角度和伸展角度均大于对照组,差异有统计学意义(P<0.001)。观察组皮肤红肿发生率低于对照组,差异有统计学意义(P<0.001)。观察组发生皮肤红肿的患者红肿严重度评分低于对照组,差异有统计学意义(P<0.001)。结论:“S”形固定法在行静脉留置针患者中的应用效果较好,能够减轻患者疼痛,改善手臂活动度,减少及减轻皮肤红肿。 展开更多

In this paper, the background, evolution, basic meaning, clinical application and the detail operating procedures of the differential insertion depth in filiform needle acupuncture were discussed based on the classical expositions of the Yellow Emperor’s Canon of Medicine. It is believed that the differential insertion depth reflects the basic idea of expelling the evil Qi from the body in the application of traditional acupuncture. Since the site of evil invasion has different shades, the position of evil Qi and correct differentiation has become the operation key points of needle insertion. Apart from this, the Yellow Emperor’s Canon of Medicine has further associated the clinical application of filiform needle insertion depth with the seasonal change of Yin and Yang, the body built of the patients, the nature of the diseases, the heat or cold pathogenic factors of the illness, the excess and deficiency of the patient, and the reinforcing and reducing function of acupuncture. These elaborations have greatly enriched the basic content of acupuncture and laid a systematic theoretical foundation of filiform needle operation. The differential insertion depth in acupuncture has its specific meaning, the emphasis of insertion depth of filiform needle with its differentiated clinical implication exemplifies the perceptual thinking features of traditional acupuncture and typical reveals the uniqueness of Chinese civilization. 展开更多

Automated Detection and Quantification of Prostate Cancer in Needle Biopsies by Digital Image Analysis

Introduction: Triple immunohistochemical (IHC) stains including antibodies specific for alpha-methylacyl-CoA-racemase and basal cell markers have been a valuable aid in accurate identification of prostate carcinoma. However, accurate quantification of minuscule areas of prostate carcinoma in biopsy specimens can often be a challenge. Here we assessed the diagnostic value and quantitative use of automated digital image analysis on triple IHC stained prostate needle biopsies. Methods: Twelve cases of prostate needle biopsy material including 75 needle cores were stained with triple-antibody cocktail (P504S + 34βE12 + p63). Slides were digitally scanned with the APERIO digital image analyzer and evaluated with the GENIE pattern and color recognition digital image analysis that we developed. A slide with known areas of adenocarcinoma, high grade prostatic intraepithelial neoplasia (PIN), benign glands and stroma was used as a training set for the automated digital image analysis platform. Results: Among 75 needle biopsy cores, 19 (25.33%) contained adenocarcinoma by histology. Digital image analysis recognized adenocarcinoma in 95% of these needle biopsies. The average area of the needle biopsy was 7.63 mm2 and overall the average area of tumor was 0.196 mm2. The smallest area of tumor recognized by the program was 0.0022 mm2 (0.0363% of the core) and the largest was 0.62 mm2 (8.17% of the core) among needle core biopsies. False positives resulted from areas of high grade PIN with patchy basal cells. The false negative was caused by uneven AMACR staining in one area of adenocarcinoma. Digital recognition of areas of interest was improved by three successive image analysis training which increased the sensitivity and specificity from 83% and 89% to 90% and 93%, respectively. Conclusions: Digital image analysis in concert with IHC triple staining may be useful for accurate detection and quantitative analysis of small foci of prostatic adenocarcinoma. Defining methods to increase the sensitivity and specificity of quantitative automated digital image analysis will likely evolve as an area of investigation. Future automated digital scanning and innovative pattern and color recognition technologies may open avenues for classifying a variety of prostate lesions. 展开更多

Correlation of S100A13 and FOXA1 expression with cell cycle and cell invasion in fine needle aspiration thyroid carcinoma tissue

