[Method] According to the related sequences of porcine H-FABP gene released in GenBank, specific primers were designed to amplify the intron 3 of porcine H-FABP gene. [ Result] The intron 3 of porcine H-FABP gene was amplified successfully. Its whole sequence was 1 350 bp in length and had been submitted to GenBank (Accession no. : DQ 002993). Objective:This study aims to investigate whether the heart fatty acid-binding protein(HFABP)in the cerebrospinal fluid(CSF)was a potential predictive biomarker for Alzheimer’s disease(AD).Methods:We evaluated the associations of CSF HFABP levels with core biomarkers,cognition,and brain structure in a sample population(n=302)from the Alzheimer’s Disease Neuroimaging Initiative(ADNI)database.Multiple linear regression and mixed-effects models were employed in the analyses.AD progression was assessed using the Kaplan–Meier survival analysis.Results:CSF HFABP was higher in patients with mild cognitive impairment and AD than the normal controls(p<0.001)and was particularly higher in those with amyloid-β(Aβ)pathologic features.CSF HFABP was associated with higher baseline CSF t-tau(p<0.001),CSF p-tau(p<0.001),and CSF t-tau/Aβ42 and CSF p-tau/Aβ42(p<0.01).Moreover,CSF HFABP was found to play predictive roles in hippocampal atrophy(p<0.01),cognitive decline(p<0.05),and the risk of AD(p<0.001).Conclusion:Our findings suggest that CSF HFABP can be a predictive biomarker of AD. 展开更多

As intracellular fatty acid(FA) carriers,FA-binding proteins(FABPs) widely participate in the absorption,transport,and metabolism of FAs.It is a key protein in insect lipid metabolism and plays an important role in various physiological activities of insects.An FABP gene(HvFABP) was cloned from the transcriptional library of Heortia vitessoides Moore(Lepidoptera:Crambidae),and its expression patterns were determined using reverse transcription quantitative PCR(RTqPCR).Stage-and tissue-specific expression profiles indicated that HvFABP highly expressed from prepupal to adult stages and in larval midgut and adult wings.HvFABP expression may be induced through starvation,mRNA expression was downregulated at 24 and 48 h and upregulated at 72 h after starvation.Furthermore,20-hydroxyecdysone can induce the upregulation of its expression.RNA interference-mediated silencing of Hv FABP significantly inhibited HvFABP expression,resulting in delayed development,abnormal molting or lethal phenotypes,and a significantly reduced survival rate.These results indicate that HvFABP plays a key role in the molting of H.vitessoides. 展开更多

Role of Heart-Type Fatty Acid Binding Protein in Early Detection of Acute Myocardial Infarction in Comparison with cTnI, CK-MB and Myoglobin 被引量:33

Heart fatty acid-binding protein (H-FABP) is supposed to be the most sensitive biomarker of early acute myocardial infarction (AMI). To evaluate the diagnostic value of H-FABP for AMI in the early stage, the plasma levels of H-FABP were measured by sandwich ELISA in 93 patients with suspected AMI at admission within 6 h after onset of chest pain and 69 normal healthy subjects. The plasma concentrations of cardiac troponin-I (cTnI), creatine kinase-MB (CK-MB) and myoglobin (Mb) were assayed at the same time by using corpuscle chemiluminescence for those patients. The patients were classified as AMI group (n=32) and non-AMI group (n=61) retrospectively. The diagnostic validity was evaluated in terms of sensitivity, specificity and receiver operating characteristic (ROC) curve analysis. The results showed the cutoff value of H-FABP for AMI was 16.8 ng/ml, and its diagnostic sensitivity for AMI was 64.29 % within 3 h and 84.38 % within 6 h after onset of chest pain, and the diagnostic specificity for non-AMI was 100 % within 3 h and 91.8 % within 6 h. H-FABP had higher sensitivity than that of cTnI and CK-MB at all time points (P<0.05), whereas there was no significant difference in specificity among the four markers. But the area under the ROC curve of H-FABP was significantly greater than that of cTnI, CK-MB and Mb within 3 h. These results revealed that H-FABP possessed high diagnostic sensitivity and specificity for AMI in early stage, especially within 3 h after onset of persistent angina pectoris. In conclusion, H-FABP can be used as a sensitive marker for AMI in the early stage. 展开更多

