Management of Immediate Postpartum Hemorrhage at the University Hospital Center of the Sino-Central African Friendship

Introduction: Immediate postpartum hemorrhages constitute a frequent maternal complication and remain at the forefront of maternal death in our countries. Objective: Contribute to improving the management of immediate postpartum hemorrhage in our context with a view to reducing maternal mortality. Patients and Methods: We conducted a retrospective descriptive and analytical study over a period of 12 months from January 1st to December 31st 2020. Results: We collected a total of 109 files on a total of 4360 deliveries. The frequency of postpartum haemorrhages was of the order of 2.5%. The most represented age group was between 20 and 24 years old. Pauciparas represented the majority of our study population. 64.5% of patients were uneducated. These hemorrhages occurred in patients who often gave birth outside the department with 63.8%. The main causes of postpartum hemorrhage were cervical tears (51.2%) and partial placental retention (30.7%). The majority of these deliveries (48.8%) were carried out by midwives followed by assistant midwives (33.2%). Active management of the third period of delivery was systematic and immediate followed by the valve examination completed by the infusion of oxytocin in case of uterine atony, tranexamic acid ans misoprostol intrarectally. In the event of persistent hemorrhage, suture of the cervical lesions, ligation of the cervical vessels or even laparotomy either for the selective ligation of the blood vessels or for the hysterectomy for hemostasis were practiced. We do not have a Nalador, nor a Bakry balloon, nor an Interventional Radiology service for embolization of the uterine arteries. We deplore 1.3% of deaths from afibrinogenemia. Conclusion: The frequency of immediate postpartum hemorrhages is lower in our health facility. This is due to the rapid handling of cases. Improving the quality of services offered to women during childbirth can further help reduce the frequency of these hemorrhages.

Clandestine Abortions and Its Complications at the University Hospital Center of the Sino-Central African Friendship

Introduction: In developed countries in general and particularly in the Central African Republic, clandestine abortions constitute a real health problem. The aim of our work was to study the method of carrying out clandestine abortions, to describe the profile of the providers and the complications that result from it. Patients and Methods: This was a descriptive cross-sectional study of patients hospitalized at the University Hospital Center for Sino-Central African Friendship (CHUASC) from 2015 to 2019 on 236 cases. Results: We identified a total of 254 patients for 8039 live births in the same period, a frequency of 3.16%. The age of the patients was between 15 years old and 48 years old with an average of 25 years old. Singles accounted for 42.5%. Induced abortions were more performed among students (32.6%) and the unemployed 53.6%. Contraception was used in 11.5% of cases. The methods used were numerous: misoprostol (32%), the traditional method (18%), the mechanical method (50%). In this method, we note the dilation of the cervix by the cassava stem;Hegar’s candles;the probes, which are infusion tubes cut and introduced into the cervix, of which 2 cases had escaped at the level of the uterine muscles and emerged under the skin at the pubis part two years later. The practice of induced abortions in our series was done by medical students in 37.3% followed by unqualified staff in 49.7%. It took place either at home or in centers managed by Non-Governmental Organizations. The aseptic conditions were not perfect. Some patients (43.2%) were aware of the ensuing complications. All complications were present: hemorrhage (60.7%), pelviperitonitis (23.6%), uterine perforation (6.6%), peritonitis (2.8%) and death (2%) related to sepsis. Conclusion: Clandestine abortion remains a real health problem for young Central African women.

船型设计是船舶总体设计中一项极其复杂且又重要的内容,船舶的结构设计、性能计算、总布置等都要以船型为依据,因此,如何实现船型参数化设计尤为重要。FRIENDSHIP系统为船型设计提供了基于Feature特征和仿真驱动设计的参数化方法和实现机制。在对船型参数化基本理论———特征参数、特征曲线和曲面生成等进行详细阐述的基础上,以某型船艉部裸船体为例,具体阐述了船型参数化的实现流程,以及以Feature、Curveengine和Meta surface为特征机制的船型参数化的具体步骤。以Feature特征为核心的船型参数化方法不仅能为船型曲面的快速建立提供技术支撑,还可以为性能分析和优化提供基础条件。

