Construction delay is a widespread issue in the construction industry of developing countries, and Nepal is no exception. These delays extend project durations and lead to cost overruns and disputes among stakeholders. To address this problem, this study aimed to identify and analyze the significant factors that contribute to construction project delays in Nepal. To gather data, a well-structured questionnaire was developed and administered to a sample of 100 participants, including contractors, consultants, and civil engineers. Various statistical tests were conducted to ensure the data’s integrity and consistency, such as reliability assessments and factor analyses. The findings of the study highlighted multiple factors contributing to delays in construction projects such as inadequate design, poor communication, and coordination among stakeholders, insufficient experience and planning by contractors, delays in material delivery and testing, labor-related problems including shortages and low qualifications, and external factors like regulatory changes and unforeseen circumstances. By identifying these major causes of construction project delays, this study presented insightful information that can contribute to the analysis and evaluation of project performance. 展开更多

Glia-derived axonal growth inhibitory proteins limit functional repair following damage to the adult cen- tral nervous system (CNS). Nogo proteins, myelin-as- sociated glycoprotein (MAG), oligodendrocyte myelin glycoprotein (OMgp) and B lymphocyte stimulator (BLyS), are 4 inhibitors that commonly interact with the neuronal receptor, Nogo receptor-1 (NgR1), lead- ing to inhibition of axonal growth. Here, we demon- strate that lateral olfactory tract usher substance (LOTUS) binds to NgR1 and blocks the binding of all four ligands to NgR1, resulting in the suppression of axonal growth inhibition induced by these NgR1 li- gands. LOTUS allows neurons to overcome NgRl-me- diated axonal growth inhibition, raising the possibility that LOTUS may be useful in future therapeutic ap- proaches as an endogenous potent inhibitor of NgR1 for promoting neuronal regeneration. 展开更多

Tooth development is a complex process that involves precise and time-dependent orchestration of multiple genetic, molecular, and cellular interactions. Ameloblastin(AMBN, also named "amelin" or "sheathlin") is the second most abundant enamel matrix protein known to have a key role in amelogenesis. Amelogenesis imperfecta(AI [MIM: 104500]) refers to a genetically and phenotypically heterogeneous group of conditions characterized by inherited developmental enamel defects. The hereditary dentin disorders comprise a variety of autosomal-dominant genetic symptoms characterized by abnormal dentin structure affecting either the primary or both the primary and secondary teeth. The vital role of Ambn in amelogenesis has been confirmed experimentally using mouse models. Only two cases have been reported of mutations of AMBN associated with non-syndromic human AI. However, no AMBN missense mutations have been reported to be associated with both human AI and dentin disorders. We recruited one kindred with autosomal-dominant amelogenesis imperfecta(ADAI) and dentinogenesis imperfecta/dysplasia characterized by generalized severe enamel and dentin defects. Whole exome sequencing of the proband identified a novel heterozygous C-T point mutation at nucleotide position 1069 of the AMBN gene, causing a Pro to Ser mutation at the conserved amino acid position 357 of the protein. Exfoliated third molar teeth from the affected family members were found to have enamel and dentin of lower mineral density than control teeth, with thinner and easily fractured enamel, short and thick roots, and pulp obliteration. This study demonstrates, for the first time, that an AMBN missense mutation causes non-syndromic human AI and dentin disorders. 展开更多

