Plexiform angiomyxoid myofibroblastic tumor of the stomach is a unique mesenchymal tumor that we first described in 2007.The tumor is very rare,and to date,only 18 cases confirmed by immunohistochemistry have been reported in the literature.The patients' ages ranged from 7 to 75 years(mean,43 years),and the male-to-female ratio was approximately 1:1.Representative clinical symptoms are ulceration,associated upper gastrointestinal bleeding(hematemesis),and anemia.The tumors are located at the antrum in all cases,and grossly,the tumor is whitish to brownish or reddish,and forms a lobulated submucosal or transmural mass.Microscopically,the tumor is characterized by a plexiform growth pattern,the proliferation of cytologically bland spindle cells,and a myxoid stroma that is rich in small vessels and positive for Alcian blue stain.Immunohistochemically,the tumor cells are positive for α-smooth muscle actin and negative for KIT and CD34.Differential diagnoses include gastrointestinal stromal tumor and other mesenchymal tumors of the gastrointestinal tract.Some authors proposed that this tumor should be designated as "plexiform fibromyxoma",but this designation might cause confusion.The tumor is probably benign and thus far,neither recurrence nor metastasis has been reported. 展开更多

目的:探讨肝脏炎性肌纤维母细胞瘤(hepatic inflammatory myofibroblastic tumor,HIMT)的临床特点及治疗方法.方法:回顾性分析近10年收治的24例HIMT患者的临床资料.结果:右上腹痛、发热为主要临床表现,术前检查白细胞、甲胎蛋白(alpha f... 目的:探讨肝脏炎性肌纤维母细胞瘤(hepatic inflammatory myofibroblastic tumor,HIMT)的临床特点及治疗方法.方法:回顾性分析近10年收治的24例HIMT患者的临床资料.结果:右上腹痛、发热为主要临床表现,术前检查白细胞、甲胎蛋白(alpha fetoprotein,AFP)、癌胚抗原(carcino-embryonic antigen,CEA)、糖类抗原19-9(carbohydrate antigen19-9,CA19-9)、癌抗原125(cancer antigen 125,CA125)多数正常,HBsAg(+)11例,合并胆囊结石1例,肝内胆管结石1例,肝吸虫感染5例,糖尿病3例.B超、CT、MRI表现多样化,增强后延迟期多表现为边缘强化或内部分隔样强化.病理检查可见胶原性基质中呈束状排列的梭形细胞伴淋巴细胞、浆细胞等炎性细胞浸润.免疫组织化学显示梭形细胞表达波形蛋白Vimentin(8/8)及平滑肌肌动蛋白(smooth muscle actin,SMA)(8/8)均为阳性.15例行肿瘤完整切除,9例仅行活检;中位随访46 mo,5例失访,19例均健康存活.结论:HIMT无特异性临床表现,CT、MRI增强扫描后的影像学表现反映了其病理学特征,对其诊断有重要价值,诊断不明或不能排除恶性肿瘤者建议手术治疗. 展开更多

Angiogenesis is a dynamic,hypoxia-stimulated and growth factor-dependent process,and is currently referred to as the formation of new vessels from preexisting blood vessels.Experimental and clinical studies have unequivocally reported that hepatic angiogenesis,irrespective of aetiology,occurs in conditions of chronic liver diseases(CLDs) characterized by perpetuation of cell injury and death,inflammatory response and progressive fibrogenesis.Angiogenesis and related changes in liver vascular architecture,that in turn concur to increase vascular resistance and portal hypertension and to decrease parenchymal perfusion,have been proposed to favour fibrogenic progression of the disease towards the end-point of cirrhosis.Moreover,hepatic angiogenesis has also been proposed to modulate the genesis of portal-systemic shunts and increase splanchnic blood flow,thus potentially affecting complications of cirrhosis.Hepatic angiogenesis is also crucial for the growth and progression of hepatocellular carcinoma.Recent literature has identified a number of cellular and molecular mechanisms governing the cross-talk between angiogenesis and fibrogenesis,with a specifi c emphasis on the crucial role of hypoxic conditions and hepatic stellate cells,particularly when activated to the myofibroblast-like pro-fibrogenic.Experimental anti-angiogenic therapy has been proven to be effective in limiting the progression of CLDs in animal models.From a clinical point of view,anti-angiogenic therapy is currently emerging as a new pharmacologic intervention in patients with advanced fibrosis and cirrhosis. 展开更多

