Endovascular treatment of delayed arterial hemorrhage after radical operation for hilar cholangiocarcinoma

Radical operation for hilar cholangiocarcinoma(HCCA)is the most effective treatment,but high rates of severe postoperative complication and death remain concerns due to the complexity of biliary and vascular anatomy of the hepatobiliary region.De-layed arterial hemorrhage(DAH)occurring>24 h postoperatively usually causes life-threatening bleeding,and relaparotomy for DAH would be difficult and hazardous because of postoperative adhe-sions and critical general condition.As endovascular technique is less invasive and has improved over the recent decades,clini-cians have begun to prefer endovascular treatment(EVT)to surgi-cal treatment of DAH after hepatobiliary pancreatic surgery.How-ever,to our knowledge,a relatively large series of patients dealing with the outcomes of EVT of DAH after surgery for HCCA has not been reported.Herein,we analyzed the technical and clinical out-comes of EVT in 17 patients to evaluate the efficacy and safety of EVT.

Multidisciplinary non-surgical treatment of advanced periodontitis:A case report

BACKGROUND Patients with severe periodontitis often require multidisciplinary treatment to achieve healthy periodontal tissue,normal occlusion,and optimal aesthetics.In the present study,we aimed to evaluate the efficacy of multidisciplinary non-surgical treatment in a patient with stage IV/grade C periodontitis,malocclusion,and dentition defects.CASE SUMMARY A 47-year-old woman visited our periodontology department due to teeth mobility problems and difficulties chewing food.The patient had no history of drug allergies or systemic disease.Initial therapy involved scaling and root planning with a Gracey scaler and periodontal ultrasonic therapeutic apparatus using a periodontal endoscope(Perioscopy,Zest Dental Solutions,United States)to control the periodontal inflammation prior to treatment.Five months later,orthodontic treatment was then performed to treat occlusion and overall aesthetics.After completion,a Maryland bridge was used to restore Nos.22,31,and 41 teeth.Florida probing(Florida probe,United States)was performed every 2-3 mo to evaluate the periodontal condition throughout treatment.Overall,multidisciplinary non-surgical treatment resulted in satisfactory aesthetic results with healthy periodontal tissue and stable occlusion.CONCLUSION In some patients with stage IV/grade C periodontitis,systematic and sequential non-surgical treatment can provide excellent therapeutic results.

Discussion on the Operation of Urban Domestic Sewage Treatment Facilities: A Case Study of Guangdong Province

Urban sewage treatment facility is an important ways to control polluted domestic water. The operation of urban sewage treatment facilities is good or not, and whether the effectiveness of pollutant emission reduction can be exerted play a significant role in reducing the pollution of living sources. Nowadays the operation of urban sewage treatment facilities is mainly evaluated by the load rate. However, due to the failure to fully implement the reformation of the rainwater and polluted water in Guangdong Province, the domestic sewage is mixed with rainwater during the rainy season and is included in the treatment of urban sewage treatment facilities. Therefore, it is not objective to evaluate the operation of sewage treatment facilities using only the load rate of sewage treatment. According to the situation of Guangdong Province, the load rates of COD, ammonia nitrogen and total phosphorus in domestic sewage and load rate of sewage treatment were included in the evaluation, and 20 urban sewage treatment facilities were selected as research objects. The operation situations and emission reduction benefits of urban sewage treatment facilities in different regions of Guangdong Province were roughly evaluated.

Non-Surgical Orthodontic Treatment of Class III Adult with Posterior Crossbite, Anterior Open Bite and Asymmetric Occlusion, Using TADs and Auxiliary Wires: A Case Report

