This results in an increased emergence of resistance to most common bacteria, higher cost of treatment, prolonged hospitalization and adverse drug reactions. Interventions measures have been instituted to avert the problem but it still persists. A systematic review was conducted to determine the effect of different interventions (education, managerial, diagnostic tests, regulatory, economic and multifaceted) on misuse of AB drugs in developing countries. A total of 722 articles were retrieved and 55 were reviewed. About 10.9% of the studies were from Africa, 63.6% from Asia, 9.1% from Latin America, and 16.4% from Southeastern Europe. A total of 52.7% of the studies were from hospital settings, 5.5% from outpatient departments, 21.8% were from public health care facilities, 12.7% from private pharmacies/drug stores, and 7.3% from the communities. Education intervention had 27.3% studies, managerial had 20%, managerial/education had 3.6%, regulatory had 9.1%, education/regulation had 9.1% and diagnostic had 3.6% studies. Multifaceted intervention had 27.3% studies, with 63% improvement in appropriate AB doses prescribed, 2.6% mean number of AB encounter reduction, 23% AB prescription reduction, 18.3% generic AB prescription improvement, 32.1% reduction in AB use, 89% reduction in AB use in acute respiratory infection, 82% in surgery, 62.7% mean reduction in deliveries, 39% in STDs, 36.3% mean reduction in diarrhea, 14.6% mean reduction AB use in malaria, and 6%-11% in the cost of treating bacteria-resistant organisms. Also noted was 6.3% reductions in mean AB encounters after 1 month of intervention, and then increased to 7.7% after 3 months thus lacking sustainability. Multifaceted interventions were effective in reducing irrational AB drug use in the various health facilities and communities as well as reduction in the emergence of resistance to the commonest bacteria in the developing countries though there was lack of sustainability or continuity of rational drug use over the time.展开更多

In recent decades, increased urbanisation and changes to lifestyle, diet and physical activity in developing countries have led to a major increase in the population incidence of chronic diseases including CHD. Poor medication adherence is one of the leading causes of failure to achieve hypertension control. The objective of this systematic review is to describe the prevalence of non-adherence to anti-hypertensive medications among hypertensive population in developing countries and identify factors associated with it. Methods: A literature search was conducted using the following scholarly electronic databases: Proquest, PubMed, JSTOR and Science Direct. The online search engine, Google Scholar was also used to search for and identify relevant papers. Peer-reviewed full-text articles published in English on hypertensive adults in developing countries that measured adherence to antihypertensive medications and their associations with different factors were eligible for inclusion. The review followed the PRISMA reporting and analytical guidelines for systematic reviews. Results: In all, 42 studies conducted across 19 developing countries were selected for the review. The mean prevalence of medication non-adherence (MNA) among the select hypertensive population was 47.34%. Very few studies were conducted in community settings and except for one, no study examined gender differences in MNA factors. The analysis revealed a range of factors that can influence MNA including low household income and socioeconomic status;knowledge and beliefs of hypertension and its management;avoiding side effects of medications;cost of medication;use of herbal preparations;absence of symptoms;irregular follow-up;and dissatisfaction with the treatment and health services provided. There was a general lack of consideration of role of health system in health care delivery, self-efficacy, cultural barriers, perceived individual risk of hypertension complications. There was also a lack of gender-specific research which is necessary at community settings given the social and economic vulnerabilities faced by women in developing countries that may affect adherence to antihypertensive medications. Conclusions: Future research in developing countries should consider individual risk perceptions, cultural barriers, gender and the role of local health system in health care delivery when assessing MNA among hypertensive population at community settings.展开更多