H-FABP is regarded as a tissue-specific protein existing only in myocardial cells. It is released from the cardiac tissue and gets into the plasma when a heart attack occurs; the myocardial infarction is a good case in point. As a resuit, the detection of H-FABP will be an early and important biomarker for the disease concerned. The objective of the study is to prepare the recombinant H-FABP by aeukaryotic expression system, pichia, to produce the protein mimicking natural H-FABP, as an immunogen for the production of the specific antibody. A gene fragment encoding H-FABP was cloned in the expressing vector pPICZα, after sequencing. The recombinant plasmid was transformed into the competent cells of the X-33 strain by means of electroporation. The expression of the target peptide induced by methanol was screened by means of Western hlotting, with the available MAb(Clone 6B6). Highly expressive engineer strains were obtained. The production of recombinant H-FABP under induction was about 0.7 g/L, with an Mr of 14.5 kDa and recognized by a commercially available MAb (Clone 6B6). The recombinant vector was successfully constructed. Following this, H-FABP was expressed in X-33, and it would become the source of the preparation of specific antibodies, to develop diagnostic kits. 展开更多

AIM To investigate feasibility of combined assessment of biochemical and electrophysiological myocardial impairment markers risk-stratifying patients with chronic heart failure(CHF). METHODS Serum levels of heart-type fatty acid binding protein(H-FABP) as a marker of ongoing myocardial damage and QRS duration on electrocardiogram were measured at admission in 322 consecutive patients with CHF. A prolonged QRS duration was defined as 120 ms or longer. The cut-off value for H-FABP level(4.5 ng/mL) was determined from a previous study. Patients were prospectively followed during a median follow up period of 534 d. The primary endpoint was cardiac deaths and rehospitalization for worsening CHF.RESULTS There were 117 primary events, including 27 cardiac deaths and 90 rehospitalizations. Patients were stratified into four groups according to H-FABP level and QRS duration(≥ 120 ms). Multivariate analysis demonstrated that high H-FABP levels [hazard ratio(HR) = 1.745, P = 0.021] and QRS prolongation(HR1.612, P = 0.0258) were independent predictors of cardiac events. Kaplan-Meier analysis demonstrated that the combination of high H-FABP levels and QRS prolongation could be used to reliably stratify patients at high risk for cardiac events(log rank test P < 0.0001).CONCLUSION Combined assessment of myocardial damage and electrical disturbance can be used to risk-stratify patients with CHF. 展开更多

Suberoylanilide hydroxamic acid(SAHA) is a histone deacetylase inhibitor that shows marked efficacy against many types of cancers and is approved to treat severe metastatic cutaneous T-cell lymphomas. In addition to its anticancer activity,SAHA has significant effects on the growth of many viruses. The effect of SAHA on replication of human cytomegalovirus(HCMV) has not, however, been investigated. Here, we showed that the replication of HCMV was significantly suppressed by treatment with SAHA at concentrations that did not show appreciable cytotoxicity. SAHA reduced transcription and protein levels of HCMV immediate early genes, showing that SAHA acts at an early stage in the viral life-cycle. RNAsequencing data mining showed that numerous pathways and molecules were affected by SAHA. Interferon-mediated immunity was one of the most relevant pathways in the RNA-sequencing data, and we confirmed that SAHA inhibits HCMV-induced IFN-mediated immune responses using quantitative Real-time PCR(qRT-PCR). Fatty acid-binding protein 4(FABP4), which plays a role in lipid metabolism, was identified by RNA-sequencing. We found that FABP4 expression was reduced by HCMV infection but increased by treatment with SAHA. We then showed that knockdown of FABP4 partially rescued the effect of SAHA on HCMV replication. Our data suggest that FABP4 contributes to the inhibitory effect of SAHA on HCMV replication. 展开更多

