Third-look endoscopy prevents delayed bleeding after endoscopic submucosal dissection under antithrombotic therapy

BACKGROUND Postoperative delayed bleeding(PDB) after gastric endoscopic submucosal dissection(ESD) is the most common adverse event in patients receiving antithrombotics even with second-look endoscopy. Moreover, with the increasing prevalence of cardiovascular and cerebrovascular diseases in an aging population with associated lifestyle-related diseases, an increasing number of patients receive antithrombotics. Several attempts have been made to prevent PDB in aging population;however, a consensus has yet to be reached.AIM To examine the efficacy of third-look endoscopy(TLE) for PDB prevention.METHODS One hundred patients with early gastric neoplasms receiving antithrombotics were prospectively enrolled and subjected to ESD with TLE between February 2017 and July 2019. The primary endpoint was PDB rate, which was compared with our preset threshold. Furthermore, we divided the bleeding period into early-and late-onset PDB(E-PDB and L-PDB, respectively) and analyzed its rate. As a secondary analysis, we compared PDB rates with those of a historical control group, using propensity score matching, and calculated the PDB rates per antithrombotic agent use in each group.RESULTS In total, 96 patients and 114 specimens were finally evaluated. The overall PDB rate was 7.9%(9/114) [90%CI: 4.7-13.1, P = 0.005], while the late-and early-onset PDB rates(L-PDB and E-PDB) were 5.3% [90%CI: 2.7-9.9, P < 0.0001] and 2.6% [90%CI: 1.1-6.4, P = 0.51], respectively. Propensity score matching generated 58 matched pairs for TLE and control groups. No differences were found in overall PDB incidence(10.3% vs 20.7%, P = 0.12), whereas L-PDB occurrence significantly differed(5.2% vs 17.2%, P = 0.04) between groups. Considering antithrombotics' use, the overall PDB rate was higher for direct oral anticoagulants and multiple antithrombotics in the control group, while L-PDB incidence was lower in the TLE group for these agents(8.7% vs 23.1% and 5.0% vs 29.4%, respectively).CONCLUSION TLE for gastric ESD reduces overall PDB, and especially L-PDB incidence, among patients receiving antithrombotics.

A comparative analysis of spontaneous fertility after myomectomy by laparotomy associated or not with a“second look”laparoscopy

Objective:The importance of a“second look”laparoscopy for adhesion formation after myomectomy by laparotomy in improving fertility is not clearly defined in our context.The aim of this study was to compare spontaneous fertility after myomectomy by laparotomy between women who underwent a“second look”laparoscopy and those who did not.Methods:We conducted an analytical cross-sectional study with retrospective data collection from January 1,2008 to December 31,2015,comparing spontaneous fertility between patients who underwent“second-look”laparoscopy after myomectomy by laparotomy to those who did not.Firstly,data was collected from the records of patients included in the study.Secondly,through phone calls,informed consent for each participant was obtained after which inquiries were made regarding their fertility status.Results:After a period of 24 months,23 patients(47.9%)with“second look”laparoscopy conceived,compared to 37 patients(35.2%)without“second look”laparoscopy(p=0.136);and 24 patients(50.0%)with“second look”had term pregnancies compared to 39 patients(37.1%)without“second look”laparoscopy(p=0.134).Conclusion:Spontaneous fertility rates seemed to be improved after a“second look”laparoscopy,however,there was no statistically significant difference between spontaneous fertility rates obtained after myomectomy by laparotomy,irrespective of the fact that the patient had undergone a“second look”laparoscopy or not at Yaounde Gyneco-Obstetric and Pediatric Hospital.

一种基于多尺度的目标检测锚点构造方法

目标检测是计算机视觉领域的研究热点和基础任务,其中基于锚点(Anchor)的目标检测已在众多领域得到广泛应用。当前锚点选取方法主要面临两个问题:基于特定数据集的先验取值尺寸固定、面对不同场景泛化能力弱。计算锚框的无监督K-means算法,受初始值影响较大,对目标尺寸较单一的数据集聚类产生的锚点差异较小,无法充分体现网络多尺度输出的特点。针对上述问题,本文提出一种基于多尺度的目标检测锚点构造方法(multi-scale-anchor,MSA),将聚类产生的锚点根据数据集本身的特性进行尺度的缩放和拉伸,优化的锚点即保留原数据集的特点也体现了模型多尺度的优势。另外,本方法应用在训练的预处理阶段,不增加模型推理时间。最后,选取单阶段主流算法YOLO(You Only Look Once),在多个不同场景的红外或工业场景数据集上进行丰富的实验。结果表明,多尺度锚点优化方法MSA能显著提高小样本场景的检测精度。

基于图像低维特征融合的航拍小目标检测模型

针对无人机航拍图像目标检测中视野变化大、时空信息复杂等问题,文中基于YOLOv5(You Only Look Once Version5)架构,提出基于图像低维特征融合的航拍小目标检测模型.引入CA(Coordinate Attention),改进MobileNetV3的反转残差块,增加图像空间维度信息的同时降低模型参数量.改进YOLOv5特征金字塔网络结构,融合浅层网络中的特征图,增加模型对图像低维有效信息的表达能力,进而提升小目标检测精度.同时为了降低航拍图像中复杂背景带来的干扰,引入无参平均注意力模块,同时关注图像的空间注意力与通道注意力;引入VariFocal Loss,降低负样本在训练过程中的权重占比.在VisDrone数据集上的实验验证文中模型的有效性,该模型在有效提升检测精度的同时明显降低复杂度.

