Introduction: Human papillomavirus (HPV) infection is the most widespread sexually transmitted infection in the world. Today, there is growing evidence that HPV can be transmitted early in life, and one potential route is mother-to-child transmission. Data on this route of HPV transmission are scarce in Africa and particularly in Burkina Faso, where no data on the subject are yet available. The aim of our study was to estimate the rate of mother-to-child transmission of HPV infection and to identify circulating genotypes. Methodology: Cervico-uterine samples were collected from 100 full-term pregnant women and, buccal samples were obtained from their newborns at Hopital Saint Camille de Ouagadougou (HOSCO) by the specialist physician. HPV DNA amplification and genotyping were performed by PCR followed by hybridization using the HPV Direct Flow Chips kit, detecting 36 genotypes including 18 high-risk and 18 low-risk. Results: The prevalence of HPV in newborns was 8% (8/100). Six (6) HPV-positive neonates had HPV-positive mothers, while 2 HPV-positive neonates had HPV-negative mothers. The vertical transmission rate was 26.09% (6/23). Mother-newborn genotypes were concordant. However, the genotype profile of the newborns was more restricted than that of the mothers. Conclusion: HPV DNA was found in 8% of newborns in our study. The genotype profile of the mother-newborn pair was concordant. Asymptomatic HPV infection in a pregnant woman could constitute a risk factor for vertical transmission. 展开更多

Research Methods: The cervical exfoliated cell specimens and their clinical data were collected. The HPV infection types of the collected specimens were detected by fluorescence quantitative PCR, and the correlation between HPV infection and clinicopathological features was analyzed statistically. Results: 725 cases were HR-HPV positive from 2605 cases, including 15 high-risk types of HPV16, 18, 31, 33, 35, 39, 45, 51, 52, 53, 56, 58, 59, 66, 68. Different histological types ranged from NILM to HSIL, and the positive rate of HPV showed an increasing trend with the aggravation of cervical lesions. Conclusion: The positive rate of 15 high-risk HPV types in the collected specimens was 27.8%. Patients with early cervical lesions could be screened for 15 high-risk HPV infection types. 展开更多

Background: The incidence of cervical cancer is high in Bangladesh and there is a high prevalence of preinvasive lower genital tract disease among women of reproductive age. Persistent high-risk Human Papilloma Virus (HPV) infection is the main underlying cause of cervical cancer and its precursor, cervical intraepithelial neoplasia (CIN). Objective: The aim of the study was to identify the subtypes of high-risk HPV infection among women with the colposcopic diagnosis of cervical intraepithelial neoplasia in Bangladesh. Methods: This cross-sectional observational study was conducted in the colposcopy clinic of Dhaka Medical College Hospital over a six-month period. A total of 100 participants were enrolled. Married women, between 30 - 60 years of age with colposcopically diagnosed cervical intra epithelial neoplasia were enrolled. Women with chronic illness, pregnancy, and women unable to consent were excluded from this study. After counselling, colposcopically directed punch biopsies were taken from each CIN case concurrently with high-risk HPV testing by polymerase chain reaction (PCR). Results: The mean age of the patients was 38.69 (SD ±7.76) years. CIN 1 was diagnosed in 57% of participants, while 24% had CIN II and 19% had CIN III lesions. High-risk HPV was present in 52 patients. HPV 16 was the most common identified in 28 (53.84%) and HPV 18 was the second most common with 20 (38.46%) either singly or in combination with other high-risk subtypes. The other HPV strains, HPV 31, 33, 35, 52, 56 and 58, were also detected either as mono or co-infections. Out of the 52 HPV positive cases, 29 (55.8%) had mono infection and 23 (44.2%) had co-infection with several subtypes. The highest incidence (50%) of oncogenic HPV infections was present among women aged 35 - 45 years. Risk factors associated with HPV positive cases were high parity (P 0.05), early age at marriage (P = 0.754) and early age of first child. Conclusion: This study identified a high prevalence of HPV 16 and 18 genotypes. HPV vaccination with the current 9-valent HPV vaccine, which contains HPV types 6, 11, 16, 18, 31, 33, 45, 52, and 58. Will be an effective public health measure to eradicate cervical cancer in Bangladesh. 展开更多

