Higher Plasma Potassium Level Reduces 10-Year Cardiovascular Disease Risk Predicted by the Framingham Risk Score among Taxi-Motorbike Drivers Residing and Working in Cotonou, Benin

Hypertension, obesity, smoking, dyslipidemia, and type 2 diabetes (T2D) are the major risk factors for developing cardiovascular diseases (CVD). Recent studies revealed that taxi-motorbike drivers (TMDs) in Cotonou had higher rates of CVD risk factors, but their impacts on cardiovascular events have rarely been studied. The Framingham risk score (FRS) is an algorithm that considers CVD risk factors and estimates the risk of developing CVD in the next 10 years. Our objectives were to assess the 10-year CVD risk predicted by the FRS, and to examine the relationships of 10-year CVD risk with plasma iron and potassium levels among TMDs. We included 134 TMDs (22 - 59 years old) who had no prior diagnosis of CVD or T2D, and not taking medications affecting iron and potassium homeostasis. Conventional cardiovascular risk factors were used to calculate the 10-year CVD risk, which was categorized as low (20%). FRS > 2%, which corresponded to the 75th percentile of FRS distribution in our study population, was used as a cut-off value to classify participants into two groups. Plasma iron and potassium levels were segregated into tertiles and their associations with 10-year CVD risk were quantified by multivariate-adjusted logistic regression to calculate the odd ratios (ORs) to being above the 75<sup>th</sup> percentile of 10-year CVD risk with the corresponding 95% confidence intervals (CIs). We found that 62.0% of participants had at least one of cardiovascular risk factors. Approximately 97.8% of TMDs had 10-year CVD risk 4.8 mmol/L led to an 83% risk reduction of having 10-year CVD risk > 2% (OR = 0.17, 95% CI: 0.04 - 0.82, P = 0.027). In conclusion, our findings showed that high plasma potassium levels associate with reduced 10-year CVD risk among TMDs. Interventions focused on monitoring of plasma potassium, particularly in those with existing cardiovascular risk factors, may help prevent CVD. 展开更多

Major and profound changes have taken place in China over the past 30 years. Rapid socioeconomic progress has exerted a great impact on lifestyle, ranging from food, clothing, working and living conditions, and means of transportation to leisure activities and entertainment. At the same time, new health problems have emerged, and health services are facing new challenges. Presently, cardiovascular diseases (CVD) are among the top health problems of the Chinese people, and pose a serious challenge to all engaged in the prevention and control of these diseases. An epidemic of CVD in China is emerging as a result of lifestyle changes, urbanization and longevity. Both national policy decision-making and medical practice urgently need an authoritative report which comprehensively reflects the trends in the epidemic of CVD and current preventive measures. Since 2005, guided by the Bureau of Disease Prevention of the Ministry of Health of the People's Republic of China and the National Center for Cardiovascular Diseases of China, nationwide experts in the fields of epidemiology, clinical medicine and health economics in the realms of CVD, cerebrovascular disease, diabetes and chronic kidney disease, completed the Report on Cardiovascular Diseases in China every year. The report aims to provide a timely review of the trend of the epidemic and to assess the progress of prevention and control of CVD. In addition, as the report is authoritative, representative and readable, it will become an information platform in the CVD field and an important reference book for government, academic institutes, medical organizations and clinical physicians. This publication is expected to play a positive role in the prevention and control of CVD in China. We present an abstract from the Report on Cardiovascular Diseases in China (2010), including trends in CVD, morbidity and mortality of major CVDs, up-to-date assessment of risk factors, as well as health resources for CVD, and a profile of medical expenditure, with the aim of providing evidence for decision-making in CVD prevention and control programs in China, and of delivering the most authoritative information on CVD prevention and control for all citizens. 展开更多

