Purpose:Delivering quality health care requires effective communication between health care providers and their patients.Nurse-patient effective communication remains a challenge in Ghana,despite criticism and concern...Purpose:Delivering quality health care requires effective communication between health care providers and their patients.Nurse-patient effective communication remains a challenge in Ghana,despite criticism and concern expressed by the public.The study,therefore,aims to assess the effective nurse caregivers’communication practices among pediatric nurses at Pentecost Hospital,Madina.Methods:A qualitative exploratory and purposive sampling technique were used.Participants were engaged in Focus group discussions or face-to-face interviews.In all,4 focus group discussions were conducted with 7 participants in each group and 15 face-face in-depth interviews,with a total sample size of 43.The data was content analyzed.Interviews were taped recorded and transcribed verbatim.Results:The data analysis yielded 3 themes and 11 subthemes.The main themes were:effective communication practices,factors influencing effective communication,and attitudes towards effective communication.The study revealed how the participants communicated with patients who are deafmute,anxious,and those who refused treatment.In addition,participants reported how they used techniques such as silence,listening,and leading cues in communicating with patients and their relatives.Conclusion:Effective communication between pediatric nurses,patients,and their parents is ascertained to be a key tool in the delivery of quality health care.However,several factors are found to hinder this therapeutic communication.Hence,further studies are needed to improve effective communication skills among pediatric nurses and their patients to enhance health care for children.展开更多
BACKGROUND Cataracts are a common ophthalmic disease and postoperative vision recovery is crucial to patient quality of life.Rational and efficient care models play an impor-tant role in promoting vision recovery.AIM ...BACKGROUND Cataracts are a common ophthalmic disease and postoperative vision recovery is crucial to patient quality of life.Rational and efficient care models play an impor-tant role in promoting vision recovery.AIM To evaluate the clinical effectiveness of procedural nursing care combined with communication intervention in vision recovery after cataract ultrasound emulsi-fication.METHODS A randomized controlled study was conducted on 100 patients with cataracts who underwent ultrasound emulsification surgery.They were randomly assigned to an experimental group or a control group.The experimental group received procedural nursing combined with Connect,Introduce,Communicate,Ask,Respond,Exit(CICARE)communication intervention,whereas the control group received conventional nursing.The effectiveness of the nursing model was assessed by comparing differences in vision recovery,pain scores,and mental health status between the two groups.RESULTS It was found that over time the visual acuity of patients in both groups gradually recovered and patients in the experimental group had lower pain scores and superior mental health status than the control group(P<0.05).CONCLUSION Procedural nursing combined with CICARE communication intervention has positive effects on vision recovery in patients after cataract ultrasound emulsification.展开更多
BACKGROUND Severe pneumonia is a common severe respiratory infection worldwide,and its treatment is challenging,especially for patients in the intensive care unit(ICU).AIM To explore the effect of communication and co...BACKGROUND Severe pneumonia is a common severe respiratory infection worldwide,and its treatment is challenging,especially for patients in the intensive care unit(ICU).AIM To explore the effect of communication and collaboration between nursing teams on the treatment outcomes of patients with severe pneumonia in ICU.METHODS We retrospectively analyzed 60 patients with severe pneumonia who were treated at the ICU of the hospital between January 1,2021 and December 31,2023.We compared and analyzed the respiratory mechanical indexes[airway resistance(Raw),mean airway pressure(mPaw),peak pressure(PIP)],blood gas analysis indexes(arterial oxygen saturation,arterial oxygen partial pressure,and oxygenation index),and serum inflammatory factor levels[C-reactive protein(CRP),procalcitonin(PCT),cortisol(COR),and high mobility group protein B1(HMGB1)]of all patients before and after treatment.RESULTS Before treatment,there was no significant difference in respiratory mechanics index and blood gas analysis index between 2 groups(P>0.05).However,after treatment,the respiratory mechanical indexes of patients in both groups were significantly improved,and the improvement of Raw,mPaw,plateau pressure,PIP and other indexes in the combined group after communication and collaboration with the nursing team was significantly better than that in the single care group(P<0.05).The serum CRP and PCT levels of patients were significantly decreased,and the difference was statistically significant compared with that of nursing group alone(P<0.05).The levels of serum COR and HMGB1 before and after treatment were also significantly decreased between the two groups.CONCLUSION The communication and collaboration of the nursing team have a significant positive impact on respiratory mechanics indicators,blood gas analysis indicators and serum inflammatory factor levels in the treatment of severe pneumonia patients in ICU.展开更多
