Objective: To describe the history of an...Background: Skin reactions and musculoskeletal injuries caused by intramuscular injection are an increased risk for skin and soft tissue infections and may culminate in sepsis. Objective: To describe the history of an elderly patient who developed a lesion in her right arm after home administration of intramuscular iron injection in this arm and its outcome. Methods: This is a case report of a patient who participated in a study in an intensive care unit. Results: This study describes a 65-year-old patient with multiple comorbidities who was admitted to the hospital complaining of pain for two months and difficulty in moving her right shoulder associated with skin and soft tissue infections in her right arm after intramuscular iron administration by a relative for the treatment of multifactorial anemia and refractory melena. The patient worsened her general condition and was transferred to the intensive care unit. Despite the therapy instituted, the patient developed sepsis of cutaneous origin, multiple organ dysfunction, and death. Conclusions: The administration of intramuscular medication requires the participation of qualified professionals, such as nurses, mainly within the scope of the Brazilian public health system which commonly comprises patients in a context of vulnerability.展开更多

Background:Gut microbiota plays a key role in health,immunity,digestion,and production in layers.Factors such as environment,diet,diseases,stress,and flock management significantly affect gut microbiota;however,it is ...Background:Gut microbiota plays a key role in health,immunity,digestion,and production in layers.Factors such as environment,diet,diseases,stress,and flock management significantly affect gut microbiota;however,it is not known how potential stressors such as intramuscular injections or feed withdrawal alter the composition of gut microbiota that result in increased the shedding level of foodborne pathogens.In the current study,the effects of intramuscular corticosterone injection and feed withdrawal were evaluated to understand their role in Salmonella Typhimurium shedding and changes in the composition of gut microbiota in layers.Results:Salmonella shedding was observed for 8 weeks post-infection.There was a significant increase in Salmonella Typhimurium count after intramuscular injection and feed withdrawal.The Salmonella infected and the negative control groups showed significant differences in the abundance of different genera in gut microbiota at week 1 and up to week 7 post infection.The infected group showed a significant reduction in alpha diversity of gut microbiota.Firmicutes reduced significantly(P<0.05)after intramuscular injection,while the feed withdrawal groups did not cause any significant changes in Proteobacteria-Firmicutes ratio.Furthermore,intramuscular injection resulted in a significant change in alpha diversity of gut microbiota.Conclusions:Exposure of chicks to relatively low dose of Salmonella Typhimurium can lead to persistent shedding in pullets.The Salmonella Typhimurium infection disrupted the gut microbiota composition immediately after infection.The potential stress of intramuscular injection and feed withdrawal significantly increased the Salmonella Typhimurium count in faeces.The intramuscular injection also resulted in a significant alteration of the Proteobacteria-Firmicutes ratio,which could increase the risk of dysbiosis.展开更多
BACKGROUND The incidence of infection with Mycobacterium abscessus(M.abscessus)has increased in recent years.This increase is partly associated with invasive cosmetic procedures.CASE SUMMARY The purpose of this case s...BACKGROUND The incidence of infection with Mycobacterium abscessus(M.abscessus)has increased in recent years.This increase is partly associated with invasive cosmetic procedures.CASE SUMMARY The purpose of this case summary is to increase clinicians'awareness of M.abscessus infection and reduce mycobacterial infection caused by cosmetic procedures.We report the case of a 45-year-old woman who received acetyl hexapeptide-8(argireline)injections in the forehead and temples,and erythema,nodules,and abscesses appeared at the injection sites after one week.The pus specimens were examined by microbiological culture and confirmed to be positive for M.abscessus.Clarithromycin 500 mg twice daily and moxifloxacin 400 mg once daily were administered for 5 mo and the lesions gradually subsided.CONCLUSION We report here for the first time a case of infection with M.abscessus after argireline injection.This condition is easily misdiagnosed as a common bacterial infection.Microbiological examinations are helpful for diagnosis and standardized cosmetic procedures can prevent infection with M.abscessus.展开更多