Objective: To access the performance of the Tellgenplex human papillomavirus(HPV) DNA test compared to the polymerase chain reaction-reverse dot blot(PCR-RDB) assay for the HPV genotyping.Methods: Sixty cervical swab samples were genotyped by the Tellgenplex HPV DNA test and the PCR-RDB assay.The Tellgenplex HPV DNA test and the PCR-RDB assay can detect 26 and 23 HPV genotypes, respectively.Each sample showed discrepancy was genotyped using sequencing.Results: The percent agreement between the two tests ranged from 83.3% to 100.0% according to different genotype.This showed perfect agreement(>0.81) for high-risk HPV genotypes(35, 39, 45, 53, 56, 59, 66, 68, and 82), substantial agreement(>0.65) for high-risk HPV genotypes(16, 18, 33, 52, and 58) and low-risk HPV genotype 43 between the two assays by the kappa analysis.The positive rates of the two assays for frequent HPV genotypes(16, 35, 39, 45, 52, 53, 58, 59, 66, and 82) were not statistically different, but the PCR-RDB assay showed higher positive rates than the Tellgenplex HPV DNA test for HPV genotypes 81(P<0.05).As for more than 10 positive results by the Tellgenplex HPV DNA test and/or the PCR-RDB assay, the PCR-RDB assay showed higher relative sensitivity and specificity than the Tellgenplex HPV DNA test for the three HPV genotypes(16, 52, and 81).All HPV genotypes that can be detected by only the Tellgenplex HPV DNA test(HPV genotypes 44 and 55) were confirmed by sequencing.Conclusions: In conclusion, our results demonstrated that the PCR-RDB assay which can detect more multiple HPV genotypes in each specimen shows higher relative sensitivity and specificity than the Tellgenplex HPV DNA test, which makes it a better option for routine clinical use. 展开更多

The specimens of 111 cervical carcinomas. 68 chronic cervicitis and 43 normal cervical exfoliated epithelial cells were examined for the presence of HSV2 DNA sequences with DNA hybridization using HSV2 BgL Ⅱ N fragment probe labelled by 32PdCTP. The result showed that the infection rates of HSV2 in the samples of cervical cancer.chronic cervicitis and normal epithelial cells were 1 4. 41 %(16/111). 27.94%( 19/68) and 25.58% ( 11/43),respectively. It was implied that early stages carcinogenesis of cervical epithelial cells might be correlated with the HSV2 infection.Sixteen HSV 2 positive samples of cervical carcinomas were also examined for the presence of the sequences homologous to human papillomavirus (HPV) type 6B/11. 16 and 18 DNA using dot blot hybridization (Tm17℃). The result indicated that 13 out of 16 were HPV 16 DNA hybridization positive accounting for 81. 2% of all HSV-2 positive samples and none of them were positive for HPV type 6B/11 and 18. HPV16L1 gene was cloned into vector pPICZ,αB. HPV16 L1 protein expression induced by methanol was screened by using sodium dedecyl sulfate-polyacrylamide gel electrophoresis (SDSPAGE) and Western blotting. The results indicate that the HPVl6 L1 protein is secreted by the recombinant P. pastoris, and the purified HPV16 L1 protein can self-assemble into vires-like particles( VLPs), which show a good immunogenicity and induces high-titer antibody in mice. 展开更多

