Systemic right ventricle complications in levo-transposition of the great arteries: A case report and review of literature

BACKGROUND Congenitally corrected levo-transposition of the great arteries(L-TGA)is a congenital heart disease in which the ventricles and great arteries are transposed from their typical anatomy.In L-TGA,the double discordance,atrioventricular and ventriculoarterial,create an acyanotic milieu which allows patients to survive their early decades,however,progressive systemic right ventricle(sRV)dys-function creates complications later in life.sRV dysfunction and remodeling predisposes patients to intracardiac thrombus(ICT)formation.CASE SUMMARY A 40-year-old male with L-TGA presented with symptoms of acute decom-pensated heart failure.In childhood,he had surgical repair of a ventricular septal defect.In adulthood,he developed sRV dysfunction,systemic tricuspid valve(sTV)regurgitation,and left-bundle branch block for which he underwent cardiac resynchronization therapy.Transthoracic echocardiogram showed a sRV ejection fraction of 40%,severe sTV regurgitation,and a newly identified sRV ICT.ICT was confirmed by ultrasound-enhancing agents and transesophageal echocardio-graphy.Our patient was optimized with guideline-directed medical therapy and diuresis.Anticoagulation was achieved with a vitamin K antagonist(VKA)and he was later referred for evaluation by advanced heart failure and heart transplant services.CONCLUSION Anticoagulation with VKA is the mainstay of treatment in the absence of conclusive data supporting direct oral anticoagulant use in ICT in patients with congenital heart disease.This case illustrates the natural history of L-TGA and highlights the importance of surveillance and monitoring with dedicated cardiac imaging to identify complications.

Hard life for sons in the nest?Sex-dependent offspring mortality in Great Tits in urban and forest areas

Sex-biased mortality can occur in birds during development,for example due to sexual differences in energy requirement and/or environmental sensitivity,or the effects of sex hormones or sex differences in the expression of mutations linked to sex chromosomes.The extent of sex-bias in mortality may also be related to environmental conditions that influence offspring development and survival.Urban areas often provide poorer conditions for nestling development resulting in higher offspring mortality compared to natural areas,which may accelerate sex differences in offspring mortality in cities.To test this hypothesis,we examined the sex ratio of dead offspring in Great Tits(Parus major),using 427 samples of unhatched eggs and dead nestlings collected in two urban and two forest sites between 2013 and 2019.The ratio of males in the whole sample of dead offspring(56.9%)was significantly higher than expected by an 1:1 ratio,and the strongest sex biases were detected in urban areas(57.6%males)and in young nestlings(<14 days old,59.0%males).However,the sex ratios of dead offspring did not differ significantly among study sites and between offspring developmental stages.29.3%of unhatched eggs contained a visible embryo,and the proportion of embryo-containing unhatched eggs did not differ significantly between urban and forest study sites.These results suggest male-biased offspring mortality in Great Tits,and highlight the need of large datasets to detect subtle differences between habitats and developmental stages.

Case Report: Surgical Correction of Transposition of the Great Arteries with Aortopulmonary Window

Introduction:Transposition of the great arteries(TGA)with aortopulmonary window is a rare type of congenital heart disease with limited experience.We reported a neonate aged 25 days receiving the arterial switch operation and assisted with extracorporeal membrane oxygenation.Conclusion:TGA with aortopulmonary window can be safely correctly with the arterial switch operation.

A 63-Year-Old Male with D-Transposition of the Great Arteries Who Had an Early Form of the Arterial Switch Operation

We describe a 63-year-old male who appears to have undergone an early form of the arterial switch operation for D-transposition of the great arteries performed in the mid-1960s.We review the clinical and imaging data that support our conclusion.He had a diagnostic cardiac catheterization which demonstrated severe pulmonary hypertension responsive to epoprostenol and oxygen.Our case may represent one example of the experimental surgical work done prior to Dr.Adibe Jatene’s description of thefirst successful arterial switch performed in 1975.

Spatial-temporal distribution and geochemistry of highly evolved Mesozoic granites in Great Xing’an Range,NE China:Discriminant criteria and geological significance