BACKGROUND In recent years,the cosmetic intervention related infections caused by nontuberculous mycobacteria(NTM)are increasing as the informal cosmetic treatments are performed.However,many dermatologists are inexpe...BACKGROUND In recent years,the cosmetic intervention related infections caused by nontuberculous mycobacteria(NTM)are increasing as the informal cosmetic treatments are performed.However,many dermatologists are inexperienced in the diagnosis and management of similar cases.Here we report a case of subcutaneous infection caused by Mycobacterium abscessus(M.abscessus)following cosmetic injections of botulinum toxin.CASE SUMMARY A 53-year-old woman presented with multiple abscesses and nodules on her forehead and both temporal sites for half a month after cosmetic injections of botulinum toxin.Her lesions did not show any alleviation after 2-wk prescription of antibiotics.Laboratory examinations indicated that she had no sign of immunodeficiency and the whole body of computed tomography did not find any systemic infection or diseases.The pathology of skin tissue showed inflammatory cell infiltration with the negative results of Periodic acid Schiff(PAS)and Acidfast staining and the culture yielded no microbiome.Afterwards,the puncture on abscess was performed and M.abscessus was successfully isolated.The pathogen was identified by acid-fast staining and DNA sequencing.The patient was treated with the strategy of clarithromycin,ofloxacin,and amikacin according to the result of drug sensitivity test and got complete remission of the lesions.CONCLUSION The case presents the whole process of diagnosis and management of NTM infection after cosmetic intervention and highlights the diagnostic thoughts.In a word,the mycobacterium infection should be aware in patients after cosmetic performance.展开更多
By using a high-intensity flying focus laser,the dephasingless[Phys.Rev.Lett.124134802(2020)]or phase-locked[Nat.Photon.14475(2020)]laser wakefield acceleration(LWFA)can be realized,which may overcome issues of laser ...By using a high-intensity flying focus laser,the dephasingless[Phys.Rev.Lett.124134802(2020)]or phase-locked[Nat.Photon.14475(2020)]laser wakefield acceleration(LWFA)can be realized,which may overcome issues of laser diffraction,pump depletion,and electron dephasing which are always suffered in usual LWFA.The scheme thus has the potentiality to accelerate electrons to Te V energy in a single acceleration stage.However,the controlled electron injection has not been self-consistently included in such schemes.Only external injection was suggested in previous theoretical studies,which requires other accelerators and is relatively difficulty to operate.Here,we numerically study the actively controlled density transition injection in phase-locked LWFA to get appropriate density profiles for amount of electron injection.The study shows that compared with LWFA driven by lasers with fixed focus,a larger plasma density gradient is necessary.Electrons experience both transverse and longitudinal loss during acceleration due to the superluminal group velocity of the driver and the variation of the wakefield structure.Furthermore,the periodic deformation and fracture of the flying focus laser in the high-density plasma plateau make the final injected charge also depend on the beginning position of the density downramp.Our studies show a possible way for amount of electron injection in LWFA driven by flying focus lasers.展开更多

文摘Background:Gut microbiota plays a key role in health,immunity,digestion,and production in layers.Factors such as environment,diet,diseases,stress,and flock management significantly affect gut microbiota;however,it is not known how potential stressors such as intramuscular injections or feed withdrawal alter the composition of gut microbiota that result in increased the shedding level of foodborne pathogens.In the current study,the effects of intramuscular corticosterone injection and feed withdrawal were evaluated to understand their role in Salmonella Typhimurium shedding and changes in the composition of gut microbiota in layers.Results:Salmonella shedding was observed for 8 weeks post-infection.There was a significant increase in Salmonella Typhimurium count after intramuscular injection and feed withdrawal.The Salmonella infected and the negative control groups showed significant differences in the abundance of different genera in gut microbiota at week 1 and up to week 7 post infection.The infected group showed a significant reduction in alpha diversity of gut microbiota.Firmicutes reduced significantly(P<0.05)after intramuscular injection,while the feed withdrawal groups did not cause any significant changes in Proteobacteria-Firmicutes ratio.Furthermore,intramuscular injection resulted in a significant change in alpha diversity of gut microbiota.Conclusions:Exposure of chicks to relatively low dose of Salmonella Typhimurium can lead to persistent shedding in pullets.The Salmonella Typhimurium infection disrupted the gut microbiota composition immediately after infection.The potential stress of intramuscular injection and feed withdrawal significantly increased the Salmonella Typhimurium count in faeces.The intramuscular injection also resulted in a significant alteration of the Proteobacteria-Firmicutes ratio,which could increase the risk of dysbiosis.

