Treatment of Protrusion of the Lumbar Intervertebral Disc by Massotherapy

Clinically, there is a higher incidence of protrusion of the lumbar intervertebral disc. It can cause a terrible pain. The author has treated 66 cases by massotherapy, who were confirmatively diagnosed as having protrusion of the lumbar intervertebral disc by X-ray film and computer-aided tomography, with satisfactory results as reported in the following.Clinical DataAmong the 66 cases in this series, 49 were male and 17 female, ranging in age from 26 to 59 years, averaging 36.2 years. All the patients had got lumbago and unilateral ischialgia, with the left side affected in 37 cases and the right side in 29 cases. The leg pain radiating to lateral malleolus was found in 28 cases, to dorsum of the foot in 24 cases, and to the toes in 14 cases. Intermittent claudication was present in 22 cases, numbness of the affected foot in 16 cases, pain exacerbated when coughing in 35 cases, scoliosis in 21 cases, and positive Lasegue's sign in 48 cases.

The authors treated 62 cases of protrusion oflumbar intervertebral disc by pulling and turningmanipulations from February 1993 to February 1998.Another 50 cases were treated by traction as controls.Statistical data showed that pulling and turningmanipulations had much better therapeuticeffectiveness than traction(P<0.05).

Effect of siRNA interference on nerve growth factor in intervertebral disc inflammation rats

Objective:To investigate the inhibition effect of siRNA interference on NGF induced by inflammatory factor IL-6,and JUL—1 so as to provide novel targets for clinical treatment of discogenic low back pain.Methods:The intervertebral disc nucleus and annulus fibrosus cells of rats were separated-The cells were co-cultured with different concentrations(10 nmol/L,20nmol/L,50 nmol/L,100 nmol/L)of IL-6 and IL-1β.The NGF-siRNA was leaded into the cocultured cells with its import ability assessed by flow cytometry instrument tests,hefore and after which the NCF mRNA expression was detected by real-time Q-PCR and the NGF content was detected by ELISA.Results:Flow cytometry instrument test results showed that the NGFsiRNA cell conversion rate was 99.8%.Real-time Q-PCR detection results showed that compared with negative control group,the NGF mRNA expression of co-cultured cells treated by 10 nmol/L,20 nmol/L,50 nmol/L,100 nmol/L IL-6 and IL-1βwere respectively raised 3.4,3.7,4.7,3.7 times which were all significantly down-regulated after the import of NGF-siRNA.EILSA detection results showed that compared with negative control group,the NGF content of cocultured medium treated by 10 nmol/L,20 nmol/L,50 nmol/L,100 nmol/L I-L6 and IL-1βwere respectively raised 2.9,3.3,4.5,7.4 times which were all significantly decreased after the import of NGF-siRNA.Conclusions:These molecular biological results suggest that inflammatory factor IL-6 and IL-1βcould stimulate NCF on intervertebral disc cells in vitro culture model and its efficiency is concentration dependent,while siRNA interference can inhibit the stimulation effect of IL-6 and IL-1βon intervertebral disc cell,which provides a new targets for the clinical treatment of discogenic low back pain.

Fifty-six Cases of Protrusion of Lumbar Intervertebral Disc Treated by Penetration and Oral Administration of Chinese Decoction Plus Traction

Fifty-six cases of the protrusion of the lumbar intervertebral disc in the treatment group were treated by drug-penetration and oral administration of traditional Chinese decoction plus traction, and the other 35 cases in the control group by oral administration of Chinese decoction and traction. The results showed that the cure rate in the treatment group was 83.9%, and that in the control group was 57.1%, with a statistically significant difference between the two groups (P<0.01), indicating that the former is a more effective therapy for protrusion of the lumbar intervertebral disc.

