Objective Laparoscopic surgery has become a routine general surgery with many advantages,such as alleviating abdominal pain.However,postoperative pain caused by abdominal drainage tubes has attracted little attention ...Objective Laparoscopic surgery has become a routine general surgery with many advantages,such as alleviating abdominal pain.However,postoperative pain caused by abdominal drainage tubes has attracted little attention from medical staff.The aim of this study was to explore the influence of a new abdominal drainage tube fixation method for 3-port laparoscopic cholecystectomy(LC)on patients’postoperative quality of life.Methods Patients who underwent 3-port LC with abdominal drainage tubes in the Department of Hepatobiliary Surgery of Linyi People’s Hospital from March 1,2023 to October 31,2023 due to gallstones with chronic cholecystitis were selected for this study.The patients were randomly divided into an experimental group and a control group.In the experimental group,the new abdominal drainage tube fixation method was used,while in the control group,the traditional method was used.Afterward,the quality of life of patient in terms of pain,activity,recovery time,and mental health status was evaluated.The exudate around the patient’s drainage tube was collected for bacterial culture and analysis.Results A total of 139 patients were randomly divided into an experimental group(70 patients)and a control group(69 patients).The patients’baseline characteristics were not significantly different.The patients in the experimental group had better outcomes in quality of life,with higher pain scores(24.03±2.37 vs.15.48±2.29,p<0.001)and activity scores(20.57±1.78 vs.14.13±1.43,p<0.001),and a shorter postoperative recovery time(2.36±0.68 d vs.2.96±1.34 d,p<0.001).The same results were shown in linear regression analysis scores of the 2 groups.The positive rate of bacterial culture in the exudate around the patient’s drainage tube in the experimental group was significantly lower than that in the control group(12.9%vs.43.5%,p<0.001);and furthermore,the positive rate of conditional pathogenic bacteria was even lower(7.1%vs.33.3%,p<0.001)in the experimental group than in the control group.Conclusion This new abdominal drainage tube fixation method can effectively promote patient rehabilitation and improve the quality of life for patient following 3-port LC with abdominal drainage tubes.展开更多

The framework of...The aims of the study were to investigate health-related quality of life (QoL) and its perceived determinants among college freshmen at an island Chinese University located between China and Taiwan. The framework of QoL given by the WHO was employed to evaluate the QoL of 404 college freshmen in January 2013, and a retrospective approach was adopted to solicit the participants' perceived determinants of their QoL. Each participant completed the WHOQOL-BREF questionnaire and specified reasons for their degree of satisfaction in each life facet addressed in the questionnaire. The participants expressed a moderate degree of satisfaction in all of the life facets measured (M = 3.29), with 3.4 in the physical, 3.3 in the psychological, 3.1 in the social, and 3.2 in the environmental domain on a 5-point scale. Among the 24 life facets assessed, sleep quality, transportation availability, medical service quality, concentration, and energy were the top five unsatisfactory facets. Determinants of the participants' QoL involved multiple factors such as the ease of day-to-day living, living environment, psychological factors, time arrangement, sleep quality, etc. The freshmen year is a transition period requiring great adjustment in lifestyle due to the change from secondary to tertiary education and from home stay with parents' care to independent dormitory life. The findings of the study indicated that college freshmen in the context of the university studied had not experienced expected satisfactory levels of life quality, and that the perceived determinants of their life quality could be further addressed for promotion of health and education quality. The study could serve as an example to call for regular examinations on students' experience and discrepancies between experience and expectation in order to address related issues for higher educational quality.展开更多

Methods: A total of 117 inpatients with COPD from the Department of Respiratory Medicine in a tertiary general hospital in Nanjing were enrolled by convenience sampling from January to December in 2016 and then were divided into intervention group (n = 60) and control group (n = 57) by random number table. Patients in the intervention group accepted both routine care and hospital– community integrated transitional care for 3 months after discharge. Assessment of quality of life by telephone follow-up or interview within a week before discharge and 1, 3, and 6 months after discharge was evaluated using the Short Form-36 (SF-36) health survey questionnaire. Results: For a total score of quality of life, there was a significant difference between the two groups (P < 0.05): 1, 3, and 6 months after discharge. In addition, for each dimension score of quality of life, there were no significant differences (P > 0.05) except vitality dimension, 1 month after discharge, and there were significant differences in all dimensions, 3 and 6 months after discharge (P < 0.05) Conclusions: Hospital–community integrated transitional care model can improve the quality of life of patients with COPD.展开更多

