Objective:To evaluate and compare the effect of semi-rigid ureterorenoscopy on post-procedural sexual function with three different operating room settings. Methods:In this prospective randomized study, consecutive sexually active patients with normal pre-operative sexual function undergoing uncomplicated semi-rigid ureterorenoscopy for distal ureteric stones were randomized into three groups, with three different operating room settings.Procedure-related anxiety and sexual function were assessed pre-operatively using Amsterdam Preoperative Anxiety and Information Scale and Brief Sexual Function Inventory(in males)and Female Sexual Function Index-6(in females),respectively. All the participants were stented following the procedure, and the stent was removed after 3 weeks.Post-procedural sexual function and general discomfort were assessed and compared between three groups at 1 week,3 weeks, and 12 weeks. The effect of surgery-related anxiety, preoperative sexual function, age, and general discomfort(including stent-related discomfort)on post-procedural sexual function were analyzed using multiple regression analysis.A p-value of less than 0.05 was considered statistically significant.Results:Totally,327 eligible patients were randomized into three groups. The group of patients who underwent the procedural with a screen separating the operating area from the patient vision, while the patient could watch the endoscopy through a separate monitor, had better post-procedural sexual function compared to those who had total vision of the operating area as well as to those whose eyes were blocked. This difference was statistically significant. This post-procedural reduction in sexual function could not be attributed to in situ stent alone. Conclusions:Our study showed that semi-rigid ureterorenoscopy can have significant negative effect on sexual function, which can be reduced with proper preoperative counseling and an ideal operating room settings. 展开更多

Objective:Surgical procedures manifest immense risks to patients. One of the adverse events that healthcare professionals see as a threat to the patient’s health is the development of complications known as surgical site infection(SSI).Although several effor ts are being under taken to determine the proper means to reduce such complications, there is still a high incidence of SSI worldwide.Surgery requires knowledge in infection control and high precision in maintaining a clean surgical site. This study tested the effectiveness of an operating room(OR)bundle of care in preventing SSI in patients undergoing elective exploratory laparotomy cholecystectomy surgery. Methods:A quasi-experimental pretest and posttest design was utilized to determine its effectiveness. The study was composed of 60 par ticipants divided into two groups:30 subjects were selected to receive the OR bundle of care, while the other 30 subjects received the usual care. The groupings were determined through a systematic random sampling technique. The OR bundle of care had three interventions,namely:(1)maintaining perioperative normothermia,(2)no pre-operative surgical site hair removal,and(3)changing gloves before abdominal wall closure. These patients were evaluated using the standard instrument,Bates–Jensen Wound Assessment Tool(BWAT)in the post-intervention phases of the wound healing process, which are as follows:hemostasis, inflammatory, and proliferative phases. To describe the difference in the patients’wound status after implementation of the OR bundle of care in each post-intervention phase,Friedman’s test was used.To describe the difference in the patients’wound status in both groups after implementation of the OR bundle of care, the Mann–Whitney U test was used. Results:The patient’s wound status was lower, indicating a more healing process.Differences between the wound status of the control and the experimental group were observed on the third postoperative day. This indicates that the experimental group’s wound status healed much faster and more effectively than the control group based on the BWAT scoring severity scoring.A significant difference in the patient’s wound status from the hemostasis phase compared with the proliferative phase was observed. Conclusions:The OR bundle of care has been shown to be effective in preventing SSI in patients who had undergone exploratory laparotomy cholecystectomy surgery in the selected hospital,if there is uniform and consistent implementation of the said intervention. 展开更多

