目的探讨血尿酸(UA)、N末端B型利钠肽原(NT-proBNP)、血小板与淋巴细胞比值(PLR)在急性脑梗死(AIS)静脉溶栓中动态变化及意义。方法选取沧州市人民医院105例AIS患者,均行静脉溶栓治疗,根据发病90 d改良Rankin量表评分(mRS)分为预后不良...目的探讨血尿酸(UA)、N末端B型利钠肽原(NT-proBNP)、血小板与淋巴细胞比值(PLR)在急性脑梗死(AIS)静脉溶栓中动态变化及意义。方法选取沧州市人民医院105例AIS患者,均行静脉溶栓治疗,根据发病90 d改良Rankin量表评分(mRS)分为预后不良组(n=26)、良好组(n=79),比较两组溶栓前、溶栓后24 h、溶栓后7 d UA、NT-proBNP、PLR水平及发病90 d mRS评分,分析预后的相关影响因素,并分析溶栓后24 h、溶栓后7 d UA、NT-proBNP、PLR水平与发病90 d mRS评分关系及预测预后不良的价值。结果不良组溶栓后24 h、溶栓后7 d UA、NT-proBNP、PLR水平均高于良好组,差异有统计学意义(P<0.05);不良组发病后90 d mRS评分高于良好组,差异有统计学意义(P<0.05);溶栓后24 h、溶栓后7 d UA、NT-proBNP、PLR水平与发病后90 d mRS评分间存在正相关关系(P<0.05),与AIS患者预后显著相关(P<0.05);溶栓后24 h、溶栓后7 d UA、NT-proBNP、PLR预测AIS患者预后不良的曲线下面积(AUC)均>0.7,溶栓后7 d各指标联合预测的AUC最大,为0.953。结论UA、NT-proBNP、PLR水平在脑梗死静脉溶栓中随着病情改善逐渐降低,与患者预后显著相关,可辅助临床预测患者预后转归方向。展开更多