目的 研究心型脂肪酸结合蛋白(H-FABP)、氨基端脑钠肽前体(NT-proBNP)对急性心力衰竭患者容量负荷状态及预后的评估。方法 本研究为前瞻性研究,以2021年1月至2022年1月于安阳市人民医院急诊住院的老年急性心力衰竭患者83例作为研究组,... 目的 研究心型脂肪酸结合蛋白(H-FABP)、氨基端脑钠肽前体(NT-proBNP)对急性心力衰竭患者容量负荷状态及预后的评估。方法 本研究为前瞻性研究,以2021年1月至2022年1月于安阳市人民医院急诊住院的老年急性心力衰竭患者83例作为研究组,另选取83例健康者作为对照组。根据相对容量平衡水平,将以上83例患者分为容量超负荷组和非容量超负荷组,对所有出院患者随访3个月,将发生死亡的患者归为死亡组,其余为生存组。比较研究组与对照组不同容量负荷、不同预后的H-FABP、NT-proBNP水平之间的差异,研究H-FABP、NT-proBNP与生存状况以及容量负荷状态的相关性。结果 研究组患者的H-FABP、NT-proBNP水平高于对照组(P<0.001);容量超负荷组患者的H-FABP、NT-proBNP水平高于非容量超负荷组(P<0.05);死亡组患者的H-FABP、NT-proBNP水平高于生存组(P<0.05)。通过相关性分析,患者的死亡以及容量超负荷与H-FABP、NT-proBNP呈现正相关(P<0.001)。结论 H-FABP、NT-proBNP与急性心力衰竭患者容量负荷状态及预后呈现相关性,有助于评估急性心力衰竭的严重程度,对预后不良患者起到积极的指导治疗作用。 展开更多

目的探究血清成纤维细胞生长因子21(FGF21)和脂肪酸结合蛋白4(FABP4)检测对急性ST段抬高型心肌梗死(STEMI)患者经皮冠状动脉介入治疗(PCI)术后心力衰竭的预测价值。方法选取2020年9月至2022年9月内蒙古医科大学附属医院接诊的113例STEM... 目的探究血清成纤维细胞生长因子21(FGF21)和脂肪酸结合蛋白4(FABP4)检测对急性ST段抬高型心肌梗死(STEMI)患者经皮冠状动脉介入治疗(PCI)术后心力衰竭的预测价值。方法选取2020年9月至2022年9月内蒙古医科大学附属医院接诊的113例STEMI患者为研究对象,依据PCI术后1年是否发生心力衰竭(心衰),将其分为心衰组(n=32)和非心衰组(n=81)。应用ELISA法测定血清FGF21、FABP4表达水平,比较两组血清FGF21、FABP4水平,多因素logistic回归分析影响STEMI患者PCI术后发生心力衰竭的相关因素,ROC曲线评估血清FGF21、FABP4水平对STEMI患者PCI术后心力衰竭发生的预测价值。结果心衰组心率次数、C反应蛋白(CRP)、心肌肌钙蛋白(cTnI)、N末端B型利钠肽原(BNP)、利尿剂使用比例均显著高于非心衰组,左心室射血分数(LVEF)显著低于非心衰组(P<0.05)。心衰组血清FGF21、FABP4表达水平均明显高于非心衰组[(228.37±33.07)ng/L比(185.68±25.52)ng/L、(34.26±5.51)ng/ml比(26.87±4.67)ng/ml,t=7.345、7.195,P<0.05]。血清FGF21(95%CI 1.371~8.191)、FABP4(95%CI 1.176~4.090)及发病到至导丝通过时间(95%CI 1.058~8.157)是影响STEMI患者PCI术后发生心力衰竭的危险因素(OR>1,P<0.05),LVEF(95%CI 0.473~0.913)是保护因素(OR<1,P<0.05)。血清FGF21、FABP4单独及二者联合预测STEMI患者PCI术后发生心力衰竭的曲线下面积(AUC)分别为0.828、0.856、0.934,二者联合优于单一(Z二者联合-FGF21=1.971、Z二者联合-FABP4=2.417,P=0.048、P=0.015)。结论STEMI患者PCI术后发生心力衰竭血清FGF21、FABP4水平均明显升高,二者联合对STEMI患者PCI术后发生心力衰竭的风险具有更高的预测价值。 展开更多

