Effect of electroacupuncture combined with rehabilitation techniques on shoulder function in patients with rotator cuff injuries

BACKGROUND The rotator cuff is located below the acromion and deltoid muscles and comprises multiple tendons that wrap around the humeral head,maintaining shoulder joint stability.AIM To explore the effect of electroacupuncture combined with rehabilitation techniques on shoulder function in patients with rotator cuff injuries.METHODS We selected 97 patients with rotator cuff injuries treated in the People's Hospital of Yuhuan from February 2020 to May 2023.Patients were grouped using the envelope method.RESULTS After treatment,the study group’s treatment effective rate was 94.90%(46/49 patients),significantly higher than that in the control group(79.17%,38/48 cases;P<0.05).Before treatment,there was no difference in Constant Murley Score(CMS)scores,shoulder mobility,or 36-Item Short Form Health Survey(SF-36)scale scores(P>0.05).Compared with those before treatment,the CMS scores(including pain,daily living ability,shoulder mobility,and muscle strength),all aspects of shoulder mobility(forward flexion,posterior extension,external rotation,internal rotation),and SF-36 scale scores(including physiological,psychological,emotional,physical,vitality,and health status)were higher in both groups after treatment and significantly higher in the study group(P<0.05).There was no difference in the occurrence of complications between the two treatment groups(P>0.05).CONCLUSION Electroacupuncture combined with rehabilitation techniques has a good treatment effect on patients with rotator cuff injuries,helps accelerate the recovery of shoulder function,improves the quality of life,and is highly safe. 展开更多

Electroacupuncture for the treatment of spinal cord iniury has a good dinical curative effect, but the underlying mechanism is unclear. In our experiments, the spinal cord of adult Sprague-Daw- ley rats was clamped for 60 seconds. Dazhui (GV14) and Mingmen (GV4) acupoints of rats were subjected to electroacupuncture. Enzyme-linked immunosorbent assay revealed that the expres- sion of serum inflammatory factors was apparently downregulated in rat models of spinal cord injury after electroacupuncture. Hematoxylin-eosin staining and immunohistochemistry results demonstrated that electroacupuncture contributed to the proliferation of neural stem cells in rat injured spinal cord, and suppressed their differentiation into astrocytes. Real-time quantitative PCR and western blot assays showed that electroacupuncture inhibited activation of the Notch signaling pathway induced by spinal cord injury. These findings indicate that electroacupuncture repaired the injured spinal cord by suppressing the Notch signaling pathway and promoting the proliferation of endogenous neural stem ceils. 展开更多

BACKGROUND:Synapses undergo high levels of plasticity within the nervous system, and cerebral ischemia induces synaptic plasticity changes.OBJECTIVE:To demonstrate the effects of electroacupuncture on ultrastructural synaptic changes in the focal cerebral ischemia marginal zone in rats using quantitative analysis of stereological measurement.DESIGN, TIME AND SETTING:A randomized, controlled, animal experiment was performed at the Experimental Animal Center and Laboratory of Electron Microscopy, Guangzhou University of Traditional Chinese Medicine from January 2008 to January 2009.MATERIALS:The G-6805 electric acupuncture apparatus was provided by Shanghai Huayi Instrument Factory, China.METHODS:A total of 90 male, Wistar rats were randomly assigned to sham-surgery, model, and electroacupuncture groups, with 30 animals in each group. Each group was subdivided into 1 hour, as well as 1, 3, 7, and 21 days post-surgery groups, with six animals assigned to each time point. Heat coagulation-induced occlusion of the middle cerebral artery was performed to establish a model of focal cerebral ischemia. Electroacupuncture was applied immediately following surgery to the electroacupuncture group [4/20 Hz, 2.0-3.0 V, 1-3 mA, to Baihui (GV 20) and Dazhui (GV 14)] for 30 minutes. Treatment was performed once a day, and experimental animals were sacrificed, at 1 hour, as well as 1, 3, 7 and 21 days post-surgery.MAIN OUTCOME MEASURES:At different time points after intervention, changes in synaptic ultrastructure, such as postsynaptic density thickness, synaptic cleft width, and synaptic interface curvature, were observed in the focal cerebral ischemia marginal zone in rats through the use of transmission electronic microscopy.RESULTS:Broken synapses were observed following cerebral ischemia, and the number of synapses was significantly decreased. Compared to the model group, synaptic ultrastructure was significantly improved in the electroacupuncture group. Compared to the sham-surgery group, postsynaptic density thickness was significantly decreased, as were synaptic cleft width and synaptic interface curvature in the electroacupuncture and model groups. However, compared to the model group, postsynaptic density thickness was significantly increased in the electroacupuncture group at the same time point post-surgery (P 〈 0.05 or P 〈 0.01). In addition, synaptic cleft width and synaptic interface curvature were significantly increased with the passage of time (P〈 0.05 or P〈 0.01).CONCLUSION:Electroacupuncture significantly ameliorated structural synapse lesion during the early stage of cerebral ischemic injury, promoted repair of synaptic structure, improved structural parameters of synapses, and increased synaptic structural plasticity, which suggested that the therapeutic effect of electroacupuncture was related to synaptic reorganization. 展开更多