BACKGROUND Cognitive decline is common among older patients with cardiovascular disease(CVD) and can decrease their self-management abilities. However, the instruments for identifying mild cognitive impairment(MCI) are not always feasible in clinical practice. Therefore, this study evaluated whether MCI could be detected using the Japanese version of the Rapid Dementia Screening Test(RDST-J), which is a simple screening tool for identifying cognitive decline.METHODS This retrospective single-center study included patients who were ≥ 65 years old and hospitalized because of CVD.Patients with a pre-hospitalization diagnosis of dementia were excluded. Each patient's cognitive function had been measured at discharge using the RDST-J and the Japanese version of the Montreal Cognitive Assessment(Mo CA-J), which is a standard tool for MCI screening. The correlation between the two scores was evaluated using Spearman's rank correlation coefficient. Receiver operating characteristic(ROC) analysis was also to evaluate whether the RDST-J could identify MCI, which was defined as a Mo CA-J score of ≤ 25 points.RESULTS The study included 78 patients(mean age: 77.2 ± 8.9 years). The RDST-J and Mo CA-J scores were strongly correlated(r = 0.835, P < 0.001). The ROC analysis revealed that an RDST-J score of ≤ 9 points provided 75.4% sensitivity and 95.2% specificity for identifying MCI, with an area under the curve of 0.899(95% CI: 0.835-0.964). The same cut-off value was identified when excluding patients with a high probability of dementia(RDST-J score of ≤ 4 points).CONCLUSIONS The RDST-J may be a simple and effective tool for identifying MCI in older patients with CVD. 展开更多

Cardiovascular diseases(CVDs)are major disease burdens with high mortality worldwide.Early prediction of cardiovascular events can reduce the incidence of acute myocardial infarction and decrease the mortality rates of patients with CVDs.The pathological mechanisms and multiple factors involved in CVDs are complex;thus,traditional data analysis is insufficient and inefficient to manage multidimensional data for the risk prediction of CVDs and heart attacks,medical image interpretations,therapeutic decision-making,and disease prognosis prediction.Meanwhile,traditional Chinese medicine(TCM)has been widely used for treating CVDs.TCM offers unique theoretical and practical applications in the diagnosis and treatment of CVDs.Big data have been generated to investigate the scientific basis of TCM diagnostic methods.TCM formulae contain multiple herbal items.Elucidating the complicated interactions between the active compounds and network modulations requires advanced data-analysis capability.Recent progress in artificial intelligence(AI)technology has allowed these challenges to be resolved,which significantly facilitates the development of integrative diagnostic and therapeutic strategies for CVDs and the understanding of the therapeutic principles of TCM formulae.Herein,we briefly introduce the basic concept and current progress of AI and machine learning(ML)technology,and summarize the applications of advanced AI and ML for the diagnosis and treatment of CVDs.Furthermore,we review the progress of AI and ML technology for investigating the scientific basis of TCM diagnosis and treatment for CVDs.We expect the application of AI and ML technology to promote synergy between western medicine and TCM,which can then boost the development of integrative medicine for the diagnosis and treatment of CVDs. 展开更多