Methods: Patients who received ultrasound-guided thyroid nodule fine needle aspiration in Haiyang People's Hospital between April 2015 and February 2017 were selected, and the tissues were divided into malignant thyroid tissue and benign thyroid nodules according to the pathological results after biopsy. The expression of S100A13, FOXA1, cell cycle molecules and cell invasion molecules were measured. Results: S100A13, FOXA1, CDK2, CyclinD1, MCM2, MCM7, SKP2, CLOCK, STAT3, STAT5, N-cadherin, MT1-MMP and ADAM17 mRNA expression in thyroid carcinoma tissue were significantly higher than those in benign thyroid nodule;CDK2, CyclinD1, MCM2, MCM7, SKP2 and CLOCK mRNA expression in thyroid carcinoma tissue with high FOXA1 expression were significantly higher than those in thyroid carcinoma tissue with low FOXA1 expression;STAT3, STAT5, N-cadherin, MT1-MMP and ADAM17 mRNA expression in thyroid carcinoma tissue with high S100A13 expression were significantly higher than those in thyroid cancer tissue with low S100A13 expression. Conclusions: High expression of S100A13 and FOXA1 in thyroid carcinoma can promote cell invasion and cell cycle progression. 展开更多

Objective:To investigate the effect of Wei’s triple nine needling therapy on the N2-P2 wave of the flash visual evoked potential(FVEP)in rats of the of the transverse directional pulling model.Methods:Thirty-six Wistar rats were randomly grouped,nine were in normal control group without any treatment,and the remaining 27 were surgically modelled in the right eyes.Eighteen of these rats were randomly divided into a Wei’s triple nine needling therapy group and a model group of nine rats each after the TON model was made using the transverse quantitative retraction method.The other 9 rats were sham-operated,and only the optic nerve was exposed without retraction.On the next day of modelling,the Wei’s triple nine needling therapy group was treated with Wei’s triple nine needling therapy for 20 minutes/1 time/1 day for 14 days.The model group,sham-operated group and normal control group were not intervened.Wei's triple nine acupoints were represented as follows:1st link:"Jingming"(BL1)and"Chengqi"(ST1);2nd link:"Sizhukong"(SJ23)penetrating"Taiyang"(EX-HN5);and the third:"Fengchi"(GB 20)and"Taichong"(LV3).The FVEP of each group was observed on 1d,7d and 14d.The FVEP of each group was observed on 1d,7d and 14d.Results:Compared with the model group,the N2 wave latency and P2 wave latency were shortened in the Wei’s triple nine needling therapy on 1d(P<0.05,P<0.01);on 7d,their N2 wave latency was shortened(P<0.01)and the N2-P2 wave amplitude was increased compared with the model group(P<0.05);on 14d,their P2 wave latency was shortened(P<0.05)and the N2-P2 wave amplitude was increased.There was no statistical difference in N2 wave latency,P2 wave latency and N2-P2 wave amplitude in the sham-operated group on 1d,7d and 14d(P>0.05).The delayed N2 and P2 wave latencies in the model group did not improve from 1d to 14d(P>0.05)and the amplitude decreased throughout,showing a significant difference on 14d compared to 1d(P<0.05).In contrast,the N2 wave latency in Wei’s triple nine needling therapy group was not significantly different until 7d to 14d(P<0.05);its P2 wave latency,although significantly delayed from 1d to 7d(P<0.05),recovered on 14d and reached a level that was not statistically different from 1d and 7d(P>0.05).There was also no significant decrease in N2-P2 wave amplitude between 1d and 14d in Wei’s triple nine needling therapy group(P>0.05).Conclusion:In this experiment,the TON rat model was successfully established by the transverse quantitative retraction method,and the treatment of TON rats with Wei’s triple nine needling therapy reduced the P2 wave delay of the FVEP electrophysiological signal and increased the N2-P2 amplitude,which had a certain positive effect on the repair of optic nerve injury,probably related to its effect of improving the conduction function of the optic nerve and protecting the retinal ganglion cells that had not been degenerated and necrosed. 展开更多

