Background:Catheter ablation is effective in restoring sinus rhythm and left atrial appendage closure(LAAC)is increasingly used for stroke prevention in patients with atrial fibrillation(AF).We aimed to observe the feasibility and safety of performing AF ablation and LAAC in a single(one-stop)procedure.Methods:Consecutive AF patients who underwent the combined procedure of AF ablation and LAAC with WATCHMAN device between March 2017 and September 2018 were prospectively enrolled.Baseline and intra-procedural parameters were evaluated.Three-month and subsequent 1-year follow-up were performed in all and earlier-enrolled subjects,respectively.Results:A total of 178 AF patients(94 males,68.9±8.1 years)underwent the one-stop procedure with CHA2DS2-VASc score 3.3±1.5 and HAS-BLED score 1.6±1.0,respectively.Pulmonary vein isolation was achieved in all patients while additional linear ablation was applied if the operator deemed necessary,yielding immediate ablation success rate of 98.9%(176/178).In the subsequent LAAC,satisfactory seal(residual leak<5 mm)was achieved in all patients.One stroke and four cardiac perforations occurred peri-operatively.At 3-month follow-up,sinus rhythm and satisfactory seal were maintained in 153/178(86.0%)and 178/178(100%)patients,respectively.One stroke and one delayed cardiac tamponade occurred,while no device-related thrombus or device migration was observed.During the 1-year follow-up for the earlier enrolled subjects,52/72(72.2%)of the patients maintained sinus rhythm.There was no stroke or systemic embolism observed.Conclusion:Combining catheter ablation and LAAC in a single procedure can be successfully and safely performed in non-valvular AF patients of Chinese population. 展开更多

A global enterprise must continuously improve the efficiency of logistic operations between supply chain collaborators. Integrating logistic services, resources, and necessary information flows in the supply chain to ensure efficiency and efficacy is critically important to these companies. Global logistic service companies face challenges from their clients to provide logistic services that are cost effective, accurate, and seamlessly integrate material, information and cash flows. In this research, an improved framework for one-stop logistic services is systematically designed, analyzed, and evaluated. The one-stop logistic service framework, defmed in four models, is developed to provide enterprises with integrated and comprehensive services within the global supply chain context. The levels of service importance are assessed using a four categories questionnaire. This research provides a case study of the implementation of one-stop logistic services in the distribution industry and demonstrates the framework operating under different demand conditions. Finally, system dynamics causal evaluation is used to evaluate the advantages of the logistic service framework. 展开更多

Coronavirus disease 2019(COVID-19),which is caused by SARS-CoV-2,varies with regard to symptoms and mortality rates among populations.Humoral immunity plays critical roles in SARS-CoV-2 infection and recovery from COVID-19.However,differences in immune responses and clinical features among COVID-19 patients remain largely unknown.Here,we report a database for COVID-19-specific IgG/IgM immune responses and clinical parameters(named COVID-ONE-hi).COVID-ONE-hi is based on the data that contain the IgG/IgM responses to 24 full-length/truncated proteins corresponding to 20 of 28 known SARS-CoV-2 proteins and 199 spike protein peptides against 2360 serum samples collected from 783 COVID-19 patients.In addition,96 clinical parameters for the 2360 serum samples and basic information for the 783 patients are integrated into the database.Furthermore,COVID-ONE-hi provides a dashboard for defining samples and a one-click analysis pipeline for a single group or paired groups.A set of samples of interest is easily defined by adjusting the scale bars of a variety of parameters.After the“START”button is clicked,one can readily obtain a comprehensive analysis report for further interpretation.COVID-ONE-hi is freely available at www.COVID-ONE.cn. 展开更多

