Methods Sixty patients with cervical cancer complicated with pelvic lymph node metastasis who were admitted to our hospital from January 2013 to January 2015 were enrolled. The patients were randomly divided into the late-course dose-increasing IMRT group and the simultaneous integrated dose-increasing IMRT group, with 30 cases included in each group, respectively. All patients were concurrently treated with cisplatin. After treatment, the clinical outcomes of the two groups were compared. Results The remission rate of symptoms in the simultaneous integrated dose-increasing IMRT group was significantly higher than that in the late-course dose-increasing IMRT group(P < 0.05). The follow-up results showed that the overall survival time, progression-free survival time, and distant metastasis time of patients in the simultaneous integrated dose-increasing IMRT group were significantly longer than those in the late-course dose-increasing IMRT group(P < 0.05). The recurrent rate of lymph nodes in the radiation field in the simultaneous integrated dose-increasing IMRT group was significantly lower(P < 0.05) than in the late-course dose-increasing IMRT group. There was no significant difference in the incidence of cervical and vaginal recurrence and distant metastasis between the two groups(P > 0.05). The radiation doses of Dmax in the small intestine, D1 cc(the minimum dose to the 1 cc receiving the highest dose) in the bladder, and Dmax in the rectum in the simultaneous integrated dose-increasing IMRT group were significantly lower(P < 0.05) than in the late-course dose-increasing IMRT group. There was no significant difference in intestinal D2 cc(the minimum dose to the 2 cc receiving the highest dose) between the two groups(P > 0.05). The incidence of bone marrow suppression in the simultaneous integrated dose-increasing IMRT group was significantly lower(P < 0.05) than in the late-course dose-increasing IMRT group.Conclusion The application of simultaneous integrated dose-increasing IMRT in the treatment of cervical cancer patients complicated with pelvic lymph node metastasis can significantly control tumor progression, improve the long-term survival time, and postpone distant metastasis time with high safety.展开更多
Based on the data from the Cost-benefit Data of Farm Produce and the China Agricultural Yearbook, this paper divided the intensity of cultivated land use into labor intensity and capital intensity, and then analyzed t...Based on the data from the Cost-benefit Data of Farm Produce and the China Agricultural Yearbook, this paper divided the intensity of cultivated land use into labor intensity and capital intensity, and then analyzed their temporal and spatial change at both national and provincial levels between 1980 and 2006. The results showed that: (1) At the national level, labor intensity on food produce decreased from 398.5 day/ha in 1980 to 130.25 day/ha in 2006; and a continuous decrease with a steep decline between 1980 and 1986, a slower decline from 1987 to 1996, and another steep decline from 1997 to 2006. On the contrary, capital intensity shows an increasing trend since 1980. As to the internal composition of capital intensity, the proportion of seed, chemical fertilizer and pesticide input decreased from 90.36% to 73.44% and the proportion of machinery increased from 9.64% to 26.56%. The less emphasis on yield-increasing input and more emphasis on labor-saving input are the main reasons for a slow increase of yield per unit area after 1996. (2) At the provincial level, the developed areas have lower labor intensity and higher capital intensity. The less developed ones have higher labor intensity but lower capital intensity. From the viewpoint of the internal composition of capital intensity, labor-saving input accounts for more proportion in the developed areas than that of other areas. The main reason is that in these developed areas, labor input has become a constraint factor in food production as more and more labors engaged in off-farm work. Farmers increase the labor-saving input for higher labor productivity. However, in the less developed areas, the major constraint is the shortage of capital; food production is still depending on labor and yield-increasing inputs.展开更多

The aim of this study was to investigate the correlation between ISI position and the outcome of surgical treatment for cervical spondylotic myelopathy (CSM) patients. Methods A retrospective study was conducted. Pre- and post-operative clinical status was evaluated by modified Japanese Orthopaedic Association (JOA) score. ISI was evaluated according to the T2-weighted sequences. The JOA score and the recovery ratios among patients with ISI in gray matter (group A), in both gray and white matter (group B), and ISI-negative group were compared. Results Totally 64 patients were enrolled in this retrospective study. Preoperative JOA score of ISI positive and negative group had significant difference, but the recovery ratios had no significant difference (the recovery ratios of the two groups in week 1, week 26, and week 104 were (21.54±14.65)%, (50.56±14.76)%, (59.23±13.08)% and (20.25±14.32)%, (54.46±3.16)% and (61.26±29.4)%, respectively; P 〉0.05). The recovery ratios of negative group and group A in week 104 were superior to group B (the recovery ratios of negative group, group A, and group B in week 104 were (61.26±29.49)%, (65.35±11.36)%, and (50.33±10.20)%, respectively; P 〈0.05). Conclusions Patients with ISI in the gray matter alone on T2-weighted MR images did not have significantly different surgical outcomes compared with those without ISI.