目的与单一标志物高敏肌钙蛋白T(cTnT)进行对比,初步探索多重心肌标志物[和肽素(copeptin)、cTnT和心脏型脂肪酸结合蛋白(HFABP)]联合检测模型在诊断4a型急性心肌梗死(AMI)中的应用价值。方法纳入2022年3月至12月期间在南京医科大学第... 目的与单一标志物高敏肌钙蛋白T(cTnT)进行对比,初步探索多重心肌标志物[和肽素(copeptin)、cTnT和心脏型脂肪酸结合蛋白(HFABP)]联合检测模型在诊断4a型急性心肌梗死(AMI)中的应用价值。方法纳入2022年3月至12月期间在南京医科大学第一附属医院接受择期经皮冠状动脉介入治疗(PCI)且术后cTnT升高超过正常参考值上限(URL)第99百分位数的非AMI患者。根据《第四版心肌梗死通用定义》,按照术后是否出现4a型AMI将患者分为非4a型AMI组和4a型AMI组。采用化学发光免疫金纳米组装体免疫传感阵列(ciGold)测定AMI生物标志物浓度。通过受试者工作特征(ROC)曲线分析单一标志物和多重联合心肌标志物诊断模型的诊断性能,从ROC曲线获得敏感性和特异性,计算ROC曲线下面积(AUCROC)评估各自诊断价值。使用Kappa分析评估多重标志物联合检测模型与《第四版心肌梗死通用定义》诊断的一致性。结果共纳入65例患者,女性占23.1%。ROC曲线分析显示,多重联合心肌标志物诊断模型的特异性为96.5%,敏感性为92.3%,符合率为94.6%,阳性预测值为92.3%,阴性预测值为96.2%,AUCROC为0.979。cTnT模型的特异性为94.2%,敏感性为100%,符合率为95.7%,阳性预测值为100%,阴性预测值为94.9%,AUCROC为0.987。虽然多重标志物联合检测模型诊断敏感性较低(P=0.011),但具有更高的特异性(P=0.016)。两种诊断模型准确性的AUCROC差异性分析提示P>0.05,诊断准确性无明显统计学差异。Kappa分析结果表明,多重联合心肌标志物检测模型与《第四版心肌梗死通用定义》诊断4a型AMI具有高度一致性,Cohen′s Kappa系数为0.818。结论多重标志物联合检测模型与cTnT在诊断4a型AMI方面性能相近,诊断一致性高,但联合检测模型具有更高的特异性优势。 展开更多

目的:探讨缺血修饰白蛋白(IMA)与心脏型脂肪酸结合蛋白(H-FABP)联合检测对不典型急性心肌梗死(AMI)患者的早期诊断价值。方法:选择2021年1月—2022年6月湖南中医药高等专科学校附属第一医院收治的发病6 h内的176例可疑AMI患者作为研究对... 目的:探讨缺血修饰白蛋白(IMA)与心脏型脂肪酸结合蛋白(H-FABP)联合检测对不典型急性心肌梗死(AMI)患者的早期诊断价值。方法:选择2021年1月—2022年6月湖南中医药高等专科学校附属第一医院收治的发病6 h内的176例可疑AMI患者作为研究对象,依照临床最终诊断结果将其分为有胸痛AMI组90例、无胸痛AMI组32例、非AMI组54例。选择同期在医院体检的30名健康者作为健康对照组。测定各组研究对象H-FABP、IMA、肌钙蛋白I(c TnI)及肌酸激酶同工酶(CK-MB)含量,计算H-FABP、IMA、H-FABP+IMA、cTnI、CK-MB对无胸痛AMI组患者的诊断性试验评价指标。结果:有胸痛AMI组及无胸痛AMI组患者IMA、H-FABP、cTnI、CK-MB水平均高于非AMI组患者及健康对照组研究对象,差异均有统计学意义(F=7.362、12.275、8.938、3.294、8.729、16.736、10.283、5.726,P<0.05);有胸痛AMI组患者与无胸痛AMI组患者IMA、H-FABP、cTnI、CK-MB水平比较,差异均无统计学意义(t=2.428、1.594、2.875、3.179,P>0.05)。IMA联合H-FABP检测诊断无胸痛AMI组患者的灵敏度、特异度、阳性预测值、阴性预测值、诊断效率均高于IMA、H-FABP、cTnI、CK-MB单独检测数据,差异均有统计学意义(χ^(2)=12.328、16.972、9.375、11.547,P<0.05)。结论:IMA与H-FABP联合检测对不典型AMI患者的早期诊断具有较高的临床价值。 展开更多