Highly evolved granite is an important sign of the mature continent crust and closely associated with deposits of rare metals.In this work,the authors undertake systematically zircon U-Pb ages and whole rock elemental data for highly evolved granitic intrusions from the Great Xing’an Range(GXR),NE China,to elucidate their discriminant criteria,spatial-temporal distribution,differentiation and geodynamic mecha-nism.Geochemical data of these highly evolved granites suggest that high w(SiO_(2))(>70%)and differentiation index(DI>88)could be quantified indicators,while strong Eu depletion,high TE_(1,3),lowΣREE and low Zr/Hf,Nb/Ta,K/Rb could only be qualitative indicators.Zircon U-Pb ages suggest that the highly evolved gran-ites in the GXR were mainly formed in Late Mesozoic,which can be divided into two major stages:Late Ju-rassic-early Early Cretaceous(162-136 Ma,peak at 138 Ma),and late Early Cretaceous(136-106 Ma,peak at 126 Ma).The highly evolved granites are mainly distributed in the central-southern GXR,and display a weakly trend of getting younger from northwest to southeast,meanwhile indicating the metallogenic potential of rare metals within the central GXR.The spatial-temporal distribution,combined with regional geological data,indicates the highly evolved Mesozoic granites in the GXR were emplaced in an extensional environ-ment,of which the Late Jurassic-early Early Cretaceous extension was related to the closure of the Mongol-Okhotsk Ocean and roll-back of the Paleo-Pacific Plate,while the late Early Cretaceous extension was mainly related to the roll-back of the Paleo-Pacific Plate.

Evidence of the Great Attractor and Great Repeller from Artificial Neural Network Imputation of Sloan Digital Sky Survey

The Sloane Digital Sky Survey (SDSS) has been in the process of creating a 3D digital map of the Universe, since 2000AD. However, it has not been able to map that portion of the sky which is occluded by the dust gas and stars of our own Milkyway Galaxy. This research builds on work from a previous paper that sought to impute this missing galactic information using Inpainting, polar transforms and Linear Regression ANNs. In that paper, the author only attempted to impute the data in the Northern hemisphere using the ANN model, which subsequently confirmed the existence of the Great Attractor and the homogeneity of the Universe. In this paper, the author has imputed the Southern Hemisphere and discovered a region that is mostly devoid of stars. Since this area appears to be the counterpart to the Great Attractor, the author refers to it as the Great Repeller and postulates that it is an area of physical repulsion, inline with the work of GerdPommerenke and others. Finally, the paper investigates large scale structures in the imputed galaxies.

A Study of Planetary Theory in the Great Expansion System(Dayan li大衍历,727 CE):The Case of Mars

The planetary theory in the Great Expansion System(Dayan li大衍历,727 CE)is investigated,with a detailed example of Mars.In ancient Chinese astrology,the position of one planet and the relative positions of different planets had important astrological significance.Thus,planetary theory is an important part of Chinese mathematical astronomy.The Great Expansion System,which was compiled by Yixing一行of the Tang唐dynasty(618–907 CE),provided many innovations in planetary theory.Based on the extant Treatises on Mathematical Harmonics and Astronomy(Lüli zhi律历志)in Chinese official histories,the Great Expansion System was the first Chinese astronomical system to include tables of the planetary equation of center and procedures for correcting the influence of the planetary equation of center on the position of a planet.It was also the first Chinese system to design a table of the planetary phases of motion for calculating the mean position of a planet,which was the basis for calculating the true position of the planet.In addition,Yixing proposed the concept of the precession of planetary perihelion and gave the values of the precession of planetary perihelion for the first time in ancient China.The innovations of the Great Expansion System regarding planetary theory established its important position in the history of Chinese astronomical systems.Mars is taken as a case study to investigate the planetary theory in the Great Expansion System,including the astronomical constants related to Mars,two important astronomical tables,namely the table of the equation of center and the table of the phase motion of Mars in one synodic period,and the procedures for calculating the position of Mars on any given day using the planetary and solar equations of center.Two questions are addressed.First,how did Yixing correct the influence of the equation of center of Mars on the time of mean conjunction and the mean position of Mars?Second,how did Yixing calculate the true position of Mars on any given day?The original text of the Great Expansion System is analyzed to show how Yixing developed the planetary theory in the Sui and early Tang periods and constructed a complete method for predicting the true positions of planets using the planetary and solar equations of center.

Explaining patterns of species dominance in the shrub steppe systems of the Junggar Basin(China) and Great Basin(USA)

Natural scientists have long recognized that regions with similar climate tend to have similar vegetation.Preliminary observations suggest that shrub steppe communities of the western US and western China may be two such regions with similar annual precipitation,temperature,land use,and vegetation.These cold dry shrub steppes have traditionally been grazed.Despite these similarities,patterns of species dominance are different.Annual species that are rare in China become dominant when introduced to the United States.The objective of this study was to investigate how climate,land use and community structure may explain these patterns of species dominance.Community structure and grazing intensity were measured at 5 sites in each region.This information was combined with a broader review of the literature describing the history of grazing in both basins.Climate was analyzed based on a spatially-gridded,interpolated weather time series （monthly records） and climatological summary （1961-1990 mean conditions） data set from the Climate Research Unit.We found that differences in summer precipitation and winter minimum temperature,land use intensity,and shrub size may all contribute to the dominance of annual species in the Great Basin,particularly Bromus tectorum.In particular,previous work indicates that summer precipitation and winter temperature drive the distribution of Bromus tectorum in the Great Basin.As a result,sites with wet summers and cold springs,similar to the Chinese sites,would not be expected to be dominated by Bromus tectorum.A history of more intense grazing of the Chinese sites,as described in the literature,also is likely to decrease fire frequency,and decreases litter and shrub dominance,all of which have been demonstrated to be important in Bromus tectorum establishment and ultimate dominance.Further research is necessary to determine if other annuals that follow the same pattern of scarcity in the Junggar Basin and dominance in the Great Basin are responding to the same influences.