目的 探讨团体认知行为干预(cognitive behavioral therapy, CBT)对宫颈高危型人乳头瘤病毒(high risk human papillomavirus, HR-HPV)持续感染患者临床转归的影响,构建针对情绪障碍的HR-HPV持续感染患者的系统性心理干预治疗方案。方法... 目的 探讨团体认知行为干预(cognitive behavioral therapy, CBT)对宫颈高危型人乳头瘤病毒(high risk human papillomavirus, HR-HPV)持续感染患者临床转归的影响,构建针对情绪障碍的HR-HPV持续感染患者的系统性心理干预治疗方案。方法 选取HR-HPV持续感染合并情绪障碍患者,按照随机数字表法分为干预组(n=44)和对照组(n=45):对照组给予健康宣教、随访监测,干预组在此基础上给予CBT干预,比较两组的临床转归及心理量表评分。结果 随访6月及12月,HR-HPV感染消退率干预组高于对照组;而HR-HPV持续感染率及进展率干预组低于对照组(P<0.05)。抑郁症筛查量表(PHQ-9)及广泛性焦虑量表(GAD-7)评分,干预结束后即刻、随访6月及12月,干预组均低于对照组(P<0.05);干预组干预结束后即刻、随访6月及12月评分均低于干预前(P<0.05);对照组干预前后评分差异无统计学意义(P>0.05)。结论 HR-HPV持续感染患者进行系统性心理干预治疗,有助于HR-HPV感染消退。 展开更多

目的分析新疆乌鲁木齐市女性人乳头瘤病毒(HPV)感染特点及HPV分型定量检测对宫颈病变的预测性能。方法收集2022年1-12月于新疆医科大学第一附属医院妇科就诊并进行HPV检测的11654例女性的HPV检测结果。根据年龄将所有就诊者分为低龄组(... 目的分析新疆乌鲁木齐市女性人乳头瘤病毒(HPV)感染特点及HPV分型定量检测对宫颈病变的预测性能。方法收集2022年1-12月于新疆医科大学第一附属医院妇科就诊并进行HPV检测的11654例女性的HPV检测结果。根据年龄将所有就诊者分为低龄组(≤29岁)、青龄组(>29~39岁)、中龄组(>39~49岁)、中老龄组(>49~59岁)和老龄组(>59岁)。其中1483例患者同时进行阴道镜活检和宫颈组织病理学检测,根据组织病理学诊断结果,将1483例患者分为宫颈炎、宫颈上皮内瘤变(CIN)Ⅰ、CINⅡ、CINⅢ和宫颈癌,将CINⅠ及以上命名为CIN阳性,CINⅡ及以上命名为CINⅡ+,CINⅢ及以上命名为CINⅢ+。采用实时荧光定量PCR检测HPV亚型及病毒载量,比较不同民族、年龄、宫颈疾病女性HPV的检测结果,比较不同宫颈病变患者主要的HPV亚型病毒载量水平,采用受试者工作特征(ROC)曲线确定主要的HPV亚型预测CINⅡ+的病毒载量阈值。结果在11654例就诊者中,HPV感染率为30.60%,单一亚型感染占77.73%。HPV16、52、58是CIN阳性患者感染的主要型别,分别占38.89%、14.90%、11.11%。汉族、维吾尔族、回族和哈萨克族女性HPV感染率分别为31.89%、24.39%、31.76%和27.88%,4个民族HPV感染率比较,差异有统计学意义(χ^(2)=40.929,P<0.001)。在11654例就诊者中,老龄组HPV感染率最高(34.04%),中龄组HPV感染率最低(26.41%),不同年龄组间HPV感染率比较,差异有统计学意义(χ^(2)=37.754,P<0.001)。在396例CIN阳性患者中,高危型HPV和单一亚型感染率随CIN级别升高而升高(χ^(2)趋势=9.994、7.233,P<0.01)。CINⅡ+患者HPV16、52、58病毒载量与宫颈炎患者相比,差异均有统计学意义(P<0.05);且HPV16、52、58病毒载量预测CINⅡ+的最佳截断值分别为4.382、3.694、4.114 copy/104个细胞(经lg转化)。结论新疆乌鲁木齐市女性HPV感染以单一亚型感染为主,HPV16、52、58是主要的感染型别,并且HPV标准化定量具有作为宫颈高级病变预测标志物的潜力。 展开更多

