Triangular fibrocartilage complex injuries are common in amateur and professional sports.These injuries are mainly caused by acute or chronic repetitive axial loads on the wrist,particularly on the ulnar side and in association with rotations or radial/ulnar deviations.In order to treat professional athletes,a detailed specific knowledge of the pathology is needed.Moreover,the clinician should fully understand the specific and unique environment and needs of the athletes,their priorities and goals,the type of sport,the time of the season,and the position played.An early diagnosis and appropriate management with the quickest possible recovery time are the uppermost goals for both the athlete and the surgeon.A compromise between conservative vs surgical indications,athletes’needs and expectations,and financial implications should be achieved.Arthroscopic procedures should be timely planned when indicated as they could allow early diagnosis and treatment at the same time.Conservative measures are often used as first line treatment when possible.Peripheral lesions are treated by arthroscopic repair,whilst central lesions are treated by arthroscopic debridement.Further procedures(such as the Wafer procedure,ulnar osteotomies,etc.)have specific indications and great implications with regard to rehabilitation. 展开更多

The Jitang metamorphic complex is key to studying the tectonic evolution of the Northern Lancangjiang zone.Through structural-lithological mapping,structural analysis and laboratory testing,the composition of the Jitang metamorphic complex was determined.The macro-and microstructural analyses of the ductile detachment shear zone(Guoxuepu ductile shear zone,2–4 km wide)between the metamorphic complex and the overlying sedimentary cap show that the shear sense of the ductile shear zones is top-to-the-southeast.The presence of various deformation features and quartz C-axis electron backscatter diffraction(EBSD)fabric analysis suggests multiple deformation events occurring at different temperatures.The average stress is 25.68 MPa,with the strain rates(έ)ranging from 9.77×10^(−14)s^(−1)to 6.52×10^(−16)s^(−1).The finite strain of the Guoxuepu ductile shear zone indicates an elongated strain pattern.The average kinematic vorticity of the Guoxuepu ductile shear zone is 0.88,implying that the shear zone is dominated by simple shear.The muscovite selected from the protomylonite samples in the Guoxuepu ductile shear zone yields a 40Ar-39Ar age of 60.09±0.38 Ma.It is suggested that,coeval with the initial Indo–Eurasian collision,the development of strike-slip faults led to a weak and unstable crust,upwelling of lower crust magma,then induced the detachment of the Jitang metamorphic complex in the Eocene. 展开更多

Orogenic peridotite is an important component of orogenic belts and retains crucial information on mantle magmatic activity,slab subduction,and melt or fluid metasomatism.To determine the source of the mantle-derived parental magma of the peridotite and to investigate the metasomatism that it experienced,we undertook an integrated study of the petrography,whole-rock major-and trace-element compositions,in situ zircon U-Pb geochronology,and mineral majorand trace-element compositions of an early Paleozoic ultramafic complex in the North Wulan area of North Qaidam.The Halihatu ultramafic-mafic complex is composed of dunite,pyroxene peridotite,and gabbro,which are characteristic of Alaskan-type complexes.The dunite yields a weighted mean^(206)Pb/^(238)U age of 479±5 Ma(MSWD=0.7),which reflects the age of the metasomatism rather than the crystallization age of the ultramafic magma.The peridotites have high Mg^(#)(89.8-91.8)and Cr contents(2419-5190 ppm),low Al_(2)O_(3)(0.20-1.68 wt%)and Ni(289-1012 ppm)contents,and high olivine Fo contents(87-91),suggesting a large degree(~15%-22%)of partial melting of lithospheric ultramafic rocks followed by variable degrees of fractional crystallization of olivine and pyroxene.This is consistent with estimates of 15%-22.3%partial melting calculated using the Cr#of spinel crystals and with the low Yb(0.04-0.33 ppm)and Y(0.72-1.29 ppm)contents of clinopyroxene crystals.Whole-rock trace-element patterns show enrichment in large ion lithophile elements and depletion in high field strength elements,along with high Al_(2)O_(3)(2.10-6.47 wt%)and low TiO_(2)(0.01-0.21 wt%)contents of clinopyroxene crystals,suggesting an arc magma cumulate trend.These features,along with the high olivine Fo contents(87-91 ppm),imply that the Halihatu peridotite is an Alaskan-type crustal cumulates derived from Mgrich hydrous basaltic melts.The high estimated f O_(2)(FMQ+1.97 to FMQ+3.81)further supports the idea that they formed in an arc setting.The Ni/Co and Ni/Mn ratios and cumulate textures of the olivine,quenched boundaries between mafic and felsic melts,and the occurrence of tremolite and phlogopite reflect interactions between the Halihatu peridotite and injected silicate and carbonatitic melts in the lower crust.Therefore,we propose a new cumulate-infiltration model for the petrogenesis of Alaskan-type ultramafic complexes,which improves our understanding of the nature of Alaskan-type continental arc root. 展开更多