Effective communication among healthcare professionals in the intensive care unit (ICU) is a particular imperative,with accurate and efficient interdisciplinary communication being a critical prerequisite for high-qua...Effective communication among healthcare professionals in the intensive care unit (ICU) is a particular imperative,with accurate and efficient interdisciplinary communication being a critical prerequisite for high-quality care.Nurses and physicians are highly important parts of the healthcare system workforce.Thus,identifying strategies that would improve communication between these two groups can provide evidence for practical improvement in the ICU,which will ultimately improve patient outcomes.This integrative literature review aimed to identify interventions that improve communication between nurses and physicians in ICUs.Three databases (Medline,CINAHL,and Science Direct) were searched between September 2014 and June 2016 using 11 search terms,namely,nurse,doctor,physician,resident,clinician,ICU,intensive care unit,communication,teamwork,collaboration,and relationship.A manual search of the reference lists of found papers was also conducted.Eleven articles met the inclusion criteria.These studies reported on the use of communication tools/checklists,team training,multidisciplinary structured work shift evaluation,and electronic situation-background-assessment -recommendation documentation templates to improve communication.Although which intervention strategies are most effective remains unclear,this review suggests that these strategies improve communication to some extent.Future studies should be rigorously designed and outcome measures should be specific and validated to capture and reflect the effects of effective communication.展开更多
Objective:This study investigates the emotional intelligence(EI),self-efficacy,and clinical communication ability of clinical nurses to explore the correlation among these three variables,and verify the mediating effe...Objective:This study investigates the emotional intelligence(EI),self-efficacy,and clinical communication ability of clinical nurses to explore the correlation among these three variables,and verify the mediating effect of self-efficacy on relationship between EI and communication skills.Methods:A total of 865 nurses were recruited and investigated using Wong and Law's Emotional Intelligence Scale,General Self-Efficacy Scale,and nurse clinical communication scale.Results:The scores for EI and self-efficacy of nurses were 14.23±2.61 and 25.36±5.67,respectively,which were lower than the international norm(p<0.01).The score for clinical communication ability of nurses was 4.14±0.53.The clinical communication competency of nurses was positively correlated with EI and general self-efficacy.Furthermore,selfefficacy played an intermediary role between EI and clinical communication commitment.Conclusion:Nursing administrators can improve the clinical communication ability of nurses by enhancing their self-efficacy and EI.展开更多

Regarding the mediator or intermediary role of PHNs in the use of CRs, PHNs are expected to be cognizant of varieties of robots for use with persons of diverse ages and health levels, as well as of robot performances. As one of the community healthcare workers who service the elderly living in the community, the role of PHNs as mediators or intermediaries is to pay attention to ethical and moral issues while working with CRs to manage the health of the community. Lack of understanding of the intermediary role might create possible ethico-moral issues in the future. Therefore, it is critical for PHNs to understand their role as intermediaries. Importantly, healthcare workers in Japan are also getting older. In order to address solutions to this problem, using robots is being realized and is starting to assist healthcare and welfare practice needs. The aim of this study was to identify the characteristics of a transactive phenomenon in relationships among older adults with dementia, with nurses as intermediaries and with a cognitive skill-aid robot. Subjects were two institutionalized older adults who were diagnosed with dementia using Hasegawa’s Dementia Scale-Revised;a Kabochan (Nodding Kabochan) fixed with a remote-controlled speaker, Pechat (Kabochan with Pechat). Measurements of autonomic nervous activity were done using wearable electrocardiography attached to the subjects for four hours. Heart Rate Variability data were assessed at various frequency bands using a HRV software tool. In a continuously recorded data, interbeat (R-R) intervals were obtained for a 1-min segment using the maximum entropy method. In this study, the two major spectral components of HRV, the variances of the low-frequency (LF: 0.04 - 0.15 Hz) band and high-frequency (HF: 0.15 - 0.4 Hz) band, were calculated. Activities and behaviors of subjects transacting with Kabochan were observed by the expert nurses and video-recorded. Throughout the transaction period, observations were recorded of patients’ behavior, words uttered and over-all contents of the transactive conversation with the Kabochan or Kabochan with Pechat. Identified characteristics were transactive relationship phenomenon;changes in autonomic nervous system activity of persons with dementia are expressed as per one minute, linguistic response of elderly people, transactive relations among Kabochan, Kabochan with Pechat, and nurses as intermediaries. These were exhibited as Figures: Situation 1, A Sense of Fear, Situation 2, A Pleasant Sensation, and Situation 3, Misunderstanding and Confusion. It also showed that autonomic nervous system activity of persons with communication skills is actively changing in older adults with dementia. Conversation of adding Pechat to the Kabochan illustrated that nurses as intermediaries showed emotional conversations as established by effectively manipulating the Pechat. Also, it was revealed that if Kabochan with Pechat uttered words about patient’s personal information in a conversation, the patient felt a sense of fear. The researchers suggest that when developing future communication robot for elderly with dementia, it is necessary to consider, for an effective conversation, transactive relationships including nurses as intermediaries. The role functions of these intermediaries have been shown of high versatility, indicating that situations such as these can also be controlled by other healthcare providers such as by occupational therapists.展开更多

