The data were derived from the Baseline Survey of Natio...The objective of this study was to examine the inpatient bed (IB) allocation equity and utilization in Chinese city commtmity health service centers (CHSCs). The data were derived from the Baseline Survey of National City Community Health Service System Building Project, which was conducted in 1917 CHSCs in 28 cities in 2007. The IB allocation was analyzed in terms oflB alloca- tion quantity and distribution equity, and the IB utilization was analyzed by the IB utilization rate and average length of stay of the CHSC inpatients. The results showed that 49.3% of the CHSCs were equipped with IB; averagely, there were 45 IBs per CHSC, 0.94 IBs per 1000 people, and 0.38 nurses and 0.57 doctors per IB; the IB Gini coefficient was 0.32; the IB utilization rate was 40.06%; and the average length of stay of inpatients was 12.24 days. The conclusions were that IB allocation among the population was equitable, but the number of nurse per IB was not up to the national standard; and the CHSC IB utilization was low as a whole, thus inpatient service was not the main health service for Chinese CHSCs.展开更多

In the last five years, China has seen the technological development of intelligent mining and the application of the longwall automation technology developed by the Longwall Automation Steering Committee. This paper ...In the last five years, China has seen the technological development of intelligent mining and the application of the longwall automation technology developed by the Longwall Automation Steering Committee. This paper summarizes this great achievement, which occurred during the 12th Five-Year Plan (2011-2015), and which included the development of a set of intelligent equipment for hydraulic-powered supports, information transfers, dynamic decision-making, performance coordination, and the achievement of a high level of reliability despite difficult conditions. Within China, the intelligent system of a set of hydraulic-powered supports was completed, with our own intellectual property rights. An intelligent mining model was developed that permitted unmanned operation and single-person inspection on the work face. With these technologies, the number of miners on the work face can now be significantly reduced. Miners are only required to monitor mining machines on the roadway or at the surface control center, since intelligent mining can be applied to extract middle-thick or thick coal seams. As a result, miners' safety has been improved. Finally, this Darter discusses theprospects and challenges of intelligent mining over the next ten years.展开更多

Since the me...This study explores the perceptions of a small group of nurses working at a newly established 24-hour community-based service enter (SC) for users with psychiatric disability using a qualitative approach. Since the mental health reform in Sweden in 1995 where the communities (in Sweden called municipalities) were given the responsibility to establish service and support to people with severe psychiatric disabilities, they have struggled in finding suitable forms of these kinds of areas. In 2010, this led to the creation and development of a new center aiming to provide services and support based on the expressed needs of people with physchiatric disability in a community located in southern Sweden. During 2011, a total of three group interviews were performed to capture the employed nurses’ perceptions of this newly established SC. The interview texts were analyzed by way of qualitative content analysis. A first reading of the interview texts revealed that the nurses’ perceptions of the service center were unwaveringly positive but that their beliefs about who the specific target group were differed. The main finding was summarized by the theme: Making a difference—on an individual, professional, and organizational level. The sub themes were: 24-hour availability, unclear assignment, and preventing mental illness. The findings indicate a need for a community round-the-clock service center in this Swedish community and a more clear definition of the target group.展开更多

However, there are no studies that comprehensively verify the stress reduction effect, cognitive function, vital signs, etc. in the elderly requiring long-term care. In this study, we focused on Himekuromoji (</span><i><span style="font-family:Verdana;">Lindera</span></i><span style="font-family:Verdana;"> <i>lancea</i></span><span style="font-family:Verdana;"> ), which grows naturally in Japan and has no verification cases and decided to verify the possibility of aromatherapy using that essential oil. As a result, olfactory stimulation by </span><i><span style="font-family:Verdana;">Lindera</span></i><span style="font-family:Verdana;"> <i>lancea</i></span><span style="font-family:Verdana;"> aroma stabilizes the mood and emotions of the client and suppressed the rise in blood pressure. In sentiment analysis, four emotions were quantified using a wearable device. Although the effects of worker emotions on labor productivity have already been elucidated using the same device as in this study, emotional quantification by aromatherapy intervention is unprecedented. There were no significant differences in the four emotions depending on the intervention period and test section, but we were able to capture changes in emotions due to differences in recreational activities. In addition, it is considered that the client was able to spend about 30% of the activity time in a relaxed manner. It was expected that this aromatherapy as complementary and alternative medicine could contribute to the good emotional control of all the people involved. The COVID 19 pandemic has led to restrictions on human-to-human interaction, so the importance of emotional control represented by stress control is increasing for all humankind. This new 24-hour support center (SC) was established in 2011 in a Swedish community to better match and facilitate the disabled persons’ needs. In order to illuminate the disabled person’s experiences individual interview was performed. A phenomenological-hermeneutical method inspired by Paul Ricoeur was used to interpret the texts. After a naive reading, a structural analysis revealed two themes: 1), becoming aware of myself as a person, and 2) having a lifeline and belongingness. The comprehensive understanding was interpreted as meaning “Making me feel almost like an ordinary person”, which incorporated the person’s past with their present together with a direction for the future and hope for a more fulfilling life.展开更多