OBJECTIVE: To summarize the development of electroacupuncture stimulation in the treatment of stroke-induced limb spasm, to provide research possibilities or clinical development, and to clarify the mechanism and therapeutic effects of electroacupuncture stimulation. DATA SOURCES: Articles relating to therapeutic effects of electroacupuncture stimulation on treatment of stroke-induced limb spasm, as well as clinical studies (which are not limited to age, sex and race), were retrieved from the China Journal Fulltext Database from January 2000 to December 2007. The key words were stroke, apoplexy, spasm, and electroacupuncture in Chinese. Other articles were obtained from specific journals or books. STUDY SELECTION: Data were first selected by title and abstract. Clinical and rehabilitative studies relating to the effects of electroacupuncture and acupuncture on the treatment of stroke-induced limb spasm were included. Clinical studies related to the rehabilitative treatment of limb spasm were excluded, unless the effects of electroacupuncture stimulation were recorded. DATA EXTRACTION: A total of 338 references were collected; however, 295 of these were excluded. Therefore, 43 articles were analyzed, comprising 40 in Chinese and 3 in English. DATA SYNTHESIS: At present, acupuncture is widely used at home to treat stroke-induced limb spasm; however, acupuncture at different acupoints might cause different clinical effects. Electroacupuncture, which is characterized by analgesia and relief of spasm, can preclude inflammatory edema, improve blood circulation and blood viscosity, inhibit platelet aggregation and adherence, relieve formation of atheromatous plaque in tunica intima of artery, and resist cerebral anoxia, so as to relieve stroke-induced limb spasm. Meanwhile, electroacupuncture combined with rehabilitation can effectively decrease the degree of spasm and improve motor function. CONCLUSION: Acupuncture and moxibustion therapy are superior in the rehabilitative treatment of stroke-induced limb spasm. Electroacupuncture is a convenient means to stimulate the acupoints. There are various ways to use this method, but it clearly has therapeutic effects, with few side effects. Electroacupuncture can effectively control limb spasms, while improving motor function and living ability. 展开更多

However, there are few reports addressing changes in neurotransmitter expression following manual acupuncture and electroacupuncture against visceral traction pain.OBJECTIVE: To explore changes in neurotransmitter expression in the ileum and protein expression in the medullary visceral zone of visceral traction pain rats undergoing pretreatment of emulational manual acupuncture, and to investigate the differences between emulational manual acupuncture and electroacupuncture.DESIGN, TIME AND SETTING: The randomized, controlled study was performed at the Biomedical Engineering Laboratory, Shanghai University of Traditional Chinese Medicine, Shanghai, China from August 2008 to July 2009.MATERIALS: G6805 electroacupuncture apparatus (Shanghai Medical Electronic Machine Factory, China) and ZSF-I acupuncture manipulation simulation therapeutic system (Chinese Medical Engineering Room, Shanghai University of Traditional Chinese Medicine, Shanghai China) were used in the present study.METHODS: A total of 40 male Sprague Dawley rats were equally and randomly assigned to sham surgery, model, emulational manual acupuncture and electroacupuncture groups. In the emulational manual acupuncture and electroacupuncture groups, emulational manual acupuncture and electroacupuncture were applied at bilateral Zusanli (ST 36) acupoints for 30 minutes, and models of visceral traction pain were established immediately.MAIN OUTCOME MEASURES: Substance P expression, c-Fos and glial fibrillary acidic protein expression were measured using immunohistochemistry. Acetylcholine esterase activity was examined utilizing a colorimetric method. Leucine-enkephalin content was detected using a radioimmune assay. Degree of pain in rats was assessed by pain score.RESULTS: Pain score, substance P expression in the ileum, acetylcholine esterase activity, expression of c-Fos protein and glial fibrillary acidic protein in the medullary visceral zone were significantly decreased following pretreatment of emulational manual acupuncture and electroacupuncture in rats with visceral traction pain (P〈0.05). Compared with the electroacupuncture group, the leucine-enkephalin content was significantly increased, and pain score was significantly diminished in the emulational manual acupuncture group (P〈0.05).CONCLUSION: Emulational manual acupuncture pretreatment decreases acetylcholine esterase activity, increases leucine-enkephalin release, downregulates expression of c-Fos protein and glial fibrillary acidic protein and ultimately inhibits visceral traction pain by reducing substance P release. The effectiveness in inhibiting visceral traction pain is greater when using emulational manual acupuncture compared with electroacupuncture. This is because emulational manual acupuncture effectively increases leucine-enkephalin release. 展开更多