BACKGROUND The timing of operative treatment for pediatric supracondylar humerus fractures(SCHF)and femoral shaft fractures(FSF)remains controversial. Many fractures previously considered to be surgical emergencies, such as SCHF and open fractures, are now commonly being treated the following day. When presented with an urgent fracture overnight needing operative treatment, the on-call surgeon must choose whether to mobilize resources for a late-night case or to add the case to an elective schedule of the following day.AIM To describe the effect of a program allowing an early operating room(OR)start for uncomplicated trauma prior to an elective day of surgery to decrease wait times for surgery for urgent fractures admitted overnight.METHODS Starting in October 2017,patients were eligible for the early slot in the OR at the discretion of the surgeon if they were admitted after 21:00 the previous night and before 05:00.We compared demographics and timing of treatment of SCHF and FSF treated one year before and after implementation as well as the survey responses from the surgical team.RESULTS Of the 44 SCHF meeting inclusion criteria,16 received treatment before imple mentation while 28 were treated after. After implementation, the mean wait time for surgery decreased by 4.8 h or 35.4%(13.4 h vs 8.7 h;P=0.001).There were no significant differences in the operative duration, time in the post anesthesia care unit, and wait time for discharge.Survey results demonstrated decreased popularity of the program among nurses and anesthesiologists relative to surgeons.Whereas 57%of the surgeons believed that the program was effective, only 9%of anesthesiologists and 16%of nurses agreed. The program was ultimately discontinued given the dissatisfaction.CONCLUSION Our findings demonstrate significantly reduced wait times for surgery for uncomplicated SCHF presenting overnight while discussing the importance of shared decision-making with the stakeholders.Although the program produced promising results,it also created new conflicts within the OR staff that led to its discontinuation at our institution.Future implementations of such programs should involve stakeholders early in the planning process to better address the needs of the OR staff. 展开更多

Noise pollution is in an increasing trend in the operating rooms, as has been the case in various other domains in life. It has been shown in studies that the main cause of this noise pollution consist of the behavior related to the operating room personel and the surgical equipment. These higher than normal noise levels may increase stress and decrease communication and performance, apart from the negative health effects on the operating team. In this study, we aimed to explore the sources of noise pollution and the place of music on these effects on anesthesiology and surgical doctors. Questions formulated through a questionnaire have been asked to the anesthesiology and surgical department physicians on a voluntary basis, and their approach to the concept of noise has been assessed. The study was planned as a descriptive study. In total, highest impact from noise pollution was found to be the aspirator (74.3%), chatting (55.4%), noises from the monitors (54.8%), alarms (48.6%), surgical material (25.9%) and music (23.8%). Noise had the most impact on the concentration of the doctors with a rate of 61.9%. Anesthesiologists, when compared to other surgical department physicians, were found to be impacted most by music with 37.2%. It is not realistic to eliminate all the noise and distraction. On the other hand, it can be minimalized through sufficient measures. Yet, general measures to keep the silence isn’t too likely to keep the noise down during long operations. Whereas music stays as a matter of personal choice. Controlled studies on whether to keep or not the music in the operating rooms aren’t yet sufficient, due to which, general recommendations do not apply. As a result of this study, we have found that while music might have performance increasing effects, it still has a potential as a distraction and interfere with the communication in the operating room. 展开更多

Here in this paper,we report a room-temperature operating infrared photodetector based on the interband transition of an In As Sb/Ga Sb quantum well. The interband transition energy of 5-nm thick In As(0.91)Sb(0.09) embedded in the Ga Sb barrier is calculated to be 0.53 e V(2.35μm),which makes the absorption range of In As Sb cover an entire range from short-wavelength infrared to long-wavelength infrared spectrum. The fabricated photodetector exhibits a narrow response range from 2.0μm to 2.3μm with a peak around 2.1μm at 300 K.The peak responsivity is 0.4 A/W under-500-m Vapplied bias voltage, corresponding to a peak quantum efficiency of 23.8%in the case without any anti-reflection coating. At 300 K,the photodetector exhibits a dark current density of 6.05×10^-3A/cm^2 under-400-m V applied bias voltage and 3.25×10^-5A/cm^2 under zero, separately. The peak detectivity is 6.91×10^10cm·Hz^1/2/W under zero bias voltage at 300 K. 展开更多

Objective:To explore the implementation effect of nursing management in operating room for hospital infection control.Methods:100 cases of patients who received surgical treatment in our hospital from 2019 to 2020 were selected and divided into observation group and control group according to different nursing management methods in operating room.Patients in the control group received routine nursing management services, while patients in the observation group received systematic and highquality comprehensive management services based on routine nursing management services. Results:The air colony count and hand colony count of the observation group were significantly better than those of the control group. At the same time, the incidence probability of infection in the observation group was also significantly lower than that of the control group, with significant difference between the groups, with statistical value(P<0.05).Conclusions:In view of the occurrence of nosocomial infection, the implementation of high-quality operating room nursing service can effectively control it,reduce the probability of occurrence, and ensure the safety of patient treatment, which is worthy of clinical application. 展开更多
