A Comparative Study of High-viscosity Cement Percutaneous Vertebroplasty vs. Low-viscosity Cement Percutaneous Kyphoplasty for Treatment of Osteoporotic Vertebral Compression Fractures

The clinical effects of two different methods–high-viscosity cement percutaneous vertebroplasty（PVP） and low-viscosity cement percutaneous kyphoplasty（PKP） in the treatment of osteoporotic vertebral compression fractures（OVCFs） were investigated. From June 2010 to August 2013, 98 cases of OVCFs were included in our study. Forty-six patients underwent high-viscosity PVP and 52 patients underwent low-viscosity PKP. The occurrence of cement leakage was observed. Pain relief and functional activity were evaluated using the Visual Analog Scale（VAS） and Oswestry Disability Index（ODI）, respectively. Restoration of the vertebral body height and angle of kyphosis were assessed by comparing preoperative and postoperative measurements of the anterior heights, middle heights and the kyphotic angle of the fractured vertebra. Nine out of the 54 vertebra bodies and 11 out of the 60 vertebra bodies were observed to have cement leakage in the high-viscosity PVP and low-viscosity PKP groups, respectively. The rate of cement leakage, correction of anterior vertebral height and kyphotic angles showed no significant differences between the two groups（P〉0.05）. Low-viscosity PKP had significant advantage in terms of the restoration of middle vertebral height as compared with the high-viscosity PVP（P〈0.05）. Both groups showed significant improvements in pain relief and functional capacity status after surgery（P〈0.05）. It was concluded that high-viscosity PVP and low-viscosity PKP have similar clinical effects in terms of the rate of cement leakage, restoration of the anterior vertebral body height, changes of kyphotic angles, functional activity, and pain relief. Low-viscosity PKP is better than high-viscosity PVP in restoring the height of the middle vertebra.

We use pins and an outer fixation frame, which keeps the injured limb in abduction and under traction and at the same time allows for functional exercise of all the joints in the extremity. Effect of Post Weld Heat Treatment on Microstructure and Fracture Toughness of Friction Welded Joint

The effect of post weld heat treatment on the microstructure and fracture toughness of friction welded joints of Ti-6.5Al-1Mo-1V-2Zr alloy was studied. The experimental results show that equiaxial grains were formed at the center of the weld metal while highly deformed grains were observed in the thermomechanically affected zone. The fracture toughness of the weld metal was lower than that of the thermomechanically affected zone under as-weld and post weld heat treatment conditions. With increasing temperature of post weld heat treatment, the fracture toughness of weld center and thermomechanically affected zone increased. The fractographic observation revealed that the friction welded joints fractured in a ductile mode.

To explore the treatment methods for scapular fractures associated with clavicle fractures.Methods Of 23 cases with scapular fractures associated with clavicle fractures,13 treated conservatively took the function exercise of shoulder joint after two months and 10 treated with open reduction and plate fixation reconstruction began the function exercise of shoulder joint four weeks after operation.The follow up averaged 12 months.Results All patients were healed up.18 cases were followed up,of them 8 cases were treated surgically,10 cases,conservatively.Of eight cases treated surgically,seven got excellent functional recovery of the shoulder joint and one satisfactory.In the meantime,of 10 cases treated conservatively,the recovery of shoulder joint was excellent in six,good in three and poor in one.Conclusion As for scapular fractures with associated clavicle fractures,open reduction and plate fixation reconstruction is a good treatment method for it can keep the stability and the balance of shoulder joint and is helpful for early functional exercise and recovery of the shoulder joint.7 refs,5 figs.

