Diffusion-weighted (DW) MR imaging is a developing technique, proved to im...Magnetic resonance (MR) imaging of the scrotum represents an important supplemental diagnostic tool in the evaluation of scrotal diseases. Diffusion-weighted (DW) MR imaging is a developing technique, proved to improve tissue characterization. We evaluated the feasibility and diagnostic performance of DW MR imaging in the detection and characterization of scrotal lesions. We retrospectively evaluated 31 scrotal lesions (23 intratesticular and 8 extratesticular) in 26 men. All MR examinations were performed on a 1.5-T unit, using a pelvic-phased array coil. DW sequences were obtained using a single shot, multislice spin echo planar diffusion pulse sequence and a b factor of 0 and 900 s mm-2. The DW MR characteristics and the apparent diffusion coefficient (ADC) values of normal scrotal contents and scrotal diseases were evaluated. Comparison between the ADC values of normal scrotum, benign lesions and scrotal malignancies was performed. The accuracy of conventional sequences, DW images alone and DW imaging combined with conventional images in differentiating benign from malignant scmtal lesions was calculated. The ADC values of testicular malignancies were different from those of normal testis and benign intratesticular lesions, and the ADC values of benign extratesticular lesions from those of normal epididymis (P〈0.05). The overall accuracy of conventional imaging, DW imaging alone and DW MR combined with conventional sequences in the characterization of intratesticular lesions was 91%, 87% and 100%, respectively. Our findings suggest that DW MR imaging and ADC values may provide valuable information in the diagnosis and characterization of scrotal diseases.展开更多

BACKGROUND Angiolipoma has been reported in many cases,and it often occurs in the skin of the trunk and limbs.However,angiolipoma in the scrotum is a rare disease with unknown etiology.This condition is difficult to d...BACKGROUND Angiolipoma has been reported in many cases,and it often occurs in the skin of the trunk and limbs.However,angiolipoma in the scrotum is a rare disease with unknown etiology.This condition is difficult to diagnosis with other lumps in the scrotum.CASE SUMMARY A 32-year-old man presented to the urinary department with a history of an enlarged left scrotum with increasing discomfort for about 5 years.Physical examination revealed that there were a palpable mass measuring about 7.0 cm×6.5 cm in the left scrotum,with smooth surfaces but without tenderness or adhesion to the skin.Ultrasound showed that there was a hyperechoic mass under the skin of the top scrotum,about 72 mm×64 mm×21 mm in size,with clear borders,uneven internal echo,and abundant blood flow signals.Serum human chorionic gonadotropin and alpha-fetoprotein were in normal level.Subcutaneous mass resection at the bottom of the left scrotum was performed under local anesthesia with 1%lidocaine.Postoperative pathological examination resulted in a diagnosis of subcutaneous angiolipoma of the scrotum.No evidence of recurrence was found at 6 mo after surgery and there were no complaints of discomfort.CONCLUSION Angiolipoma is an extremely rare type of benign tumor extremely rarely found in the scrotum,but needs to be considered when evaluating scrotal masses especially when the mass is solid.According to the characteristics of angiolipoma,surgical resection is the best treatment strategy and it is not prone to recurrence after resection.展开更多

The aim of the study is to describe the diagnostic and therapeutic approach of a total unilateral ectopy of the right scrotum. Methods: This is a clinical observation of a 46-year-old patient with a right abdominal mass evolving from birth. Analyzing the cluster of arguments clinically, operatively and anatomopathologically, allowed us to retain the diagnosis of a total unilateral ectopy of the right scrotum. The informed consent of the patient was obtained before the use of the images for scientific purposes. Conclusion: the unilateral total ectopy of the right scrotum is a rare congenital malformation of the external genitalia, which clinical diagnosis is difficult. Its management is surgical.展开更多

Objective:To design and manufacture medical scrotal support shorts for patients with enlarged scrotum and observe its application and effect.Methods:40 patients with enlarged scrotum admitted to the basic surgery depa...Objective:To design and manufacture medical scrotal support shorts for patients with enlarged scrotum and observe its application and effect.Methods:40 patients with enlarged scrotum admitted to the basic surgery department from February 2021 to March 2023 were selected and divided into a test group and a control group according to their time of admission,with the test group using scrotal support shorts and the control group using ordinary shorts without scrotal support pockets or diapers.Results:The complication rate of skin injury in the scrotum and the surrounding inguinal area of the patients in the test group was significantly lower than that of the control group(P<0.01).The medical cost covered by patients in the test group was significantly lower than that of the control group(P<0.01)and the hospitalization satisfaction of the patients in the test group was significantly higher than that of the control group(P<0.01).The difference in the therapeutic effect of the test groups was statistically significant when compared with the control group(P<0.05).Conclusion:Homemade medical scrotal support shorts reduced the local enlargement of the scrotum and bleeding,but also protected the scrotum and the surrounding skin to prevent secondary injuries.The process of patient care was simple and promoted their recovery.The length of hospitalization was also decreased,the burden of health care costs was reduced,and the overall comfort and satisfaction of the patient was improved.展开更多

Physical exam revealed an ulcerating lesion with central necrosis and eschar surrounded by a halo of erythema on the inferior aspect of the left scrotum. The condition indicated an early necrofizing soft tissue infection. The patient was started on broad-spectrum antibiotics and taken to the operating room for a wound debridement. Blood and tissue cultures grew Pseudomonas aeruginosa, which confirmed the diagnosis of ecthyma gangrenosum of the scrotum. The fever resolved, and the wound healed without further progression after wet to dry dressing changes.展开更多