早期宫颈癌术后HPV持续感染的危险因素分析

目的分析早期宫颈癌根治术后高危型人乳头瘤病毒(HR-HPV)持续感染的相关危险因素。方法选取2021年1月至2023年11月之间,在昆明市妇幼保健院行开腹或腹腔镜下宫颈癌根治术的159例早期宫颈癌患者,根据术后1a返院复查HR-HPV持续感染情况分... 目的分析早期宫颈癌根治术后高危型人乳头瘤病毒(HR-HPV)持续感染的相关危险因素。方法选取2021年1月至2023年11月之间,在昆明市妇幼保健院行开腹或腹腔镜下宫颈癌根治术的159例早期宫颈癌患者,根据术后1a返院复查HR-HPV持续感染情况分为2组:HR-HPV阳性组(n=41)和HR-HPV转阴组(n=118)。对2组患者的一般资料、阴道微生态情况、手术入路途径、术后病检结果等分析,研究早期宫颈癌患者术后HR-HPV持续阳性的相关危险因素。结果2组的单因素和多因素回归分析显示,绝经、手术期别晚、阴道微生态失调/亚失调是早期宫颈癌患者术后HR-HPV持续阳性的危险因素(P<0.05),而腹腔镜微创手术不是其危险因素。其中阴道微生态异常OR为12.574,绝经OR为3.510。结论绝经、阴道微生态失调为影响早期宫颈癌患者术后HR-HPV清除的重要危险因素。 展开更多

目的:调查备孕女性人乳头瘤病毒(HPV)感染情况并分析HPV疫苗接种影响因素。方法:收集2022年1月-2023年1月本院健康检查的211例备孕女性宫颈样本,采用聚合酶链式反应-反向点杂交法进行HPV感染亚型分型,通过问卷调查备孕女性HPV疫苗接种情... 目的:调查备孕女性人乳头瘤病毒(HPV)感染情况并分析HPV疫苗接种影响因素。方法:收集2022年1月-2023年1月本院健康检查的211例备孕女性宫颈样本,采用聚合酶链式反应-反向点杂交法进行HPV感染亚型分型,通过问卷调查备孕女性HPV疫苗接种情况,分析影响备孕女性HPV疫苗接种影响因素。结果:备孕女性HPV感染率为22.7%,其中高危型HPV感染率为81.25%,主要HPV感染亚型为HPV52(18.8%)、HPV16(10.4%)。HPV单一型感染率为20.6%,多重感染率为14.6%。211例HPV疫苗接种率为14.7%,文化水平低、不知道HPV疫苗是影响备孕女性HPV疫苗接种的影响因素(P<0.05)。结论:本次调查的备孕女性HPV感染率为22.7%,以HPV52、HPV16为主要感染亚型;HPV疫苗接种率较低,应加强对备孕女性相关知识宣传,提高HPV疫苗接种率。 展开更多