Objectives:The motives that lead to nurses’workplace silence behaviour are essential to understand,as the failure to voice their concerns and maintain silence may cause various adverse outcomes,including burnout,turn...Objectives:The motives that lead to nurses’workplace silence behaviour are essential to understand,as the failure to voice their concerns and maintain silence may cause various adverse outcomes,including burnout,turnover,job dissatisfaction,low productivity,and mental and moral distress.This study aimed to identify the level and influencing motives of the workplace silence behaviour among nurses at the National Guard Hospital in Madinah.Methods:A descriptive cross-sectional survey was conducted in January and February 2023.The participants completed a questionnaire with two parts:the sociodemographic data form and the Workplace Silence Behavior Nursing Motives Scale.Data analysis was performed by descriptive statistics and the chi-square test.Results:The overall level for all workplace silence behavior motives is moderate(3.29±0.69),with Avoidance Motives dimension receiving the highest scores(3.62±0.75)and Organization-Oriented Motives dimension receiving the lowest(3.15±0.86).Chi-square test showed that Saudi nationality(X^(2)=0.015,P=0.019)and monthly income(X^(2)=0.045,P?0.013)are factors in high-moderate levels of silence behaviour.Conclusions:The overall level of workplace silence behaviour motives was found to be moderate,nurses have higher monthly salaries and non-Saudi nationals tend to have higher levels of workplace silence behaviour motives.The study results provide a reference for nursing managers to pay attention to nurses’workplace silence behavior and take corresponding intervention measures.展开更多
Caring for cancer patients requires both technical and psychosocial nursing skills.The aim of this study was to determine the psychosocial care skill levels of nurses and affecting factors.This multicenter,cross-secti...Caring for cancer patients requires both technical and psychosocial nursing skills.The aim of this study was to determine the psychosocial care skill levels of nurses and affecting factors.This multicenter,cross-sectional study was conducted with 1,189 nurses providing direct care to adult cancer patients in 32 hospitals in 12 geographical regions of Turkey.A questionnaire,the Psychosocial Skills Form,and the Professional Quality of Life Scale were used to collect the data.Nurses’psychosocial skill level was in the range of 2.72±0.98 and 2.47±0.89 out of four points.Communication skills such as empathic response,active listening,and the ability to provide information were found to be at a higher level than skills such as the activation of social support systems,therapeutic touch,and development of coping methods.Approximately 40%of nurses had received psychosocial care training,and 87%were interested in receiving additional psychosocial training.Gender,educational status,previous training in psychosocial care,and work experience with cancer patients were shown to affect psychosocial skill levels.There was a positive relationship between the level of psychosocial skills and the level of compassion satisfaction,and a negative relationship between the level of psychosocial skills and the level of burnout and compassion fatigue(p<0.05).Nurses perceive themselves as having a medium to high level of psychosocial skills yet desire additional training.The results of this study may contribute to the development of training programs according to the needs of nurses who care for cancer patients.展开更多

However, specific therapeutic communication methods have not been systematized. The purpose of this study was to clarify therapeutic communication methods for families/family members from the perspectives of verbal communication and non-verbal communication through a review of existing literature. Methods: We conducted a search using the medical literature databases PubMed and Ichushi-Web using the keywords “therapeutic communication”. Analysis was performed on seven articles from PubMed and 14 articles from Ichushi-Web that described therapeutic communication methods performed by healthcare professionals for families/family members. Through directed content analysis, therapeutic communication methods were subcategorized, and classified into three categories: verbal communication, non-verbal communication, and verbal/non-verbal communication. Results: A total of 23 subcategories were extracted. Verbal communication included 11 subcategories, such as “asking questions using the communicatee’s words as they are”. Non-verbal communication included five subcategories, such as “noticing changes in the content of the communicatee’s story”. And verbal/non-verbal communication featured seven subcategories, such as “making the communicatee aware of one’s own beliefs”. Conclusion: Therapeutic communication methods included basic care/caring in family interviews/meetings, as well as verbal communication and non-verbal communication that act on family/family members’ beliefs. It is believed that changes in family/family members’ beliefs can be used to eliminate, reduce, or improve problematic conditions in the family. .展开更多
Background:To explore the application effect of Acknowledge-Introduce-Duration-Explanation-Thank you(AIDET)communication mode in reducing anxiety and improving the care services satisfaction of the patients.Methods:Th...Background:To explore the application effect of Acknowledge-Introduce-Duration-Explanation-Thank you(AIDET)communication mode in reducing anxiety and improving the care services satisfaction of the patients.Methods:The 100 cases undergoing cataract daytime operation from February to July 2019 were divided into test group and control group.The test group conducted health education and nursing care for patients during admission,pre-operation,post-operation,and discharge according to the AIDET communication mode.And the control group was given health education and nursing care to the patients according to the traditional way of communication.Results:The difference of the socioeconomic information between these two groups showed no statistical significance.The care services satisfaction score in the test group was statistically higher than that in the control group(P<0.05).The Self-Rating Anxiety Scale(SAS)score in the test group was significantly lower than that in the control group(P<0.01).This result indicated that the patients suffered less anxiety with the AIDET mode of communication.Conclusions:The application of AIDET communication mode to the nursing service of cataract daytime operation can reduce the tension and anxiety of the patients from the treatment and improve their satisfaction to care services.展开更多