乌鲁木齐市35~75岁体检人群心血管病高危预测模型的建立与验证

目的了解乌鲁木齐市35~75岁体检人群心血管病(CVD)高危人群检出情况并建立多因素Logistic回归模型与决策树模型预测CVD高危人群,对比分析模型预测效果及准确率,为CVD的预防控制提供参考依据。方法于2023年6月-2024年1月采用便利抽样方... 目的了解乌鲁木齐市35~75岁体检人群心血管病(CVD)高危人群检出情况并建立多因素Logistic回归模型与决策树模型预测CVD高危人群,对比分析模型预测效果及准确率,为CVD的预防控制提供参考依据。方法于2023年6月-2024年1月采用便利抽样方法在乌鲁木齐市某三甲医院体检中心抽取40364名35~75岁体检者进行问卷调查和体格检查,分析CVD高危人群检出情况;应用多因素Logistic回归分析方法以及决策树模型建立CVD高危预测模型,并对模型的预测效果和准确性进行比较和分析。结果40364名35~75岁体检人群中,检出CVD高危人群10858名,CVD高危人群检出率为26.9%。多因素Logistic回归分析结果显示,年龄>40岁、商业服务业、退(离)休人员、吸烟、饮食中肉食摄入过多或过少是35~75岁体检人群CVD高危人群检出的危险因素。女性、初高中以上文化程度、专业技术人员、办事人员或职员、农林牧渔生产人员以及生产运输设备操作工人、每周运动1次及以上是乌鲁木齐市35~75岁体检人群CVD高危人群检出的保护因素。CVD高危人群体质指数、收缩压、舒张压、臀围、腰围、空腹血糖、总胆固醇、甘油三酯、高密度脂蛋白和低密度脂蛋白胆固醇中除高密度脂蛋白低于非高危人群外,其余均高于非高危人群,差异有统计学意义(均P<0.001)。以人口学特征(年龄、吸烟习惯、饮食习惯)和体检指标(收缩压、舒张压、空腹血糖、丙氨酸氨基转移酶、门冬氨酸氨基转移酶、总胆固醇、甘油三酯、高密度脂蛋白、低密度脂蛋白)为自变量,通过多因素Logistic回归以及决策树模型建立CVD高危预测模型,两个模型均具有较好的预测准确率,多因素Logistic回归模型与决策树模型的AUC值均为0.867(95%CI=0.864~0.871,P<0.001)。结论乌鲁木齐市35~75岁体检人群CVD高危人群检出率相对较高。构建的多因素Logistic回归模型以及可视化的决策树模型均具有较好的预测效果。可以预测个体未来成为心血管病高危人群的可能性,为识别CVD高危人群提供了筛查工具,对推进心血管疾病的零级预防有一定的指导作用。 展开更多

目的探讨代谢健康状态转变与体质指数(BMI)交互作用及与心血管疾病(CVD)的关联。方法基于“新疆多民族自然人群队列建设及健康随访研究”和田调查现场,进行前瞻性队列研究。经过3年随访,根据代谢状态转变将研究对象分为保持代谢健康(MH... 目的探讨代谢健康状态转变与体质指数(BMI)交互作用及与心血管疾病(CVD)的关联。方法基于“新疆多民族自然人群队列建设及健康随访研究”和田调查现场,进行前瞻性队列研究。经过3年随访,根据代谢状态转变将研究对象分为保持代谢健康(MH)、代谢健康转为代谢不健康(MH-MU)、保持代谢不健康(MU)、代谢不健康转为代谢健康(MU-MH)4组,多因素Logistic回归分析不同组代谢状态对CVD发生的影响。应用限制性立方样条探讨BMI为连续变量与CVD风险的关联,分析代谢状态转变和BMI交互作用与CVD的关联。结果共10232人纳入本研究,平均年龄为(52.43±10.29)岁。在随访3年期间新发心血管疾病349例(3.41%);其中MH、MH-MU、MU和MU-MH 4组心血管疾病发生率分别为2.48%、4.21%、5.13%、3.30%。与MH组相比,MH-MU、MU组心血管疾病发生风险分别为OR=1.42(95%CI:1.05~1.93)和OR=1.41(95%CI:1.03~1.92)。限制性立方样条分析表明,基线BMI与心血管疾病发生为非线性关联(P=0.023)。BMI分组与代谢状态转变存在交互作用(χ2=26.025,P=0.001),其归因于交互效应比例为3%[RERI=0.032(95%CI:0.005~0.06)、AP=0.031(95%CI:0.007~0.05)、S=2.157(95%CI:1.194~3.897)]。结论保持或转变为代谢不健康状态均可增加心血管疾病的风险。保持或转为代谢不健康状态与超重、肥胖交互作用能够增加心血管疾病的风险。 展开更多

