内镜下STER术和ESE术治疗上消化道固有肌层肿瘤效果对比

本文主要是比较两类治疗术在对人体消化道固有肌层的肿瘤治疗过程中的安全程度以及治疗效果,这两类治疗术一是STER术,二是ESE术通过进行回顾性分析,我们发现,在一年的时间里,接受STER和ESE 治疗的患者数量都在28例,而且年龄也相似。然而,我们发现,这些患者的肿瘤病理特征和年龄之间的差异没有显著的统计学意义。在这些案例中,只有39例得到了彻底的手术治疗,但是能够获得全面治疗的可能性却是百分之百。在利用ESE术进行治疗的病患里,在剥离期间出现穿孔现象的有两例,经金属钛夹封闭的方式妥善处理。经过研究,我们发现,无论采取哪种治疗,住院时长和肿瘤的大小都没有明显的变化。然而,STER组的术后出血量明显低于ESE组,两者的比较结果都有一定的相关性(P<0.05)。经研究发现,无论采用STER术或ESE术,均能够取得良好的治疗效果,因此,它们均被认定为最佳的治疗策略。然而,ESE术的优势显而易见,它的术式更加简单,而且术后的恢复期也更加缩短。STER和ESE相比,STER更为安全、有效。

利用ASTER数据反演陆面温度的研究进展

目前利用ASTER数据反演陆面温度的算法可划分为3类:ASTER TES(Temperature/Emissivi-ty Separation)算法,多通道算法,辐射传输算法。对上述3类方法进行了分析比较,提出了今后工作中要:实现遥感的综合反演、地表发射率的精确获取以及反演实用价值更大的组分温度的努力方向。

文章介绍了使用MicReD公司的热测试仪T3Ster测量元器件内部热特性的方法。T3Ster测试仪可以测试各类IC、LED、散热器、热管等电子器件的热特性以及PCB、导热材料等的热阻、热容及导热系数、接触热阻等热特性。使用T3Ster测试仪对某航天器用电子元器件内部热特性进行了测量,并与器件资料中的热特性数据进行比对,二者相对误差为0.07%,验证了T3Ster测试仪具有测试高可靠度要求的宇航级电子元器件热特性的能力。为宇航电子元器件的热设计与热分析提供重要的试验依据。

STER术治疗食管固有肌层肿瘤23例

目的:探讨经黏膜下隧道内镜切除术(submucosal tunneling endoscopic resection,STER)治疗来源于食管固有肌层肿瘤的疗效和安全性.方法:对23例经超声内镜和计算机断层扫描(computed tomography,CT)诊断为来源于食管固有肌层肿瘤患者全麻下行STER术切除肿瘤,观察并记录肿瘤大小、病理类型、是否穿孔、手术时间、手术并发症情况.结果:23例食管固有肌层肿瘤中来源于固有肌层浅层者15例,深层者8例,STER术均完整切除肿瘤,术后瘤体直径范围在0.6-1.8 cm,平均为1.0 cm±0.4 cm.完整缝合创面所需钛夹4-8枚,平均为6.2枚±2.4枚,手术时间在32-115 min,平均为62.0 min±24.5 min.术后病理确诊为平滑肌瘤19例,间质瘤4例,切缘均阴性.术中穿孔2例,皮下气肿3例,无迟发性出血.术后住院日4-11 d,平均为5.8 d±3.4 d.结论:STER术治疗来源于固有肌层的食管黏膜下肿瘤是安全、有效的.

ESD与STER在上消化道黏膜下肿物切除中的选择与对比研究

目的评价内镜黏膜下剥离术(ESD)和黏膜下隧道肿瘤切除术(STER)在上消化道黏膜下肿物切除中的选择与应用,以指导临床选择合理的内镜技术。方法回顾性收集2012年6月-2018年10月在广州市第一人民医院消化疾病中心就诊并接受ESD或STER治疗的所有患者,筛选出上消化道黏膜下肿物患者共64例,其中ESD组34例,STER组30例,详细分析并归纳患者临床资料及术后随访资料。结果①ESD组及STER组在治疗效果及预后方面无差异,两组肿物完整切除率均为100%,术后随访两组均无局部复发及远处转移;②ESD组内镜下表现以腔内生长及肿物形态较规则为主,瘤体主要位于胃底、胃体大弯;STER组内镜下表现以腔外生长及肿物形态不规则为主,瘤体主要位于食管、胃体小弯。③STER组手术时间少于ESD组,差异有统计学意义。结论对于上消化道黏膜下肿瘤患者,ESD和STER是一种安全和有效的治疗手段;若瘤体形态欠规则及腔外生长为主,位于食管、胃体小弯,STER为优先选择。

消化道黏膜下肿物的内镜下诊断及治疗

目的探讨消化道黏膜下肿物的内镜下诊断、治疗及其安全性。方法对261例消化道黏膜下肿物进行内镜及超声内镜检查,并采用内镜黏膜下挖除术(endoscopic submucosal excavation,ESE)、胃镜与腹腔镜双镜联合以及内镜黏膜下隧道肿瘤切除术(submucosal tunneling endoscopic resection,STER)进行肿物切除。结果内镜下通过ESE切除黏膜下肿物213例,胃镜腹腔镜双镜联合切除42例,STER切除6例,确诊胃肠道间质瘤86例、平滑肌瘤85例、脂肪瘤35例、类癌32例、异位胰腺15例、神经鞘瘤4例、错构瘤3例和血管球瘤1例。超声内镜对各种黏膜下肿物的诊断符合率分别为:胃肠道间质瘤(93%)、平滑肌瘤(87%)、脂肪瘤(100%)、类癌(78%)、异位胰腺(80%)、神经鞘瘤(0)、错构瘤(0)和血管球瘤(100%)。所有患者均顺利完整切除瘤体,有3例GIST在内镜下剥离过程中发生了胃壁微小穿孔,通过内镜下钛夹修补达到缝合。另有1例患者发生术后感染,经抗感染治疗后痊愈。所有患者均无术中大出血、术后迟发性出血及死亡病例发生。结论 ESE、胃镜腹腔镜双镜联合以及STER是目前治疗黏膜下肿物微创、有效且安全的方法。

超声内镜是否足以精准诊断上消化道黏膜下肿物?

目的评估超声内镜(endoscopic ultrasonography,EUS)诊断上消化道黏膜下隆起病变的准确率。方法收集四川省人民医院2014年1月至2016年6月经胃镜检查提示上消化道黏膜下隆起者共108例,顺序以EUS拟诊、内镜下切除术(ESD/STER等)、病理结果评估EUS诊断的准确率。以病理结果为金标准,计算EUS的真阳性率及假阳性率。结果 EUS真阳性率为72.2%(78/108),假阳性率为27.8%(30/108)。来源于黏膜肌层及固有肌层隆起病变的假阳性率为23.6%(21/89)。食管及贲门隆起病变多为平滑肌瘤;除贲门外其他上消化道黏膜下隆起病变则以间质瘤为主,真阳性率高达80.3%。来源于固有肌层的钙化性纤维瘤,EUS下极易被误诊为间质瘤。来源于黏膜下层的低回声病灶,如神经内分泌肿瘤、深在性囊性胃炎、神经鞘瘤、脉管瘤等,EUS的假阳性率则高达47.3%(9/19)。结论 EUS对于上消化道黏膜下隆起病变的诊断有较高的临床价值,但对于黏膜下层低回声病灶的精准诊断,EUS仍存在较多困难。因此,对照术后病理结果复习EUS图像,对于提高EUS诊断、鉴别诊断的能力,把握内镜切除指征具有重要的临床意义。