目的:探究血清心型脂肪酸结合蛋白(heart-type fatty acid binding protein,H-FABP)、生长分化因子-15(growth and differentiation factor-15,GDF-15)表达对ST段抬高型心肌梗死(ST-segment elevation myocardial infarction,STEMI)患... 目的:探究血清心型脂肪酸结合蛋白(heart-type fatty acid binding protein,H-FABP)、生长分化因子-15(growth and differentiation factor-15,GDF-15)表达对ST段抬高型心肌梗死(ST-segment elevation myocardial infarction,STEMI)患者经皮冠状动脉介入治疗(percutaneous coronary intervention,PCI)后不良心血管事件发生的预测价值。方法:采用前瞻性研究,选择2020年6月—2023年6月于赣州市人民医院接受PCI的140例STEMI患者作为研究对象,术前检测患者血清H-FABP、GDF-15,行PCI,术后随访3个月,统计患者术后不良心血管事件发生情况,分析血清H-FABP、GDF-15与STEMI患者术后不良心血管事件发生的关系,同时绘制受试者工作特征(receiver operating characteristic,ROC)曲线,探究血清H-FABP、GDF-15对STEMI患者PCI后不良心血管事件发生的预测价值。结果:随访3个月期间,140例患者均无脱落病例;随访期间,140例STEMI患者PCI后发生不良心血管事件占比为23.57%(33/140),未发生不良心血管事件占比为76.43%(107/140)。两组心肌梗死面积、高血压、既往吸烟史比较,差异均有统计学意义(P<0.05);两组性别、年龄、病变支数、既往饮酒史、肌红蛋白(myoglobin,Myo)、心肌肌钙蛋白I(cardiac troponin I,cTnI)、肌酸激酶同工酶(creatine kinase-MB,CK-MB)、心室舒张末期内径(left ventricular end diastolic dimension,LVEDD)比较,差异均无统计学意义(P>0.05)。发生组血清H-FABP、GDF-15均高于未发生组,差异均有统计学意义(P<0.05)。点二列相关性分析显示,血清H-FABP、GDF-15与STEMI患者PCI后不良心血管事件发生呈正相关(r>0,P<0.05)。ROC曲线结果显示,血清H-FABP、GDF-15单独预测的AUC>0.7,联合预测的AUC>0.8,联合预测价值更高。结论:STEMI患者PCI前血清H-FABP、GDF-15水平越高,术后不良心血管事件发生风险越大,且临床可以将血清H-FABP、GDF-15作为STEMI患者PCI后不良心血管事件发生的有效预测指标。 展开更多

