A total of 28,016 women on the territory of the Republic of Bulgaria were tested for serum AMH levels with a median age of 37.0 years (interquartile range 32.0 to 41.0). For women aged 20 - 29 years, the Bulgarian population has relatively high median levels of AMH, similar to women of Asian origin. For women aged 30 - 34 years, our results are comparable to those of women living in Western Europe. For women aged 35 - 39 years, our results are comparable to those of women living in the territory of India and Kenya. For women aged 40 - 44 years, our results were lower than those for women from the Western European and Chinese populations, close to the Indian and higher than Korean and Kenya populations, respectively. Our results for women of Bulgarian origin are also comparable to US Latina women at age 30, 35 and 40 ages. On the base on constructed a statistical model to predicting the decline in AMH levels at different ages, we found non-linear structure of AMH decline for the low AMH 3.5) the dependence of the decline of AMH on age was confirmed as linear. In conclusion, we evaluated the serum level of AMH in Bulgarian women and established age-specific AMH percentile reference values based on a large representative sample. We have developed a prognostic statistical model that can facilitate the application of AMH in clinical practice and the prediction of reproductive capacity and population health.展开更多

To reduce the uncertainty of safety evaluation, this study developed a composite cloud model, which improved the combination weights of the decision-making trial and evaluation laboratory (DEMATEL) and criteria importance through intercriteria correlation (CRITIC) methods. A safety evaluation system was developed according to in situ monitoring data. The backward cloud generator was used to calculate the numerical characteristics of a cloud model of quantitative indices, and different virtual clouds were used to synthesize some clouds into a generalized one. The synthesized numerical characteristics were calculated to comprehensively evaluate the safety of toppling rock slopes. A case study of a toppling rock slope near the Huangdeng Hydropower Station in China was conducted using monitoring data collected since operation of the hydropower project began. The results indicated that the toppling rock slope was moderately safe with a low safety margin. The composite cloud model considers the fuzziness and randomness of safety evaluation and enables interchange between qualitative and quantitative knowledge. This study provides a new theoretical method for evaluating the safety of toppling rock slopes. It can aid in the predication, control, and even prevention of disasters.展开更多

However, reasonable drought simulations have remained a challenge, and the related performances of the current state-of-the-art Coupled Model Intercomparison Project phase 6(CMIP6) models remain unknown. Here, both the strengths and weaknesses of CMIP6 models in simulating droughts and corresponding hydrothermal conditions in drylands are assessed. While the general patterns of simulated meteorological elements in drylands resemble the observations, the annual precipitation is overestimated by ~33%(with a model spread of 2.3%–77.2%), along with an underestimation of potential evapotranspiration(PET) by ~32%(17.5%–47.2%). The water deficit condition, measured by the difference between precipitation and PET, is 50%(29.1%–71.7%) weaker than observations. The CMIP6 models show weaknesses in capturing the climate mean drought characteristics in drylands, particularly with the occurrence and duration largely underestimated in the hyperarid Afro-Asian areas. Nonetheless, the drought-associated meteorological anomalies, including reduced precipitation, warmer temperatures, higher evaporative demand, and increased water deficit conditions, are reasonably reproduced. The simulated magnitude of precipitation(water deficit) associated with dryland droughts is overestimated by 28%(24%) compared to observations. The observed increasing trends in drought fractional area, occurrence, and corresponding meteorological anomalies during 1980–2014 are reasonably reproduced. Still, the increase in drought characteristics, associated precipitation and water deficit are obviously underestimated after the late 1990s,especially for mild and moderate droughts, indicative of a weaker response of dryland drought changes to global warming in CMIP6 models. Our results suggest that it is imperative to employ bias correction approaches in drought-related studies over drylands by using CMIP6 outputs.展开更多