文摘BACKGROUND The incidence of infection with Mycobacterium abscessus(M.abscessus)has increased in recent years.This increase is partly associated with invasive cosmetic procedures.CASE SUMMARY The purpose of this case summary is to increase clinicians'awareness of M.abscessus infection and reduce mycobacterial infection caused by cosmetic procedures.We report the case of a 45-year-old woman who received acetyl hexapeptide-8(argireline)injections in the forehead and temples,and erythema,nodules,and abscesses appeared at the injection sites after one week.The pus specimens were examined by microbiological culture and confirmed to be positive for M.abscessus.Clarithromycin 500 mg twice daily and moxifloxacin 400 mg once daily were administered for 5 mo and the lesions gradually subsided.CONCLUSION We report here for the first time a case of infection with M.abscessus after argireline injection.This condition is easily misdiagnosed as a common bacterial infection.Microbiological examinations are helpful for diagnosis and standardized cosmetic procedures can prevent infection with M.abscessus.
文摘BACKGROUND In recent years,the cosmetic intervention related infections caused by nontuberculous mycobacteria(NTM)are increasing as the informal cosmetic treatments are performed.However,many dermatologists are inexperienced in the diagnosis and management of similar cases.Here we report a case of subcutaneous infection caused by Mycobacterium abscessus(M.abscessus)following cosmetic injections of botulinum toxin.CASE SUMMARY A 53-year-old woman presented with multiple abscesses and nodules on her forehead and both temporal sites for half a month after cosmetic injections of botulinum toxin.Her lesions did not show any alleviation after 2-wk prescription of antibiotics.Laboratory examinations indicated that she had no sign of immunodeficiency and the whole body of computed tomography did not find any systemic infection or diseases.The pathology of skin tissue showed inflammatory cell infiltration with the negative results of Periodic acid Schiff(PAS)and Acidfast staining and the culture yielded no microbiome.Afterwards,the puncture on abscess was performed and M.abscessus was successfully isolated.The pathogen was identified by acid-fast staining and DNA sequencing.The patient was treated with the strategy of clarithromycin,ofloxacin,and amikacin according to the result of drug sensitivity test and got complete remission of the lesions.CONCLUSION The case presents the whole process of diagnosis and management of NTM infection after cosmetic intervention and highlights the diagnostic thoughts.In a word,the mycobacterium infection should be aware in patients after cosmetic performance.

文摘By using a high-intensity flying focus laser,the dephasingless[Phys.Rev.Lett.124134802(2020)]or phase-locked[Nat.Photon.14475(2020)]laser wakefield acceleration(LWFA)can be realized,which may overcome issues of laser diffraction,pump depletion,and electron dephasing which are always suffered in usual LWFA.The scheme thus has the potentiality to accelerate electrons to Te V energy in a single acceleration stage.However,the controlled electron injection has not been self-consistently included in such schemes.Only external injection was suggested in previous theoretical studies,which requires other accelerators and is relatively difficulty to operate.Here,we numerically study the actively controlled density transition injection in phase-locked LWFA to get appropriate density profiles for amount of electron injection.The study shows that compared with LWFA driven by lasers with fixed focus,a larger plasma density gradient is necessary.Electrons experience both transverse and longitudinal loss during acceleration due to the superluminal group velocity of the driver and the variation of the wakefield structure.Furthermore,the periodic deformation and fracture of the flying focus laser in the high-density plasma plateau make the final injected charge also depend on the beginning position of the density downramp.Our studies show a possible way for amount of electron injection in LWFA driven by flying focus lasers.