Design: Open non-comparative study....Objective: To evaluate the quality of life in patients with endometriosis pelvic pain before and after the application of the levonorgestrel-releasing intrauterine system (LNG-IUS). Design: Open non-comparative study. Setting: Obstetrics and Gynecology Hospital at Monterrey, Mexico. Sample: 29 women aged 18 to 40 years with pelvic pain associated with endometriosis confirmed by laparoscopy. Methods: After laparoscopy but before LNG-IUS insertion (basal visit) and 6 months afterwards, modified Endometriosis Health Profile (EHP-30) was applied. Main outcomes measures: Size of change of questionnaire scores, need of additional analgesic therapy and adverse effects. Statistical Analysis: Differences in the questionnaire scores before and after intervention were analyzed by Student t-test. Results: Final analysis set included 29 women aged 31.7 ± 4.7 years years. The ASRM surgical staging of endometriosis was mild in 19.3 moderate in 13.7 and severe in 76% of the patients. The general perception of quality of life improved from 52 at baseline to 98% at six months (p < 0.001). Adverse events were mild in nature, 19 patients reported no adverse events during the study (65.5%). Two patients (6.9%) required the use of concomitant therapy with non-steroidal analgesics for relief of pain. Conclusion: The application of LNG-IUS in patients with pelvic pain associated with endometriosis improved significatively all aspects related with quality of life as measured with Endometriosis Health Profile (EHP-30). We concluded that LNGIUS may be an effective and convenient therapeutic alternative for the management of pain associated with endometriosis.展开更多
Background:The purpose of this study was to use the modified Delphi method to identify the influencing factors of health-related quality of life(HRQoL)in patients with unruptured intracranial aneurysms(UIAs)after endo...Background:The purpose of this study was to use the modified Delphi method to identify the influencing factors of health-related quality of life(HRQoL)in patients with unruptured intracranial aneurysms(UIAs)after endovascular treatment.Methods:A modified Delphi method to obtain expert consensus on the content of potential influencing factors of HRQoL in patients with UIAs treated by endovascular intervention was employed.The research team consists of three neuroradiologists and one epidemiologist from Xuanwu Hospital of Capital Medical University.They randomly selected 21 well-known experts in cerebrovascular disease diagnosis and treatment as participating experts.The importance of the indicator is based on the 5-Likert scale.The standard deviation(SD),coefficient of variation(CV),mean(x),and minimum and maximum scores of each indicator were calculated.The consistency was described by Kendall coefficient of concordance with a p value<0.05 indicating that the expert consistency was high.Result:Twenty-one and 18 questionnaires were responded in 2 rounds,with effective response rates of 85.7%and 100.0%,respectively.The average authoritative coefficient(Cr)of all 21 experts was 0.88,familiarity with the indicators(Cs)was 0.82,and the judgment basis of the indicators(Ca)was 0.94.Eventually,the x values of arterial puncture hematoma,hyperlipidemia,gender,marital status,and hospitalization for other diseases were lower than 3.5;CV for marital status and gender was higher than 0.35.The Kendall coefficient of concordance in the first round was 0.19(p<0.001),and the second round was 0.15(p<0.001).Conclusion:In this study,the factors affecting the recovery of HRQoL after endovascular treatment in patients with UIAs were analyzed by the modified Delphi method,which provided a valuable evidence for the clinical management and daily life guidance for UIAs patients.展开更多

Although acupuncture is widely used, until now Western acupuncture studies on RA have not shown conclusive positive results. Acupuncture is regarded as a reflex therapy that has effects on the human autonomic nervous system. By establishing a traditional Chinese medicine(TCM) diagnosis first, the practitioner is able to choose acupoints according to the state of each individual patient.METHODS/DESIGN: We are interested if acupuncture, using a classical diagnostic procedure to allocate acupoints to the patient according to the Shang Han Lun theory, can be effective in relieving pain, improving hand function and increasing health-related quality of life in RA.The authors intend to harmonize TCM diagnosis according to clinical and genetic profiles. Patients with the TCM diagnosis of a so-called Turning Point syndrome will be followed up in a randomized, prospective, double-blind, placebo-controlled, multicenter and three-armed parallel-group study with a standardized treatment in order to optimize potential therapeutic effects of acupuncture on pain, strength and muscle function of patients with RA as well as the influence on inflammation and quality of