文摘BACKGROUND Previous studies have validated the efficacy of both magnetic compression and surgical techniques in creating rabbit tracheoesophageal fistula(TEF)models.Magnetic compression achieves a 100%success rate but requires more time, while surgery, though less frequently successful, offers rapid model establishment and technical maturity in larger animal models.AIM To determine the optimal approach for rabbit disease modeling and refine the process.METHODS TEF models were created in 12 rabbits using both the modified magnetic compression technique and surgery.Comparisons of the time to model establishment, success rate, food and water intake, weight changes, activity levels, bronchoscopy findings, white blood cell counts, and biopsies were performed. In response to the failures encountered during modified magnetic compression modeling,we increased the sample size to 15 rabbit models and assessed the repeatability and stability of the models, comparing them with the original magnetic compression technique.RESULTS The modified magnetic compression technique achieved a 66.7%success rate, whereas the success rate of the surgery technique was 33.3%.Surviving surgical rabbits might not meet subsequent experimental requirements due to TEF-related inflammation. In the modified magnetic compression group, one rabbit died, possibly due to magnet corrosion, and another died from tracheal magnet obstruction.Similar events occurred during the second round of modified magnetic compression modeling, with one rabbit possibly succumbing to aggravated lung infection. The operation time of the first round of modified magnetic compression was 3.2±0.6 min, which was significantly reduced to 2.1±0.4 min in the second round, compared to both the first round and that of the original technique.CONCLUSION The modified magnetic compression technique exhibits lower stress responses,a simple procedure,a high success rate, and lower modeling costs, making it a more appropriate choice for constructing TEF models in rabbits.
文摘Transport analysis and impact evaluations are important input for decisions about infrastructure projects. The impacts on transport from fjord crossing tunnels or bridges are the foundation for the cost benefit analysis, and also the basis for estimating the income from toll collection.Based on experiences from concept evaluations of several fixed link projects on E39,and an ongoing overall analysis,we question the results from transport analysis made by the official tools for such analysis:the RTM(regional transport model)which estimates the demand for trips below 10 km,the NTM(national transport model),for trips of 10 km or more, and the freight transport model. Both the NTM and the freight transport model are integrated in the RTM in the net assignment stage. We will demonstrate strengths and weaknesses in the transport models by showing contra intuitive or questionable results using the model as it is.The following questions arose as the initial results from the transport model were presented:Are the transport models able to capture immediate as well as long-term impacts? How would different assumptions about the monetary costs on these projects affect the forecasted demand and the cost benefit analysis? Are there other and wider ranges of impacts,if the analysis covers the total coastal highway as a whole, compared to evaluating impacts of each fixed link project individually?Do we have enough data to include transport effects of wider impacts of the fixed link projects? We had to deal with these questions in the concept evaluations carried out for the various fixed links project and in the current overall evaluation. We would like to suggest improvements in the analysis tools and emphasize requirements for knowledge about impacts of fixed links projects.

文摘Background:The Mongolian gerbil is an excellent laboratory animal for preparing the cerebral ischemia model due to its inherent deficiency in the circle of Willis. However, the low incidence and unpredictability of symptoms are caused by numerous complex variant types of the circle.Additionally, the lack of an evaluation system for the cer-ebral ischemia/reperfusion(I/R)model of gerbils has shackled the application of this model. Methods:We created a symptom-oriented principle and detailed neurobehavioral scoring criteria. At different time points of reperfusion,we analyzed the alteration in locomotion by rotarod test and grip force score, infarct volume by triphenyltetrazo-lium chloride(TTC)staining, neuron loss using Nissl staining, and histological charac-teristics using hematoxylin-eosin(H&E)straining.Results:With a successful model rate of 56%,32 of the 57 gerbils operated by our method harbored typical features of cerebral I/R injury, and the mortality rate in the male gerbils was significantly higher than that in the female gerbils. The suc-cessfully prepared I/R gerbils demonstrated a significant reduction in motility and grip strength at 1 day after reperfusion;formed obvious infarction;exhibited typi-cal pathological features, such as tissue edema, neuronal atrophy and death, and vacuolated structures;and were partially recovered with the extension of reperfu-sion time. Conclusion:This study developed a new method for the unilateral common carotid artery ligation I/R model of gerbil and established a standardized evaluation system for this model, which could provide a new cerebral I/R model of gerbils with more practical applications.
文摘BACKGROUND Liver metastases(LM)is the primary factor contributing to unfavorable outcomes in patients diagnosed with gastric cancer(GC).The objective of this study is to analyze significant prognostic risk factors for patients with GCLM and develop a reliable nomogram model that can accurately predict individualized prognosis, thereby enhancing the ability to evaluate patient outcomes.AIM To analyze prognostic risk factors for GCLM and develop a reliable nomogram model to accurately predict individualized prognosis, thereby enhancing patient outcome assessment.METHODS Retrospective analysis was conducted on clinical data pertaining to GCLM(type III),admitted to the Department of General Surgery across multiple centers of the Chinese PLA General Hospital from January 2010 to January 2018.The dataset was divided into a development cohort and validation cohort in a ratio of 2:1.In the development cohort,we utilized univariate and multivariate Cox regression analyses to identify independent risk factors associated with overall survival in GCLM patients.Subsequently,we established a prediction model based on these findings and evaluated its performance using receiver operator characteristic curve analysis, calibration curves, and clinical decision curves.A nomogram was created to visually represent the prediction model, which was then externally validated using the validation cohort.RESULTS A total of 372 patients were included in this study, comprising 248 individuals in the development cohort and 124 individuals in the validation cohort.Based on Cox analysis results, our final prediction model incorporated five independent risk factors including albumin levels, primary tumor size, presence of extrahepatic metastases, surgical treatment status, and chemotherapy administration. The 1-,3-,and 5-years Area Under the Curve values in the development cohort are 0.753,0.859,and 0.909,respectively;whereas in the validation cohort, they are observed to be 0.772,0.848,and 0.923.Furthermore,the calibration curves demonstrated excellent consistency between observed values and actual values. Finally, the decision curve analysis curve indicated substantial net clinical benefit.CONCLUSION Our study identified significant prognostic risk factors for GCLM and developed a reliable nomogram model, demonstrating promising predictive accuracy and potential clinical benefit in evaluating patient outcomes.