Traditional practical teaching has problems on low reusability, high cost and low collaboration ability. To solve these problems, this paper proposes a solution that is to build and use a novel practical teaching platform based on cloud computing. By utilizing advanced information technology, for example, cloud computing, this solution can present a unified service system providing practical teaching resource management, innovation resource management and knowledge management both inside and outside class. Moreover, an instance implementing the cloud-based practical teaching unified services system, Yun Hai, would be detailedly discussed in this paper. The system deployment, resource configuration and matched practice teaching mode will be presented. Yun Hai is independently developed by State Key Laboratory of Networking and Switching Technology and has been deployed and employed in many institutions. To verify the effectiveness of this teaching platform, we take Data Mining, a representative course of computer science, as an example, and analyze how the system would perform when applied to such course which contains cross-disciplinary knowledges. The practice indicates that this teaching system and practical teaching mode can improve convenience and flexibility on practical teaching resource. The Corona Virus Disease 2019(COVID-19) pandemic has taught us many valuable lessons regarding the importance of our physical and mental health. Even with so many technological advancements, we still lag in developing a system that can fully digitalize the medical data of each individual and make it readily accessible for both the patient and health worker at any point in time. Moreover, there are also no ways for the government to identify the legitimacy of a particular clinic. This study merges modern technology with traditional approaches,thereby highlighting a scenario where artificial intelligence(AI) merges with traditional Chinese medicine(TCM), proposing a way to advance the conventional approaches. The main objective of our research is to provide a one-stop platform for the government, doctors,nurses, and patients to access their data effortlessly. The proposed portal will also check the doctors’ authenticity. Data is one of the most critical assets of an organization, so a breach of data can risk users’ lives. Data security is of primary importance and must be prioritized. The proposed methodology is based on cloud computing technology which assures the security of the data and avoids any kind of breach. The study also accounts for the difficulties encountered in creating such an infrastructure in the cloud and overcomes the hurdles faced during the project, keeping enough room for possible future innovations. To summarize, this study focuses on the digitalization of medical data and suggests some possible ways to achieve it. Moreover, it also focuses on some related aspects like security and potential digitalization difficulties. 展开更多

Assessing the impact of TB/HIV services integration on TB treatment outcomes and their relevance in TB/HIV monitoring in Ghana 被引量:4

Background:The impact of the human immunodeficiency virus(HIV)on tuberculosis(TB),and the implications for TB and HIV control,is a public health challenge in Ghana-almost a quarter(23%)of all TB cases were HIV positive in 2010.The integration of TB/HIV services has therefore emerged as an essential component of the national response to TB and HIV.The aim is to reduce fragmentation,improve access,enhance efficiency and improve quality of care.Ghana’s TB/HIV policy comprises three linked sets of activities:effective implementation of the Stop TB Strategy for TB control,improved HIV prevention and care,and the implementation of additional TB/HIV activities.Different models of service delivery with increasing integration of TB/HIV activities are expected to provide greater access to more comprehensive care.The objective of this paper is to assess the impact of TB/HIV integration on TB treatment outcomes and to explore the usefulness of TB treatment outcomes as TB/HIV indicators.Methods:A before-and-after study to observe the introduction of TB/HIV activities into TB programmes in three hospitals with different levels of integration was conducted.Anonymised patient data was collated from TB registers from each facility,and analysed to determine if TB treatment outcomes changed significantly after integration.Results:TB treatment success was 50%(95%CI 49-52)prior to,and 69%(95%CI 65-73)after,integration(Χ243.96,p<0.00).Treatment success increased from 43%to 53%at the one-stop shop(OSS),from 69%to 78%at the partially integrated site(PIS)and substantially from 46%to 78%at the referral site(RS)(Χ264.54;p<0.01).Defaults and cases transferred out reduced from 14.3%and 15.3%prior to integration,to 1.4%and 9.0%after integration,respectively,accounting for a significant increase in treatment success.Death rates remained high at 18%in all cases studied and 25%in HIV-associated cases after integration.Conclusion:TB/HIV integration may improve TB treatment success,but its exact impact is difficult to ascertain due to non-specificity and design limitations.TB mortality may be more useful as an indicator for monitoring TB/HIV activities in Ghana. 展开更多

Investigating the genetic and developmental characteristics,infection transmission attributes,and epidemiological trends of pathogens using genomic data represents the foundation for pathogen surveillance and is a crucial prerequisite for guaranteeing global health security.To meet the analytical demands of research relating to pathogen prevention and control,we designed a secure visualization system capable of pathogen genome assembly,annotation,species identification,sequence typing,antibiotic resistance and virulence analysis,genomic mobile element and transferable resistance gene annotation,and phylogenetic tree reconstruction.For highly pathogenic organisms requiring complete data protection,we have developed a secure computing tool that utilizes a trusted execution environment,is combined with blockchain and privacy computing technologies,and is specifically designed for nucleotide basic local alignment search tool(BLASTn)comparison analysis.This technological advancement offers scientific support for in‐depth investigations into pathogen transmission and epidemiological mechanisms,environmental adaptability,evolutionary trends,and immune evasion mechanisms,as well as the identification of new or emerging pathogen strains.This,in turn,aids efforts in infectious disease prevention,treatment,and research. 展开更多
