Importance/Objective: Adverse Drug Reactions (ADRs) are unavoidable, but recognizing and addressing ADRs early can improve wellness and prevent permanent injury. We suggest that available medical information and digital/electronic methods could be used to manage this major healthcare problem for individual patients in real time. Methods: We searched the available digital applications and three literature databases using the medical subject heading terms, adverse drug reaction reporting systems or management, filtered by clinical trial or systemic reviews, to detect publications with data about ADR identification and management approaches. We reviewed the reports that had abstract or summary data or proposed or implemented methods or systems with potential to identify or manage ADRs in clinical settings. Results: The vast majority of the 481 reports used retrospectively collected data for groups of patients or were limited to surveying one population group or class of medication. The reports showed potential and definite associations of ADRs for specific drugs and problems, mostly, but not exclusively, for patients in hospitals and nursing homes. No reports described complete methods to collect comprehensive data on ADRs for individual patients in a healthcare system. The digital applications have ADR information, but all are too cumbersome or incomplete for use in active clinical settings. Several studies suggested that providing information about potential ADRs to clinicians can reduce these problems. Conclusion and Relevance: Although investigators and government agencies agree with the need, there is no comprehensive ADR management program in current use. Informing the patient’s healthcare practitioners of potential ADRs at the point of service has the potential for reduction of these complications, which should improve healthcare and reduce unneeded costs. 展开更多

Introduction: The need to address the problem of patient safety has been a focus of World Health Assembly (WHA) meetings of 2002, 2019 and 2021. The 2019 WHA Resolution urged the Member States to take action on patient safety. We aimed to review patient safety efforts in Tanzania from 2002 to 2022 to inform improvement efforts towards the 2030 target. Methods: A rapid literature review was conducted between January 2002 and April 2022. We searched Google, PubMed and PubMed Central in April and May 2022 using the following search terms: PubMed—“patient safety Tanzania”, “blood safety in Tanzania”, “safe surgery Tanzania”, and “healthcare-associated infections Tanzania”;Google—“blood safety in Tanzania”, injection safety in Tanzania”, “infection prevention and control”, “radiation safety in health facilities in Tanzania”;and PubMed Central—“injection safety in Tanzania. Results: The search identified 4160 articles, of which 4053 were removed in initial screening;21 were duplicates, giving 86 relevant articles for full screening. Of the 86 articles, 04 were removed after the full screening, hence remaining with 82 articles. Among the 82 eligible articles, 27 are on IPC, 26 on safe surgery, 12 on blood safety, 07 on radiation safety, 06 on injection safety, and 02 on medication safety. One article was relevant to—blood safety, IPC and injection safety;and one article was relevant to—IPC and injection safety. Conclusion: Most of the eligible literature was on IPC and safe surgery, followed by blood safety, radiation safety, injection safety and medication safety. The literature on IPC has highlighted the need to strengthen efforts to address AMR. Findings from the implementation of the safe surgery 2020 intervention warrants for its scale-up to other zones. There is a need to strengthen hemovigilance and pharmacovigilance functions;and strengthen quality management and assurance systems and regulatory functions to ensure radiation safety. 展开更多

Medication safety improvement strategies require a better understanding of the safety culture specifically related to medicines. In healthcare, safety climate questionnaires are often used as a proxy measure of the underlying safety culture. However, there are currently not known instruments to assess medication safety climate. The study therefore aimed to develop and evaluate a medication safety climate questionnaire for healthcare staff in UK hospitals. Two validated patient safety climate instruments were adapted to develop a Medication Safety Climate (MSC) questionnaire. Data was collected from 510 healthcare professionals (response rate 9.4%); routinely involved with prescribing, dispensing, administering and monitoring medication; in two acute NHS hospitals in London. Confirmatory factor analysis and reliability analyses were conducted to determine the psychometric properties of the MSC questionnaire. Results showed that the 50-item MSC questionnaire contained nine factors--teamwork climate, safety climate, job satisfaction, stress recognition, perceptions of management, working conditions, organisational learning, feedback and communication about error and management support for medication safety. Internal consistency reliability scores for eight of the nine factors were 〉 0.7 and ranged from 0.64 to 0.9. Correlations between eight factors showed a moderate relationship between the factors; ranging from 0.232 to 0.669. One factor, Stress recognition, had a weak and negative correlation with all other factors. Confirmatory factor analysis achieved an almost adequate model fit (x2/df ratio 2.572; root mean square error of approximation (RMSEA) 0.069; comparative fit index (CFI) 0.791). The MSC questionnaire demonstrated reasonable psychometric properties. Further refinement using exploratory factor analysis is, however, required to improve the questionnaire's validity. This is the first known instrument to measure mediation safety climate in the UK and could be used to inform medication safety improvement strategies and monitor change in healthcare staff perceptions, related to medication safety, over time. 展开更多

The purpose of this manuscript is to present research findings based on the reported cases of medical information breaches due to Social Media (SM) usage, in selected medical institutions in Uganda. The study employed online survey techniques. Altogether, 710 questionnaires (Google forms) were developed, and operationalized. The main respondents included 566 medical students, and 143 medical staff from Mbarara University of Science and Technology (MUST), and Kampala International University (KIU), accordingly. Using SPSS, the main statistical analysis tools employed include frequency distribution summary, and Chi-square (x2) test. According to the frequency distribution summary, 27% to 42% of the respondents within categorical divides acknowledged occurrence of medical information breaches due to SM usage. Notably, higher levels of the breaches were reported among male students (64%), age-group 18 to 35 years (68%), and WhatsApp users (63%). On the other hand, Chi-square results showed significant levels (p p > 0.05) between medical institutions and medical information breaches. Overall, the vulnerable areas of the breaches identified would serve as important reference points in the process of rationalizing SM usage in medical institutions. Nevertheless, further studies could focus on identification of the key SM usage factors associated with medical information breaches in medical institutions in Uganda. 展开更多