AIM: To determine the causative agents of spontaneous bacterial peritonitis (SBP) in children with liver disease and ascites in our center. METHODS: During a 2.5 year period, from September 2003 to March 2006, 12 patients with 13 episodes of SBP were studied. In all cases at the time of admission serum albumin and glucose, urinalysis and urine culture was performed. Analysis [white blood cell (WBC) count with differential, albumin, glucose], gram stain, culture by BACTEC method and antibiogram was done on ascitic fluids. Abdominal paracentesis was repeated after 48 h of antibiotic therapy for bacteriologic assay. The patients were followed for at least three months in a gastroenterology clinic. RESULTS: There were 7 girls (58%) and 5 boys (42%) with a median age of 5.2 years (range, 6 mo to 16 years). All cases had positive ascitic fluid culture. Gram stain was positive in 5 (38.5%) of them. The isolated organisms were S. pneumoniae in 5 (38.5%), E. coli in 2 (15.3%), S. viridans in 2 (15.3%), and K. pneumoniae, H. influenza, Enterococci, and nontypable Streptococcus each in one (7.7%). All of them except Enterococci were sensitive to ciprofloxacin and ceftriaxone. All ascitic fluid cultures were negative after 48 h of antibiotic therapy. CONCLUSION: S. pneumoniae is the most common cause of SBP in the pediatric age group and we recommend a third generation cephalosporine (e.g., Ceftriaxlone or Cefotaxime) for empirical therapy in children with SBP. 展开更多

Policymakers and international organizations are making the conscious effort to address climate change through afforestation and sustainable ecosystem management. Economic activities including agriculture, mining, and infra-structure improvement to meet basic human needs continuously degrade the natural and forest resources. The rate of deforestation in Ghana is alarming due to over-reliance on forest resources by forest-dependent communities. Perceived causes of deforestation differ from individuals and groups perspec-tive. This depends on factors including environmental knowledge, education level, market demand and socio-economic activities. Simple random sampling and key informant interview with the aid of semi-structured questionnaire was used to elicit information from the indigenes of Sissala West District to determine their perceptions of causes of deforestation. The study indicated that poverty, high illiteracy, population growth and lack of alternative source of livelihood were the indirect causes which trigger livelihood economic activities such as farming, charcoal burning, wood logging and hunting leading to degradation of the ecosystem. It was also realized that majority of the indigenes are uneducated and this contributes to their unawareness of rate of de-forestation. Recommendations suggested to address challenges were enforce-ment of bye-laws and stringent government environmental policies to deter people from degrading the forest. Education, agroforestry, afforestation, and provision of alternative livelihood were also good interventions suggested. 展开更多

AIM: To map Usher phenotype in a consanguineous Pakistani family and identify disease-associated mutation in a causative gene to establish phenotype-genotype correlation.· METHODS: A consanguineous Pakistani family in which Usher phenotype was segregating as an autosomal recessive trait was ascertained. On the basis of results of clinical investigations of affected members of this family disease was diagnosed as Usher syndrome(USH). To identify the locus responsible for the Usher phenotype in this family, genomic DNA from blood sample of each individual was genotyped using microsatellite Short Tandem Repeat(STR) markers for the known Usher syndrome loci. Then direct sequencing was performed to find out disease associated mutations in the candidate gene.· RESULTS: By genetic linkage analysis, the USH phenotype of this family was mapped to PCDH15 locus on chromosome 10q21.1. Three different point mutations in exon 11 of PCDH15 were identified and one of them,c.1304AC was found to be segregating with the disease phenotype in Pakistani family with Usher phenotype. This, c.1304 A C transversion mutation predicts an amino-acid substitution of aspartic acid with an alanine at residue number 435(p.D435A) of its protein product. Moreover, in silico analysis revealed conservation of aspartic acid at position 435 and predicated this change as pathogenic.·CONCLUSION:Theidentificationofc.1304ACpathogenic mutation in PCDH15 gene and its association with Usher syndrome in a consanguineous Pakistani family is thefirst example of a missense mutation of PCDH15 causing USH1 phenotype. In previous reports, it was hypothesized that severe mutations such as truncated protein of PCDH15 led to the Usher I phenotype and that missense variants are mainly responsible for non-syndromic hearing impairment. 展开更多