The land use pattern will not ensure everywhere enough land for certain cropping if spatial allocation just according to land use suitability. To solve this problem, a subzone and a pre-allocation for each land use are added in spatial allocation module, and land use suitability and area optimi- zation module are incorporated to constitute a whole agricultural land use optimal allocation (ALUOA) system. The system is developed on the platform .Net 2005 using ArcGIS Engine (version 9.2) and C# language, and is tested and validated in Yili watershed of Xinjiang Region on the newly reclaimed area. In the case study, with the help of soil data obtained from 69 points sampled in the fieldwork in 2008, main river data supplied by the Department of Water Resources of Xinjiang Uygur Autonomous Region in China, and temperature data provided by Data Center for Resources and Environmental Sciences, Chinese Academy of Sciences, land use suitability on eight common crops are evaluated one by one using linear weighted summation method in the land use suitability model. The linear pro- gramming (LP) model in area optimization model succeeds to give out land area target of each crop under three scenarios. At last, the land use targets are allotted in space both with a six subzone file and without a subzone file. The resuits show that the land use maps with a subzone not only ensure every part has enough land for every crop, but also gives a more fragmental land use pattern, with about 87.99% and 135.92% more patches than the one without, while at the expense of loss between 15.30% and 19.53% in the overall suitability at the same time.展开更多

However, the process of developing this system of nature reserves was not quite smooth. Various kinds of information related to nature reserves, such as annual growth, the type of nature reserves established, motives behind building a nature reserve, legal instructions and the constitution of government sectors, were collected and analyzed in order to highlight the characteristics of the development of our nature reserve system. As a result, we identify three phases and their characteristics, i.e., the initial phase from 1956 to 1978, the establishment phase from 1979 to 1996 and the management standardization phase from 1997 to 2009, could be clearly identified. It is suggested that since the establishment of this comprehensive structural framework in China, the system should be entering a new developing phase. We recommend that more attentions should be paid on how to guarantee effective management and how nature reserves are able to maintain their value and fulfill their objectives in the future.展开更多

Yet bringing intelligent healthcare informatics to bear on the dual problems of reducing healthcare costs and improving quality and outcomes is a challenge even in countries with a reasonably developed technology infrastructure. This paper focused at how appropriate an ap-plication of Medical Diagnosis Expert System (MDES) is to manage diseases in developing countries. MDES is usually designed to enable clinicians to identify diseases and describe methods of treatment to be carried out taking into account the user capability. The MDES described here is implemented using the C Language Integrated Production System (CLIPS). The CLIPS is an expert system, which has a shell composed of four modules: the user interface, the explanation system, the inference engine and the knowledge base editor. In the system, a number of patient cases will be selected as prototypes and stored in a separate database. The knowledge is acquired from literature review, human experts and the internet of the specific domain and is used as a base for analysis, diagnosis and recommendations.展开更多

Vaccinating girls between 9 and 14 years old is supposed to be the most cost-effective public health approach against cervical cancer. This systematic review aims to assess the application and coverage of the HPV vaccine in developing countries and identify the main challenges for the introduction of the vaccine in these settings. Eligible studies were selected according to the defined inclusion and exclusion criteria. To determine the quality of the studies was employed the STROBE checklist. This review included seven studies, encompassing the analysis of 19 countries and 112,116 girls aged from 9 to 18 years old. The coverage of HPV vaccination ranged from 13.8% to 107.4%, with most of the programs having more than 60% of coverage, which reflects a high percentage of vaccinated girls. The main challenges were lack of knowledge and worries about the vaccine, insufficient financial resources and staff workers, lack of community involvement and dissemination of important information about HPV. In developing countries that implemented the HPV vaccine, high coverage rates were achieved, despite sociocultural, economic and political challenges. In the future, studies that analyze the coverage rates after the elimination of the barriers and the repercussions on the mortality rates should be conducted, so that more developing countries have the opportunity to efficiently implement the vaccine.展开更多