目的探讨维持性血液透析(maintenance hemodialysis,MHD)患者外周血单个核细胞(peripheralblood mononuclear cell,PBMC)核因子-κB(nuclear factor kappa B,NF-κB)活性与心血管疾病(cardiovasculardisease,CVD)的关系。方法选取MHD治... 目的探讨维持性血液透析(maintenance hemodialysis,MHD)患者外周血单个核细胞(peripheralblood mononuclear cell,PBMC)核因子-κB(nuclear factor kappa B,NF-κB)活性与心血管疾病(cardiovasculardisease,CVD)的关系。方法选取MHD治疗3个月以上的稳定患者(n=32),以体检健康人为对照组(n=12),采用酶联免疫吸附法检测受试者PBMC的NF-κB活性及血浆五聚素3(pentraxin 3,PTX3)。用Pearson相关和线性回归分析PBMC的NF-κB活性与其他指标的相关性。用二分类Logistic回归分析PBMC的NF-κB活性与CVD的关系。用受试者工作特征(receiver operating characteristic,ROC)曲线比较PBMC的NF-κB活性、PTX3和高敏C反应蛋白(high sensitivity C-reactive protein,hsCRP)与CVD的相关性。结果 MHD患者PBMC的NF-κB活性、血浆PTX3和血清hsCRP皆显著高于对照组;单次透析后MHD患者PBMC的NF-κB活性和血浆PTX3显著升高;合并CVD患者PBMC的NF-κB活性显著高于非CVD患者。Pearson相关分析显示PBMC的NF-κB活性与白细胞计数(r=0.454,P=0.009)、血清hsCRP(r=0.590,P<0.001)、血浆PTX3(r=0.680,P<0.001)、心肌肌钙蛋白T(r=0.409,P=0.020)及颈动脉内膜中层厚度(r=0.541,P=0.011)呈显著正相关。线性回归分析显示白细胞计数(β=0.338,P=0.028)、血清hsCRP(β=0.440,P=0.005)、血浆PTX3(β=0.487,P=0.004)及颈动脉内膜中层厚度(β=0.453,P=0.032)与PBMC的NF-κB活性独立相关。Logistic回归分析显示PBMC的NF-κB活性升高是CVD的独立危险因素(OR=8.47,P=0.022)。受试者工作特征曲线(receiver operating characteristic curve,ROC曲线)分析显示,PBMC的NF-κB活性与CVD的相关性高于PTX3和hsCRP,三者的ROC曲线下面积分别为0.791(95%CI:0.633~0.950,P=0.006)、0.785(95%CI:0.628~0.943,P=0.007)和0.711(95%CI:0.514~0.907,P=0.046)。结论 MHD患者PBMC的NF-κB活性显著升高;血液透析(hemodialysis,HD)过程可激活PBMC的NF-κB;MHD患者PBMC的NF-κB活性与CVD相关,可作为患者炎症和CVD危险因素的标志物。 展开更多

目的:探讨慢性肾脏病(CKD)患者合并睡眠呼吸暂停低通气综合征(SAHS)对CKD患者心血管疾病(CVD)发病情况的影响。方法:调查92例CKD患者合并SAHS的情况,并分析CKD病情严重程度与SAHS患病率的关系;采用Logistic回归分析法分析CKD合并SAHS时... 目的:探讨慢性肾脏病(CKD)患者合并睡眠呼吸暂停低通气综合征(SAHS)对CKD患者心血管疾病(CVD)发病情况的影响。方法:调查92例CKD患者合并SAHS的情况,并分析CKD病情严重程度与SAHS患病率的关系;采用Logistic回归分析法分析CKD合并SAHS时对其CVD发病情况的影响。结果:92例CKD患者合并SAHS的患病率为83.70%(77/92),以中枢性睡眠呼吸暂停(CSA)为主(49.35%,38/77);终末期肾脏病患者合并SAHS的患病率明显高于非终末期肾脏病患者(P<0.05)。Logistic回归分析显示,SAHS是CKD患者并发CVD的预测危险因子(OR=9.41,95%CI:2.16-41.03);CKD合并SAHS患者CVD患病率明显高于CKD未合并SAHS者(P<0.05)。结论:CKD患者容易合并SAHS,且以CSA为主;同时,SAHS可以增加CKD患者并发CVD的风险。 展开更多

