Globe luxation may prevent myopia in a child:A case report

BACKGROUND Globe luxation is rare and is mostly due to direct orbital trauma with fractures of the medial and floor walls,which displace the globe into the maxillary sinus.Only a few cases have been reported;moreover,patients who suffer global luxation rarely achieve eyesight recovery.CASE SUMMARY This report describes the treatment and prognosis of global luxation occurring in a child.A 6-year-old boy presented with left globe luxation that occurred after a sudden stop on a tricycle,without any injury to the orbital or maxillofacial bony structures.After admission to the hospital,an external canthus incision,globe repositioning,orbital exploration and temporary blepharoplasty were performed.Finally,the child completely recovered and maintained good eyesight in his left eye even though the right eye developed myopia after four years of follow-up.CONCLUSION The aim of this study was to report the special occurrence of globe luxation in the child and share some experience of the treatment.Immediate surgical management plays an important role in the recovery of visual function,and globe luxation may prevent nearsightedness by reducing the distortion of the eyeball,shortening the axis and improving ciliary function. 展开更多

Using tripelvic osteotomy and acetabular reconstruction for the management of long term traumatic hip luxation in a mongrel dog

To describe surgical methods for the management of long term traumatic hip luxation. One client-owned 11 year old mongrel dog with 3 month craniodorsal hip luxation.A tripelvic osteotomy was performed through the standard normal procedures under general anesthesia. Pubic wedge osteotomy was done first then the ischial osteotomy was performed after. Approach to the craniodorsal aspect of the hip joint was through a craniolateral incision. The soft tissue in the acetabulum was debrided similar to the preparation for total hip replacement. The luxated hip joint was reduced. Iliac osteotomy was performed and thirty degree tripelvic plate was used to fix the osteotomic site. To close the vastus lateralis and deep gluteal muscles,tensor fasciae latae,subcutaneous tissue,and skin used standard methods. Exercise restrained for three months.The dog returned to use the affected limb three days after surgery. There was no any lameness observed at three months after surgery. The dog could jump and run at four months postoperatively. There was no complications and excellent activity one year later.In this case report,tripelvic osteotomy and acetabular reconstruction together can be used for the management of long term traumatic hip luxation. The surgical techniques can be an option for veterinary surgeon to manage long term hip luxation instead of other surgical methods. 展开更多

This retrospective non-comparative consecutive case series study was conducted at Azienda Ospedaliera Universitaria Careggi, Florence, Italy and describes a useful intraocular lens(IOL) repositioning technique using iris sutures. In our study, 41 consecutive cases of posteriorly dislocated IOLs were surgically treated between January 2015 and May 2017. Six of the cases were post-traumatic luxations, and 20 patients had pseudoexfoliation syndrome. All the patients underwent pars plana vitrectomy and same IOL repositioning using iris sutures. The mean followup was 12.2 mo. The mean preoperative best corrected visual acuity(BCVA) was 0.10±0.15 logMAR, whereas the mean postoperative BCVA was 0.08±0.14 logMAR. The mean postoperative BCVA did not change significantly from the preoperative BCVA. The final mean spherical equivalent was-0.44±0.49 SD. Three lenses(7.31%) were found tilted during post-operative follow-up. Two eyes(4.87%) had postoperative cystoid macular edema. No eyes had endophthalmitis, hypotony, retinal or choroidal detachment. The iris fixation technique seems to be a safe and valid option for the management of dislocated IOLs. 展开更多

Objective: To study the local inflammatory stress response and pain mediator secretion after impacted wisdom tooth extraction with high-speed turbine drill combined with original luxator. Methods: Patients who received impacted wisdom tooth extraction in the First People's Hospital ofYunnan Province between March 2014 and June 2017 were selected as the research objects and randomly divided into the observation group who accepted impacted wisdom tooth extraction with high-speed turbine drill combined with original luxator and the control group who accepted impacted wisdom tooth extraction with traditional chisel. The contents of inflammatory mediators, stress mediators and pain mediators in gingival crevicular fluid were measured before surgery and 3 days after surgery. Results: Three days after surgery, PTX3, ICAM1, MPO, PGE2, PPARγ, ROS, MDA, 5-HT, CGRP, SP, Gal and ATP contents in gingival crevicular fluid of both groups were higher than those before surgery whereas GPx and T-AOC contents were significantly lower than those before surgery, and PTX3, ICAM1, MPO, PGE2, PPARγ, ROS, MDA, 5-HT, CGRP, SP, Gal and ATP contents in gingival crevicular fluid of observation group were lower than those of control group whereas GPx and T-AOC contents were significantly higher than those of control group. Conclusions: The impacted wisdom tooth extraction with high-speed turbine drill combined with original luxator can reduce the local inflammatory stress response and inhibit the secretion of pain mediators. 展开更多
