Three evaluation scales are available, including the Fatigue Impact Scale (FIS). Yet none of these has been proven to be of use in the evaluation of psychological quality after post-stroke fatigue. OBJECTIVE: To introduce FIS into China, and to evaluate the validity and reliability of the Chinese Version in patients with cerebral infarction. DESIGN: Scale evaluation. SETTING: Department of Rehabilitation, Beijing Friendship Hospital, Capital Medical University. PARTICIPANTS: A total of 330 patients with cerebral infarction were selected from the Department of Neurology, Beijing Friendship Hospital, Capital Medical University from June 2005 to December 2006. All patients met diagnostic standards from the Fourth National Cerebrovascular Disease Academic Meeting of China Medical Association and were diagnosed with CT or MRI examination. Patients who were not willing to accept scale evaluation were excluded. METHODS: The English version of FIS was translated into Chinese and subsequently translated back to English again. FIS was given to the discharged patients, and they returned them upon completion. In-patients completed and immediately returned the FIS. Validity and reliability of the retrieved scales were ascertained with descriptive and inference analysis. MAIN OUTCOME MEASURES: Total scores and subscale scores of FIS. RESULTS: Out of 330 questionnaires, 214 valid questionnaires were deemed valid in total. Six components were extracted in factor analysis, and the total cumulative contribution was 73.919%, which suggested the questionnaire was valid. After correlation analysis, 6 components were divided into 3 subscales, including cognitive, physical, and social, which was the same as the English version. The Cronbach α value for the three subscales was 0.937 7, 0.918 8, and 0.940 6, respectively (〉 0.7). CONCLUSION: The reliability and validity of the Chinese version of FIS met with satisfaction and appeared to be adaptable to cerebral infarction patients in China.展开更多

On the basis of short-term field measurement results of structural response, research is carried out on the fatigue analysis of hinge joints at the upper part of the Soft Yoke single point Mooring System (SYMS) by simultaneously monitoring the environmental load and considering the design criteria of offshore structure. Through analysis of finite element modeling, the time-histories of typical stress response are obtained, and then the assessment of fatigue damage at key hinge joints is conducted. The simulation results indicate that the proposed method can accurately analyze the fatigue damage of offshore engineering structure caused by the effect of wave load. The present analytical method of fatigue characteristics can be extended on other offshore engineering structures subjected to meta-ocean load.展开更多

By analyzing the characteristic parameter which is easy to be detected from the supporting shaft’s exterior, the time series model parameter which is hypersensitive to the situation of fatigue crack in ulterior place of the supporting shaft is the target input of neural network, and the fatigue crack’s degree value of supporting shaft is the output. The BP network model can be built and net-work can be trained after the structural parameters of network are selected. Furthermore, choosing the other two different group data can test the network. The test result will verify the validity of the BP network model. The result of experiment shows that the method of time series and neural network are effective to diagnose the occurrence and the development of the fatigue crack’s degree in ulterior place of the supporting shaft.展开更多

OBJECTIVE: To dynamically monitor the changes in mean cerebral blood flow velocity in different brain regions of high performance fighter pilots during mental arithmetic tasks and consecutive performance tasks. DESIGN, TIME AND SETTING: The present neurophysiological trial, based on controlled observation, was performed at the Laboratory of Neurophysiology, Institute of Aviation Medicine, Air Force of China between January 2003 and December 2005. PARTICIPANTS: Forty-five males, high performance fighter pilots, averaging (27.6±2.5) years, were recruited for this study. METHODS: The mean cerebral blood flow velocity in the anterior cerebral artery, middle cerebral artery, and posterior cerebral artery of subjects was dynamically tested using transcranial Doppler during 5- hour mental arithmetic tasks and during 5- hour consecutive performance tasks. The neurobehavioral ability index was analyzed throughout each trial according to the number of correct responses, false responses, and lost responses. Simultaneously, cerebral cognitive fatigue-induced lethargy was assessed by the Stanford Sleepiness Scale. MAIN OUTCOME MEASURES: Changes in mean cerebral blood flow velocity in the anterior cerebral artery, middle cerebral artery, and posterior cerebral artery; neurobehavioral ability index of mental arithmetic and consecutive performance tasks; Stanford Sleepiness Scale scores. RESULTS: During mental arithmetic tasks, the mean cerebral blood flow velocity in the anterior cerebral artery increased during hour 2 and decreased after hour 4. There was no significant change in mean cerebral blood flow velocity in the middle cerebral artery and posterior cerebral artery. During hour 4, cerebral cognitive fatigue was observed and, simultaneously, Stanford Sleepiness Scale scores demonstrated the presence of fatigue. During such a stage, the neurobehavioral ability index decreased, indicating a decline in performance ability. During consecutive performance tasks, the mean cerebral blood flow velocity declined earlier in the posterior cerebral artery compared to the middle cerebral artery. CONCLUSION: Five- hour mental arithmetic tasks have few effects on cerebral functions and do not lead to a significant change in mean cerebral blood flow velocity. Five-hour consecutive performance tasks can induce cerebral cognitive fatigue, and a marked decline in mean cerebral blood flow velocity.展开更多