life.DISCUSSION: The findings of this study will provide important clinical information about the feasibility and efficacy of acupuncture treatment for RA patients. Objective:To study the application effects of teach-back method combined with WeChat education in patients with chronic obstructive pulmonary disease(COPD).Methods:Convenience sampling was used to select 103 patients ...Objective:To study the application effects of teach-back method combined with WeChat education in patients with chronic obstructive pulmonary disease(COPD).Methods:Convenience sampling was used to select 103 patients with COPD hospitalized in the department of respiratory medicine of a tertiary-level hospital from March to June 2021 as study subjects.Randomized grouping was carried out using the random number table method.The routine care was given in the control group,and the teach-back method combined with WeChat tutorials on the basis of routine care was applied in the intervention group.In the follow-up three months after discharge,the changes in pulmonary function,self-care ability,and quality of life of patients in the two groups were observed and compared.Results:After the intervention,the pulmonary function indexes of both groups improved significantly,and the improvement effect was more significant in the intervention group(P<0.05);after the intervention,the self-care ability scores of the patients in the intervention group were significantly higher than those of the control group(P<0.05);the quality-of-life scores of the patients in the two groups decreased at 3 months after discharge,and the scores of the patients in the intervention group were significantly lower than those of the patients in the control group(P<0.05).Conclusion:The teach-back method combined with WeChat education can effectively improve the pulmonary function,self-care ability,and quality of life of patients with COPD.展开更多
BACKGROUND Systemic lupus erythematosus(SLE)is a heterogeneous autoimmune disorder with varied clinical courses and prognoses,not only did the patients suffer from physical impairment,but also various physical and psy...BACKGROUND Systemic lupus erythematosus(SLE)is a heterogeneous autoimmune disorder with varied clinical courses and prognoses,not only did the patients suffer from physical impairment,but also various physical and psychiatric comorbidities.Growing evidence have suggested that mental disorders in SLE patients,can lead to various adverse consequences.AIM To explored the features and influencing factors of mental health in patients with SLE and clarifying the correlations between mental health and personality characteristics and perceived social support.The results would provide a basis for psychological intervention in patients with SLE.METHODS The clinical data of 168 patients with SLE admitted at the First Affiliated Hospital of Hainan Medical University between June 2020 and June 2022 were collected.Psychological assessment and correlation analysis were conducted using the Symptom Checklist-90(SCL-90)and Perceived Social Support Scale,and the collected data were compared with the national norms in China.The relevant factors influencing mental health were identified by statistical analysis.A general information questionnaire,the Revised Life Orientation Test,and Short-Form 36-Item Health Survey were employed to assess optimism level and quality of life(QoL),respectively.RESULTS Patients with SLE obtained higher scores for the somatization,depression,anxiety,and phobic anxiety subscales than national norms(P<0.05).A correlation was identified between total social support and total SCL-90 score or each subscale(P<0.05).The factors significantly affecting patients’mental health were hormone dosage and disease activity index(DAI)(P<0.05).The average optimism score of patients with SLE was 14.36±4.42,and 30 cases were in the middle and lower levels.A positive correlation was found between optimism level and QoL scores.CONCLUSION Patients with SLE develop psychological disorders at varying degrees,which are significantly influenced by hormone dosage and DAI.Patients’mental health should be closely monitored during clinical diagnosis and treatment and provided adequate support in establishing positive,healthy thinking and behavior patterns and improving their optimism level and QoL.展开更多
BACKGROUND Psilocybin,a naturally occurring psychedelic compound found in certain species of mushrooms,is known for its effects on anxiety and depression.It has recently gained increasing interest for its potential th...BACKGROUND Psilocybin,a naturally occurring psychedelic compound found in certain species of mushrooms,is known for its effects on anxiety and depression.It has recently gained increasing interest for its potential therapeutic effects,particularly in patients with advanced cancer.This systematic review and meta-analysis aim to evaluate the effects of psilocybin on adult patients with advanced cancer.AIM To investigate the therapeutic effect of psilocybin in patients with advanced cancer.METHODS A comprehensive search of electronic databases was conducted in PubMed,Cochrane Central Register of Controlled Trials,and Google Scholar for articles published up to February 2023.The reference lists