宫颈病变患者HPV基因分型检测及其临床意义分析

目的:分析宫颈病变患者人乳头瘤病毒(HPV)基因分型检测及临床意义。方法:回顾性分析2021年5月—2023年5月泰兴市人民医院收治的125例宫颈病变患者的临床资料,均进行HPV基因分型检测。统计125例宫颈病变患者HPV感染阳性率、HPV基因分型情... 目的:分析宫颈病变患者人乳头瘤病毒(HPV)基因分型检测及临床意义。方法:回顾性分析2021年5月—2023年5月泰兴市人民医院收治的125例宫颈病变患者的临床资料,均进行HPV基因分型检测。统计125例宫颈病变患者HPV感染阳性率、HPV基因分型情况(多重感染重复计算)。比较不同程度宫颈病变患者中HPV感染阳性率、高危型HPV感染阳性率。以病理检查为“金标准”,分析并比较液基薄层细胞学检查(TCT)单独诊断、TCT联合HPV基因分型诊断子宫颈癌的准确度、敏感度、特异度。结果:125例宫颈病变患者中,HPV感染阳性率为51.20%(64/125),HPV感染阳性患者中HPV基因型总数为92株,其中高危型HPV为63.04%(58/92),低危型HPV为36.96%(34/92)。高危型HPV感染阳性率最高的前5位基因型分别为HPV16(16.30%)、HPV18(11.96%)、HPV58(8.70%)、HPV53(6.52%)、HPV51(4.35%);低危型HPV感染阳性率最高的前3位基因型分别为HPV6(8.70%)、HPV11(7.61%)、HPV40(4.35%)。不同级别宫颈病变患者HPV感染阳性率、高危型HPV感染阳性率比较,差异有统计学意义(P<0.05)。TCT单独诊断子宫颈癌的准确度为78.40%(98/125),敏感度为66.67%(18/27),特异度为81.63%(80/98),阳性预测值为50.00%(18/36),阴性预测值为89.89%(80/89),Kappa值为0.431;TCT联合HPV基因分型诊断子宫颈癌的准确度为88.00%(110/125),敏感度为59.26%(16/27),特异度为95.92%(94/98),阳性预测值为80.00%(16/20),阴性预测值为89.52%(94/105),Kappa值为0.609;TCT联合HPV基因分型诊断子宫颈癌的准确度、特异度均显著高于TCT单独诊断,差异有统计学意义(P<0.05);TCT联合HPV基因分型诊断子宫颈癌的敏感度低于TCT单独诊断,但差异无统计学意义(P>0.05)。结论:宫颈病变类型与HPV基因分型密切相关,且不同宫颈病变恶性程度患者HPV感染阳性率、高危型HPV感染阳性率存在显著差异,因此HPV基因分型用于辅助子子宫颈癌筛查具有重要意义。 展开更多

目的:探讨锌指蛋白(ZNF)772甲基化及高危型人乳头瘤病毒(HR-HPV)检测在意义不明的非典型鳞状细胞(ASCUS)病人分流中的意义。方法:选取初筛ASCUS病人195例,均接受HC2-HPV-DNA检测及ZNF772基因DNA甲基化检测;比较宫颈上皮内瘤样变(CIN)Ⅱ... 目的:探讨锌指蛋白(ZNF)772甲基化及高危型人乳头瘤病毒(HR-HPV)检测在意义不明的非典型鳞状细胞(ASCUS)病人分流中的意义。方法:选取初筛ASCUS病人195例,均接受HC2-HPV-DNA检测及ZNF772基因DNA甲基化检测;比较宫颈上皮内瘤样变(CIN)Ⅱ及以上与CINⅡ以下、CINⅢ及以上及CINⅢ以下病人ZNF772基因位点甲基化水平的差异,并评估ZNF772甲基化水平及HR-HPV对ASCUS病人CINⅡ及以上、CINⅢ及以上的分流价值。结果:病理学检查显示,195例ASCUS病人中炎症80例(41.03%)、CINⅠ64例(32.82%)、CINⅡ29例(14.87%)、CINⅢ/CIS/AIS 18例(9.23%)、SCC/AC 4例(2.05%)。HR-HPV(HC2)阳性141例(72.31%)、HR-HPV(HC2)阴性54例(27.69%);CINⅡ及以上病人HR-HPV(HC2)阳性检出率高于CINⅡ以下(P<0.05);CINⅢ及以上病人HR-HPV(HC2)阳性检出率高于CINⅢ以下(P<0.05)。CINⅡ及以上、CINⅢ及以上病人ZNF772基因-420、-422位点甲基化水平分别高于CINⅡ以下、CINⅢ以下病人(P<0.05)。ZNF772基因-420、-422位点甲基化水平及HR-HPV诊断CINⅡ及以上的AUC(95%CI)分别为0.867(0.811~0.911)、0.806(0.743~0.859)、0.777(0.712~0.834),诊断CINⅢ及以上的AUC(95%CI)分别为0.891(0.838~0.931)、0.868(0.813~0.912)、0.780(0.716~0.836)。ZNF772基因-420位点甲基化水平诊断CINⅡ及以上、CINⅢ及以上的AUC面积最大。结论:相较于HC2-HPV-DNA检测,ZNF772甲基化检测更能准确地识别ASCUS中CINⅡ及以上、CINⅢ及以上病变病人,有望成为分流ASCUS的有效替代手段。 展开更多