Jasmonic acid(JA)signaling pathway plays an important role in tanshinone and phenolic acid biosynthesis in Salvia miltiorrhiza.However,the specific regulatory mechanism remains largely unclear.Previous work showed that a JASMONATE ZIM-domain(JAZ)protein,SmJAZ9,acted as a repressor of tanshinone production in S.miltiorrhiza.In this study,we revealed that SmJAZ9 reduced both phenolic acid accumulation and related biosynthetic gene expression,confirming that SmJAZ9 also negatively affected phenolic acid biosynthesis.Then,we identified a novel MYB transcription factor,SmMYB76,which interacted with SmJAZ9.SmMYB76 repressed phenolic acid biosynthesis by directly downregulating SmPAL1,Sm4CL2,and SmRAS1.Further investigation demonstrated that JA mediated phenolic acids biosynthesis via SmJAZ9-SmMYB76 complex.Taken together,these findings state the molecular mechanism that SmJAZ9-SmMYB76 regulated phenolic acid biosynthesis at the transcriptional and protein levels,which provided new insights into JA signaling pathway regulating plant metabolism. 展开更多

Accretionary complex study provides important knowledge on the subduction and the geodynamic processes of the oceanic plate,which represents the ancient ocean basin extinction location.Nevertheless,there exist many disputes on the age,material source,and tectonic attribute of the Lancang Group,located in Southwest Yunnan,China.In this paper,the LA-ICP-MS detrital zircon U‒Pb chronology of nine metamorphic rocks in the Lancang Group was carried out.The U‒Pb ages of the three detrital zircons mainly range from 590-550 Ma,980-910 Ma,and 1150-1490 Ma,with the youngest detrital zircons having a peak age of about 560 Ma.The U‒Pb ages of the six detrital zircons mainly range from 440-460 Ma and 980-910 Ma,and the youngest detrital zircon has a peak age of about 445 Ma.In the Lancang Group,metamorphic acidic volcanic rocks,basic volcanic rocks,intermediate-acid intrusive rocks,and high-pressure metamorphic rocks are exposed in the form of tectonic lens in schist,rendering typical melange structural characteristics of“block+matrix”.Considering regional deformation and chronology,material composition characteristics,and the previous data,this study thinks the Lancang Group may be an early Paleozoic tectonic accretionary complex formed by the eastward subduction of the Changning-Menglian Proto-Tethys Ocean,which provides an important constraint for the Tethys evolution. 展开更多

Purpose: In recent years, multidrug-resistant Acinetobacter baumannii has appeared and caused outbreaks of hospital infections all over the world. Close monitoring of this pathogen and other A. baumanii complex species is considered of critical importance to public health organizations. The reliable identification method able to distinguish A. baumannii from genetically close Acinetobacter species is needed, because these species are unable to be differentiated by phenotypic or biochemical methods. The purpose of the present study was to design species-specific primers in order to identify and detect A. baumanii complex species, and Acinetobacter lwoffii which is frequently detected from the human specimens, and to investigate the distribution of these organisms in dental hospital using a multiplex PCR. Methods: Polymerase chain reaction (PCR) primers were designed based on partial sequences of the 16S ribosomal RNA (16S rRNA) gene and DNA gyrase subunit B (gyrB) of each species of A. baumanii complex species. Swab samples were collected from ten dental spittoon units in dental hospital, and the distribution of A. baumanii complex species was investigated using a multiplex PCR. Results: These primers were able to distinguish each species of A. baumanii complex species clearly. A. baumanii and A. calcoaceticus were detected at 20.0% and 10.0% in ten swab samples, respectively. On the other hand, A. nosocomialis, A. lowffii, and A. pittii were detected from no sample. Conclusion: Our developed one-step multiplex PCR method is accurate, specific, cost-effective, time-saving, and worked without requiring DNA extraction. 展开更多