The modern power system has evolved into a cyber-physical system with deep coupling of physical and information domains, which brings new security risks.Aiming at the problem that the“information-physical”cross-domain attacks with key nodes as springboards seriously threaten the safe and stable operation of power grids,a risk propagation model considering key nodes of power communication coupling networks is proposed to study the risk propagation characteristics of malicious attacks on key nodes and the impact on the system.First, combined with the complex network theory,a topological model of the power communication coupling network is established, and the key nodes of the coupling network are screened out by Technique for Order Preference by Similarity to Ideal Solution(TOPSIS)method under the comprehensive evaluation index based on topological characteristics and physical characteristics.Second,a risk propagation model is established for malicious attacks on key nodes to study its propagation characteristics and analyze the state changes of each node in the coupled network. Then, two loss-causing factors:the minimum load loss ratio and transmission delay factor are constructed to quantify the impact of risk propagation on the coupled network. Finally, simulation analysis based on the IEEE 39-node system shows that the probability of node being breached(α)and the security tolerance of the system(β)are the key factors affecting the risk propagation characteristics of the coupled network,as well as the criticality of the node is positively correlated with the damage-causing factor. The proposed methodological model can provide an effective exploration of the diffusion of security risks in control systems on a macro level. 展开更多

The clinical picture of actinomycosis was first described in 1878.Actinomycosis agents are found in the natural flora of the oral cavity, upper gastrointestinal system and female genital systems. Actinomyces israelii is usually responsible for the infections and causes chronic suppurative and granulomatous infections. The most common disease form is cervicofascial infection. Liver is involved in 5% of all actinomycosis infections. Liver involvement secondary to any primary infection site in the abdomen frequently occurs. In 15% of abdominal infections the liver is also involved. However, in very rare cases, a primary focus cannot be found, and these cases are called primary or isolated hepatic actinomycosis (IHA). IHA which is frequently seen as a solitary abscess can be confused with a malignancy both clinically and radiologically. 展开更多

In this study, the characteristics and preliminary causes of tropical cyclone remote precipitation(TRP)over China during the period from 1979 to 2020 are investigated. Results indicated that approximately 72.42%of tropical cyclones(TCs)in the Western Pacific produce TRP over China. The peak months for TRP are July and August. The four key regions of TRP are the adjacent areas between the Sichuan and Shaanxi Provinces, the northern coast of the Bohai Sea,the coast of the Yellow Sea,and the southern coast area. The typical distance between the station with TRP and the TC center ranges from 1500 to 2500 km.Most of these stations are situated north to 60°west of north of the TC.The south–west water vapor transportation on the west side of the TC is crucial to TRP.TRP has a decreasing trend because of the decrease in the number of TCs that generate TRP.From the perspective of large-scale environmental conditions,a decrease in the integrated horizontal water vapor transport in China' Mainland, the weakening of upward motion at approximately 25°–35°N,which is inconducive to convection, and an increase in low-level vertical wind shear, which is unfavorable for the development of TC in areas with high frequencies of TRP-related TCs,are the factors that result in the decreasing trend of TRP. 展开更多

Fine particulate matter(PM_(2.5))and ozone(O_(3))double high pollution(DHP)events have occurred frequently over China in recent years, but their causes are not completely clear. In this study, the spatiotemporal distribution of DHP events in China during 2013–20 is analyzed. The synoptic types affecting DHP events are identified with the Lamb–Jenkinson circulation classification method. The meteorological and chemical causes of DHP events controlled by the main synoptic types are further investigated. Results show that DHP events(1655 in total for China during 2013–20)mainly occur over the North China Plain,Yangtze River Delta,Pearl River Delta,Sichuan Basin, and Central China. The occurrence frequency increases by 5.1%during 2013–15,and then decreases by 56.1%during 2015–20.The main circulation types of DHP events are“cyclone”and“anticyclone”,accounting for over 40%of all DHP events over five main polluted regions in China, followed by southerly or easterly flat airflow types, like“southeast”,“southwest”,and“east”.Compared with non-DHP events,DHP events are characterized by static or weak wind, high temperature(20.9℃ versus 23.1℃)and low humidity(70.0%versus 64.9%).The diurnal cycles of meteorological conditions cause PM_(2.5)(0300–1200 LST,Local Standard Time=UTC+8 hours)and O_(3)(1500–2100 LST)to exceed the national standards at different periods of the DHP day.Three pollutant conversion indices further indicate the rapid secondary conversions during DHP events, and thus the concentrations of NO_(2),SO_(2) and volatile organic compounds decrease by 13.1%,4.7%and 4.4%,respectively.The results of this study can be informative for future decisions on the management of DHP events. 展开更多