目的:探讨p16/Ki-67双染检测、HPVE6/E7mRNA检测在宫颈病变诊断中的价值。方法:选择2021年4月至2023年2月我院收治的605例高危型HPV阳性患者为研究对象,所有患者入院后均进行阴道镜检查与病理活检,取患者细胞学样本分别进行p16/Ki-67双... 目的:探讨p16/Ki-67双染检测、HPVE6/E7mRNA检测在宫颈病变诊断中的价值。方法:选择2021年4月至2023年2月我院收治的605例高危型HPV阳性患者为研究对象,所有患者入院后均进行阴道镜检查与病理活检,取患者细胞学样本分别进行p16/Ki-67双染检测、HPVE6/E7mRNA检测。比较两种方法单独检查与联合检测对宫颈病变阳性检出率的差异;比较三种检查方法对宫颈病变诊断效能的差异。结果:联合检测检出CIN1~3阳性率均高于p16/Ki-67双标记、HPVE6/E7mRNA检测(P<0.05);三种检测方式的浸润癌阳性检出率比较无明显差异(P>0.05)。Kappa检验分析结果显示,p16/Ki-67双标记与HPVE6/E7mRNA检查结果与病理诊断结果具有较好的一致性(Kappa值=0.719,P<0.05);联合检测结果与病理诊断结果具有高度的一致性(Kappa值=0.894,P<0.05)。联合检测对宫颈病变的诊断灵敏度、特异性与准确率高于p16/Ki-67双标记与HPVE6/E7mRNA检查(P<0.05)。结论:p16Ki-67双染与HPVE6/E7mRNA联合检测可准确检出宫颈病变程度,对宫颈病变疾病类型的鉴别可提供充足可靠的数据支持,联合检查的诊断效能较好,对患者后续相关治疗开展有积极指导意义。 展开更多

血清Th1/Th2、Hmga1、PKM2表达水平及其联合HPV诊断宫颈癌的价值

目的探讨辅助性T细胞1(Th1)/辅助性T细胞2(Th2)、高迁移率族蛋白A1(Hmga1)、丙酮酸激酶M2型(PKM2)水平及联合人乳头状瘤病毒(HPV)对宫颈癌的诊断价值。方法选取2022年3月至2023年3月在河南省中医院就诊的186例宫颈疾病患者作为研究对象... 目的探讨辅助性T细胞1(Th1)/辅助性T细胞2(Th2)、高迁移率族蛋白A1(Hmga1)、丙酮酸激酶M2型(PKM2)水平及联合人乳头状瘤病毒(HPV)对宫颈癌的诊断价值。方法选取2022年3月至2023年3月在河南省中医院就诊的186例宫颈疾病患者作为研究对象,根据疾病类型分为宫颈炎组(n=62)、宫颈癌前病变组(n=58)和宫颈癌组(n=66),另选取60例同期健康体检者作为对照组,比较四组及宫颈癌前病变组和宫颈癌组合并、未合并HPV感染患者入院时Th1/Th2因子[血清干扰素γ(IFN-γ)、白细胞介素-2(IL-2)、白细胞介素-4(IL-4)、白细胞介素-10(IL-10)]、Hmga1、PKM2水平,分析血清Th1/Th2、Hmga1、PKM2水平表达与肿瘤病理特征的关系及联合HPV对宫颈癌合并HPV感染的诊断价值。结果四组受检者入院(体检)时的血清IFN-γ、IL-2、IL-4、IL-10、Hmga1、PKM2水平比较,宫颈癌组>宫颈癌前病变组>宫颈炎组>对照组,差异均有统计学意义(P<0.05);与未合并HPV感染患者比较,宫颈癌前病变组、宫颈癌组合并HPV感染患者入院时的血清IFN-γ、IL-2、IL-4、IL-10、Hmga1、PKM2水平较高,差异均有统计学意义(P<0.05);不同肿瘤分化程度、临床分期、有无淋巴结转移宫颈癌合并HPV感染患者入院时的血清IFN-γ、IL-2、IL-4、IL-10、Hmga1、PKM2水平比较差异均有统计学意义(P<0.05);HPV感染对宫颈癌诊断灵敏度为84.85%(54/66),特异度为72.41%(16/28),准确度为56.45%(70/124);血清IFN-γ、IL-2、IL-4、IL-10、Hmga1、PKM2、HPV感染对宫颈癌、宫颈癌前病变诊断AUC分别为0.761、0.730、0.745、0.806、0.819、0.707、0.786,HPV感染联合各血清指标诊断AUC为0.908,大于单一指标诊断。结论血清Th1/Th2、Hmga1、PKM2水平检测对宫颈癌具有一定的诊断价值,临床可通过其联合HPV感染情况早期辅助诊断宫颈病变,为临床针对性制定干预方案提供参考。 展开更多