of the included studies were also searched to retrieve possible additional studies.RESULTS A total of 7 studies met the inclusion criteria for the systematic review,comprising 132 participants.The results revealed significant improvements in quality of life,pain control,and anxiety relief following psilocybin-assisted therapy,specifically results on anxiety relief.Pooled effect sizes indicated statistically significant reductions in symptoms of anxiety at both 4 to 4.5 months[35.15(95%CI:32.28-38.01)]and 6 to 6.5 months[33.06(95%CI:28.73-37.40)].Post-administration compared to baseline assessments(P<0.05).Additionally,patients reported sustained improvements in psychological well-being and existential distress fo-llowing psilocybin therapy.CONCLUSION The findings provided compelling evidence for the potential benefits of psilocybin-assisted therapy in improving quality of life,pain control,and anxiety relief in patients with advanced cancer.展开更多

Effective treatment for RA is not yet available because current research does...BACKGROUND: Rheumatoid arthritis (RA), as a common systemic inflammatory autoimmune disease, affects approximately 1 in 100 individuals. Effective treatment for RA is not yet available because current research does not have a clear understanding of the etiology and pathogenesis of RA. Xinfeng Capsule, a patent Chinese herbal medicine, has been used in the treatment of RA in recent years. Despite its reported clinical efficacy, there are no large-sample, multicenter, randomized trials that support the use of Xinfeng Capsule for RA. Therefore, we designed a randomized, double-blind, multicenter, placebo-controlled trial to assess the efficacy and safety of Xinfeng Capsule in the treatment of RA. METHODS AND DESIGN: This is a 12-week, randomized, placebo-controlled, double-blind, multicenter trial on the treatment of RA. The participants will be randomly assigned to the experimental group and the control group at a ratio of 1:1. Participants in the experimental group will receive Xinfeng Capsule and a pharmaceutical placebo (imitation leflunomide). The control group will receive leflunomide and an herbal placebo (imitation Xinfeng Capsule). The American College of Rheumatology (ACR) Criteria for RA will be used to measure the efficacy of the Xinfeng Capsule. The primary outcome measure will be the percentage of study participants who achieve an ACR 20% response rate (ACR20), which will be measured every 4 weeks after randomization. Secondary outcomes will include the ACR50 and ACR70 responses, the side effects of the medications, the Disease Activity Score 28, RA biomarkers, quality of life, and X-rays of the hands and wrists. The first four of the secondary outcomes will be measured every 4 weeks and the others will be measured at baseline and after 12 weeks of treatment. DISCUSSION: The result of this trial will help to evaluate whether Xinfeng Capsule is effective and safe in the treatment of RA. BACKGROUND Study showed that systemic holistic care not only aids in disease treatment and physical recovery to a certain extent but also effectively enhances patient psychological well-being,social support,and overal...BACKGROUND Study showed that systemic holistic care not only aids in disease treatment and physical recovery to a certain extent but also effectively enhances patient psychological well-being,social support,and overall quality of life(QoL).AIM To assess systematic holistic care impact on the recovery and well-being of postoperative patients with colon cancer.METHODS Our randomized controlled trial included 98 postoperative patients with colon cancer admitted to our hospital from June 2021 to June 2022.Patients were divided into control and study groups.The control group received conventional postoperative nursing care,whereas the study group received systematic holistic nursing care.We monitored gastrointestinal function recovery,and recorded changes in serum albumin(ALB),prealbumin(PA),psychological state,selfmanagement,self-efficacy,QoL,and the occurrence of complications in patients before,at discharge,and 2 wk post-discharge.Spearman analysis assessed correlations between psychological state,self-management,self-efficacy,and QoL of patients in the study group 2 wk post-discharge.RESULTS Following the nursing intervention,we observed significantly shorter postoperative bowel sound recovery time,anal exhaust time,and defecation time in the study group than in the control group(P<0.05).Patient ALB and PA levels,psychological status,self-management ability,self-efficacy and QoL at discharge and 2 wk post-discharge significantly improved,with greater improvements observed in the study group(P<0.05).Both groups experienced complications post-interventions,but the intervention group had significantly lower complication rate(3/49,6.12%)(P<0.05).In the study group,patient anxiety,depression,self-management and QoL scores at 2 wk post-discharge exhibited a significant negative correlation(3/49,6.12%)with QoL scores,with correlation coefficients of r=-0.273,-0.522,-0.344,and P<0.01,respectively.Conversely,patient self-efficacy scores 2 wk postdischarge showed a positive correlation with QoL scores(r=0.410,P=0.000).CONCLUSION Systemic holistic nursing significantly benefits postoperative patients with colon cancer by promoting gastrointestinal recovery,improving post-operation well-being,reducing complications,and enhancing QoL.展开更多