Abstract Objective To observe apoptotic process in various cardiovascular disorders with a particular attention to the ultrastructural morphology of apoptosis in cardiomyocytes, fibroblasts, vascular endothelial cells and smooth muscle cells. Methods Transmission electron microscopic observations of the tissue specimens obtained from endomyocardial biopsies or surgical excisions of left ventricular myocardium or calcified aortic valves were carried out in 50 patients with various cardiovascular diseases. Results The ultrastructural features of apoptosis was consistently observed in cardiomyocytes, fibroblasts, vascular endothelial cells and smooth muscle cells in all diseased tissues. In cardiomyopathies and rheumatic heart diseases apoptosis was commonly observed in the cardiomyocytes. It was often found that fibroblasts underwent apoptosis in calcific aortic valve tissues. Apoptosis of arterial smooth muscle cells was a frequent finding in renal arterial stenosis due to Takayasu's arteritis and fibromuscular dysplasia. Regardless of the cell types, the nuclear hallmarks of apoptosis were identical with minor modifications of the fragmentation of the condensed cells into apoptotic bodies. Conclusions Based on electron microscopic findings, it is suggested that the underlying disease processes determine which type of cells predominantly undergoes apoptotic changes in various cardiovascular disorders. In addition, different cells with similar structural morphology may have common ultrastructural features of apoptosis. 展开更多

Heart Rate Variability Applied to Short-Term Cardiovascular Event Risk Assessment

Cardiovascular disease (CVD) risk assessment is an important instrument to enhance the clinical decision in the daily practice as well as to improve the preventive health care promoting the transfer from the hospital to patient’s home. Due to its importance, clinical guidelines recommend the use of risk scores to predict the risk of a cardiovascular disease event. Therefore, there are several well known risk assessment tools, unfortunately they present some limitations.This work addresses this problem with two different methodologies:1) combination of risk assessment tools based on fusion of Bayesian classifiers complemented with genetic algorithm optimization;2) personalization of risk assessment through the creation of groups of patients that maximize the performance of each risk assessment tool. This last approach is implemented based on subtractive clustering applied to a reduced-dimension space.Both methodologies were developed to short-term CVD risk prediction for patients with Acute Coronary Syndromes without ST segment eleva-tion (ACS-NSTEMI). Two different real patients’ datasets were considered to validate the developed strategies:1) Santa Cruz Hospital, Portugal, N=460 patients;2)LeiriaPombal Hospital Centre, Portugal, N=99 patients.This work improved the performance in relation to current risk assessment tools reaching maximum values of sensitivity, specificity and geometric mean of, respectively, 80.0%, 82.9%, 81.5%. Besides this enhancement, the proposed methodologies allow the incorporation of new risk factors, deal with missing risk factors and avoid the selection of a single tool to be applied in the daily clinical practice. In spite of these achievements, the CVD risk assessment (patient stratification) should be improved. The incorporation of new risk factors recognized as clinically significant, namely parameters derived from heart rate variability (HRV), is introduced in this work. HRV is a strong and independent predictor of mortality in patients following acute myocardial infarction. The impact of HRV parameters in the characterization of coronary artery disease (CAD) patients will be conducted during hospitalization of these patients in the Leiria-Pombal Hospital Centre (LPHC). 展开更多

Background:Most duck eggs possess a fishy odor,indicating that ducks generally exhibit impaired trimethylamine(TMA)metabolism.TMA accumulation is responsible for this unpleasant odor,and TMA metabolism plays an essen-tial role in trimethylaminuria(TMAU),also known as fish odor syndrome.In this study,we focused on the unusual TMA metabolism mechanism in ducks,and further explored the unclear reasons leading to the debilitating TMA metabolism.Methods:To achieve this,transcriptome,proteome,and metagenome analyses were first integrated based on the constructed duck populations with high and low TMA metabolism abilities.Additionally,further experiments were conducted to validate the hypothesis regarding the limited flavin-containing monooxygenase 3(FMO3)metabolism ability of ducks.Results:The study demonstrated that liver FMO3 and cecal microbes,including Akkermansia and Mucispirillum,par-ticipated in TMA metabolism in ducks.The limited oxidation ability of FMO3 explains the weakening of TMA metabo-lism in ducks.Nevertheless,it decreases lipid deposition and increases antibacterial activity,contributing to its survival and reproduction during the evolutionary adaptation process.Conclusions:This study demonstrated the function of FMO3 and intestinal microbes in regulating TMA metabolism and illustrated the biological significance of FMO3 impairment in ducks. 展开更多