不同治疗方案对宫颈高危型HPV感染患者的治疗效果分析

目的:探讨多种治疗方案对宫颈高危型人乳头瘤病毒(HPV)感染患者的临床治疗效果,为HPV患者的临床治疗提供新思路。方法:采用回顾性研究方法纳入2022年1月—2023年1月在宜昌市中心人民医院治疗的宫颈高危型HPV感染患者501例,根据患者选取... 目的:探讨多种治疗方案对宫颈高危型人乳头瘤病毒(HPV)感染患者的临床治疗效果,为HPV患者的临床治疗提供新思路。方法:采用回顾性研究方法纳入2022年1月—2023年1月在宜昌市中心人民医院治疗的宫颈高危型HPV感染患者501例,根据患者选取的治疗方法分为三组,A组患者给予保妇康栓治疗(n=167),B组患者给予重组人干扰素α-2b凝胶治疗(n=167),C组给予抗HPV生物蛋白敷料治疗(n=167)。比较三组HPV患者治疗有效率、HPV-DNA水平、治疗后HPV阳性率和治疗过程中不良反应发生率,评价临床治疗效果。结果:C组患者治疗总有效率明显高于A组(90.42%vs 74.85%)和B组(90.42%vs 77.25%)(均P<0.05)。三组患者治疗3个月及6个月HPV-DNA水平较治疗前明显下降,且治疗3个月及6个月时,C组患者的HPV-DNA水平较A组和B组明显降低(均P<0.05)。治疗后C组HPV阳性率明显低于A组(6.60%vs 22.75%)和B组(6.60%vs 20.96%)(均P<0.05)。三组患者不良反应发生情况无明显统计学差异(P>0.05)。结论:抗HPV生物蛋白敷料对于宫颈高危型HPV感染患者的治疗效果优于保妇康栓及重组人干扰素α-2b凝胶,且不良反应发生少,值得临床上进一步推广。 展开更多