Objective:To evaluate nursing students’assessment and communication skills through an objective structured clinical examination(OSCE)within a high-fidelity simulation using a student-simulated patient(SSP)and analyze...Objective:To evaluate nursing students’assessment and communication skills through an objective structured clinical examination(OSCE)within a high-fidelity simulation using a student-simulated patient(SSP)and analyze the validity,reliability,degree of difficulty,and discriminability of the OSCE.Methods:A descriptive study was conducted at one nursing school in Macao.All scenarios were designed using a high-fidelity simulator combined with SSPs.A convenience sample of 54 baccalaureate nursing students completed the OSCE.The nursing assessment OSCE checklist(NAOC)and the communication evaluation rubric(CER)were used as observational measurements with three-point Likert scales(2=Achieved,1=Partly achieved,0=Not completed/Incorrect).Results:Difficulty coefficients of the exam were 0.63 for nursing assessment skills and 0.56 for communication skills.The discrimination index of the majority items of the NAOC(86.4%)was>0.20,showing a better to good discriminability.The items of the CER had satisfactory indexes of item discrimination(from 0.38 to 0.84).Students received high scores in conducting blood oxygen saturation and cardiac and lung auscultation but low scores in vomiting and diarrhea assessment.Students presented good communication skills in eye contacting and listening,but culture assessment needs to be improved.The students with experiences in simulation or simulated patient(SP)interactions had better assessment and communication skills than students without those experiences.There was a positive relationship between nursing assessment and communication skills(r=0.67,P=0.000).Conclusions:SSPs were involved in enhancing the realism of interactions in simulated scenarios.Nursing students can conduct nursing assessments specific to patient conditions,explain the conditions to the patient,and ensure that the patient remains informed at all times of the precautions to be taken.However,students’cultural awareness and some communication skills need further training.With moderate difficulty and high discrimination index,OSCE showed satisfactory reliability and validity.展开更多

This study was carried out at College of Applied Medical Sciences-Bshia University. Data Collection: demographic data, communication skill, self-efficacy and clinical competence questionnaires. Analysis is done by SPSS version 20 software. Results: Participants who received the simulation-based training, showed statistical significant improvement in communication skill, self-efficacy, and clinical competence scores after participation in the simulation program (t = −32.64, p = 0.001;t = −19.9, p = 0.001;16.4, p = 0.001). Also, there are significant relation between gender and clinical competency (t = 2.768, p Conclusion: Simulation-based training in medical courses is effective in enhancing communication skill, self-efficacy and clinical competence. Methods: We systematically searched PubMed, Science Direct and Google Scholar, for relevant articles published in English between 2010 and 2019. We included the studies that investigated humor between nurses and their patient’s relationship. Results: From the initial 287 articles found, 15 were included for final revision. Data allowed retrieving information on humor definition;its applicability as a nursing intervention;humor as a tool to improve nurse-patient communication and relationship;influencing factors;humor benefits in health care context and the limits that come against its use. Conclusion: Humor is considered a powerful communication tool, it promotes well-being, relieves anxiety and stress, helps deal with stressful situations but it should be used cautiously in order to prevent undesired consequences.展开更多

A methodology of quantitative study was used with 30 nurses and 30 blind people who participated in the study, divided in control and experimental groups. 30 nursing appointments took place;they were recorded and analyzed by three judges. The experimental group had excellent performance in following protocol (95.6%);taking notes informing the reason for silence (93.4%);and avoiding long silence (100%). In interventions, it presented excellence in all the items, in the evaluation stage, there were no bad/terrible actions. In none of the actions the control group presented better performance than the experimental one. The use of the Model is recommended in nursing appointment with blind patient.展开更多

文摘This study explores the practical application of nursing management led by head nurses to enhance team communication and collaboration within clinical settings.By integrating leadership theories with nursing practice,this research adopts a qualitative methodology to examine the effects of strategic communication and teamwork enhancement initiatives.Through interviews,observations,and the analysis of case studies in various hospital departments,the study identifies key barriers to effective team communication and collaboration,including hierarchical structures,lack of standardized communication protocols,and insufficient training.Solutions implemented involve targeted communication skills training,the establishment of interdisciplinary teamwork protocols,and leadership workshops for head nurses.The outcomes indicate significant improvements in team efficiency,patient care quality,and staff satisfaction.This research underscores the importance of head nurses in fostering an environment conducive to open communication and collaborative practice,thereby advancing patient care and team performance.
