A total of 20 low-back and leg pain patients with lumbar disc herniation were treated with balanced acupuncture or body acupuncture. Central mechanisms of vaded acupunctures were compared using resting-state functional MRI. Patients from both groups received functional MRI before and after acupuncture. Functional connectivity in brain regions that were strongly associated with the bilatera amygdala was analyzed utilizing AFNI software. Visual analogue scale scores were greater in the balanced acupuncture group compared with the body acupuncture group. Function of the endogenous pain regulation network was enhanced in patients in the balanced acupuncture group, but was not changed in the body acupuncture group. This result indicates that the analgesic effects of body acupuncture do not work through the central nervous system. These data suggest that balanced acupuncture exerts analgesic effects on low-back and leg pain patients with lumbar disc herniation by regulating the function of the endogenous pain regulation network.展开更多

It involves removal of subcutaneous stagnant blood through suction cups after making superficial incisions at particular area of the body. This study was undertaken to evaluate the efficacy of hijama in sciatica pain at Aligarh Shifa hospital. 92 patients with the history of sciatica were selected randomly between 18 - 75 years of age and hijama cups were applied generally at C7, T2 and L5/S1 vertebrae, while two cups were also applied bilaterally on L4/L5 vertebrae, four cups were additionally applied on hip joint, back of thigh, knee and calf muscle, all cups were applied thrice at an interval of 15 days between each session. The decrease in sciatic pain was assessed after three sessions of Hijama by numeric pain rating scale there was overall significant reduction in pain with 67 percent patients showing relief in pain up to varying degree. Present study suggests that hijama has been found to be effective in relieving pain and improving quality of life in majority of the patient’s, hence may be used as effective alternative tool to alleviate pain.展开更多
The lack of truly robust analgesics for chronic pain is owed,in part,to the lack of an animal model that reflects the clinical pain state and of a mechanismbased,objective neurological indicator of pain.The present st...The lack of truly robust analgesics for chronic pain is owed,in part,to the lack of an animal model that reflects the clinical pain state and of a mechanismbased,objective neurological indicator of pain.The present study examined stimulus-evo ked brain activation with functional magnetic resonance imaging in male and female cynomolgus macaques following unilateral L7 spinal nerve ligation and the effects of clinical analgesics pregabalin,duloxetine,and morphine on brain activation in these macaques.A modified straight leg raise test was used to assess pain severity in awake animals and to evo ke regional brain activation in anesthetized animals.The potential effects of clinical analgesics on both awake pain behavior and regional brain activation were examined.Following spinal nerve ligation,both male and female macaques showed significantly decreased ipsilateral straight leg raise thresholds,suggesting the presence of radicula rlike pain.Morphine treatment increased straight leg raise thresholds in both males and females whereas duloxetine and pregabalin did not.In male macaques,the ipsilateral straight leg raise activated contralateral insular and somatosensory cortex(Ins/SII),and thalamus.In female macaques,the ipsilateral leg raise activated cingulate cortex and contralateral insular and somatosensory cortex.Straight leg raises of the contralateral,unligated leg did not evoke brain activation.Morphine reduced activation in all brain regions in both male and female macaques.In males,neither pregabalin nor duloxetine decreased brain activation compared with vehicle treatment.In females,however,pregabalin and duloxetine decreased the activation of cingulate cortex compared with vehicle treatment.The current findings suggest a diffe rential activation of brain areas depending on sex following a peripheral nerve injury.Diffe rential brain activation observed in this study could underlie qualitative sexual dimorphism in clinical chronic pain perception and responses to analgesics.Future pain management approaches for neuropathic pain will need to consider potential sex differences in pain mechanism and treatment efficacy.展开更多

Background As one of the most common musculoskeletal ailments,chronic nonspecific low-back pain(CNLBP)causes persistent disability and substantial medical expenses.Epidemiological evidence shows that the incidence rat...Background As one of the most common musculoskeletal ailments,chronic nonspecific low-back pain(CNLBP)causes persistent disability and substantial medical expenses.Epidemiological evidence shows that the incidence rate of CNLBP in young and middle-aged people who are demanded rapidly recovery and social contribution is rising.Recent guidelines indicate a reduced role for medicines in the management of CNLBP.Objective The present study investigates the short-term effects of cupping and scraping therapy using a medicated balm,compared to nonsteroidal anti-inflammatory drug(NSAID)with a capsaicin plaster,in the treatment of CNLBP.Design,setting,participants and interventions We designed a prospective multicenter randomized clinical trial enrolling patients from January 1,2022 to December 31,2022.A total of 156 patients with CNLBP were randomized into two parallel groups.Diclofenac sodium-sustained release tablets were administered orally to participants in the control group for one week while a capsaicin plaster was applied externally.Patients in the test group were treated with cupping and scraping using a medical device and medicated balm.Main outcome measures Primary outcome was pain recorded using the visual analogue scale(VAS).Two secondary outcomes were recorded using the Japanese Orthopedic Association low-back pain scale(JOA)and the traditional Chinese medicine(TCM)syndrome integral scale(TCMS)as assessment tools.Results Between baseline and postintervention,all changes in outcome metric scales were statistically significant(P<0.001).Compared to the control group,patients in the test group had a significantly greater treatment effect in all outcome variables,as indicated by lower VAS and TCMS scores and higher JOA scores,after the one-week intervention period(P<0.001).Further,according to the findings of multivariate linear regression analysis,the participants’pain(VAS score)was related to their marital status,age,smoking habits and body mass index.No adverse reactions were reported for any participants in this trial.Conclusion The effectiveness of TCM combined with the new physiotherapy tool is superior to that of NSAID combined with topical plasters,regarding to pain intensity,TCM symptoms and quality of life.The TCM plus physiotherapy also showed more stable and long-lasting therapeutic effects.展开更多