Objective: Precise prevention is more desired for cervical cancer due to the huge population, high prevalence of human papillomavirus(HPV) infection in China and the vision of screen-and-treat strategies in low-and middleincome countries(LMICs). Considerations of combining type-specific prevalence and attribution proportion to high-grade cervical intraepithelial neoplasia are informative to more precise and effective region-specific cervical cancer prevention and control programs. The aim of the current study was to determine the genotype distribution of HPV and attribution to cervical precancerous lesions among women from rural areas in North China.Methods: A total of 9,526 women participated in the cervical cancer screening project in rural China. The samples of women who tested positive for HPV were retested with a polymerase chain reaction(PCR)-based HPV genotyping test. The attribution proportion of specific high-risk human papillomavirus(HR-HPV) types for different grades of cervical lesions was calculated by using the type contribution weighting method.Results: A total of 22.2%(2,112/9,526) of women were HR-HPV positive and HPV52(21.7%) was the most common HR-HPV genotype, followed by HPV58(18.2%), HPV53(18.2%) and HPV16(16.2%). The top three genotypes detected in HR-HPV-positive cervical intraepithelial neoplasia(CIN)1 were HPV16(36.7%), HPV58(20.4%), HPV56(15.3%). Among CIN2+, the most frequent genotypes were HPV16(75.6%), HPV52(17.8%),HPV58(16.7%). HPV16, 56, 58, 53, 52, 59, 68, and 18 combined were attributed to 84.17% of all CIN1 lesions,and HPV16, 58, and 52 combined were attributed to 86.98% of all CIN2+ lesions.Conclusions: The prevalence of HR-HPV infection among women from rural areas in North China was high and HPV16, HPV58, HPV52 had paramount attributable fraction in CIN2+. Type-specific HPV prevalence and attribution proportion to cervical precancerous lesions should be taken into consideration in the development of vaccines and strategy for screening in this population. 展开更多

Objective:To study the expression of E6 and E7 mRNA in high-risk human papillomavirus(HPV) HPV-18 and the relationship between the expression of invasive gene and cervical carcinoma.Methods:A total of 119 patients with cervical cancer,cervical erosion and cervical HPV infection who were diagnosed in our hospital were selected and randomly divided into two groups:cervical cancer group(n= 58) and non-cancerous group(n= 61).Another 60 patients with uterine leiomyoma were selected as normal control group.Detection of HPV18 E6,E7 mRNA expression and invasion,migration,proliferation inhibition genes,epithelial mesenchymal transition genes and proliferation related protein content.Results:The relative expression of E6 and E7 HPV-18 in cervical cancer group was significant higher than that in non-cancerous group and control group(mRNA)(P<0.05).The content of TRAF6 and c-FLIP in invasive cervical cancer group was significantly higher than that in non-cancerous group and control group(P<0.05).The mR NA content of CD44v6 and MMP-9 in cervical cancer group was significantly higher than that in non-cancerous group and control group(P<0.05).The content of DEC-1,IKK16,MBP-1 in cervical cancer group was significant lower than that in non-cancerous group and control group(P<0.05).The mR NA content of beta-catenin and Vimentin in cervical cancer group was significantly lower than that in non cancerous group and control group(P<0.05).The proliferation related protein E2F1 of cervical cancer group was significantly lower than that of non-cancerous group and control group,Bmi-1 content was significantly higher than non-cancerous group and control group(P<0.05).Conclusions:The expression of the detection of cervical cancer in high-risk human papilloma virus HPV-18 E6 and E7 mRNA,and the invasion,migration,proliferation inhibition gene,epithelial mesenchymal transition and proliferation related gene protein content,HPV expression rate of mR NA increased with the development of cervical cancer,the expression is also enhanced.The expression has a certain correlation between the level and development of cervical cancer.Through the above indicators,the development of cervical cancer monitoring and treatment to provide important clinical guidance. 展开更多

AIM:To investigate the relationship between human papillomavirus (HPV) infection and concurrent esophagus and gastric cardia cancer from the same patient (CC) and examine the significance of P16 INK4A protein expression.METHODS:Polymerase chain reaction was used to detect the presence of HPV type16 (HPV16).The expression of P16 INK4A protein was detected using immunohistochemistry.RESULTS:Among the CC specimens,HPV16-DNA was found in eight cases of esophageal squamous cell carcinoma (ESCC) and five cases of gastric cardia adenocarcinoma (GCA),respectively (47% vs 29%),and two of both ESCC and GCA.P16 INK4A was highly expressed in both ESCC and GCA.In the HPV-associated positive CC,higher P16 INK4A expression was observed in the GCA than in the ESCC (75% vs 25%,P < 0.05).CONCLUSION:HPV16 as a correlated risk factor may play an important role in the development of ESCC and GCA.P16 INK4A may be a screening index in the HPVassociated carcinoma of gastric cardia. 展开更多