Currently,chemicals and waste are recognized as key drivers of habitat degradation and biodiversity loss in aquatic ecosystems.To ensure vibrant habitats for aquatic species and maintain a sustainable aquatic food sup...Currently,chemicals and waste are recognized as key drivers of habitat degradation and biodiversity loss in aquatic ecosystems.To ensure vibrant habitats for aquatic species and maintain a sustainable aquatic food supply system,Japan promulgated its Environmental Quality Standards for the Conservation of Aquatic Life(EQS-CAL),based on its own aquatic life water quality criteria(ALWQC)derivation method and application mechanism.Here we overview Japan's EQS-CAL framework and highlight their best practices by examining the framework systems and related policies.Key experiences from Japan's EQS-CAL system include:(1)Classifying six types of aquatic organisms according to their adaptability to habitat status;(2)Using a risk-based chemical screening system for three groups of chemical pollutants;(3)Recommending a five-step method for determining ALWQC values based on the most sensitive life stage of the most sensitive species;(4)Applying site-specific implementation mechanisms through a series of Plan-Do-Check-Act loops.This paper offers scientific references for other jurisdictions,aiding in the development of more resilient ALWQC systems that can maintain healthy environments for aquatic life and potentially mitigate ongoing threats to human societies and global aquatic biodiversity.展开更多

The findings of the study indicated that college freshmen in the context of the university studied had not experienced expected satisfactory levels of life quality, and that the perceived determinants of their life quality could be further addressed for promotion of health and education quality. The study could serve as an example to call for regular examinations on students' experience and discrepancies between experience and expectation in order to address related issues for higher educational quality.

文摘Objective: To explore the effects of “hospital–community integrated transitional care” model on quality of life in patients with chronic obstructive pulmonary disease (COPD). Methods: A total of 117 inpatients with COPD from the Department of Respiratory Medicine in a tertiary general hospital in Nanjing were enrolled by convenience sampling from January to December in 2016 and then were divided into intervention group (n = 60) and control group (n = 57) by random number table. Patients in the intervention group accepted both routine care and hospital– community integrated transitional care for 3 months after discharge. Assessment of quality of life by telephone follow-up or interview within a week before discharge and 1, 3, and 6 months after discharge was evaluated using the Short Form-36 (SF-36) health survey questionnaire. Results: For a total score of quality of life, there was a significant difference between the two groups (P < 0.05): 1, 3, and 6 months after discharge. In addition, for each dimension score of quality of life, there were no significant differences (P > 0.05) except vitality dimension, 1 month after discharge, and there were significant differences in all dimensions, 3 and 6 months after discharge (P < 0.05) Conclusions: Hospital–community integrated transitional care model can improve the quality of life of patients with COPD.
文摘Objective: To evaluate the quality of life in patients with endometriosis pelvic pain before and after the application of the levonorgestrel-releasing intrauterine system (LNG-IUS). Design: Open non-comparative study. Setting: Obstetrics and Gynecology Hospital at Monterrey, Mexico. Sample: 29 women aged 18 to 40 years with pelvic pain associated with endometriosis confirmed by laparoscopy. Methods: After laparoscopy but before LNG-IUS insertion (basal visit) and 6 months afterwards, modified Endometriosis Health Profile (EHP-30) was applied. Main outcomes measures: Size of change of questionnaire scores, need of additional analgesic therapy and adverse effects. Statistical Analysis: Differences in the questionnaire scores before and after intervention were analyzed by Student t-test. Results: Final analysis set included 29 women aged 31.7 ± 4.7 years years. The ASRM surgical staging of endometriosis was mild in 19.3 moderate in 13.7 and severe in 76% of the patients. The general perception of quality of life improved from 52 at baseline to 98% at six months (p < 0.001). Adverse events were mild in nature, 19 patients reported no adverse events during the study (65.5%). Two patients (6.9%) required the use of concomitant therapy with non-steroidal analgesics for relief of pain. Conclusion: The application of LNG-IUS in patients with pelvic pain associated with endometriosis improved significatively all aspects related with quality of life as measured with Endometriosis Health Profile (EHP-30). We concluded that LNGIUS may be an effective and convenient therapeutic alternative for the management of pain associated with endometriosis.