文摘Traditional tumor models do not tend to accurately simulate tumor growth in vitro or enable personalized treatment and are particularly unable to discover more beneficial targeted drugs. To address this, this study describes the use of threedimensional(3D)bioprinting technology to construct a 3D model with human hepatocarcinoma SMMC-7721 cells(3DP-7721)by combining gelatin methacrylate(GelMA)and poly(ethylene oxide)(PEO)as two immiscible aqueous phases to form a bioink and innovatively applying fluorescent carbon quantum dots for long-term tracking of cells. The GelMA(10%,mass fraction)and PEO(1.6%,mass fraction)hydrogel with 3:1 volume ratio offered distinct pore-forming characteristics, satisfactorymechanical properties, and biocompatibility for the creation of the 3DP-7721 model.Immunofluorescence analysis and quantitative real-time fluorescence polymerase chain reaction(PCR)were used to evaluate the biological properties of the model.Compared with the two-dimensional culture cell model(2D-7721)and the 3D mixed culture cell model(3DM-7721),3DP-7721 significantly improved the proliferation of cells and expression of tumor-related proteins and genes. Moreover,we evaluated the differences between the three culture models and the effectiveness of antitumor drugs in the three models and discovered that the efficacy of antitumor drugs varied because of significant differences in resistance proteins and genes between the three models. In addition, the comparison of tumor formation in the three models found that the cells cultured by the 3DP-7721 model had strong tumorigenicity in nude mice.Immunohistochemical evaluation of the levels of biochemical indicators related to the formation of solid tumors showed that the 3DP-7721 model group exhibited pathological characteristics of malignant tumors, the generated solid tumors were similar to actual tumors, and the deterioration was higher. This research therefore acts as a foundation for the application of 3DP-7721 models in drug development research.
文摘The present study aims to establish a relationship between serum AMH levels and age in a large group of women living in Bulgaria, as well as to establish reference age-specific AMH levels in women that would serve as an initial estimate of ovarian age. We have developed a prognostic statistical model that can facilitate the application of AMH in clinical practice and the prediction of reproductive capacity and population health.

文摘We have proposed a methodology to assess the robustness of underground tunnels against potential failure. This involves developing vulnerability functions for various qualities of rock mass and static loading intensities. To account for these variations,we utilized a Monte Carlo Simulation(MCS)technique coupled with the finite difference code FLAC^(3D),to conduct two thousand seven hundred numerical simulations of a horseshoe tunnel located within a rock mass with different geological strength index system(GSIs)and subjected to different states of static loading. To quantify the severity of damage within the rock mass,we selected one stress-based(brittle shear ratio(BSR))and one strain-based failure criterion(plastic damage index(PDI)).Based on these criteria,we then developed fragility curves.Additionally,we used mathematical approximation techniques to produce vulnerability functions that relate the probabilities of various damage states to loading intensities for different quality classes of blocky rock mass. The results indicated that the fragility curves we obtained could accurately depict the evolution of the inner and outer shell damage around the tunnel. Therefore,we have provided engineers with a tool that can predict levels of damages associated with different failure mechanisms based on variations in rock mass quality and in situ stress state. Our method is a numerically developed,multi-variate approach that can aid engineers in making informed decisions about the robustness of underground tunnels.

