There is rising incidence and prevalence of inflammatory bowel disease(IBD) in India topping the Southeast Asian(SEA) countries. The common genes implicated in disease pathogenesis in the West are not causal in Indian...There is rising incidence and prevalence of inflammatory bowel disease(IBD) in India topping the Southeast Asian(SEA) countries. The common genes implicated in disease pathogenesis in the West are not causal in Indian patients and the role of "hygiene hypothesis" is unclear. There appears to be a North-South divide with more ulcerative colitis(UC) in north and Crohn's disease(CD) in south India. IBD in second generation Indian migrants to the West takes the early onset and more severe form of the West whereas it retains the nature of its country of origin in migrants to SEA countries. The clinical presentation is much like other SEA countries(similar age and sex profile, low positive family history and effect of smoking, roughly similar disease location, use of aminosalicylates for CD, low use of biologics and similar surgical rates) with some differences(higher incidence of inflammatory CD, lower perianal disease, higher use of aminosalicylates and azathioprine and lower current use of corticosteroids). UC presents more with extensive disease not paralleled in severity clinically or histologically, follows benign course with easy medical control and low incidence of fulminant disease, cancer, complications, and surgery. UC related colorectal cancer develop in an unpredictable manner with respect to disease duration and site questioning the validity of strict screening protocol. About a third of CD patients get antituberculosis drugs and a significant number presents with small intestinal bleed which is predominantly afflicted by aggressive inflammation. Biomarkers have inadequate diagnostic sensitivity and specificity for both. Pediatric IBD tends to be more severe than adult. Population based studies are needed to address the lacunae in epidemiology and definition of etiological factors. Newer biomarkers and advanced diagnostic techniques(in the field of gastrointestinal endoscopy, molecular pathology and genetics) needs to be developed for proper disease definition and treatment.展开更多

METHODS: Polymerase chain reaction (PCR) amplification using Y-specific STS of azoospermia factor (AZF) regions i.e., SY 84 for AZFa, SY 127 for AZFb and SY 254 for AZFc. RESULTS: Of the 20 infertile subjects 3 (15 %), one azoospermic and two oligozoospermic, showed microdeletions in the AZF region of Y-chromosome. CONCLUSION: The frequency of deletions involving AZF region of the Y-chromosome is 15 % in azoospermic and severely oligozoospermic infertile men. PCR amplification of AZF locus is useful for the diagnosis of microdeletions in the Y-chromosome.展开更多

We report an Indian family where two siblings were homozygous for a known mutation of the AAAS gene and presented with the classical triad of symptoms. The mother and the brother were heterozygous and asymptomatic. The affected siblings had iron deficiency anemia and the younger sister had pes cavus and palmoplantar keratosis. Neurological symptoms were absent in both affected children. Recognition of this syndrome can lead to early treatment of adrenal insufficency and genetic counselling.展开更多

The case report also shows that when treated, the benefits to the patient and their family are lifelong and disproportionately large compared to the degree of effort and cost of doing the reconstruction;and puts forth the case for educating physicians to maximize their societal benefit through work in resource-constrained settings.展开更多

Self-transcendence can be re...Since India's philosophical discussion on the correlation between the mind and the body, the goal achieved through practice has been imbued with much meaning under the influences of yoga. Self-transcendence can be reached through self-discipline that is founded upon the fundamental change and potential for existential practice. The mind and the body do not comprise a binary relationship but instead maintain an intimate relationship bound by the medium of organic energy of breathing, breath, and pr^0a. Of these medium that form the relationship, the pr^-0a has a natural healing tendency, which has been well documented within classics such as Upani.sads and Bhagavad G^ta. Classic yoga explains that pr^0a imbues within it a vivacious passion and energy that will connect the human body with the human mind. Thus, the pr^0a functions as bio-energy. From ancient Indian philosophy to Buddhism, yoga therapy actively utilizes practice as a main element. In particular, the Patafijali yoga's methodology details the organic correlation within its eight limbs (支) yoga practices. The prana is breath. This is the only sense in which Patafijali has used this word. Breath is called prana because it moves on continuously throughout the body and reaches the basic circle of the individual self. It pervades mouth, nostrils, heart, navel region and toes. As a result, it is a healing method that achieves far more than the scientific healing methods common in society.展开更多