基于ID-YOLO的数字仪表检测方法

当前针对数字式仪表检测算法在边缘设备具有实时性差、泛化性差的问题,对此提出一种采用ID-YOLO(instrument detection-you only look once)模型的变电站数字仪表检测识别方法。所提算法以YOLOv5模型为基础,首先设计轻量骨干网络(light weight-YOLO, LW-YOLO)提取图像特征,降低网络参数,提高检测实时性;然后设计了一种双级路由注意力模块(bi-level routing attention moudle, BRAM),提高网络对小数点的检测精度以及网络的鲁棒性和泛化性;最后,引入损失函数α-IoU,通过设定不同的可调节参数α数值得到更准确的真实框与预测框的交并比计算,可以提高模型的检测精度。结果表明:相比于其他基于深度学习的数字仪表检测识别方法,所提方法在不同显示方式的数字仪表识别任务上具有更好的准确性和泛化性,而且可以在检测准确率领先的情况下,将模型在边缘设备上的检测速度从6.87帧/s提升至8.77帧/s,其实时性和检测精度均能够满足实际变电站智能数据采集、检测识别的工程需要。

基于深度学习YOLOX算法的混凝土构件裂缝智能化检测方法

针对现有混凝土构件裂缝人工检测操作不仅费时、费力,而且易出现错检、误检、漏检,以及部分位置难以开展检测的问题,提出一种基于深度学习YOLOX(You Only Look Once)算法的混凝土构件裂缝智能化检测方法;首先采集、整理包含各类混凝土构件的典型裂缝图像,并通过图像数据增强建立Pascal VOC数据集,然后基于Facebook公司开发的深度学习框架Pytorch,利用数据集训练YOLOX算法,并进行裂缝识别和验证;将训练完成后YOLOX算法移植至搭载安卓系统的手机端,进行现场实时检测操作。结果表明:在迭代次数为700时,混凝土构件裂缝识别精度可达88.84%,能有效筛分混凝土构件表面裂缝,并排除其他干扰项,证明了所提出的方法对裂缝具有较高的识别精度和广泛的适用性;经试验测试,移植至手机端的YOLOX算法能在提升便携性的同时保证高效、准确的检测效果,具有良好的应用前景。

Uphold the Original Intention and Work together for Future Development——Speech at the 35th Anniversary Symposium of BES

Time flies,in the blink of an eye,it is already 2023.The journal Biomedical and Environmental Sciences(BES)has been established for thirty-five years.Today,I am honored to participate in the BES’s 35th anniversary symposium,organized by the Chinese Center for Disease Control and Prevention(China CDC).Seeing Director Hongbing Shen,Deputy Director Xiaoming Shi,along colleagues from the center’s affiliated units and departments,as well as fellow researchers,we have come together for this symposium under the theme of“Looking back and looking forward”,I am not only deeply appreciative and thankful,but also envisioning a bright future for BES in the next 35 years.I would like to share my thoughts and hopes under the theme of“Review and Development”.

Benchmarking YOLOv5 models for improved human detection in search and rescue missions

Drone or unmanned aerial vehicle(UAV)technology has undergone significant changes.The technology allows UAV to carry out a wide range of tasks with an increasing level of sophistication,since drones can cover a large area with cameras.Meanwhile,the increasing number of computer vision applications utilizing deep learning provides a unique insight into such applications.The primary target in UAV-based detection applications is humans,yet aerial recordings are not included in the massive datasets used to train object detectors,which makes it necessary to gather the model data from such platforms.You only look once(YOLO)version 4,RetinaNet,faster region-based convolutional neural network(R-CNN),and cascade R-CNN are several well-known detectors that have been studied in the past using a variety of datasets to replicate rescue scenes.Here,we used the search and rescue(SAR)dataset to train the you only look once version 5(YOLOv5)algorithm to validate its speed,accuracy,and low false detection rate.In comparison to YOLOv4 and R-CNN,the highest mean average accuracy of 96.9%is obtained by YOLOv5.For comparison,experimental findings utilizing the SAR and the human rescue imaging database on land(HERIDAL)datasets are presented.The results show that the YOLOv5-based approach is the most successful human detection model for SAR missions.