Class III malocclusion associated with posterior crossbite, anterior open bite pattern with asymmetric occlusion in adults can be a challenging orthodontic problem, especially for the nonsurgical treatment. Skeletal Class III anteroposterior discrepancies in adult patients are generally managed either by surgical-orthodontic treatment or by orthodontic camouflage through dentoalveolar compensation. Although correction with surgery may be the most effective and stable way, many patients refuse surgical treatment plan because of the costs and traumas it may bring. This case report describes the successful use of TADs with expansion auxiliary wire to treat a 24-year-old man with skeletal class III malocclusion, posterior crossbite, anterior open-bite and asymmetric occlusion, mild upper and lower dental spacing and a chief manifestation of anterior crossbite. The patient refused surgery. A treatment plan was formulated consisting of using auxiliary expansion wire to expand the maxillary arch, 8 mm mini-screws between the roots of the mandibular canines and first premolars, preadjusted edgewise brackets to align the teeth, Class III and asymmetric elastics to correct the canines, premolars, and molars relationship and midline deviation, reverse curve of the nickel-titanium wire combined with anterior vertical elastics to intrude molars and correct the anterior open-bite. In this case, without going through surgery, the posterior cross-bite was corrected, and ideal overjet and overbite relationships, midline coincidence and functional occlusion were all achieved. Satisfactory occlusal, functional, esthetic and stable results were obtained.

Non-Surgical Treatment with Chemical Peeling and Subsequent Vitamin C Iontophoresis for Rolling Scars in Acne Vulgaris

A 19-year-old man had atrophic scars on bilateral temple for several years. The atrophic scars were composed of rolling scars (three depressions, 8 × 4 mm in diameter), ice pick scars and boxcar scar. The patient was treated with chemical peeling (CP) using 20% glycolic acid (GA) (pH 3.2) and subsequent iontophoresis with vitamin C derivative at one month’s interval twice. Remarkable improvement was observed;the rolling scars almost disappeared with traces of hypopigmented macule. However, the ice pick scars and boxcar scar did not improve. CP and subsequent vitamin C iontophoresis can be an alternative non-surgical and non-invasive treatment for rolling scar in acne.

Conventional operation and laser therapy in the treatment of varicose veins

Objective:To evaluate the effectiveness of endovenous laser therapy and conventional thera- py on the varicose of great saphenous vein.Methods:Thirty-two patients received endovenous laser thera- py and 32 patients were operated by conventional therapy(high ligation and stripping).The observation results of great saphenous vein(GSV)were recorded by clinical evaluation and duplex ultrasound examina- tion.And the operating time,intraoperative blood loss,time to become moveable,duration of hospitaliza- tion and degree of feeling pain were compared between the two groups.Results:Compared with conven- tional therapy,laser therapy had good curative effect with less complications and no scars and no pains. There were significant differences between the two groups(P<0.01).Conclusion:Laser therapy for varicose of great saphenous vein is better than the conventional therapy.It deserves to be widely used in clinical treatment.

Concurrent Multi-Modality Treatment of Keloids (CMTK) Not Manageable by Conventional Postoperative Radiotherapy

Objective: To design and test a treatment regimen which is clinically responsive, readily available, cost effective, and applicable especially to children and women of child bearing age. Design Setting: A prospective cohort study. Setting: Two major postgraduate teaching hospitals: one in Tripoli, Libya and the other in Jeddah, Saudi Arabia. Participants: Fifty-seven patients with 79 keloids, referred from Plastic Surgery Units between April 1996 and January 2005. Main Outcome Measure: Degree of flattening of the keloidal lesion and symptomatic recovery. Results: Result of treatment has been analyzed using unified set criteria. Seventy-seven percent of this cohort had complete response. 19% of cases had partial response, 50% acknowledged the treatment outcome had been “satisfactory” and 44% had an “acceptable” outcome. There was no significant acute or delayed reaction. Conclusion: The technique appears universally adaptable, cost effective, and can safely be prescribed for children and women of child-bearing age. In spite of prolonged treatment course, compliance was excellent.