Conventional geothermal syste...This paper provides an overview of conventional geothermal systems and unconventional geothermal developments as a common reference is needed for discussions between energy professionals. Conventional geothermal systems have the heat, permeability and fluid, requiring only drilling down to °C, normal heat flow or decaying radiogenic granite as heat sources, and used in district heating. Medium-temperature (MT) 100°C - 190°C, and high-temperature (HT) 190°C - 374°C resources are mostly at plate boundaries, with volcanic intrusive heat source, used mostly for electricity generation. Single well capacities are °C - 500°C) and a range of depths (1 m to 20 Km), but lack permeability or fluid, thus requiring stimulations for heat extraction by conduction. HVAC is 1 - 2 m deep and shallow geothermal down to 500 m in wells, both capturing °C, with °C are either advanced by geothermal developers at <7 Km depth (Enhanced Geothermal Systems (EGS), drilling below brittle-ductile transition zones and under geothermal fields), or by the Oil & Gas industry (Advanced Geothermal Systems, heat recovery from hydrocarbon wells or reservoirs, Superhot Rock Geothermal, and millimeter-wave drilling down to 20 Km). Their primary aim is electricity generation, relying on closed-loops, but EGS uses fractures for heat exchange with earthquake risks during fracking. Unconventional approaches could be everywhere, with shallow geothermal already functional. The deeper and hotter unconventional alternatives are still experimental, overcoming costs and technological challenges to become fully commercial. Meanwhile, the conventional geothermal resources remain the most proven opportunities for investments and development.展开更多

For this, the relevant literature on the EIA system and process was reviewed and evaluated through the application of quantitative and qualitative approaches, including the assessment of legal instruments and the adoption of EIA methodologies in developing countries. EIA, initiated in the 1970s in the US and Australia, expanded to developing countries and was amended in East and Southeast Asia from the 1970s to the 2000s. The evaluation assessed that the South Asian countries follow UNEP and IAIA guidelines, utilizing national laws and expert consultations, with screening criteria and administrative processes based on established environmental legislation. Ad hoc, checklist, matrix, network, overlay, cost-benefit analysis, and predictive or simulation in EIA practice are used to assess the environmental impacts of development activities. Failure to recommend major projects undermines public trust and prevents mitigation measures from being implemented. Most developing countries have followed EIA to fulfill the legal requirement with shadow-off monitoring and follow-up rather than to upset, reduce, or compensate for the project impacts as per size, location, and severity of the project area. The research and guidelines outlined in the IAIA principles and process have synthesized the best EIA practices worldwide. Public participation, impact coverage, scientific mitigation, transparent evidence-based approaches, monitoring, follow-up, legitimate approaches, and future appraisal opportunities are major concerns to be included in best EIA practice.展开更多

Multifaceted interventions were effective in reducing irrational AB drug use in the various health facilities and communities as well as reduction in the emergence of resistance to the commonest bacteria in the developing countries though there was lack of sustainability or continuity of rational drug use over the time.
文摘Background: Coronary heart disease (CHD), a complication of hypertension, is one of the most important and common causes of morbidity, hospitalisation, and mortality among hypertensive population. Conclusions: Future research in developing countries should consider individual risk perceptions, cultural barriers, gender and the role of local health system in health care delivery when assessing MNA among hypertensive population at community settings.

文摘This paper examines the nutrition impacts of using non-solid cooking fuel on under-five children in developing countries. We draw on data from more than 1.12 million children in 62 developing countries from the Demographic and Health Surveys(DHS).Results from both fixed effects(FE)and instrumental variable(IV)estimates show that using non-solid cooking fuel significantly improves the nutrition outcomes of under-five children.Compared with their peers from households mainly using solid fuel, children from households mainly using non-solid fuel exhibit a lower probability of experiencing stunting(by 5.9 percentage points)and being underweight(by 1.2 percentage points). Our further investigation provides evidence for several underlying mechanisms, such as improved indoor air quality, induced reduction in children’s respiratory symptoms, benefits on maternal health, and reduction in maternal time spent on fuel collection or cooking.Heterogenous analyses suggest that the nutrition benefits of using non-solid cooking fuel are more prominent among boys, children above three years old,and those from households of lower socioeconomic status, rural areas, and Southeast Asia.

文摘In developing countries, land productivity involves little market, where the agricultural land use is mainly determined by the food demands as well as the land suitability. The resuits show that the land use maps with a subzone not only ensure every part has enough land for every crop, but also gives a more fragmental land use pattern, with about 87.99% and 135.92% more patches than the one without, while at the expense of loss between 15.30% and 19.53% in the overall suitability at the same time.

