The objective of this study was to evaluate the effects of a Pilates exercise program on postural stability and balance confidence in people with IPD. Ten IPD patients (Hoehn & Yahr Stage 1-3) with a history of falls or nearfalls had the following assessments before and after completion of a 6-week supervised Pilates exercise program: Activities-Specific Balance Confidence Scale (ABC);Berg Balance Scale (BBS);Schwab and England Scale (SES);Unified Parkinson’s Disease Rating Scale (UPDRS);pull-test;timed-up-and-go (TUG);5-metre walk;static and dynamic posturography. There were significant improvements in BBS score, 5-metre walk time and TUG after the training program, as well as improvement trends in some posturographic measures. Participants also reported improved balance confidence with Activities of Daily Living (ADLs). Our findings suggest that Pilates therapy can be beneficial in IPD and warrants further evaluation in a larger study.展开更多

Participants will undergo multiple assessments through all phases of the study which incorporates an A-B-A withdrawal design. The initial baseline period will be randomised from 5 to 7 weeks duration, then participants will enter the intervention period for 8 weeks followed by a withdrawal period of 5 weeks. Methods: The literature databases such as the Cochrane Library, EMbase, PubMed, Web of Science, Chinese Biomedicine Database (CBM), China National Knowledge Infrastructure (CNKI), VIP Database for Chinese Technical Periodicals (VIP) and Wan Fang database (Wan Fang) were retrieved. Randomized controlled trials (RCTs) regarding the effects of Pilates exercise on balance, walking and lower limbs motor function of stroke patients were retrieved. Data were collected and evaluated independently by two researchers. Then, we adopted RevMan 5.3 software to perform a meta-analysis. Results: A total of 5 RCTs were included, including 182 subjects were chosen. The result showed that compared with the routine care group, Pilates exercise can effectively improve the balance of stroke patients when they have trained eight weeks [MD=4.71, 95%CI (2.14, 7.28), P=0.0003], shorten the time of the Timed Up and Go test (TUG)[MD=-1.63, 95%CI (-2.69,-0.56), P=0.003], increase the stride velocity [MD=3.83, 95%CI (1.43, 6.23), P=0.002]. However, the evidence did not support the effectiveness of Pilates exercise on increasing the score of Fugl-Meyer Assessment-Lower Extremity (FMA-LE) of stroke patients [MD=6.70, 95%CI (-3.46, 16.86), P=0.20]. Conclusion: The findings suggest that Pilates could improve the balance and walking speed of stroke patients. However, there still is the lack of the evidence in improving lower limb function. Due to the limitation of the quality of the included studies. highquality studies still should be conducted to investigate the effect of Pilates exercise in stroke patients.展开更多

The upper rectus abdominis (URA) and lower rectus abdominis (LRA) were independently examined and intra-group and inter-group comparison were done. Findings demonstrate a significant increase in RA strength after both protocols, relative to baseline. Significantly increased electrical activation was seen in the URA after CK. Training with PME, although as efficient as CK in strength improvement, produced decreased activation level of muscles. CK training induced an opposite result. The differences after training suggest that CK and PME training programmers may strength RA, but the neuromuscular activation strategies for that are not the same. PME can be incorporated into protocols to improve abdominals strength, and trunk stabilization.展开更多