THE OPERATIVE TREATMENT OF SPINAL FRACTURE-DISLOCATION WITHOUT NEUROLOGIC DEFICITS

To evaluate the results of operative treatment of spinal fracture dislocation without neurologic deficits. Methods.Eighteen patients with spinal fracture dislocation were neurologically intact at the time of injury, and all were treated operatively. The fracture sites were:8 cases in cervical spine, 3 cases in thoracic spine, and 7 cases in lumbar spine. Eight patients with cervical injuries had variant degrees of forward slide and kyphotic deformity. Of the 10 thoracic and lumbar fractures, one had lateral dislocation, 4 cases with kyphotic deformities, 5 cases with spinal canal compromise averaged 50% (ranging from 40% to 70%). Results.The average period of follow up was 4.4 years with a range of 11 months to 13 years. All the patients returned to full time work. No patient developed neurologic deterioration. Kyphotic deformity was corrected in the 4 cases, and no progressive kyphosis was noted. There was no operation related complication. The averaged post operative hospitalization time was 13 days. Conclusions. Despite the rare incidence of spinal fracture dislocation without neurologic deficits, we suggested that kind of fracture be considered unstable fracture because of its potential risk of delayed neurologic deterioration and kyphotic deformity, and be treated operatively to restore the sagittal alignment and the stability of the spine.

Essential prerequisite of laparoscopic operation is establishing and maintaining pneumoperitoneum with a certain pressure. But under certain circumstances CO2-gas insuffiating could induce pneumothorax , pneumomediastinum and subcutaneous emphysema during the operation. to probe the mechanism of the above complications. 2 typical cases were reported with attempt to probe the mechanism of the development of such applications. We believe that high intraabdominal pressure during operation is the primary cause of the complications. The essentials of prevention and management of such complications were proposed.

Controlling of crustacean zooplankton reproduction in biological activated carbon (BAC) filters by strengthen operation and management of conventional process

To counter the mass reproduction and penetration of crustacean zooplankton in Biological Activated Carbon(BAC)filters which may result in the presence of organisms in potable water and water pollution,this paper analyzed the factors affecting organisms' reproduction in BAC filters.A comparative study was performed on the density and composition of crustacean zooplankton of the concerned water treatment units of two advanced water plants(Plant A and B)which with the same raw water and the same treatment technique in southern China.The results obtained show that the crustaceans' density and composition was very different between the sand filtered water of Plant A and Plant B.which Harpacticoida bred sharply in the sediment tanks and penetrated sand filter into BAC filters was the primary reason of crustaceans reproduce in BAC filters of Plant A.For prevention of the organisms reproduction in BAC,some strengthen measures was taken including pre-chlorination,cleaning coagulation tanks and sediment tanks completely,increasing sludge disposal frequency to stop organisms enter BAC filters,and the finished water quality was improved and enhanced.

Combined Endovascular and Surgical Treatment for Brain Arteriovenous Malformations in Biplanar Hybrid Operating Room

Objective Combined surgical and endovascular treatment for vascular disorders has become prevalent in recent years.However,reports on one-session hybrid surgery for arteriovenous malformations(AVMs)are relatively rare.The safety and efficiency of combined treatment for brain AVMs were analyzed in biplanar hybrid operating room(OR)at one stage.Methods We retrospectively analyzed 20 patients with AVMs undergoing combined surgical and endovascular treatment from October 2015 to June 2018.The data for resection rate,microcatheter adhesion,surgical position and postoperative outcomes were analyzed.Total resection or near-total resection was achieved in all cases.Results A total of 13 patients were under combined endovascular and surgical procedures,and 7 experienced surgery with intraoperative digital subtraction angiography.Sitting position was applied in 3 of them;2 niduses in cerebellum,and 1 in parietal lobe.Compared with admission modified Rankin Scale(mRS)in all patients,postoperative 12-month mRS showed a significant decline.Besides,3 patients experienced microcatheter adhesion after endovascular embolization,thereafter underwent surgical adhesion removal while nidus resection was done.Conclusion Combined endovascular and surgical modality in a hybrid OR at one stage provides a safe strategy for the treatment of AVMs.The biplanar hybrid neurointerventional suite is endowed with unconstrained operating angle which enables combined endovascular and surgical treatment in sitting position.It also reduces the risk of microcatheter adhesion,which enables interventional radiologists to perform aggressively.