文摘Sox9–gliogenesis and beyond:Neurons and glial cells are the major neuroectodermal cell types of the vertebrate central nervous system(CNS).Their generation from common progenitor cells takes place mostly during embryonic and early postnatal development. After closure of the neural tube, neural epithelial progenitor cells(NEPs)establish the ventricular zone(VZ).By asymmetrical cell division,NEPs first give rise to neuronal precursor cells(NPs)that then differentiate into various types of neurons.Later,NEPs predominantly produce glial precursor cells that become either astroglia or oligodendroglia.

文摘Up until 2009, 2541 nature reserves had been established in China. We recommend that more attentions should be paid on how to guarantee effective management and how nature reserves are able to maintain their value and fulfill their objectives in the future.
文摘There is a strong need for cost-effective technologies to manage disease processes and thus reduce morbidity and mortality in the developing countries. The knowledge is acquired from literature review, human experts and the internet of the specific domain and is used as a base for analysis, diagnosis and recommendations.
文摘This paper demonstrates the general character of some technology designing processes through their analysisand introduces a developing tool for the expert system in technology design. With this tool a particular domain ex-pert can directly establish a knowledge base so as to form a practical expert system, and thus enhanca correctness ofknowledge experssion and the developing efficiency of the expert system. It also discusses in detail the mech-anism of reasoning, interpreting and knowledge obtaining. The knowledge base consists of three parts:classifyingrules, essential data, and regulating rules. It can be formed by means of the expert dialouge and edition. In its ap-plication,the knowledge base can constantly accumulate successful experience to achieve itsself-study function. The paper shows the way to describe the knowledge in a particular domain and the process of applying this tool ina particular domain. The tool is written in Turb-Prolog language,And an expert system for cocoon cooking isprovided.
文摘The problem of power system planning, due to its complexity and dimensionality, is one of the most challenging problems facing the electric power industry in developing as well as developed countries. In planning phase, two of the most important decision-making parameters are the reliability and costs. The latter includes both system investment costs and outages costs. In this paper, these parameters are described and the interrelation between them is evaluated. Some previous approaches and developed techniques will he applied to a particular planning problem in a developing country and some aspects having a significant impact on the decision making process in the planning phase will be considered.
文摘Cervical cancer is the second most common cancer in women living in developing countries that account high HPV incidence and mortality rates. In the future, studies that analyze the coverage rates after the elimination of the barriers and the repercussions on the mortality rates should be conducted, so that more developing countries have the opportunity to efficiently implement the vaccine.
文摘This paper provides an overview of conventional geothermal systems and unconventional geothermal developments as a common reference is needed for discussions between energy professionals. Meanwhile, the conventional geothermal resources remain the most proven opportunities for investments and development.
文摘Environmental impact assessment (EIA), a decision-making process for project appraisal and sustainability adopted globally as an administrative process to identify, predict, evaluate, and monitor projects from their feasibility, preconstruction, construction, and operation stages to mitigate the adverse impacts and enhance the beneficial impacts for the protection of the affected environment, The study objective is to explore global EIA systems and processes and find shortcomings and implications for making the best instrument or tool to protect the natural environment from man-made activities over the project cycle. Public participation, impact coverage, scientific mitigation, transparent evidence-based approaches, monitoring, follow-up, legitimate approaches, and future appraisal opportunities are major concerns to be included in best EIA practice.
文摘Taiwan is located in the subtropical island monsoon climate area which also in the Eurasian plate and the Philippine intersectionplace. The natural disasters occur quite frequently in Taiwan. In recent years, global warming and climate abnormal phenomenahave become seriously. Extreme weather happens frequently. The common disasters will also directly or indirectly affect other disasters. If these disasters combine together the damage willbecame serious than a single disaster. The natural disasters may also be accompanied by a power outage, etc. so that the localdisaster can't notify the information immediately. Then lead to the disaster spread, causing significant loss of social and public andthe economy, in view of this paper, in order to avoid the limitations of a variety of irresistible when emergency issue happened thatcan informed the disaster information immediately. Therefore, the development version of iOS "Environmental DisasterManagement Information System Mobile Communications Software" (Emergency Response System Application hereinafter ERSAPP), allowing users can be quick and easy notification of the disaster losses, upload photo and request support in the affectedareas. Also due to the disaster area often located tough and no network environment as well as cell phone battery performance willbe discussed among this article.