目的 分析支原体肺炎患儿血清心脏型脂肪酸结合蛋白(fatty acid-binding protein 3,FABP3)、多配体蛋白聚糖-4(Syndecan-4,SDC-4)水平与心肌损害的关系。方法 选取本院2022年6月到2023年6月期间收治的支原体肺炎患儿98例作为此次研究对... 目的 分析支原体肺炎患儿血清心脏型脂肪酸结合蛋白(fatty acid-binding protein 3,FABP3)、多配体蛋白聚糖-4(Syndecan-4,SDC-4)水平与心肌损害的关系。方法 选取本院2022年6月到2023年6月期间收治的支原体肺炎患儿98例作为此次研究对象,根据患儿的病情严重程度分为轻症组(58例)和重症组(40例),另选取同期健康体检儿童98例作为对照组。根据心肌损害情况将患儿分为心肌损害组(n=43)和非心肌损害组(n=55);血清FABP3、SDC-4水平检测采用酶联免疫吸附法(Enzyme-linked immunosorbent assay,ELISA);使用全自动生化分析仪和电化学发光法检测心肌酶谱指标;采用Pearson法分析血清FABP3、SDC-4水平以及与心肌酶谱相关指标的相关性;采用Logistic回归分析支原体肺炎患儿的影响因素。结果 重症组和轻症组血清FABP3、SDC-4水平显著高于对照组(P<0.05)。重症组血清FABP3、SDC-4水平显著高于轻症组(P<0.05)。心肌损害组和非心肌损害组FABP3、SDC-4、谷草转氨酶(aspartate transferase,AST)、乳酸脱氢酶(lactate dehydrogenase,LDH)、α-羟丁酸脱氢酶(α-hydroxybutyrate dehydrogenase,α-HBDH)、磷酸肌酸激酶(creatine phosphokinase,CK)、肌酸激酶同工酶(creatine kinase isoenzyme,CK-MB)以及超敏肌钙蛋白I(high sensitive troponin I,hs-cTnI)水平显著高于对照组(P<0.05),且心肌损害组FABP3、SDC-4、AST、LDH、α-HBDH、CK、CK-MB以及hs-cTnI水平显著高于非心肌损害组(P<0.05)。根据Pearson相关性分析得知,支原体肺炎患儿血清FABP3、SDC-4水平呈正相关(P<0.05),血清FABP3、SDC-4水平均与AST、LDH、α-HBDH、CK、CK-MB以及hs-cTnI水平呈正相关(P<0.05)。根据Logistic回归分析得知,FABP3、SDC-4、AST、LDH、α-HBDH、CK、CK-MB以及hs-cTnI升高是影响支原体肺炎合并心肌损害的危险因素(P<0.05)。结论 FABP3、SDC-4在支原体肺炎患儿血清中显著升高,二者与心肌损害密切相关。 展开更多

基于无缝管理理念的MDT模式对CHF患者的临床疗效及其H-FABP、cTnI水平与心功能的关系研究

目的:探讨无缝管理理念下的多学科协作(MDT)模式对慢性心力衰竭(CHF)患者的临床疗效,并分析心型脂肪酸结合蛋白(H-FABP)、心肌肌钙蛋白I(cTnI)水平与心功能指标的相关性。方法:纳入2020年11月-2021年11月本院收治的222例CHF患者,根据随... 目的:探讨无缝管理理念下的多学科协作(MDT)模式对慢性心力衰竭(CHF)患者的临床疗效,并分析心型脂肪酸结合蛋白(H-FABP)、心肌肌钙蛋白I(cTnI)水平与心功能指标的相关性。方法:纳入2020年11月-2021年11月本院收治的222例CHF患者,根据随机数字表法分为对照组(111例,采用MDT模式干预)和实验组(111例,采用无缝管理理念联合MDT模式干预)。干预一个月后,采用心力衰竭自我管理量表评价两组患者临床疗效,并比较两组干预前后明尼苏达心力衰竭生活质量问卷(MLHFQ)评分、H-FABP、cTnI水平及心功能指标,采用Pearson相关分析法分析H-FABP、cTnI水平与心功能指标的相关性。结果:干预一个月后,与对照组比较,实验组心力衰竭自我管理量表得分[(50.04±6.08)分比(72.07±5.65)分]、左室射血分数(LVEF)[(36.23±3.68)%比(43.42±3.36)%]、6min步行距离(6MWD)[(228.35±28.78)m比(337.29±32.31)m]均显著增加,MLHFQ评分[(40.14±3.25)分比(33.05±4.61)分]、H-FABP[(5.97±0.57)ng/ml比(2.77±0.59)ng/ml]、cTnI[(3.34±0.33)ng/ml比(2.45±0.61)ng/ml]水平均显著降低(P均<0.001)。CHF患者H-FABP、cTnI水平随心功能分级提升呈显著上升趋势(P均<0.001)。Pearson相关分析显示,H-FABP、cTnI与LVEF、6MWD呈显著负相关(r=-0.745~-0.676,P均<0.001),与BNP水平呈显著正相关(r=0.705、0.668,P均<0.001)。结论:无缝管理理念的MDT模式应用于CHF的临床治疗,能显著改善患者生活质量及心功能,联合H-FABP、cTnI检测对指导CHF患者预后评估及心功能监测具有重要临床意义。 展开更多