The aim of the present study was to construct the multidimensional Post-Myocardial Infarction Fatigue (PMIF) scale, based on empirical data gathered in coronary heart disease contexts. Methods: The construction of the post-MI fatigue scale was conducted in seven steps, ending in a psychometric evaluation of the internal structure of the scale, using confirmatory factor analysis, as well as testing of convergent and discriminant validity, using correlational analysis, in a sample of 141 persons treated for MI. Results: The results showed that the PMIF scale represents three dimensions: physical, cognitive and emotional fatigue. It was also found that the scale is suitable for summing the items to obtain a total score representing a post-MI global fatigue dimension. Conclusion: The PMIF is a brief and easily completed scale for identifying persons experiencing post-MI fatigue. Early identification of fatigue, together with health behavior support, might prevent progression toward a more severe state of fatigue.展开更多

METHODS: Sixteen women with primary biliary cirrhosis without comorbidity and 13 healthy women matched for age and body mass index (BMI) completed the selfreported questionnaire fatigue impact scale. All subjects underwent surface electromyography assessment of peripheral fatigability. Anterior tibial muscle isometric voluntary contraction was executed for 20 s at 80% of maximal voluntary isometric contraction. During the exercise electromyographic signal series were recorded and root mean square (expression of central drive) as well as mean and median of electromyographic signal frequency spectrum (estimates of muscle fatigability) were com- puted. Each subject executed the trial two times. EMG parameters were normalized, then linear regression was applied and slopes were calculated. RESULTS: Seven patients were fatigued (median fatigue impact scale score: 38, range: 26-66) and 9 were not fatigued (median fatigue impact scale score: 7, range: 0-17). The maximal voluntary isometric contraction was similar in patients (82, 54-115 N) and controls (87, 74-101 N), and in patients with high (81, 54-115 N) and low fatigue impact scale scores (86, 65-106 N). Root mean square as well as mean and median of frequency spectrum slopes were compared with the Mann-Whitney U test, and no significant difference was found between fatigued and non-fatigued patients and controls. CONCLUSION: No instrumental evidence of peripheral fatigability can be found in women with primary biliary cirrhosis but no comorbidity, suggesting that fatigue in such patients may be of central origin.展开更多

We conducted this study to estimate the prevalence of fatigue among taxi drivers and to assess the determinant of fatigue among taxi drivers. In this cross-sectional study, 300 urban and rural taxi drivers from Fez, Morocco were enrolled from January to May 2013. The mean score of Pichot scale was 13.59 ± 8.23. The mean duration of sleep was 6.78 ±1.47 h per day. The mean rate of accident was 2.78 ± 1.12 per year. At the final model of the multiple linear regression we found an association statistically significant between Pichot scale and income, cardiovascular disorders, tobacco use, problems with colleagues, regularity of sleep and sleeping at the wheel. This study opened the door to many discussions about the professional drivers' conditions of work and we can argue that we should ameliorate the professional drivers conditions of work in Morocco.展开更多

The primary goal of the current study was to assess the relation between sleep disorders and accidents by using Epworth sleepiness scale. In this cross-sectional study, 300 urban and rural taxi drivers from Fez, Morocco were enrolled from January to May 2013. All participants were male and their mean age was 42.51 ± 10.81 (range 18-74) years. Their experience in driving was 8.34± 3.56 years on average; which had no significant relationship with their crash history and was not found a confounding factor in this series. The mean duration of sleep was 6.78± 1.47 h per day. The mean rate of accident was 2.78 ± 1.12 per year. The mean Epworth index was 13.23 ± 5.59. The simple linear regression showed a significant correlation between the Epworth sleepiness scale and education (P = 0.030), age (P = 0.017), marital status (P = 0.011), coffee consumption (P = 0.006), and water pipe (chicha) use (P = 0.01 ), but there was no significant correlation with history of crash in past years. The multivariate analysis (multiple linear regressions) found a significant association between Epworth scale and age (P = 0.024), coffee consumption (P = 0.023) and water pipe use (P = 0.027). In Morocco where road accidents are very frequent and number of serious injuries and death is very high. This is considered as one of the major public health problems and hence public health officials, legislators and the police should collaborate in multidisciplinary approach to have effective preventives measures. This study can be the start of many debates about the most efficient preventive measure to ameliorate professional drivers' work condition and to reduce road accident and hence save thousands of lives from the "traffic war" in Morocco.展开更多