The perioperative management of 81 cases with pheochromocytoma was reviewed including blood pressure and arrhythmia control, blood volume expansion, patient′s general condition improvement and vital signs monitoring. It was stressed that pre and intra operative management was of prime importance in the treatment of pheochromocytoma.

Countermeasures on improving the charge rules and long-acting operation mechanism for the treating residential domestic sewage and household garbage in rural Yunnan

Improving the rural living environment in rural China is one of the key tasks for the country to accomplish its goal of building a moderately prosperous society by 2020. Yunnan has highly focused on the task of the treatment of residential domestic sewage and household garbage in rural area. Many efforts and resources have been put into this field in Yunnan since 2016. Progress has been made to increase the coverage rate of the sewage and household garbage treatment facilities. Seventy-five percent of total administrative villages have built up garbage transportation system and treatment facilities. Sixty-three percent of towns have collected and treated the residential domestic sewage by constructing various scale sewage treatment stations. However, the lack of the long-acting operation mechanism and the imperfection of the charge rules for the sewage and garbage treatment facilities have become problems that would hinder the achieving of the environmental goals in Yunnan. The reasons were elaborated on the basis of the local actual situations. Therefore, it is significant to improve the charge rules and frame the long-acting operation mechanism by strengthening the governance capacity, frame an overall mechanism and encourage the mass to be involved in the improvements of the living environment in rural Yunnan.

Emergency Treatment of Testicular Torsion and Postoperative Follow-up: A 71 Case Report

This study presented our experience in the treatment of testicular torsion,which may help achieve early diagnosis and improve therapeutic effects.A retrospective analysis was conducted in 71 patients with testicular torsion who were treated in our hospital from October 2007 to April 2011.The age of the patients ranged from 16 days to 34 years.All the patients had unilateral testicular torsion,which took place on the left side in 43 cases and on the right side in 28 cases.The course of the disease varied between three hours to 30 days.Post-operative follow-up was conducted until October 2011.Items examined included signs and symptoms at their first clinical visit,ultrasound findings,treatment in emergency surgery,and post-operative follow-up.In this study,the 71 patients were diagnosed with testicular torsion by color Doppler sonography,7 had testicular fixation,63 patients received orchiectomy,while 1 patient did not undergo surgery due to pressure from family members.Post-operative follow-up showed that the one patient’s testicle,which had been reserved,atrophied,while all the other survived.No recurrence was found during the follow-up visits.It is concluded that an early diagnosis and surgery is important in improving the survival rate of testicular torsion,and the diagnosis and treatment by the first attending clinician is of critical importance.

CLINICAL OBSERVATION ON ACUPOINT DIGITAL PRESSING FOR TREATING IRRITABLE SYMPTOMS AND SIGNS OF POSTOPERATION OF MYOPORTHOSIS

Objective: To explore a new effective approach for treatment of the local irritable symptoms and signs resulted from myoporthosis operation. Methods: 263 cases of radial keratotomy outpatients were divided into control group (n=49, 90 eyes) and treatment group (n=214, 412 eyes). Patients of control group were treated with 0.5% Chloramphenicol eye drops, 3 times daily, 1-2 drops/time, and with 0.5% Erythromycine eye ointment every night. In treatment group, patients were treated with acupoint and hand reflection area digital pressing method except with the same eye drops and eye ointment. Body acupoints used were Cuanzhu (BL 2), Yuyao (EX HN 4), Sizhukong (TE 23), Sibai (ST 2) and Taiyang (EX HN 5). The hand reflex areas used were Eye Area, Liver Area, Kidney Area, Spleen Area, Heart Area and Lung Area. Five days constituted a therapeutic course. Results: Following 3 courses (15 days) of treatment, the markedly effective rate and total effective rate of treatment group and control group were 74.51% and 38.89%, 94.17% and 66.67% respectively. There was a significant difference between two groups in the total effective rate (P<0.05), showing the therapeutic effect of treatment group being better than that of control group. Conclusion: Acupoint digital pressing is rather effective in treatment of myoporthosis post operation irritable symptoms and signs.