Clinical study of polydloxanone suture tension band fixation for treatment of patellar fractures

To explore biomechanic characteristics and clinical outcome of absorbable polydloxanone (PDS) treating patellar fractures.Methods The tensile strength test of PDS 0-0 or PDS 1-0 sutures were made in WDW305 electro-universal tester.A total of 60 cases were classified randomly and equally into two groups:treatment group (treated with the absorbable PDS tension hand) and Kirschner wire metals tension band control group (control group).Anatomy reduction was performed with towel clip fixation during operation.Treatment group was managed with diameter 1.5 mm Kirschner wires drilling two pores on the near or distal segment of patellar fracture end are penetrating through two strands of absorbable PDS 0-0 or PDS 1-0 sutures for a satisfactory fracture fixation.Control group was treated with Kirschner wires (2.0 mm in diameter) and steel wire (1.0 mm in diameter) of type “8” tension band fixation.Results Tensile strength testing showed that the biggest tensile of a strand PDS 0-0 sutures was 71.50 N and that of PDS 1-0 97.48 N,when the length was three times more than the initial.Two groups were followed up and the mean fracture healing time was two months,without broken wire or dislocation.Treatment group showed excellent clinical results in 21 cases,good in six and fair in three;while control group showed excellent clinical results in 19 cases,good in seven and fair in four,with no statistical difference compared with treatment group (χ2=0.32,P>0.05).Conclusion Absorbable PDS tension band has better characteristics of mechanics and creepage in treating patellar fractures.It can avoid defects of secondary operation and complications like needle-tail pain and sharped skin resulted from metal tension band fixation.Meanwhile,it is economic and worthy of further clinical application.16 refs,4 figs.

Usage of buttress plate internal fixation associated with autografting of fibula and iliac bone for the treatment of distal femoral C_3 type fracture

To assess the effect of using buttress plate associated with antografting of fibula and iliac bone for the treatment of distal femoral C3 type fracture.Methods Seventeen cases of distal femoral C3 type fracture using buttress plate associated with antografting of fibula and iliac bone were analyzed retrospectively.Results All cases were followed up for an average of 24 months（8～55 months）.The average time of octets bridge forming were 4 months（3～5 months） while the average time for bone union were 8 months （6～14 months）.According to Shelbourne rating system,result of all 18 cases were excellent and no malunion,infection were found.Conclusion Buttress plate associated with antografting of fibula and iliac bone is an effective alternative for the treatment of distal femoral C3 type fracture.It can provide more stable fixation to the bone and earlier functional exercises can be achieved.5 refs,3 figs,1 tab.

Ulnar nerve injury associated with displaced distal radius fracture:Two case reports

BACKGROUND Ulnar nerve injury subsequent to a fracture of the distal radius is extremely rare compared to median nerve injury.Treatment of ulnar nerve injury after closed distal radial fracture is controversial.Reasonable surgical planning and careful postoperative management can improve the prognosis of patients.CASE SUMMARY We report two cases of ulnar nerve injury subsequent to fracture of the distal radius.Both patients were admitted to hospital.Both patients had persistent ulnar nerve compression syndromes.The first patient achieved rapid recovery by early nerve decompression surgery,while the second patient had no recovery at 2-3 mo after injury and had more severe symptoms.At 10 wk after injury,the second patient agreed to nerve decompression surgery.The second patient finally achieved a successful outcome after nerve decompression and neurolysis,although she still has residual symptoms.CONCLUSION For patients with ulnar nerve compression syndrome related to acute wrist fracture,if symptoms persist and signs of recovery are not observed,early release is necessary to prevent permanent neurological damage.