Background: Wrinkles, pigmented spots, and roughness are representative parameters reflecting facial skin aging. Sulci cutis connecting to follicular orifices frequently form perifollicular cracks, which join together adjacent sulci. This follicle-sulcus-crack complex (FSCC) is exacerbated by dehydration. Purpose: Although dehydration is associated with facial skin aging, FSCC’s implications in facial skin aging remain unresolved. Method: We measured facial skin parameters in 1999 and 2010 in 108 Japanese women, and in 2006 and 2016 in 182 Chinese women. We addressed whether expansion of FSCC is associated with other skin aging parameters. We also examined whether skin moisturizer can reverse the expanded FSCC. Results: In both clinical studies, FSCC severity as well as wrinkles, spots, and roughness significantly deteriorated with age. Notably, FSCC significantly increased as early as in subjects in their 20s, whereas wrinkles, spots, and roughness became conspicuous in those in their 40s or older. Moreover, subjects with more severe FSCC in the initial measurement exhibited significantly worse wrinkles, spots, and roughness in the second measurement a decade later. Dehydration was significantly correlated with expanded FSCC. Furthermore, FSCC expansion was reversed after appropriate moisturization by daily application of Galactomyces ferment filtrate (GFF)-containing skincare product (SK-II Skin Power Cream) for 4 weeks in 37 women. Conclusion: The expanded FSCC is likely to be an early indicator of facial skin aging. Appropriate moisturization may reduce FSCC formation and decelerate facial skin aging. 展开更多

BACKGROUND Schizophrenia afflicts 1%of the world population.Clinical studies suggest that schizophrenia patients may have an imbalance of mitochondrial energy metabolism via inhibition of mitochondrial complex I activity.Moreover,recent studies have shown that ERVWE1 is also a risk factor for schizophrenia.Nevertheless,there is no available literature concerning the relationship between complex I deficits and ERVWE1 in schizophrenia.Identifying risk factors and blood-based biomarkers for schizophrenia may provide new guidelines for early interventions and prevention programs.AIM To address novel potential risk factors and the underlying mechanisms of mitochondrial complex I deficiency caused by ERVWE1 in schizophrenia.METHODS Quantitative polymerase chain reaction(qPCR)and enzyme-linked immunosorbent assay were used to detect differentially expressed risk factors in blood samples.Clinical statistical analyses were performed by median analyses and Mann-Whitney U analyses.Spearman’s rank correlation was applied to examine the correlation between different risk factors in blood samples.qPCR,western blot analysis,and luciferase assay were performed to confirm the relationship among ERVWE1,cytoplasmic polyadenylation element-binding protein 1(CPEB1),NADH dehydrogenase ubiquinone flavoprotein 2(NDUFV2),and NDUFV2 pseudogene(NDUFV2P1).The complex I enzyme activity microplate assay was carried out to evaluate the complex I activity induced by ERVWE1.RESULTS Herein,we reported decreasing levels of CPEB1 and NDUFV2 in schizophrenia patients.Further studies showed that ERVWE1 was negatively correlated with CPEB1 and NDUFV2 in schizophrenia.Moreover,NDUFV2P1 was increased and demonstrated a significant positive correlation with ERVWE1 and a negative correlation with NDUFV2 in schizophrenia.In vitro experiments disclosed that ERVWE1 suppressed NDUFV2 expression and promoter activity by increasing NDUFV2P1 level.The luciferase assay revealed that ERVWE1 could enhance the promoter activity of NDUFV2P1.Additionally,ERVWE1 downregulated the expression of CPEB1 by suppressing the promoter activity,and the 400 base pair sequence at the 3′terminus of the promoter was the minimum sequence required.Advanced studies showed that CPEB1 participated in regulating the NDUFV2P1/NDUFV2 axis mediated by ERVWE1.Finally,we found that ERVWE1 inhibited complex I activity in SH-SY5Y cells via the CPEB1/NDUFV2P1/NDUFV2 signaling pathway.CONCLUSION In conclusion,CPEB1 and NDUFV2 might be novel potential blood-based biomarkers and pathogenic factors in schizophrenia.Our findings also reveal a novel mechanism of ERVWE1 in the etiology of schizophrenia. 展开更多