文摘Background:The purpose of this study was to use the modified Delphi method to identify the influencing factors of health-related quality of life(HRQoL)in patients with unruptured intracranial aneurysms(UIAs)after endovascular treatment.Methods:A modified Delphi method to obtain expert consensus on the content of potential influencing factors of HRQoL in patients with UIAs treated by endovascular intervention was employed.The research team consists of three neuroradiologists and one epidemiologist from Xuanwu Hospital of Capital Medical University.They randomly selected 21 well-known experts in cerebrovascular disease diagnosis and treatment as participating experts.The importance of the indicator is based on the 5-Likert scale.The standard deviation(SD),coefficient of variation(CV),mean(x),and minimum and maximum scores of each indicator were calculated.The consistency was described by Kendall coefficient of concordance with a p value<0.05 indicating that the expert consistency was high.Result:Twenty-one and 18 questionnaires were responded in 2 rounds,with effective response rates of 85.7%and 100.0%,respectively.The average authoritative coefficient(Cr)of all 21 experts was 0.88,familiarity with the indicators(Cs)was 0.82,and the judgment basis of the indicators(Ca)was 0.94.Eventually,the x values of arterial puncture hematoma,hyperlipidemia,gender,marital status,and hospitalization for other diseases were lower than 3.5;CV for marital status and gender was higher than 0.35.The Kendall coefficient of concordance in the first round was 0.19(p<0.001),and the second round was 0.15(p<0.001).Conclusion:In this study,the factors affecting the recovery of HRQoL after endovascular treatment in patients with UIAs were analyzed by the modified Delphi method,which provided a valuable evidence for the clinical management and daily life guidance for UIAs patients.
文摘BACKGROUND: Rheumatoid arthritis(RA) is a systemic inflammatory disease characterized by functional disability and pain. Although acupuncture is widely used, until now Western acupuncture studies on RA have not shown conclusive positive results. Acupuncture is regarded as a reflex therapy that has effects on the human autonomic nervous system. By establishing a traditional Chinese medicine(TCM) diagnosis first, the practitioner is able to choose acupoints according to the state of each individual patient.METHODS/DESIGN: We are interested if acupuncture, using a classical diagnostic procedure to allocate acupoints to the patient according to the Shang Han Lun theory, can be effective in relieving pain, improving hand function and increasing health-related quality of life in RA.The authors intend to harmonize TCM diagnosis according to clinical and genetic profiles. Patients with the TCM diagnosis of a so-called Turning Point syndrome will be followed up in a randomized, prospective, double-blind, placebo-controlled, multicenter and three-armed parallel-group study with a standardized treatment in order to optimize potential therapeutic effects of acupuncture on pain, strength and muscle function of patients with RA as well as the influence on inflammation and quality of life.DISCUSSION: The findings of this study will provide important clinical information about the feasibility and efficacy of acupuncture treatment for RA patients. 文摘Objective:To study the application effects of teach-back method combined with WeChat education in patients with chronic obstructive pulmonary disease(COPD).Methods:Convenience sampling was used to select 103 patients with COPD hospitalized in the department of respiratory medicine of a tertiary-level hospital from March to June 2021 as study subjects.Randomized grouping was carried out using the random number table method.The routine care was given in the control group,and the teach-back method combined with WeChat tutorials on the basis of routine care was applied in the intervention group.In the follow-up three months after discharge,the changes in pulmonary function,self-care ability,and quality of life of patients in the two groups were observed and compared.Results:After the intervention,the pulmonary function indexes of both groups improved significantly,and the improvement effect was more significant in the intervention group(P<0.05);after the intervention,the self-care ability scores of the patients in the intervention group were significantly higher than those of the control group(P<0.05);the quality-of-life scores of the patients in the two groups decreased at 3 months after discharge,and the scores of the patients in the intervention group were significantly lower than those of the patients in the control group(P<0.05).Conclusion:The teach-back method combined with WeChat education can effectively improve the pulmonary function,self-care ability,and quality of life of patients with COPD.