文摘Surface waves have a considerable effect on vertical mixing in the upper ocean. In the past two decades, the vertical mixing induced through nonbreaking surface waves has been used in ocean and climate models to improve the simulation of the upper ocean. Thus far, several nonbreaking wave-induced mixing parameterization schemes have been proposed;however,no quantitative comparison has been performed among them.In this paper,a one-dimensional ocean model was used to compare the performances of five schemes, including those of Qiao et al.(Q),Hu and Wang(HW),Huang and Qiao(HQ),Pleskachevsky et al.(P),and Ghantous and Babanin(GB).Similar to previous studies, all of these schemes can decrease the simulated sea surface temperature(SST),increase the subsurface temperature, and deepen the mixed layer, thereby alleviating the common thermal deviation problem of the ocean model for upper ocean simulation.Among these schemes, the HQ scheme exhibited the weakest wave-induced mixing effect, and the HW scheme exhibited the strongest effect;the other three schemes exhibited roughly the same effect. In particular, the Q and P schemes exhibited nearly the same effect. In the simulation based on observations from the Ocean Weather Station Papa, the HQ scheme exhibited the best performance, followed by the Q scheme. In the experiment with the HQ scheme, the root-mean-square deviation of the simulated SST from the observations was 0.43℃,and the mixed layer depth(MLD)was 2.0 m.As a contrast, the deviations of the SST and MLD reached 1.25℃ and 8.4 m,respectively,in the experiment without wave-induced mixing.
文摘BACKGROUND Hypertension is a major risk factor for cardiovascular disease and stroke, and its prevalence is increasing worldwide.Health education interventions based on the health belief model(HBM)can improve the knowledge, attitudes, and behaviors of patients with hypertension and help them control their blood pressure.AIM To evaluate the effects of health education interventions based on the HBM in patients with hypertension in China.METHODS Between 2021 and 2023,140 patients with hypertension were randomly assigned to either the intervention or control group. The intervention group received health education based on the HBM,including lectures, brochures, videos, and counseling sessions, whereas the control group received routine care.Outcomes were measured at baseline, three months, and six months after the intervention and included blood pressure, medication adherence,self-efficacy,and perceived benefits, barriers, susceptibility, and severity.RESULTS The intervention group had significantly lower systolic blood pressure[mean difference(MD):-8.2 mmHg,P<0.001]and diastolic blood pressure(MD:-5.1 mmHg,P=0.002)compared to the control group at six months. The intervention group also had higher medication adherence(MD:1.8,P<0.001),self-efficacy(MD:12.4,P<0.001),perceived benefits(MD:3.2,P<0.001),lower perceived barriers(MD:-2.6,P=0.001),higher perceived susceptibility(MD:2.8,P=0.002),and higher perceived severity(MD:3.1,P<0.001)than the control group at six months.CONCLUSION Health education interventions based on the HBM effectively improve blood pressure control and health beliefs in patients with hypertension and should be implemented in clinical practice and community settings.

文摘Safety evaluation of toppling rock slopes developing in reservoir areas is crucial. It can aid in the predication, control, and even prevention of disasters.

文摘Both the attribution of historical change and future projections of droughts rely heavily on climate modeling. Our results suggest that it is imperative to employ bias correction approaches in drought-related studies over drylands by using CMIP6 outputs.