Objective:Qualitative research on therapeutic components is necessary to evaluate the efficacy of complex interventions in healthcare.As few qualitative syntheses have been conducted,this study aimed to derive a new conceptual framework for understanding the components of complex interventions and provide evidence for the implementation and evaluation of complex healthcare interventions.Methods:A systematic search of seven databases was conducted to identify qualitative studies that explored components of complex healthcare interventions.Meta-ethnography was used to analyze the data and thematic analysis was used to build the conceptual framework.Results:Of the 35 included studies,most complex interventions were non-pharmacological,with cancer accounting for 22%,mental health for 14%,and stroke for 8%.Half of the studies were conducted in the United Kingdom.Three main categories emerged:what should healthcare workers do?what qualifications should they have?and what should patients do?Five main themes were identified:psychological,biological,cognitive and behavioral,environmental,and social support.Conclusion:This analysis provides a reference for designing components of complex interventions in further studies. 展开更多

BACKGROUND Lisfranc injuries have not received much attention by orthopedic doctors in the past,and there is little related research on the diagnosis and treatment of these injuries.In recent years with the rise in foot and ankle surgery,doctors are now paying more attention to this type of injury.However,there is still a high rate of missed diagnosis due to insufficient attention causing treatment delays or inadequate treatments,which eventually result in greater sequelae;including long-term pain,arthritis,foot deformity etc.In particular,for cases with a mild Lisfranc joint complex injury,the incidence of sequelae is higher.AIM To select an active surgical treatment for an atypical Lisfranc joint complex injury and to evaluate the clinical efficacy of the surgical treatment.METHODS The clinical data of 18 patients,including 10 males and 8 females aged 20-64 years with Lisfranc injuries treated in our department from January 2017 to September 2019 were retrospectively analyzed.All patients were treated with an open reduction and internal fixation method using locking titanium mini-plates and hollow screws or Kirschner wires.X-ray images were taken and follow-up was performed monthly after the operation;the internal fixation was then removed 4-5 mo after the operation;and the American Orthopedic Foot and Ankle Society(AOFAS)score was used for evaluation on the last follow-up.RESULTS All patients were followed up for 6-12 mo.A good/excellent AOFAS score was observed in 88.9%of patients.CONCLUSION For atypical Lisfranc joint complex injuries,active open reduction and internal fixation can be performed to enable patients to obtain a good prognosis and satisfactory functional recovery. 展开更多

Quantitative analysis of retinal intermediate and deep capillary plexus in patients with retinal deep vascular complex ischemia

AIM: To quantitatively analyze the retinal intermediate and deep capillary plexus(ICP and DCP) in patients with retinal deep vascular complex ischemia(RDVCI), using 3D projection artifacts removal(3D PAR) optical coherence tomography angiography(OCTA).METHODS: RDVCI patients and gender-and agematched healthy controls were assessed and underwent OCTA examinations. The parafoveal vessel density(PFVD) of retinal deep vascular complex(DVC), ICP, and DCP were analyzed, and the percentage of reduction(PR) of PFVD was calculated.RESULTS: Twenty-four eyes in 22 RDVCI patients(20 in acute phase and 4 in chronic phase) and 24 eyes of 22 healthy subjects were enrolled as the control group. Significant reduction of PFVD in DVC, ICP, and DCP was observed in comparison with the controls(DVC: acute: 43.59%±6.58% vs 49.92%±5.49%, PR=12.69%;chronic: 43.50%±3.33% vs 51.20%±3.80%, PR=15.04%. ICP: acute: 40.28%±7.91% vs 46.97%±7.14%, PR=14.23%;chronic: 41.48%±2.87% vs 46.43%±3.29%, PR=10.66%. DCP: acute: 45.44%±8.27% vs 51.51%±9.97%, PR=11.79%;chronic: 37.78%±3.48% vs 51.73%±5.17%, PR=26.97%;all P<0.05). No significant PR difference was found among DVC, ICP, and DCP of RDVCI in acute phase(P=0.812), but significant difference in chronic phase(P=0.006, DVC vs DCP, ICP vs DCP). No significant difference in PR between acute and chronic phases in the DVC(P=0.735) or ICP(P=0.681) was found, but significant difference in the DCP(P=0.041).CONCLUSION: The PFVD of DVC, ICP, and DCP in RDVCI is significantly decreased in both acute and chronic phases. ICP impairment is stabilized from acute to chronic phase in RDVCI, whereas subsequent DCP impairment is uncovered and can be explained by ischemia-reperfusion damage. 展开更多