目的探讨参芎葡萄糖治疗慢性阻塞性肺疾病(COPD)合并肺源性心脏病(PHD)的临床效果。方法选取2022年1月—2024年1月收治的228例COPD合并PHD,采用随机数字表法随机将其分为观察组和对照组2组各114例。观察组给予参芎葡萄糖联合常规治疗,... 目的探讨参芎葡萄糖治疗慢性阻塞性肺疾病(COPD)合并肺源性心脏病(PHD)的临床效果。方法选取2022年1月—2024年1月收治的228例COPD合并PHD,采用随机数字表法随机将其分为观察组和对照组2组各114例。观察组给予参芎葡萄糖联合常规治疗,对照组给予常规治疗。比较2组血常规指标(红细胞、血红蛋白、红细胞比容、血小板计数)、凝血指标[D-二聚体(D-D)、凝血酶原时间(PT)、活化部分凝血酶原时间(APTT)、纤维蛋白原(FIB)]、肝肾功能[血尿素(BUN)、血肌酐(Scr)、天冬氨酸转氨酶(AST)、丙氨酸转氨酶(ALT)、总蛋白(TP)]、血气指标[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、动脉血氧饱和度(SaO_(2))]、心型脂肪酸结合蛋白(H-FABP)、肌钙蛋白I(cTnI)、N末端B型脑钠肽前体(NT-proBNP)、肺动脉收缩压(PASP)、急性生理学及慢性健康状况评估系统Ⅱ(APACHEⅡ)评分,并统计入院28 d病死情况、治疗期间不良反应。结果治疗后,2组红细胞、血红蛋白、红细胞比容、血小板计数、D-D、FIB、PaCO_(2)、H-FABP、cTnI、NT-proBNP、PASP、APACHEⅡ低于治疗前,且观察组低于对照组;2组PT、APTT、PaO_(2)、SaO_(2)高于治疗前,且观察组高于对照组(P<0.05,P<0.01)。治疗前后,2组组间和组内BUN、Scr、AST、ALT、TP比较无明显差异(P>0.05)。观察组入院28 d病死率和治疗期间不良反应发生率(7.02%,8/114;7.89%,9/114)与对照组(10.53%,12/114;5.26%,6/114)比较无明显差异(P>0.05)。结论参芎葡萄糖可改善COPD合并PHD患者血常规、凝血指标及血气指标,并降低H-FABP、cTnI、NT-proBNP水平,进而减轻心肌损伤程度,缓解患者病情。 展开更多

目的旨在研究心脏型脂肪酸结合蛋白(H-FABP)与前体B型钠尿肽(pro-BNP)在快速心房纤颤(简称房颤)中的诊断和评估价值。方法应用双抗夹心酶联免疫吸附试验(ELISA)对2021年1月至2022年12月间,潍坊市坊子区人民医院急诊收治的79例快速房颤... 目的旨在研究心脏型脂肪酸结合蛋白(H-FABP)与前体B型钠尿肽(pro-BNP)在快速心房纤颤(简称房颤)中的诊断和评估价值。方法应用双抗夹心酶联免疫吸附试验(ELISA)对2021年1月至2022年12月间,潍坊市坊子区人民医院急诊收治的79例快速房颤患者的血浆中H-FABP和pro-BNP进行检测。进一步结合患者的临床表现、心功能检测进行综合分析。结果在阵发性、持续性和永久性房颤患者中,血浆H-FABP的平均含量分别为(7.0±1.7)ng/mL、(8.3±2.2)ng/mL和(9.5±3.0)ng/mL,而pro-BNP的平均含量则分别为(819±125)pg/mL、(1630±215)pg/mL和(2189±165)pg/mL。阵发性房颤患者经过胺碘酮转复心律后,H-FABP和pro-BNP水平均显著下降至(6.2±1.1)ng/mL和(561±117)pg/mL。根据NYHA心功能分级,Ⅲ级和Ⅳ级患者的H-FABP和pro-BNP含量显著高于心功能较好的患者。结论H-FABP和pro-BNP的血浆水平与快速房颤患者的心功能状态显著关联,因此有潜力作为房颤诊断和心功能评估的生物标志物。 展开更多