文摘BACKGROUND Systemic lupus erythematosus(SLE)is a heterogeneous autoimmune disorder with varied clinical courses and prognoses,not only did the patients suffer from physical impairment,but also various physical and psychiatric comorbidities.Growing evidence have suggested that mental disorders in SLE patients,can lead to various adverse consequences.AIM To explored the features and influencing factors of mental health in patients with SLE and clarifying the correlations between mental health and personality characteristics and perceived social support.The results would provide a basis for psychological intervention in patients with SLE.METHODS The clinical data of 168 patients with SLE admitted at the First Affiliated Hospital of Hainan Medical University between June 2020 and June 2022 were collected.Psychological assessment and correlation analysis were conducted using the Symptom Checklist-90(SCL-90)and Perceived Social Support Scale,and the collected data were compared with the national norms in China.The relevant factors influencing mental health were identified by statistical analysis.A general information questionnaire,the Revised Life Orientation Test,and Short-Form 36-Item Health Survey were employed to assess optimism level and quality of life(QoL),respectively.RESULTS Patients with SLE obtained higher scores for the somatization,depression,anxiety,and phobic anxiety subscales than national norms(P<0.05).A correlation was identified between total social support and total SCL-90 score or each subscale(P<0.05).The factors significantly affecting patients’mental health were hormone dosage and disease activity index(DAI)(P<0.05).The average optimism score of patients with SLE was 14.36±4.42,and 30 cases were in the middle and lower levels.A positive correlation was found between optimism level and QoL scores.CONCLUSION Patients with SLE develop psychological disorders at varying degrees,which are significantly influenced by hormone dosage and DAI.Patients’mental health should be closely monitored during clinical diagnosis and treatment and provided adequate support in establishing positive,healthy thinking and behavior patterns and improving their optimism level and QoL.
文摘BACKGROUND Psilocybin,a naturally occurring psychedelic compound found in certain species of mushrooms,is known for its effects on anxiety and depression.It has recently gained increasing interest for its potential therapeutic effects,particularly in patients with advanced cancer.This systematic review and meta-analysis aim to evaluate the effects of psilocybin on adult patients with advanced cancer.AIM To investigate the therapeutic effect of psilocybin in patients with advanced cancer.METHODS A comprehensive search of electronic databases was conducted in PubMed,Cochrane Central Register of Controlled Trials,and Google Scholar for articles published up to February 2023.The reference lists of the included studies were also searched to retrieve possible additional studies.RESULTS A total of 7 studies met the inclusion criteria for the systematic review,comprising 132 participants.The results revealed significant improvements in quality of life,pain control,and anxiety relief following psilocybin-assisted therapy,specifically results on anxiety relief.Pooled effect sizes indicated statistically significant reductions in symptoms of anxiety at both 4 to 4.5 months[35.15(95%CI:32.28-38.01)]and 6 to 6.5 months[33.06(95%CI:28.73-37.40)].Post-administration compared to baseline assessments(P<0.05).Additionally,patients reported sustained improvements in psychological well-being and existential distress fo-llowing psilocybin therapy.CONCLUSION The findings provided compelling evidence for the potential benefits of psilocybin-assisted therapy in improving quality of life,pain control,and anxiety relief in patients with advanced cancer.