BACKGROUND Total knee arthroplasty(TKA)can improve pain, quality of life, and functional outcomes.Although uncommon, postoperative complications are extremely consequential and thus must be carefully tracked and communicated to patients to assist their decision-making before surgery.Identification of the risk factors for complications and readmissions after TKA,taking into account common causes, temporal trends, and risk variables that can be changed or left unmodified, will benefit this process.AIM To assess readmission rates, early complications and their causes after TKA at 30 days and 90 days post-surgery.METHODS This was a prospective and retrospective study of 633 patients who underwent TKA at our hospital between January 1,2017,and February 28,2022.Of the 633 patients,28 were not contactable, leaving 609 who met the inclusion criteria. Both inpatient and outpatient hospital records were retrieved, and observations were noted in the data collection forms.RESULTS Following TKA,the 30-day and 90-day readmission rates were determined to be 1.1%(n=7)and 1.8%(n=11),respectively.The unplanned visit rate at 30 days following TKA was 2.6%(n=16)and at 90 days was 4.6%(n=28).At 90 days, the unplanned readmission rate was 1.4%(n=9).Reasons for readmissions included medical(27.2%,n=3)and surgical(72.7%,n=8).Unplanned readmissions and visits within 90 days of follow-up did not substantially differ by age group(P=0.922),body mass index(BMI)(P=0.633),unilateral vs bilateral TKA(P=0.696),or patient comorbidity status(30-day P=0.171 and 90-day P=0.813).Reoperation rates after TKA were 0.66%(n=4)at 30 days and 1.15%(n=8)at 90 days. The average length of stay was 6.53 days.CONCLUSION In this study, there was a low readmission rate following TKA.There was no significant correlation between readmission rate and patient factors such as age,BMI,and co-morbidity status. 展开更多

Hyperlipidemia is a kind of pancreatitis caused by high triglyceride levels in the blood. The morbidity and mortality of this disease continue to increase worldwide, and it has become one of the most common gastrointestinal diseases in developed countries worldwide. Although many studies have been conducted, the pathogenesis still cannot be defined. Many studies have shown that this may be related to the triglyceride decomposition products free fatty acids are the main toxic substances, which can directly damage pancreatic acinar cells and vascular endothelial cells, causing tissue ischemia and acidic environment. Therefore, this paper focuses on the correlation of triglycerides and their decomposition products in plasma and provides evidence on the pathogenesis of AP and the disease progression of AP. Finally, the future potential to prevent and treat acute pancreatitis by some new drugs to reduce plasma triglycerides is summarized. 展开更多

Introduction: Neonatal mortality represents a global health problem that has been at the core of programs developed by the World Health Organization (WHO) for more than 30 years. In our country, it represents a real scourge and remains high despite the efforts made by the Ministry of Health. The goal of this study was to investigate the prevalence and causes of neonatal mortality at CHU-MEL of Cotonou in 2023. Material and Methods: This was a cross-sectional and descriptive study with retrospective data collection, carried out in the neonatology unit of the Mother and Child Teaching Hospital of Lagune (CHU-MEL) in Cotonou over a period of six months from January 1, 2023 to June 30, 2023. All neonates, premature or full-term, born alive and who died during hospitalization in the unit were included. Results: 211 cases of neonatal deaths were recorded among the 2884 neonates hospitalized in the unit during the study period, representing a hospital prevalence of 7.31%. Early neonatal mortality represented 81.5% of cases. The average age at admission was 4.6 days ± 5.3. The average weight of deceased neonates was 1609.08 ± 798.35 g. The most frequent reasons for hospitalization were represented by prematurity (60.66%) and respiratory distress (23.22%), respectively. Prematurity was the leading cause of neonatal mortality (41.7%), followed by neonatal infections (29.4%) and perinatal asphyxia (10.9%). Conclusion: The prevalence of neonatal mortality in the neonatology unit of CHU-MEL is high. Efforts to improve the quality of perinatal care departments must be intensified to reduce this prevalence. 展开更多