Treatment of 150 Cases of Lumber Intervertebral Disk Displacement by Acupuncture plus Traction

益肾强骨汤联合一指禅推拿治疗急性期腰椎间盘突出症疗效观察

目的:探讨益肾强骨汤联合一指禅推拿治疗急性期腰椎间盘突出症的临床疗效。方法:将120例急性期腰椎间盘突出症患者随机分为对照组和观察组各60例。对照组采用塞来昔布口服联合一指禅推拿治疗,观察组采用益肾强骨汤联合一指禅推拿治疗。观察两组患者临床疗效、日本骨科协会评分(Japanese orthopaedic association scores JOA)及血清炎症因子[白细胞介素1β(Interleukin-1β,IL-1β)、白细胞介素6(Interleukin-6,IL-6)、肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)]水平。结果:观察组总有效率为93.33%(56/60),高于对照组的83.33%(50/60)(P<0.05);治疗后观察组JOA评分高于对照组(P<0.05);治疗后两组患者IL-1β、IL-6、TNF-α水平均降低,观察组降低更明显(P<0.05)。结论:益肾强骨汤联合一指禅推拿可降低急性期腰椎间盘突出症患者血清IL-1β、IL-6、TNF-α水平,改善患者腰椎功能。

Exploration of the Clinical Rule of Acupuncturemoxibustion Treatment for Lumbar Intervertebral Disk Herniation

Objective： To review year 1956-2007 literature on acupuncture and moxibustion and explore the regularity of point selection and methods for acupuncture-moxibustion treatment of lumbar intervertebral disk herniation. Methods： A comprehensive analysis was made by a method of metrological analysis according to the data from ＂China modem acupuncture-moxibustion information databank＂. Results： Points Huantiao （GB 30）, Weizhong （BL 40）, Yanglingquan （GB 34）, and Jiaji （Ex-B 2） were most frequently used. Acupuncture was the most frequently used therapy and electroacupuncture, the next one. Conclusion： Electroacupuncture is a common and effective way to treat lumbar intervertebral disk herniation. Points Huantiao （GB 30）, Weizhong （BL 40）, Yanglingquan （GB 34）, and Jiaji （Ex-B 2） can be used as a basic prescription for acupuncture-moxibustion treatment of lumbar intervertebral disk herniation. Other acupoints are selected according to the syndrome.

Therapeutic Observation on Treatment of 68 Cases of Prolapse of Lumbar Intervertebral Disc with Lumber Vertebral Traction plus Ding＇s Rolling Technique

To observe the clinical therapeutic efficacy of treatment of prolapse of lumbar intervertebral disc with lumber vertebral traction plus Ding＇s Rolling Technique. Methods： 133 cases of prolapse of lumbar intervertebral disc were randomly divided into two groups. There were 68 cases in the treatment group, which were treated with lumber vertebral traction plus Tuina. There were 65 cases in the control group, which were treated with simple lumber vertebral traction. Results： There were statistic differences in the two groups （X^2=6.86, P〈0.01）. The therapeutic efficacy in the treatment group was better than the control group. Conclusion： The clinical therapeutic efficacy treated with lumber vertebral traction plus Ding＇s Rolling Technique was better than simple lumber vertebral traction in treating prolapse of lumbar intervertebral disc.

Treatment of Lumbar Intervertebral Disk Herniation with Electric Acupuncture

Objective： To make objective evaluation on treatment of lumbar intervertebral disc herniation with electric acupuncture. Method： All 200 cases were randomly allocated into an observation group （100 cases） and a control group （100 cases）. Cases in the observation group were treated with electric acupuncture. Cases in the control group were treated with oral administration of Mobic. Result： The total effective rate in the observation group was 86.5%, and the total effective rate of the control group was 75.0%. There was a significant difference between the two groups （P〈0.01）. Conclusion： Electric acupuncture is a safe and remarkably effective way for lumbar intervertebral disc herniation.