基于Friendship的波浪能滑翔器母船设计

波浪能滑翔器作为近年来一种利用海洋能的创新型设备,在民用和军事方面都有着重要的研究意义和应用价值。本文应用船体设计领域特有的CAD-CFD集成平台Friendship软件,通过其Feature二次开发功能,研究了波浪能滑翔器母船曲面的全参数化建模方法。并以此为基础,采用Sobol优化算法,对母型船进行了多方案优化计算,并对影响阻力的关键参数进行分析,同时对艇体的重心位置和挂点位置的范围做了初步分析。通过优化设计,波浪能滑翔器母船的快速性能得到显著提升,STAR-CCM+的计算结果也佐证了优化的有效性。该方法为波浪能滑翔器母船的快速设计提供了有效途径。

Ebola virus disease reemerged in Western Africa in 2014.Chinese Center for Disease Control and Prevention dispatched the first Ebola virus（EBOV）detection team to run newly established Sierra Leone-China Friendship Biological Safety Laboratory.The aims of study were to understand epidemiology,clinical manifestations and survival time of EBOV in patient＇s blood.A total of 913specimens were tested between March 11 and April20, 2015. EBOV positivity occurred in 7.37% of the blood and 0.53% in throat swabs.

The role of drinking in new and existing friendships across high school settings

We use 9 Add Health high schools with longitudinal network data to assess whether adolescent drinkers choose friends who drink, prefer friends whose friends drink, if selection differs between new and existing friendships, and between schools. Utilizing dynamic social network models that control for friend influences on individual alcohol use, the results show that drinkers do not strongly prefer friends who drink. Instead, they favor close friends whose friends’ drink, suggesting that alcohol matters for selection on the social groups and environments that friends connect each other to. The role of alcohol use differs by whether friendships are new or existing, however, with bridging connections being less stable. Moreover, selection processes, and the implications of alcohol use for friendship, vary in important ways between schools.

A study of nurses’ job satisfaction: The relationship to professional commitment and friendship networks

We suggest that employees’ job satisfaction has relationship to friendship network other than professional commitment, and argue that friendship network in the same ward and across wards will have different effects on employees’ job satisfaction. A cross-sectional survey design utilizing questionnaires was selected to fulfill the research objectives. All of the 405 nurses in the En Chou Kong Hospital were surveyed. Three hundred and three nurses completed the questionnaire representing a response rate of 74.8%. The instruments included friendship network nomination, professional commitment scale, and nurses’ job satisfaction scale (NJSS). The regression model of job satisfaction was constructed, using friendship network variables in the ward and across wards and professional commitment as independent variables. R square for each model is 0.22-0.36 for the four dimensions of job satisfaction. Professional commitment is the robust predictor. The efficiency of friendship network in the ward is a good predictor, while it is negative related to satisfaction of work load. Further, the indegree in the ward is negative related to work load. Implication was discussed.

Potential friendship discovery in social networks based on hybrid ensemble multiple collaborative filtering models in a 5G network environment

At present, 5G network technology is being applied to various social network modes, and it can provide technical and traffic support for social networks. Potential friendship discovery technology in 5G-enabled social networks is beneficial for users to make potential friends and expand their range of activities and social hierarchy, which is highly sought after in today's social networks and has great economic and application value. However, the sparsity of the dominant user association dataset in 5G-enabled social networks and the limitations of traditional collaborative filtering algorithms are two major challenges for the friend recommendation problem. Therefore, in order to overcome these problems regarding previous models, we propose a Hybrid Ensemble Multiple Collaborative Filtering Model (HEMCF) for discovering potential buddy relationships. The HEMCF model draws on a special autoencoder method that can effectively exploit the association matrix between friends and additional information to extract a hidden representation of users containing global structural information. Then, it uses the random walk-based graph embedding algorithm DeepWalk to extract another hidden representation of users in the buddy network containing local structural information. Finally, in the output module, the HEMCF model stacks and multiplies the two types of hidden representations of users to ensure that the information mentioned above is concentrated in the final output to generate the final prediction value. The magnitude of the prediction value represents the probability of the users being friends, with larger values representing a high probability of the two users being friends, and vice versa. Experimental results show that the proposed method boosts the accuracy of the relationship prediction over baselines on 3 real-world public datasets dramatically.