Effects of 8-OHdG,H-FABP and CRP on the Development of Ischemic Cardiomyopathy

[Objectives]To analyze the relationship between serum 8-hydroxydeoxyguanosine(8-OHdG),heart fatty acid-binding protein(H-FABP),C-reactive protein(CRP)levels and clinical efficacy and short-term prognosis in patients with ischemic cardiomyopathy.[Methods]The clinical data of 100 patients with ischemic cardiomyopathy from October 2021 to November 2022 were retrospectively analyzed,and the serum levels of 8-OHdG,H-FABP and CRP were compared before and one week after treatment.The patients were followed up for 12 months after discharge,and the incidence of major adverse cardiovascular events(MACE)was counted during the follow-up period.Univariate and multivariate Logistic regression analysis were used to analyze the prognostic factors of patients with ischemic cardiomyopathy in the near future,and the predictive value of serum 8-OHdG,H-FABP and CRP levels for the prognosis of patients was evaluated by ROC curve.[Results]After 1 week of treatment,the serum levels of 8-OHdG,H-FABP and CRP in patients with ischemic cardiomyopathy were significantly lower than those before treatment(P<0.05).During the follow-up period,34 patients developed MACE;the serum levels of 8-OHdG,H-FABP and CRP in the MACE group were higher than those in the non-MACE group,and the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that 8-OHdG,H-FABP and CRP were the risk factors of MACE in patients with ischemic cardiomyopathy(P<0.05).ROC curve analysis showed that the combined prediction of 8-OHdG,H-FABP and CRP for MACE in patients with ischemic cardiomyopathy was higher than that of CRP,H-FABP and 8-OHdG alone(P<0.05).[Conclusions]8-OHdG,H-FABP and CRP are closely related to the clinical efficacy and short-term prognosis of patients with ischemic cardiomyopathy,and the detection of serum 8-OHdG,H-FABP and CRP levels can help to evaluate the clinical efficacy and short-term prognosis of patients with ischemic cardiomyopathy. 展开更多

慢性心力衰竭患者血清sFGL2、AFABP与心力衰竭严重程度及预后的相关性研究

目的探究慢性心力衰竭患者血清可溶性纤维蛋白原2(sFGL2)、脂肪细胞型脂肪酸结合蛋白(AFABP)与心力衰竭严重程度及预后的相关性研究。方法选取2020年6月至2022年6月于唐山市人民医院(下称该院)治疗的102例确诊的慢性心力衰竭患者为患病... 目的探究慢性心力衰竭患者血清可溶性纤维蛋白原2(sFGL2)、脂肪细胞型脂肪酸结合蛋白(AFABP)与心力衰竭严重程度及预后的相关性研究。方法选取2020年6月至2022年6月于唐山市人民医院(下称该院)治疗的102例确诊的慢性心力衰竭患者为患病组,根据病情严重程度分为Ⅱ级组(28例)、Ⅲ级组(35例)和Ⅳ级组(39例),根据预后是否出现不良心血管事件(MACE)进行分组,分为MACE组(44例)和无MACE组(58例),另选取102例同期于该院体检的健康者为对照组。比较各组血清sFGL2、AFABP的水平;Logistic回归分析患者发生预后不良的影响因素;受试者工作特征(ROC)曲线分析血清sFGL2、AFABP水平对患者发生预后不良的预测价值。结果与对照组相比,患病组血清sFGL2水平显著降低(P<0.05),AFABP水平显著升高(P<0.05);Ⅱ级组、Ⅲ级组、Ⅳ级组血清sFGL2水平依次降低(P<0.05),AFABP水平依次升高(P<0.05);MACE组的心率、sFGL2水平显著低于无MACE组(P<0.05),心房颤动占比、AFABP水平显著高于无MACE组(P<0.05);心率、心房颤动、AFABP为患者发生MACE的独立危险因素(P<0.05),sFGL2为患者发生MACE的独立保护因素(P<0.05);血清sFGL2、AFABP预测患者发生MACE的曲线下面积(AUC)分别为0.842、0.858,二者联合预测的AUC为0.943,二者联合预测优于各自单独预测(Z二者联合-sFGL2=2.898,P=0.004;Z二者联合-AFABP=2.608,P=0.009)。结论慢性心力衰竭患者血清sFGL2水平下调,AFABP水平上调,且为患者发生MACE的影响因素,二者联合预测患者预后具有更高效能。 展开更多
