Objective: The purpose of this study was to clarify the collaborative activities and mutual recognition between community comprehensive care unit nurses (Ns) and care managers (CM) in supporting the discharge of the elderly from the hospital. Methods: A total of 300 nurses working in community comprehensive care wards and 360 care managers working in B City in A Prefecture were surveyed using an anonymous self-administered questionnaire. Results: The highest percentage of responses regarding necessary collaborative activities with multiple professions in supporting hospital discharge were the same for Ns and CMs. The items regarding practice with the highest percentages were “relationship as a team” for Ns, and “user-centered awareness” for CM. While these professionals were willing to share information about their patients’ lives after discharge, the percentage of those explaining their expertise was low. It is thought that collaborative activities focusing on these aspects would lead to more appropriate discharge support. 展开更多

The Effect of Personalized Comprehensive Care on the Nursing Care of Severe Pneumonia Patients

Objective:To explore the value of receiving personalized comprehensive care for patients with severe pneumonia.Methods:73 patients with severe pneumonia who visited the clinic from February 2020 to February 2023 were included in this study.The patients were randomly grouped into Group A and Group B.Group A received personalized comprehensive care whereas Group B received conventional care.The value of care was compared.Results:The duration of mechanical ventilation time,the time taken for fever and dyspnea relief,and the hospitalization time of Group A were shorter than those in Group B(P<0.05).The blood gas indexes such as PaO_(2),PaCO_(2),and blood pH of Group A were better than those of Group B(P<0.05).The pulmonary function indexes such as peak expiratory flow(PEF),forced vital capacity(FVC),and forced expiratory volume in 1 second(FEV_(1))of Group A were better than those of Group B,P<0.05.Moreover,the patients in Group A were generally more satisfied with the care given compared to the patients in Group B(P<0.05).Conclusion:Personalized comprehensive care improves blood gas indexes,enhances lung function,accelerates the relief of symptoms,and also enhances patient satisfaction in severe pneumonia patients. 展开更多

The Preventive Effect of Comprehensive Nursing Intervention on Arteriovenous Fistula Failure in Patients in the Hemodialysis Room

Objective:To explore the preventive effect of comprehensive nursing intervention on arteriovenous fistula failure in patients in the hemodialysis room.Methods:82 patients with arteriovenous fistula in the hemodialysis chamber treated in our hospital from July 2022 to October 2023 were selected as the research subjects.The random number table method divided them into a control group and an experimental group of 41 cases each.The control group received general nursing intervention measures,while the experimental group underwent comprehensive nursing intervention.The incidence of complications(internal fistula failure,bleeding,thrombosis,infection),psychological emotions(SAS scale,SDS scale),quality of life(physical pain,physiological function,emotional function,social function),and nursing satisfaction(very satisfied,basically satisfied,satisfied,dissatisfied)were compared between the two groups of patients.Results:The incidence of complications in the experimental group(4,9.76%)was significantly lower than that in the control group(29,70.73%);the SAS scores and SDS scores of the patients in the experimental group after intervention were both lower than those in the control group;the quality of life score(physical pain,physiological function,emotional function,and social function)was all higher than those of the control group;the post-intervention nursing satisfaction of the experimental group(40,97.57%)was also significantly higher than that of the control group(29,73.17%);and the listed differences were statistically significant(P<0.01).Conclusion:For patients with hemodialysis ventricular arteriovenous fistula failure,comprehensive nursing intervention can reduce the incidence of complications,relieve anxiety and stress,improve quality of life and nursing satisfaction,and achieve better preventive effects. 展开更多

Objective: To assess comprehensive care in the elderly population, as well as the quality of care in Primary Health Care. Methods: This is an exploratory descriptive study with a quantitative approach, conducted from July to December 2012, in the city of Santa Cruz, Rio Grande do Norte, Brazil. A sample of 130 subjects chosen by drawn was calculated, and data collection was performed at their homes. Results: There were interviewed 130 people, 92 (70.8%) women and 38 men (29.2%), with a minimum age of 60 and maximum of 96 years, with a mean of 72.8, median of 72.0 and a standard deviation of 8.3. Regarding the quality of care ratings of the PHC team, 48.5% (n = 63) of respondents stated this to be good, while 32.3% (n = 42) rated this as fair. Conclusions: In this perspective, one of the most appreciated meanings that were given to comprehensive care by health care professionals refers to holistic knowledge of each patient, resulting in the non-fragmentation of care. Thus, it is noticed that comprehensiveness has some weaknesses that need to be corrected, which shows the need for education and training of professionals assigned to primary health care services. 展开更多

Influence of nutritional care during pregnancy on weight control and maternal and infant outcomes in patients with pregnancy-induced hypertension

Background Preeclampsia is a common disease in pregnant women,characterized by clinical manifestations such as hypertension and proteinuria,which not only seriously affect the safety of pregnant women,but also jeopardize the safety of the fetus,threatening maternal and infant safety.Currently,in addition to routine hypoglycemic and antihypertensive treatments,auxiliary treatment methods such as exercise and diet are gradually gaining clinical recognition.Therefore,this study aimed to observe the effects of comprehensive nutrition care during pregnancy on weight control during pregnancy and maternal and infant outcomes in patients with pregnancy-induced hypertension syndrome.Methods A total of 100 cases of preeclampsia patients in our hospital from August 2021 to May 2022 were selected and randomly divided into a nutrition group and a conventional group,with 50 cases in each group.The conventional group received routine care,while the nutrition group received additional comprehensive nutrition care during pregnancy based on routine care.The weight control and blood pressure control during pregnancy and maternal and infant outcomes of the two groups were observed.Results The weight control during pregnancy in the nutrition group was significantly better than that in the conventional group(P<0.05).There was no significant difference in systolic and diastolic blood pressure between the two groups before intervention(P>0.05),but both systolic and diastolic blood pressure in both groups were lower after intervention,with the nutrition group significantly lower than the conventional group(P<0.05).The incidence of neonatal asphyxia,placental abruption,postpartum hemorrhage,and cesarean section in the nutrition group was significantly lower than that in the conventional group(P<0.05).Conclusions Comprehensive nutrition care during pregnancy can effectively control weight and blood pressure levels in patients with preeclampsia,and improve maternal and infant outcomes.[S Chin J Cardiol 2024;25(3):156-161] 展开更多

HIV/AIDS has brought to light the challenge of incorporating the many influences between living conditions, social characteristics and health services performance to an adequate care for PLWHA (people living with AIDS). Vulnerability of these populations is under the responsibility of specialized care units whose assistance does not always occur according to their real needs and demands. Therefore, this study aimed to analyze demographic, social and clinical profiles of PLWHA, as well as their follow-up in SS (Specialized Health Services) in Ribeir^o Preto, Brazil. It is a descriptive study conducted by the application of structured questionnaires to 253 patients with HIV/AIDS in follow-up during the years of 2012-2013. Variables were analyzed by descriptive statistics procedures. The findings pointed out gender parity, aging population, low education and economic predominance of class C. Regarding clinical characteristics, there was a predominance of asymptomatic individuals, with no clinical manifestations of AIDS or major comorbidities. The main mode of transmission was through sexual contact. The results led to the need of adequating the assistance provided to the specificities inherent to PLWHA. The care provision should cross an interdisciplinary perspective, targeting recognition of problems and ensuring comprehensive health care adequate to users' needs and demands. 展开更多