文摘Purpose:Delivering quality health care requires effective communication between health care providers and their patients.Nurse-patient effective communication remains a challenge in Ghana,despite criticism and concern expressed by the public.The study,therefore,aims to assess the effective nurse caregivers’communication practices among pediatric nurses at Pentecost Hospital,Madina.Methods:A qualitative exploratory and purposive sampling technique were used.Participants were engaged in Focus group discussions or face-to-face interviews.In all,4 focus group discussions were conducted with 7 participants in each group and 15 face-face in-depth interviews,with a total sample size of 43.The data was content analyzed.Interviews were taped recorded and transcribed verbatim.Results:The data analysis yielded 3 themes and 11 subthemes.The main themes were:effective communication practices,factors influencing effective communication,and attitudes towards effective communication.The study revealed how the participants communicated with patients who are deafmute,anxious,and those who refused treatment.In addition,participants reported how they used techniques such as silence,listening,and leading cues in communicating with patients and their relatives.Conclusion:Effective communication between pediatric nurses,patients,and their parents is ascertained to be a key tool in the delivery of quality health care.However,several factors are found to hinder this therapeutic communication.Hence,further studies are needed to improve effective communication skills among pediatric nurses and their patients to enhance health care for children.
文摘BACKGROUND Cataracts are a common ophthalmic disease and postoperative vision recovery is crucial to patient quality of life.Rational and efficient care models play an impor-tant role in promoting vision recovery.AIM To evaluate the clinical effectiveness of procedural nursing care combined with communication intervention in vision recovery after cataract ultrasound emulsi-fication.METHODS A randomized controlled study was conducted on 100 patients with cataracts who underwent ultrasound emulsification surgery.They were randomly assigned to an experimental group or a control group.The experimental group received procedural nursing combined with Connect,Introduce,Communicate,Ask,Respond,Exit(CICARE)communication intervention,whereas the control group received conventional nursing.The effectiveness of the nursing model was assessed by comparing differences in vision recovery,pain scores,and mental health status between the two groups.RESULTS It was found that over time the visual acuity of patients in both groups gradually recovered and patients in the experimental group had lower pain scores and superior mental health status than the control group(P<0.05).CONCLUSION Procedural nursing combined with CICARE communication intervention has positive effects on vision recovery in patients after cataract ultrasound emulsification.
文摘BACKGROUND Severe pneumonia is a common severe respiratory infection worldwide,and its treatment is challenging,especially for patients in the intensive care unit(ICU).AIM To explore the effect of communication and collaboration between nursing teams on the treatment outcomes of patients with severe pneumonia in ICU.METHODS We retrospectively analyzed 60 patients with severe pneumonia who were treated at the ICU of the hospital between January 1,2021 and December 31,2023.We compared and analyzed the respiratory mechanical indexes[airway resistance(Raw),mean airway pressure(mPaw),peak pressure(PIP)],blood gas analysis indexes(arterial oxygen saturation,arterial oxygen partial pressure,and oxygenation index),and serum inflammatory factor levels[C-reactive protein(CRP),procalcitonin(PCT),cortisol(COR),and high mobility group protein B1(HMGB1)]of all patients before and after treatment.RESULTS Before treatment,there was no significant difference in respiratory mechanics index and blood gas analysis index between 2 groups(P>0.05).However,after treatment,the respiratory mechanical indexes of patients in both groups were significantly improved,and the improvement of Raw,mPaw,plateau pressure,PIP and other indexes in the combined group after communication and collaboration with the nursing team was significantly better than that in the single care group(P<0.05).The serum CRP and PCT levels of patients were significantly decreased,and the difference was statistically significant compared with that of nursing group alone(P<0.05).The levels of serum COR and HMGB1 before and after treatment were also significantly decreased between the two groups.CONCLUSION The communication and collaboration of the nursing team have a significant positive impact on respiratory mechanics indicators,blood gas analysis indicators and serum inflammatory factor levels in the treatment of severe pneumonia patients in ICU.
文摘Effective communication among healthcare professionals in the intensive care unit (ICU) is a particular imperative,with accurate and efficient interdisciplinary communication being a critical prerequisite for high-quality care.Nurses and physicians are highly important parts of the healthcare system workforce.Thus,identifying strategies that would improve communication between these two groups can provide evidence for practical improvement in the ICU,which will ultimately improve patient outcomes.This integrative literature review aimed to identify interventions that improve communication between nurses and physicians in ICUs.Three databases (Medline,CINAHL,and Science Direct) were searched between September 2014 and June 2016 using 11 search terms,namely,nurse,doctor,physician,resident,clinician,ICU,intensive care unit,communication,teamwork,collaboration,and relationship.A manual search of the reference lists of found papers was also conducted.Eleven articles met the inclusion criteria.These studies reported on the use of communication tools/checklists,team training,multidisciplinary structured work shift evaluation,and electronic situation-background-assessment -recommendation documentation templates to improve communication.Although which intervention strategies are most effective remains unclear,this review suggests that these strategies improve communication to some extent.Future studies should be rigorously designed and outcome measures should be specific and validated to capture and reflect the effects of effective communication.