文摘Dispersed computing is a new resourcecentric computing paradigm.Due to its high degree of openness and decentralization,it is vulnerable to attacks, and security issues have become an important challenge hindering its development. The trust evaluation technology is of great significance to the reliable operation and security assurance of dispersed computing networks. In this paper,a dynamic Bayesian-based comprehensive trust evaluation model is proposed for dispersed computing environment.Specifically,in the calculation of direct trust,a logarithmic decay function and a sliding window are introduced to improve the timeliness. In the calculation of indirect trust,a random screening method based on sine function is designed, which excludes malicious nodes providing false reports and multiple malicious nodes colluding attacks. Finally, the comprehensive trust value is dynamically updated based on historical interactions, current interactions and momentary changes.Simulation experiments are introduced to verify the performance of the model.Compared with existing model, the proposed trust evaluation model performs better in terms of the detection rate of malicious nodes, the interaction success rate, and the computational cost.

文摘A quantitative evaluation model that integrates kerogen adsorption and clay pore adsorption of shale oil was proposed, and the evaluation charts of adsorption-swelling capacity of kerogen(Mk)and adsorbed oil capacity of clay minerals(Mc)were established, taking the 1st member of Cretaceous Qingshankou Formation in the northern Songliao Basin as an example. The model and charts were derived from swelling oil experiments performed on naturally evolved kerogens and adsorbed oil experiments on clays(separated from shale core samples). They were constructed on the basis of clarifying the control law of kerogen maturity evolution on its adsorption-swelling capacity, and considering the effect of both the clay pore surface area that occupied by adsorbed oil and formation temperature. The results are obtained in four aspects:(1)For the Qing 1 Member shale, with the increase of maturity,Mk decreases.Given Ro of 0.83%–1.65%,Mk is about 50–250 mg/g.(2)The clay in shale adsorbs asphaltene.Mc is 0.63 mg/m^(2),and about 15%of the clay pore surface is occupied by adsorbed oil.(3)In the low to medium maturity stages, the shale oil adsorption is controlled by organic matter. When Ro>1.3%,the shale oil adsorption capacity is contributed by clay pores.(4)The oil adsorption capacity evaluated on the surface at room temperature is 8%–22%(avg.15%)higher than that is held in the formations. The proposed evaluation model reveals the occurrence mechanisms of shale oils with different maturities, and provides a new insight for estimating the reserves of shale oil under formation temperature conditions.
文摘BACKGROUND Gestational diabetes mellitus(GDM)is a condition characterized by high blood sugar levels during pregnancy. The prevalence of GDM is on the rise globally, and this trend is particularly evident in China, which has emerged as a significant issue impacting the well-being of expectant mothers and their fetuses.Identifying and addressing GDM in a timely manner is crucial for maintaining the health of both expectant mothers and their developing fetuses. Therefore, this study aims to establish a risk prediction model for GDM and explore the effects of serum ferritin, blood glucose, and body mass index(BMI)on the occurrence of GDM.AIM To develop a risk prediction model to analyze factors leading to GDM,and evaluate its efficiency for early prevention.METHODS The clinical data of 406 pregnant women who underwent routine prenatal examination in Fujian Maternity and Child Health Hospital from April 2020 to December 2022 were retrospectively analyzed.According to whether GDM occurred, they were divided into two groups to analyze the related factors affecting GDM.Then, according to the weight of the relevant risk factors, the training set and the verification set were divided at a ratio of 7:3.Subsequently,a risk prediction model was established using logistic regression and random forest models, and the model was evaluated and verified.RESULTS Pre-pregnancy BMI,previous history of GDM or macrosomia, hypertension, hemoglobin(Hb)level, triglyceride level, family history of diabetes, serum ferritin, and fasting blood glucose levels during early pregnancy were determined. These factors were found to have a significant impact on the development of GDM(P<0.05).According to the nomogram model’s prediction of GDM in pregnancy, the area under the curve(AUC)was determined to be 0.883[95%confidence interval(CI):0.846-0.921],and the sensitivity and specificity were 74.1%and 87.6%,respectively.The top five variables in the random forest model for predicting the occurrence of GDM were serum ferritin, fasting blood glucose in early pregnancy,pre-pregnancy BMI,Hb level and triglyceride level. The random forest model achieved an AUC of 0.950(95%CI:0.927-0.973),the sensitivity was 84.8%,and the specificity was 91.4%.The Delong test showed that the AUC value of the random forest model was higher than that of the decision tree model(P<0.05).CONCLUSION The random forest model is superior to the nomogram model in predicting the risk of GDM.This method is helpful for early diagnosis and appropriate intervention of GDM.