基于Langer线的穿刺体位对肿瘤化疗患者输液港难免性医用黏胶相关皮肤损伤的预防效果研究

目的探究基于Langer线的无损伤针穿刺体位对肿瘤化疗患者输液港难免性黏胶相关皮肤损伤的预防效果。方法通过平行胸部Langer线对床头抬高60~90°和平卧位穿刺进行皮肤软组织力学、表皮屏障功能理论分析,然后选取2019年5月至2020年5... 目的探究基于Langer线的无损伤针穿刺体位对肿瘤化疗患者输液港难免性黏胶相关皮肤损伤的预防效果。方法通过平行胸部Langer线对床头抬高60~90°和平卧位穿刺进行皮肤软组织力学、表皮屏障功能理论分析,然后选取2019年5月至2020年5月南京医科大学第一附属医院肿瘤科79例植入胸壁输液港、既往应用Ⅳ3000及液体敷料皮肤保护性屏障仍出现输液港黏胶相关性皮肤损伤的患者为研究对象。采用前瞻性自身前后对照,干预前采用平卧位穿刺,干预后床头抬高60~90°穿刺,比较两种穿刺体位对输液港黏胶相关性皮肤损伤预防效果和带针期间舒适度的影响。结果干预后患者输液港黏胶相关性皮肤相关性损伤总发生率较干预前显著降低(P<0.001),机械性损伤发生率、接触性皮炎发生率较干预前显著降低(均P<0.05),输液港带针期间舒适度较干预前明显提高(P<0.001),且不增加患者穿刺时的疼痛感(P=0.692)。结论床头抬高60~90°穿刺可降低肿瘤化疗患者输液港难免性黏胶相关皮肤损伤,提高患者带针期间的舒适度。 展开更多

目的:观察多针透刺法联合揿针治疗急性期贝尔面瘫的临床疗效。方法:将符合纳入标准的60例急性期贝尔面瘫患者依据随机数字表随机分为对照组和观察组,每组各30例。对照组给予常规西药治疗,观察组给予多针透刺法联合揿针治疗,两组均治疗4... 目的:观察多针透刺法联合揿针治疗急性期贝尔面瘫的临床疗效。方法:将符合纳入标准的60例急性期贝尔面瘫患者依据随机数字表随机分为对照组和观察组,每组各30例。对照组给予常规西药治疗,观察组给予多针透刺法联合揿针治疗,两组均治疗4周。观察治疗前后两组患者Sunnybrook面神经系统评分、面部残疾指数(FDI)评分、Portmann积分以及SAS焦虑评分变化,并比较临床疗效。结果:治疗后,两组患者患侧Sunnybrook评分(P<0.01)、Portmann评分(P<0.01)、FDI指数升高(P<0.01),SAS焦虑水平降低(P<0.01),且观察组优于对照组。观察组临床总有效率93.3%(28/30)明显高于对照组76.7%(23/30),差异具有统计学意义(P<0.05)。结论:多针透刺法联合揿针治疗急性期贝尔面瘫患者,可有效改善急性期贝尔面瘫患者的临床症状,降低负面情绪,提高生活质量。 展开更多

目的观察刘氏毫火针结合骨骼肌冲击波治疗卒中后肩手综合征(SHS)的临床疗效。方法将90例卒中后SHS患者按照随机数字表法分为毫火针组、冲击波组和联合组。3组均接受常规治疗,在此基础上,3组分别采用刘氏毫火针治疗、骨骼肌冲击波治疗和... 目的观察刘氏毫火针结合骨骼肌冲击波治疗卒中后肩手综合征(SHS)的临床疗效。方法将90例卒中后SHS患者按照随机数字表法分为毫火针组、冲击波组和联合组。3组均接受常规治疗,在此基础上,3组分别采用刘氏毫火针治疗、骨骼肌冲击波治疗和刘氏毫火针与骨骼肌冲击波联合治疗,均治疗1个月。治疗前和治疗1个月时分别采用改良Rankin量表(mRS)、脑卒中生活质量量表(SS-QOL)、数字等级评定量表(NRS)和肩关节被动活动度(PROM)评估临床疗效。结果治疗1个月后,联合组的总有效率(96.67%)明显优于毫火针组(80.00%)、冲击波组(73.33%)(P<0.05)。联合组的mRS、SS-QOL、NRS评分及肩关节PROM优于冲击波组(P<0.05),联合组mRS、SS-QOL、NRS评分及肩关节外展PROM优于毫火针组(P<0.05),肩关节前屈PROM与毫火针组差异不明显(P>0.05)。结论刘氏毫火针联合骨骼肌冲击波治疗卒中后SHS能有效减轻疼痛和改善肩关节功能,提高患者的生活质量。 展开更多