文摘Objective:This study investigates the emotional intelligence(EI),self-efficacy,and clinical communication ability of clinical nurses to explore the correlation among these three variables,and verify the mediating effect of self-efficacy on relationship between EI and communication skills.Methods:A total of 865 nurses were recruited and investigated using Wong and Law's Emotional Intelligence Scale,General Self-Efficacy Scale,and nurse clinical communication scale.Results:The scores for EI and self-efficacy of nurses were 14.23±2.61 and 25.36±5.67,respectively,which were lower than the international norm(p<0.01).The score for clinical communication ability of nurses was 4.14±0.53.The clinical communication competency of nurses was positively correlated with EI and general self-efficacy.Furthermore,selfefficacy played an intermediary role between EI and clinical communication commitment.Conclusion:Nursing administrators can improve the clinical communication ability of nurses by enhancing their self-efficacy and EI.
文摘Robot technology is expected to reduce the burden of medical professionals, a concern in the super-aged society. The aim of this article is to determine the intermediary roles of Public Health Nurses (PHNs) in using Communication Robots (CRs) in Community Health care. It is necessary to consider who will manage, and how to draw lines of responsibility when a problem occurs when using robots in the future. Regarding the mediator or intermediary role of PHNs in the use of CRs, PHNs are expected to be cognizant of varieties of robots for use with persons of diverse ages and health levels, as well as of robot performances. As one of the community healthcare workers who service the elderly living in the community, the role of PHNs as mediators or intermediaries is to pay attention to ethical and moral issues while working with CRs to manage the health of the community. Lack of understanding of the intermediary role might create possible ethico-moral issues in the future. Therefore, it is critical for PHNs to understand their role as intermediaries. In doing so, it is expected that older people at home will be able to live peaceful lives, as well as be physically and mentally healthy.
文摘Healthcare for older adults with dementia is an important healthcare problem in Japan and in other developed countries. Importantly, healthcare workers in Japan are also getting older. In order to address solutions to this problem, using robots is being realized and is starting to assist healthcare and welfare practice needs. The aim of this study was to identify the characteristics of a transactive phenomenon in relationships among older adults with dementia, with nurses as intermediaries and with a cognitive skill-aid robot. Subjects were two institutionalized older adults who were diagnosed with dementia using Hasegawa’s Dementia Scale-Revised;a Kabochan (Nodding Kabochan) fixed with a remote-controlled speaker, Pechat (Kabochan with Pechat). Measurements of autonomic nervous activity were done using wearable electrocardiography attached to the subjects for four hours. Heart Rate Variability data were assessed at various frequency bands using a HRV software tool. In a continuously recorded data, interbeat (R-R) intervals were obtained for a 1-min segment using the maximum entropy method. In this study, the two major spectral components of HRV, the variances of the low-frequency (LF: 0.04 - 0.15 Hz) band and high-frequency (HF: 0.15 - 0.4 Hz) band, were calculated. Activities and behaviors of subjects transacting with Kabochan were observed by the expert nurses and video-recorded. Throughout the transaction period, observations were recorded of patients’ behavior, words uttered and over-all contents of the transactive conversation with the Kabochan or Kabochan with Pechat. Identified characteristics were transactive relationship phenomenon;changes in autonomic nervous system activity of persons with dementia are expressed as per one minute, linguistic response of elderly people, transactive relations among Kabochan, Kabochan with Pechat, and nurses as intermediaries. These were exhibited as Figures: Situation 1, A Sense of Fear, Situation 2, A Pleasant Sensation, and Situation 3, Misunderstanding and Confusion. It also showed that autonomic nervous system activity of persons with communication skills is actively changing in older adults with dementia. Conversation of adding Pechat to the Kabochan illustrated that nurses as intermediaries showed emotional conversations as established by effectively manipulating the Pechat. Also, it was revealed that if Kabochan with Pechat uttered words about patient’s personal information in a conversation, the patient felt a sense of fear. The researchers suggest that when developing future communication robot for elderly with dementia, it is necessary to consider, for an effective conversation, transactive relationships including nurses as intermediaries. The role functions of these intermediaries have been shown of high versatility, indicating that situations such as these can also be controlled by other healthcare providers such as by occupational therapists.

文摘Objective:To explore the clinical significance of strengthening nurse patient communication in oncology department.Methods:The study was carried out in our hospital.Taking 100 patients selected from May 2020 to May 2021 as an example,all patients were cancer patients.When they were treated,they cooperated with nursing,carried out grouping experiments,adopted different nursing methods,and compared and analyzed the recovery of patients under different nursing interventions.Results:There were significant differences in nursing satisfaction and compliance between the experimental group and the control group,expressed as P<0.05.In contrast,the experimental group had higher satisfaction,better compliance and better effect.Conclusion:Strengthening nurse patient communication in oncology patients is helpful to improve patients’understanding,improve patients’bad mood and promote patients’compliance.It has positive significance and remarkable effect on patients.