文摘BACKGROUND: Rheumatoid arthritis (RA), as a common systemic inflammatory autoimmune disease, affects approximately 1 in 100 individuals. Effective treatment for RA is not yet available because current research does not have a clear understanding of the etiology and pathogenesis of RA. Xinfeng Capsule, a patent Chinese herbal medicine, has been used in the treatment of RA in recent years. Despite its reported clinical efficacy, there are no large-sample, multicenter, randomized trials that support the use of Xinfeng Capsule for RA. Therefore, we designed a randomized, double-blind, multicenter, placebo-controlled trial to assess the efficacy and safety of Xinfeng Capsule in the treatment of RA. METHODS AND DESIGN: This is a 12-week, randomized, placebo-controlled, double-blind, multicenter trial on the treatment of RA. The participants will be randomly assigned to the experimental group and the control group at a ratio of 1:1. Participants in the experimental group will receive Xinfeng Capsule and a pharmaceutical placebo (imitation leflunomide). The control group will receive leflunomide and an herbal placebo (imitation Xinfeng Capsule). The American College of Rheumatology (ACR) Criteria for RA will be used to measure the efficacy of the Xinfeng Capsule. The primary outcome measure will be the percentage of study participants who achieve an ACR 20% response rate (ACR20), which will be measured every 4 weeks after randomization. Secondary outcomes will include the ACR50 and ACR70 responses, the side effects of the medications, the Disease Activity Score 28, RA biomarkers, quality of life, and X-rays of the hands and wrists. The first four of the secondary outcomes will be measured every 4 weeks and the others will be measured at baseline and after 12 weeks of treatment. DISCUSSION: The result of this trial will help to evaluate whether Xinfeng Capsule is effective and safe in the treatment of RA. 文摘BACKGROUND Study showed that systemic holistic care not only aids in disease treatment and physical recovery to a certain extent but also effectively enhances patient psychological well-being,social support,and overall quality of life(QoL).AIM To assess systematic holistic care impact on the recovery and well-being of postoperative patients with colon cancer.METHODS Our randomized controlled trial included 98 postoperative patients with colon cancer admitted to our hospital from June 2021 to June 2022.Patients were divided into control and study groups.The control group received conventional postoperative nursing care,whereas the study group received systematic holistic nursing care.We monitored gastrointestinal function recovery,and recorded changes in serum albumin(ALB),prealbumin(PA),psychological state,selfmanagement,self-efficacy,QoL,and the occurrence of complications in patients before,at discharge,and 2 wk post-discharge.Spearman analysis assessed correlations between psychological state,self-management,self-efficacy,and QoL of patients in the study group 2 wk post-discharge.RESULTS Following the nursing intervention,we observed significantly shorter postoperative bowel sound recovery time,anal exhaust time,and defecation time in the study group than in the control group(P<0.05).Patient ALB and PA levels,psychological status,self-management ability,self-efficacy and QoL at discharge and 2 wk post-discharge significantly improved,with greater improvements observed in the study group(P<0.05).Both groups experienced complications post-interventions,but the intervention group had significantly lower complication rate(3/49,6.12%)(P<0.05).In the study group,patient anxiety,depression,self-management and QoL scores at 2 wk post-discharge exhibited a significant negative correlation(3/49,6.12%)with QoL scores,with correlation coefficients of r=-0.273,-0.522,-0.344,and P<0.01,respectively.Conversely,patient self-efficacy scores 2 wk postdischarge showed a positive correlation with QoL scores(r=0.410,P=0.000).CONCLUSION Systemic holistic nursing significantly benefits postoperative patients with colon cancer by promoting gastrointestinal recovery,improving post-operation well-being,reducing complications,and enhancing QoL.

文摘Currently,chemicals and waste are recognized as key drivers of habitat degradation and biodiversity loss in aquatic ecosystems.To ensure vibrant habitats for aquatic species and maintain a sustainable aquatic food supply system,Japan promulgated its Environmental Quality Standards for the Conservation of Aquatic Life(EQS-CAL),based on its own aquatic life water quality criteria(ALWQC)derivation method and application mechanism.Here we overview Japan's EQS-CAL framework and highlight their best practices by examining the framework systems and related policies.Key experiences from Japan's EQS-CAL system include:(1)Classifying six types of aquatic organisms according to their adaptability to habitat status;(2)Using a risk-based chemical screening system for three groups of chemical pollutants;(3)Recommending a five-step method for determining ALWQC values based on the most sensitive life stage of the most sensitive species;(4)Applying site-specific implementation mechanisms through a series of Plan-Do-Check-Act loops.This paper offers scientific references for other jurisdictions,aiding in the development of more resilient ALWQC systems that can maintain healthy environments for aquatic life and potentially mitigate ongoing threats to human societies and global aquatic biodiversity.

文摘A Bayesian sequential testing method is proposed to evaluate system reliability index with reliability growth during development.The method develops a reliability growth model of repairable systems for failure censored test,and figures out the approach to determine the prior distribution of the system failure rate by applying the reliability growth model to incorporate the multistage test data collected from system development.Furthermore,the procedure for the Bayesian sequential testing is derived for the failure rate of the exponential life system,which enables the decision to terminate or continue development test.Finally,a numerical example is given to illustrate the efficiency of the proposed model and procedure.