目的探讨头穴丛刺结合跑台训练对坐骨神经损伤大鼠轴突再生及S-100蛋白表达的影响。方法清洁级健康雄性Sprague-Dawley大鼠90只,随机分为空白组、假手术组、模型组、跑台组和头穴丛刺结合跑台组,每组18只,每组分为7 d、14d、21 d三个亚... 目的探讨头穴丛刺结合跑台训练对坐骨神经损伤大鼠轴突再生及S-100蛋白表达的影响。方法清洁级健康雄性Sprague-Dawley大鼠90只,随机分为空白组、假手术组、模型组、跑台组和头穴丛刺结合跑台组,每组18只,每组分为7 d、14d、21 d三个亚组。钳夹法造模,假手术组暴露坐骨神经但不钳夹。造模3 d后进行干预,跑台组进行跑台训练,头穴丛刺结合跑台组行头穴丛刺治疗结合跑台训练,空白组、假手术组及模型组除定时抓取外不做任何干预。各组相应时间点测定坐骨神经功能指数(SFI);取材后坐骨神经HE染色观察轴突生长情况,免疫组化染色检测S-100蛋白表达情况。结果各时间点跑台组及头穴丛刺结合跑台组SFI高于模型组(P<0.05),但低于假手术组和空白组;头穴丛刺结合跑台组SFI高于跑台组(P<0.05)。各时间点跑台组和头穴丛刺结合跑台组轴突生长情况均优于模型组,头穴丛刺结合跑台组优于跑台组。各时间点空白组和假手术组S-100蛋白均呈少量表达,跑台组、头穴丛刺结合跑台组S-100蛋白表达均高于模型组(P<0.05),头穴丛刺结合跑台组S-100蛋白表达高于跑台组(P<0.05)。结论头穴丛刺结合跑台训练能够促进S-100蛋白的表达,促进损伤神经轴突再生,提高坐骨神经功能。 展开更多

浮针结合悬吊训练对腰背肌筋膜炎疗效及血清IL-1β、NLRP3及β-EP表达的影响

目的:明确浮针结合悬吊训练(SET)技术对腰背肌筋膜炎的临床疗效,并分析其对患者血清中IL-1β、NLRP3及β内啡肽(β-EP)含量的影响。方法:选取在东营市人民医院康复医学科门诊治疗的90例腰背肌筋膜炎患者,对患者采用随机数字表法随机分组... 目的:明确浮针结合悬吊训练(SET)技术对腰背肌筋膜炎的临床疗效,并分析其对患者血清中IL-1β、NLRP3及β内啡肽(β-EP)含量的影响。方法:选取在东营市人民医院康复医学科门诊治疗的90例腰背肌筋膜炎患者,对患者采用随机数字表法随机分组,分为浮针组、SET组和浮针联合SET组(简称联合组),各30例。浮针组给予浮针治疗,1次/2 d,SET组给予SET治疗,1次/d,联合组给予浮针联合SET治疗,频率与浮针组及SET组相同。3组治疗以6 d为1个疗程,共治疗2个疗程。在治疗前及治疗后采用VAS量表评分、Oswestry功能障碍评分对患者进行评定,并检测血清中IL-1β、NLRP3及β-EP的含量,进行临床疗效分析。在治疗结束后1个月时采用VAS量表进行随访。结果:经2周治疗后,3组患者VAS及Oswestry评分均较治疗前有明显改善,差异有统计学意义(P<0.05),联合组VAS及Oswestry评分低于浮针组和SET组,差异有统计学意义(P<0.05);联合组总有效率为96.7%(29/30),浮针组为73.3%(22/30),悬吊训练组为80.0%(24/30),联合组高于浮针组和悬吊训练组,差异有统计学意义(P<0.05)。治疗后3组患者血清IL-1β、NLRP3均低于治疗前,差异有统计学意义(P<0.01),β-EP水平高于治疗前,差异有统计学意义(P<0.01),且联合组血清IL-1β、NLRP3含量低于浮针组和SET组,血清β-EP表达高于浮针组和SET组,差异有统计学意义(P<0.01)。3组患者在治疗结束后1个月时VAS评分较治疗结束时均未见明显变化,联合组VAS评分仍低于浮针组和SET组,差异有统计学意义(P<0.05)。结论:浮针结合SET技术对腰背肌筋膜炎疗效确切,可显著改善患者的疼痛及腰椎活动功能,且远期疗效较好,其机制可能与降低血清中IL-1β、NLRP3等炎症因子表达和提高β-EP的表达有关。 展开更多