ESTABLISHMENT OF SURVEYING SYSTEM OF THE GREAT WALL STATION

It is the main task of the antarctic surveying expedition to establish the surveying system. In three times of the antarctic surveying research expeditions, a complete and accurate surveying system has been established in the Chinese Great Wall Station area. The surveying system includes geodetic coordinate system, elevation system and gravity reference system. In this paper, the surveying methods and the mathematical models for establishing these systems are discussed, and the accuracy of results is analysed.

Systematic review and meta-analysis of outcomes of anatomic repair in congenitally corrected transposition of great arteries

BACKGROUND Treatment of congenitally corrected transposition of great arteries(cc-TGA)with anatomic repair strategy has been considered superior due to restoration of the morphologic left ventricle in the systemic circulation.However,data on long term outcomes are limited to single center reports and include small sample sizes.AIM To perform a systematic review and meta-analysis for observational studies reporting outcomes on anatomic repair for cc-TGA.METHODS MEDLINE and Scopus databases were queried using predefined criteria for reports published till December 31,2017.Studies reporting anatomic repair of minimum 5 cc-TGA patients with at least a 2 year follow up were included.Metaanalysis was performed using Comprehensive meta-analysis v3.0 software.RESULTS Eight hundred and ninety-five patients underwent anatomic repair with a pooled follow-up of 5457.2 patient-years(PY).Pooled estimate for operative mortality was 8.3%[95%confidence interval(CI):6.0%-11.4%].0.2%(CI:0.1%-0.4%)patients required mechanical circulatory support postoperatively and 1.7%(CI:1.1%-2.4%)developed post-operative atrioventricular block requiring a pacemaker.Patients surviving initial surgery had a transplant free survival of 92.5%(CI:89.5%-95.4%)per 100 PY and a low rate of need for pacemaker(0.3/100 PY;CI:0.1-0.4).84.7%patients(CI:79.6%-89.9%)were found to be in New York Heart Association(NYHA)functional class I or II after 100 PY follow up.Total re-intervention rate was 5.3 per 100 PY(CI:3.8-6.8).CONCLUSION Operative mortality with anatomic repair strategy for cc-TGA is high.Despite that,transplant free survival after anatomic repair for cc-TGA patients is highly favorable.Majority of patients maintain NYHA I/II functional class.However,monitoring for burden of re-interventions specific for operation type is very essential.

Recent Vegetation Cover Dynamics and Climatic Parameters Evolution Study in the Great Green Wall of Senegal

The drought recorded in 1970s and 1980s, particularly in the Sahara and Sahel region has greatly affected the population as well as the economies and the eco-systems of this area. In 2007, the African Union launched a Pan-African program, the Great Green Wall for the Sahara, the Sahel Initiative (GGWSSI) to reverse land degradation and desertification by planting a wall of trees stretching from Dakar to Djibouti. The objective is to improve food security, and support local people to adapt to climate change. This paper aims to evaluate the impacts of the reforestation program in Senegal, fifteen years after it was launched. This study uses a time series of satellite-derived vegetation cover and climatic parameters data to analyze the sustainability of these interventions. Change detection approaches were applied to identify and characterize the drives of the eventual changes. A comparative analysis of reforestation on climatic parameters was explored through the temporal analysis of the vegetation index over the periods 2000-2008 and 2009-2020. An increase in vegetation activity was noted through the NDVI at the interannual (+2% to +8%) and seasonal (+1.5% to 7% for the wet season and 1% to 4% for the dry season) scale and a positive and significant evolution is noted on the trace of the GGW. Also, the period 2009-2020 recorded an increase in rainfall of 2% to 8% of the average value 2000-2020 and 4% to 8% of the rainy season. Soil moisture is the climatic parameter that has increased the most, with an increase of 25% to 54% of the 2000-2020 average, i.e. between 20 mm and 70 mm more. This study shows a significant improvement in the relationship between NDVI and climate parameters after the different reforestation actions of the GGW.