BACKGROUND Hand in hand with technological advancements,treatment modalities continue to grow.With the turn of the century,the internet has become the number one source of information for almost every topic.Thus,many patients look toward the internet as their primary source of information to learn about their respective medical conditions.The American Medical Association and National Institute of Health strongly recommend that online medical information be written at the 6th to 8th-grade level to aid comprehension by patients of all literacy backgrounds.AIM To assess the readability of online information regarding Jones fracture.Our hypothesis is that the reading level of medical information published on websites far exceeds the recommended reading level of 6th-8th grade as proposed by the American Medical Associate and National Institute of Health.The result of this study can help us formulate improved recommendations for publishing more comprehensible material and,thus,eventually improve patient compliance and clinical outcomes.METHODS The exact phrase“Jones fracture”was queried on the three most common search engines,Google,Yahoo!,and Bing,on December 28,2022.As of December 2022,Google held 84%,Bing held 9%,and Yahoo!held 2%of the worldwide search engine market share.Web pages uniform resource locator from the first three pages of search results were recorded from each search engine.These web pages were classified according to academic,physician-sponsored,governmental and non-government organizations(NGO),commercial,and unspecified as per formally defined categories.Websites associated with an educational institution or medical organization were classified as academic.Websites with products for sale,corporate sponsorship,or advertisements were classified as commercial.Governmental websites or NGOs comprised those that received government subsidies or grants.Webpages that were independently owned by physicians or physician groups were respectively classed as physician sponsored.The remainder of websites that did not fall under the above categories were classified as unspecified.RESULTS A total of 93 websites were analyzed for reading assessment.A whopping 44%of websites were commercial,followed by 22%of physician-sponsored websites.Third place belonged to non-government organization websites holding a 15%share.The academic website held a meager 9%portion,while unspecified sites were 3%.The table illustrates mean readability scores,along with average cumulative grade level.The average grade level was 10.95±2.28 for all websites,with a range of 6.18 to 18.90.Since P values were more than 0.05,there was not a significant statistical difference between the first page results and the results of all pages.Thus,we can rationalize that readability scores are consistent throughout all pages of a website.CONCLUSION Hand in hand with technological advancements,treatment modalities continue to grow.With the turn of the century,the internet has become the number one source of information for almost every topic.Thus,many patients look towards the internet as the primary source of information to learn about their respective medical conditions.Our study demonstrates that current online medical information regarding Jones fracture is written at an extraordinarily high-grade level,with an average grade level of all websites at 10.95,nearly an 10th-grade educational level.The American Medical Association and National Institute of Health strongly recommend that online medical information should be written at the 6th to 8th-grade level to aid comprehension by patients of all literacy backgrounds.On the contrary,most of the medical information evaluated was at an 10th-grade level,which far exceeds recommendations by AMA and NIH.This is particularly relevant because readability scores are directly proportional to the level of comprehension attained by readers,thus directly impacting patient outcomes.In conclusion,we suggest and encourage that all online reading materials should be re-written at the 6th to 8th-grade level in a public service effort to increase compliance with treatment goals and raise awareness of preventive measures.

Effect of Aging on Fracture Toughness and Stress Corrosion Cracking Resistance of Forged 7475 Aluminum Alloy

The effects of two-stage aging and retrogression and reaging heat treatment on the fracture toughness and stress corrosion cracking resistance of 7475 alloy were studied. The fracture toughness, conductivity and strength of samples of nine groups under duplex aging conditions and three retrogression and reaging heat treatments were also measured. Incorporating the microstructure and property, we found that when the condition of the first order aging kept identical, the fracture toughness and stress corrosion cracking resistance increase with aging time and the second aging temperature. The optimal treatment conditions are （ 115℃×7h ＋ 185 ℃×13h） among all tested two-stage aging treatments. Although the 7475 alloy treated by RRA method shows the highest strength and its stress corrosion cracking resistance after twenty minutes retrogression can also reach the same level as those by the optimal treatment of （115℃×7h＋ 185℃×13h ）, the fracture toughness is even low.

Tensile properties and fracture behavior of friction stir welded joints of Fe-32Mn-7Cr-1Mo-0.3N steel at cryogenic temperature

This study investigates the cryogenic tensile properties and fracture behavior of fiction stir welded and post-weld heat-treated joints of 32Mn-7Cr-1Mo-0.3N steel. Cryogenic brittle fracture, which occurred in the as-welded joint, is related to the residual particles that contain tungsten in the joint band structure. Post-weld water toughening resulted in the cryogenic intergranular brittleness of the joint, which is related to the non-equilibrium segregation of solute atoms during the post-weld water toughening. Annealing at 55OC for 30rain can effectively inhibit the cryogenic intergranular brittleness of the post- weld water-toughened joint. The yield strength, ultimate tensile strength, and uniform elongation of the annealed joint are approximately 95%, 87%, and 94% of the corresponding data of the base metal.