文摘Objectives:The motives that lead to nurses’workplace silence behaviour are essential to understand,as the failure to voice their concerns and maintain silence may cause various adverse outcomes,including burnout,turnover,job dissatisfaction,low productivity,and mental and moral distress.This study aimed to identify the level and influencing motives of the workplace silence behaviour among nurses at the National Guard Hospital in Madinah.Methods:A descriptive cross-sectional survey was conducted in January and February 2023.The participants completed a questionnaire with two parts:the sociodemographic data form and the Workplace Silence Behavior Nursing Motives Scale.Data analysis was performed by descriptive statistics and the chi-square test.Results:The overall level for all workplace silence behavior motives is moderate(3.29±0.69),with Avoidance Motives dimension receiving the highest scores(3.62±0.75)and Organization-Oriented Motives dimension receiving the lowest(3.15±0.86).Chi-square test showed that Saudi nationality(X^(2)=0.015,P=0.019)and monthly income(X^(2)=0.045,P?0.013)are factors in high-moderate levels of silence behaviour.Conclusions:The overall level of workplace silence behaviour motives was found to be moderate,nurses have higher monthly salaries and non-Saudi nationals tend to have higher levels of workplace silence behaviour motives.The study results provide a reference for nursing managers to pay attention to nurses’workplace silence behavior and take corresponding intervention measures.

文摘Caring for cancer patients requires both technical and psychosocial nursing skills.The aim of this study was to determine the psychosocial care skill levels of nurses and affecting factors.This multicenter,cross-sectional study was conducted with 1,189 nurses providing direct care to adult cancer patients in 32 hospitals in 12 geographical regions of Turkey.A questionnaire,the Psychosocial Skills Form,and the Professional Quality of Life Scale were used to collect the data.Nurses’psychosocial skill level was in the range of 2.72±0.98 and 2.47±0.89 out of four points.Communication skills such as empathic response,active listening,and the ability to provide information were found to be at a higher level than skills such as the activation of social support systems,therapeutic touch,and development of coping methods.Approximately 40%of nurses had received psychosocial care training,and 87%were interested in receiving additional psychosocial training.Gender,educational status,previous training in psychosocial care,and work experience with cancer patients were shown to affect psychosocial skill levels.There was a positive relationship between the level of psychosocial skills and the level of compassion satisfaction,and a negative relationship between the level of psychosocial skills and the level of burnout and compassion fatigue(p<0.05).Nurses perceive themselves as having a medium to high level of psychosocial skills yet desire additional training.The results of this study may contribute to the development of training programs according to the needs of nurses who care for cancer patients.
文摘Background and Purpose: Therapeutic communication is a new term in family health care nursing, defined by Hohashi (2019) as a method of family intervention, and characterized by inclusion of not only verbal conversation but also nonverbal interaction. However, specific therapeutic communication methods have not been systematized. The purpose of this study was to clarify therapeutic communication methods for families/family members from the perspectives of verbal communication and non-verbal communication through a review of existing literature. Methods: We conducted a search using the medical literature databases PubMed and Ichushi-Web using the keywords “therapeutic communication”. Analysis was performed on seven articles from PubMed and 14 articles from Ichushi-Web that described therapeutic communication methods performed by healthcare professionals for families/family members. Through directed content analysis, therapeutic communication methods were subcategorized, and classified into three categories: verbal communication, non-verbal communication, and verbal/non-verbal communication. Results: A total of 23 subcategories were extracted. Verbal communication included 11 subcategories, such as “asking questions using the communicatee’s words as they are”. Non-verbal communication included five subcategories, such as “noticing changes in the content of the communicatee’s story”. And verbal/non-verbal communication featured seven subcategories, such as “making the communicatee aware of one’s own beliefs”. Conclusion: Therapeutic communication methods included basic care/caring in family interviews/meetings, as well as verbal communication and non-verbal communication that act on family/family members’ beliefs. It is believed that changes in family/family members’ beliefs can be used to eliminate, reduce, or improve problematic conditions in the family. .
文摘Background:To explore the application effect of Acknowledge-Introduce-Duration-Explanation-Thank you(AIDET)communication mode in reducing anxiety and improving the care services satisfaction of the patients.Methods:The 100 cases undergoing cataract daytime operation from February to July 2019 were divided into test group and control group.The test group conducted health education and nursing care for patients during admission,pre-operation,post-operation,and discharge according to the AIDET communication mode.And the control group was given health education and nursing care to the patients according to the traditional way of communication.Results:The difference of the socioeconomic information between these two groups showed no statistical significance.The care services satisfaction score in the test group was statistically higher than that in the control group(P<0.05).The Self-Rating Anxiety Scale(SAS)score in the test group was significantly lower than that in the control group(P<0.01).This result indicated that the patients suffered less anxiety with the AIDET mode of communication.Conclusions:The application of AIDET communication mode to the nursing service of cataract daytime operation can reduce the tension and anxiety of the patients from the treatment and improve their satisfaction to care services.