Treatment of 52 Cases of Lumbar Intervertebral Disk Herniation by Acupuncture plus Herbal Medicine

Lumbar intervertebral disk herniation is a commonly and frequently encountered disease in the orthopedic department, manifested by a series of symptoms such as low back pain, numbness and pain in one or two lower limbs, due to compression to the lumbosacral nerves by protrusion of the pulpiform nucleus after rupture of the fibrous rings. The author has treated 52 cases of this disease by acupuncture plus herbal medicine. Now, the report is given as follows.

Intervention of CT Localization plus TCM Therapy on Prolapse of Lumbar Intervertebral Disc

Purpose To observe the curative effect of interventional therapy by CT localization combined traditional Chinese medicine on prolapse of lumbar intervertebral disc.Method The inpatients of lumbar intervertegroup and control group. Interventional therapy by CT localization combined TCM was used in the treatment group while in the control group TCM therapy was taken only. The treatment lasted for 10 days in both groups.Results The curative effect was 94.3% in the treatment group while 76.8% in the control one (P<0.05), which had statistical meaning.Conclusion Interventional therapy by CT localization combined TCM on prolapse of lumbar intervertebral disc had better effect and shorter treatment course. Treatment of Protrusion of Cervical Intervertebral Disc with Tuina Therapy plus Acupuncture

In application of the bone-setting Tuina therapy plus acupuncture on Jiaji (Ex-B 2), Tianzong (SI 11). 臭氧治疗腰椎间盘突出症600例临床疗效分析

目的探讨臭氧椎间盘注射术对腰椎间盘突出症的疗效.方法 600例患者经CT 或MR检查证实为腰椎间盘突出症,主要表现为腰背痛及下肢麻痛.其中椎间盘膨出234例,突出361例,脱垂5例.在透视监视下采用21 G穿刺针经皮穿刺至腰椎间盘,实施盘内及椎旁间隙臭氧注射术.盘内注射3～5 ml,椎旁间隙注射10～15 ml,O3 浓度为30～50 μg/ml.结果随访6～54个月,总有效率为80.2%.显效409例(68.2%),有效72例(12%),无效119例(19.8%),无任何严重并发症发生.结论经皮穿刺O2-O3 混合气体注射术是治疗腰椎间盘突出症的一种安全、有效的方法.

夹脊电针配合激光针刀治疗腰椎间盘突出症:多中心随机对照试验

目的:观察夹脊电针配合激光针刀治疗腰椎间盘突出症的临床效应。方法:将120例腰椎间盘突出症患者按随机数字表法分为夹脊电针组、激光针刀组和综合组(夹脊电针加激光针刀),采用SF-MPQ积分观察治疗前后不同时段症状变化。结果:115例完成全部研究,综合组SF-MPQ积分明显低于激光针刀组和夹脊电针组(P<0.01,P<0.05),激光针刀组在治疗2周末SF-MPQ积分与夹脊电针组相比差异有非常显著性意义(P<0.01),在治疗1周和3周末则无统计学差异(P>0.05)。综合组半年后复发率明显低于激光针刀组和夹脊电针组(P<0.05)。结论:夹脊电针配合激光针刀可明显提高临床疗效,减轻患者疼痛,降低复发。

腹针为主治疗腰椎间盘突出症50例疗效观察

目的:探讨腹针治疗腰椎间盘突出症的临床疗效。方法:选择我科1998年8月～2001年8月收治的腰椎间盘突出症患者98例,随机分为腹针组和电针组。结果:腹针组痊愈率与电针组相比较有极显著差异(P<0.01),疗程相比较有显著差异(P<0.05)。结论:腹针治疗腰椎间盘突出症优于电针。

圆利针结合推拿治疗腰椎间盘突出症临床研究

目的探寻治疗腰椎间盘突出症的较佳疗法。方法将116例腰椎间盘突出症患者随机分为治疗组(圆利针+推拿组)和对照组(毫针+推拿组)各58例,对治疗前后症状的改善情况进行对照观察。结果治疗组有效率为98.2%,对照组为82.8%,两组疗效比较(P<0.01)。结论圆利针结合推拿治疗腰椎间盘突出症能取得更好的疗效。
