髋臼周围加股骨近端减压截骨术治疗儿童中晚期Perthes病

目的 :介绍一种治疗Perthes病新术式—髋臼周围加股骨近端减压截骨。方法 :1997年 8月～ 2 0 0 0年 8月 ,2 1例中晚期儿童接受此术式治疗 ,共 2 1髋 ,随访时间 2 4～ 3 6个月 ,X线分期 ,Ⅲ期 17例 ,Ⅳ期 4例 ,均合并有半脱位和扁平髋畸形。结果 :术前髋臼指数平均 2 2 .45° ,骺核高度 5 .46mm ,CE角 6.3 8° ,术后髋臼指数 6.67° ,骺核高度 6.5 6mm ,CE角 19.95°。功能评价 :优 19例 ,良 2例。结论 :新术式优于原始术式 ,并同时解决了压力、肢短、头臼覆盖及预防远期骨性关节炎的目的。另外 ,强调合理和准确的适应证选择是治疗中晚期Perthes病和降低病残率的关键。