目的 探讨儿童腹部肌纤维母细胞瘤(IMT)的CT表现。方法 收集2008~2013年经病理证实的IMT患儿10例,术前均经CT平扫及增强检查,对照术后病理结果 并分析肿瘤的CT表现。结果 10例患儿均为单发病例,发病部位肠系膜3例,小网膜1例,肝... 目的 探讨儿童腹部肌纤维母细胞瘤(IMT)的CT表现。方法 收集2008~2013年经病理证实的IMT患儿10例,术前均经CT平扫及增强检查,对照术后病理结果 并分析肿瘤的CT表现。结果 10例患儿均为单发病例,发病部位肠系膜3例,小网膜1例,肝脏1例,腹膜后2例,腹腔2例,乙状结肠1例。CT平扫表现为实性肿块8例,囊实混合性肿块2例。增强扫描显示实性部分均呈明显不均匀强化,动脉期无明显强化1例,其余9例患儿均呈明显不均匀强化。静脉期10例患儿均呈明显强化,强化后CT值均大于95 Hu,且呈逐渐强化的趋势。病理结果 显示肿瘤组织由梭形细胞组成。结论 小儿腹部IMT的CT表现有一定特点,能提供有价值的诊断信息。 展开更多

【目的】观察阿托伐他汀对肺纤维化大鼠的肺成纤维细胞增殖的影响及其对TGF-β1诱导的大鼠肺成纤维细胞表型分化的影响。【方法】建立Wistar大鼠肺纤维化模型,1周后取肺组织进行成纤维细胞原代培养,细胞经鉴定后以不同浓度的阿托伐他汀... 【目的】观察阿托伐他汀对肺纤维化大鼠的肺成纤维细胞增殖的影响及其对TGF-β1诱导的大鼠肺成纤维细胞表型分化的影响。【方法】建立Wistar大鼠肺纤维化模型,1周后取肺组织进行成纤维细胞原代培养,细胞经鉴定后以不同浓度的阿托伐他汀对试验组细胞进行干预,采用MTT法测定细胞增殖情况;TGF-β1诱导细胞分化,同时给予不同浓度阿托伐他汀干预,免疫细胞化学染色(SP)法检测药物对肌成纤维标志物-αSMA表达的影响。【结果】MTT结果显示:各剂量组中不同干预天数所测得的OD值之间存在显著差异(P<0.01)。各时间点所测OD值与不同用药物剂量之间存在交互作用(P<0.01)。各剂量组对细胞的抑制作用之间存在显著差别(P<0.01)。免疫细胞化学染色结果显示:药物因素对-αSMA的表达量有影响(P<0.01),区组因素对-αSMA的表达量有影响(P<0.01)。【结论】阿托伐他汀可抑制肺纤维化大鼠肺成纤维细胞的增殖,同时可抑制TGF-β1诱导的成纤维细胞的分化,这主要表现在抑制细胞增殖、改变其形态、减少肌成纤维细胞的标志性产物-αSMA的产生上。 展开更多

Myofibroblasts,recognized classically by-smooth muscle actin(-SMA)expression,play a key role in the wound-healing process,promoting wound closure and matrix deposition.Although a body of evidence shows that keratinocytes explanted onto a wound bed promote closure of a skin injury,the underlying mechanisms are not well understood.The basal layer of epidermis is rich in undifferentiated keratinocytes(UKs).We showed that UKs injected into granulation tissue could switch into-SMA positive cells,and accelerate the rate of skin wound healing.In addition,when the epidermis sheets isolated from foreskin cover up the wound bed or are induced in vitro,keratinocytes located at the basal layers or adjacent sites were observed to convert into myofibroblast-like cells.Thus,UKs have a potential for myofibroblastic transition,which provides a novel mechanism by which keratinocyte explants accelerate skin wound healing. 展开更多