In the mission of on-orbit servicing, as fuel is expended, or the payloads are added or removed, the center of mass will be changed in certain axe; consequently, some thrusters' directions are deviated from the center of mass(CM) in certain plane. The CM of assembled spacecraft estimation and thruster direction control are studied. Firstly, the attitude dynamics of the assembled spacecraft is established based on the Newton-Euler method. Secondly, the estimation can be identified by the least recursive squares algorithm. Then, a scheme to control the thrusters' directions is proposed. By using the gimbal installed at the end of the boom, the angle of the thruster is controlled by driving the gimbal; therefore, thrusters can be directed to the CM again. Finally, numerical simulations are used to verify this scheme. Results of the numerical simulations clearly show that this control scheme is rational and feasible.展开更多
Concerns regarding the environmental impacts of hospitals have made the healthcare sector one of the most prominent settings for the green building movement.Despite these environmental concerns,the number of studies t...Concerns regarding the environmental impacts of hospitals have made the healthcare sector one of the most prominent settings for the green building movement.Despite these environmental concerns,the number of studies that document the benefits of sustainable healthcare facilities is limited.In this study,the authors used the Medicare and Medicaid cost report data to compare the financial performance of 14 Leadership in Energy and Environmental Design(LEED)certified hospitals against the average performance of non-green facilities.In line with studies conducted in other settings,the authors found that there are low and high performers among green hospitals.For this study,green hospitals had higher facility operating expenses than an average non-green hospital and spent more on plant operations.While green hospitals included in this study generated more revenue,the incomes were not high enough to validate the high operating expenses.展开更多

This bundle protocol, titled SEP-1 is a multicomponent 3 h and 6 h resuscitation treatment for p...Background: In 2018, the Centers for Medicaid and Medicare Services (CMS) issued a protocol for the treatment of sepsis. This bundle protocol, titled SEP-1 is a multicomponent 3 h and 6 h resuscitation treatment for patients with the diagnosis of either severe sepsis or septic shock. The SEP-1 bundle includes antibiotic administration, fluid bolus, blood cultures, lactate measurement, vasopressors for fluid-refractory hypotension, and a reevaluation of volume status. We performed a retrospective analysis of patients diagnosed with either severe sepsis or septic shock comparing mortality outcomes based on compliance with the updated SEP-1 bundle at a rural community hospital. Methods: Mortality outcome and readmission data were extracted from an electronic medical records database from January 1, 2019, to June 30, 2020. International Classification of Diseases (ICD)-10 codes were used to identify patients with either severe sepsis or septic shock. Once identified, patients were separated into four populations: patients with severe sepsis who met SEP-1, patients with severe sepsis who failed SEP-1, patients with septic shock who met SEP-1, and patients with septic shock who failed SEP-1. A patient who met bundle criteria (SEP-1 criteria) received each component of the bundle in the time allotted. Using chi-squared test of homogeneity, mortality outcomes for population proportions were investigated. Two sample proportion summary hypothesis test and 95% confidence intervals (CI) determined significance in mortality outcomes. Results: Out of our 1122 patient population, 437 patients qualified to be measured by CMS criteria. Of the 437 patients, 195 met the treatment bundle and 242 failed the treatment bundle. Upon comparing the two groups, we found the probable difference in mortality rate between the met(14.87%) and failed bundle(27.69%) groups to be significant(95% CI: 5.28-20.34, P = 0.0013). However, the driving force of this result lies in the subgroup of patients with severe sepsis with septic shock, which show a higher mortality rate compared to the subgroup with just severe sepsis. The difference was within the range of 3.31% to 29.71%. Conclusion: This study shows that with septic shock obtained a benefit, decreased mortality, when the SEP-1 bundle was met. However, meeting the SEP-1 bundle had no benefit for patients who had the diagnosis of severe sepsis alone. The significant difference in mortality, found between the met and failed bundle groups, is primarily due to the number of patients with septic shock, and whether or not those patients with septic shock met or failed the bundle.展开更多