文摘Dear Editor,Wei,Ming,Cheng,Bian,Ming,&Wei(2018)[1],state that specific communication problems were linked to 17.5%of all complaints,but in reality other categories,totalling 83.7%of all complaints,have a clear(verbal and non-verbal)communication component;uncaring attitudes(26.7%),unsatisfactory quality of treatment and competence(26.5%)and processes of care(13.0%).Highlighting this point are the examples given by patients in relation to complaints categorised as uncaring attitudes,included"displayed indifferent expressions,used unfriendly language tones,were impatient with patients'questions,or did not give thorough explanations when answering questions".The authors suggest theory-guided strategies to assist with nursing care.We advocate the need for this to be instilled in nursing education.Education providers for health professionals aim to improve communication skills through formal education[2-4].Wei et al.(2018)[1]state the Chinese curriculum for medicine focuses on science more than humane or caring qualities.Few education providers are able to effectively educate health professionals of the critical nature of communication for their future practice and work place requirements[5].

文摘Objective:To evaluate nursing students’assessment and communication skills through an objective structured clinical examination(OSCE)within a high-fidelity simulation using a student-simulated patient(SSP)and analyze the validity,reliability,degree of difficulty,and discriminability of the OSCE.Methods:A descriptive study was conducted at one nursing school in Macao.All scenarios were designed using a high-fidelity simulator combined with SSPs.A convenience sample of 54 baccalaureate nursing students completed the OSCE.The nursing assessment OSCE checklist(NAOC)and the communication evaluation rubric(CER)were used as observational measurements with three-point Likert scales(2=Achieved,1=Partly achieved,0=Not completed/Incorrect).Results:Difficulty coefficients of the exam were 0.63 for nursing assessment skills and 0.56 for communication skills.The discrimination index of the majority items of the NAOC(86.4%)was>0.20,showing a better to good discriminability.The items of the CER had satisfactory indexes of item discrimination(from 0.38 to 0.84).Students received high scores in conducting blood oxygen saturation and cardiac and lung auscultation but low scores in vomiting and diarrhea assessment.Students presented good communication skills in eye contacting and listening,but culture assessment needs to be improved.The students with experiences in simulation or simulated patient(SP)interactions had better assessment and communication skills than students without those experiences.There was a positive relationship between nursing assessment and communication skills(r=0.67,P=0.000).Conclusions:SSPs were involved in enhancing the realism of interactions in simulated scenarios.Nursing students can conduct nursing assessments specific to patient conditions,explain the conditions to the patient,and ensure that the patient remains informed at all times of the precautions to be taken.However,students’cultural awareness and some communication skills need further training.With moderate difficulty and high discrimination index,OSCE showed satisfactory reliability and validity.
文摘Background: Simulation-based training is a new strategy in teaching that gives the students good opportunities to learn and apply what they learn in nursing care safely. Aim: This study conducted to evaluate the effects of simulation-based training on nursing students’ communication skill, self-efficacy and clinical competence in practice. Subjects and Methods: Quiz-experimental design was used in this study (pre-posttest intervention), it was carried out on 100 nursing students first semester in 2019 using low and high-fidelity simulators. This study was carried out at College of Applied Medical Sciences-Bshia University. Data Collection: demographic data, communication skill, self-efficacy and clinical competence questionnaires. Analysis is done by SPSS version 20 software. Results: Participants who received the simulation-based training, showed statistical significant improvement in communication skill, self-efficacy, and clinical competence scores after participation in the simulation program (t = −32.64, p = 0.001;t = −19.9, p = 0.001;16.4, p = 0.001). Also, there are significant relation between gender and clinical competency (t = 2.768, p Conclusion: Simulation-based training in medical courses is effective in enhancing communication skill, self-efficacy and clinical competence. Multiple-patient simulations as a teaching-learning strategy in the nursing curriculum are highly recommended.
文摘Objective: To identify the importance of using humor in patient/nurse relationship, its factors, its conditions and the barriers that might limit its use. Methods: We systematically searched PubMed, Science Direct and Google Scholar, for relevant articles published in English between 2010 and 2019. We included the studies that investigated humor between nurses and their patient’s relationship. Results: From the initial 287 articles found, 15 were included for final revision. Data allowed retrieving information on humor definition;its applicability as a nursing intervention;humor as a tool to improve nurse-patient communication and relationship;influencing factors;humor benefits in health care context and the limits that come against its use. Conclusion: Humor is considered a powerful communication tool, it promotes well-being, relieves anxiety and stress, helps deal with stressful situations but it should be used cautiously in order to prevent undesired consequences.
文摘It was aimed at validating the applicability of the Model of Verbal Communication with blind people in nursing appointment in the stages of the nursing process. A methodology of quantitative study was used with 30 nurses and 30 blind people who participated in the study, divided in control and experimental groups. 30 nursing appointments took place;they were recorded and analyzed by three judges. The experimental group had excellent performance in following protocol (95.6%);taking notes informing the reason for silence (93.4%);and avoiding long silence (100%). In interventions, it presented excellence in all the items, in the evaluation stage, there were no bad/terrible actions. In none of the actions the control group presented better performance than the experimental one. The use of the Model is recommended in nursing appointment with blind patient.