The total effective rate was 90% in the acupuncture group, and 86.7% in the control group. Methods: A total of 114 cases (38 males and 76 females) of Meniere's disease were treated with EA of Ermen(耳门 TE 21), Tinggong (听宫 SI 19), Tinghui (听会 GB 2), Yifeng (医风 TE 17), Fengchi (风池 GB 20) and Baihui (百会 GV 20). The treatment was given 3 times a week,9 times altogether. Follow-up was made at the end of the 3rd, 6th and 12th months after treatment. Results; Follow-up in 114 patients showed that after EA treatment, 63 cases (55.26%) were cured, 30 (26.31%) had remarkable improvement in their symptoms, 10 (8. 77%) responded with improvement and 11 (9.65%) failed in the treatment, with a total effective rate of 90.35%. Conclusion: EA treatment was effective in controlling Meniere's disease patients' symptoms and reducing attack. 展开更多

Methods: 76 patients were randomly divided into treatment group (n=43) and control group (n=33).In teatment group, penetration needling from Qiuxu (丘墟 GB 40) to Zhaohai (照海 KI 6) was performed, combined with electrical stimulation for 30 min. Patients of control group were ordered to take Antinfan (50 mg,b.i.d.),supplemented with local external application of Votalin cream (b.i.d.).After 14 treatments (two courses), the therapeutic effect was assessed. Results: Following two courses of treatment, of the 43 cases and 33 cases in treatment and control groups,33 (76.7%) and 15 (45.5%) were cured, 4 (9.3%) and 7 (21.2%) had marked improvement in their symptoms, 3 (7.0%) and 2 (6.1%) had improvement, and 3 (7.0%) and 9(27.3%) failed, with the effective rates being 93.0% and 72.7% respectively. The therapeutic effect of treatment group was significantly superior to that of control group (P<0.05). Conclusion: Penetrative needling plus EA is significantly superior to medication in relieving chronic ankle spain patient’s clinical symptoms and signs. 展开更多

Main acupoints were Qugu (CV 2), Henggu (KI 11), Shuidao (ST 28) and Guanyuan (CV 4) in combination with auxiliary acupoints Zhonglushu (BL 29), Huiyang (BL 35) and Sanyinjiao (SP 6) which were stimulated with EA Therapeutic Apparatus for 30min. After treatment, of the 58 cases, 36 were cured, accounting for 62%, 20 had marked effect, accounting for 34%, and 2 had no any effectiveness, accounting for 3%. In the present paper, 176 cases of lumbar intervertebral disc protrusion are treated with electroacupuncture (EA) and topical heat irradiation. Local tender-point is used as the main acupoint, combined with Tunzhong, Tiaoyue, Weizhong (BL 40) and Yanglingquan (GB 34). The treatment is conducted once daily, with 15 sessions being a therapeutic course. Following treatment, of 176 cases, 46 (26. 1%) are cured, 90 (51. 1%) have prominent improvement, 35 (19.8%) have amelioration and 5 (2.8%) have no apparent changes, with a total effective rate of 97.0%. 展开更多

METHOD: Electro-acupuncture therapy was simultaneously applied during chemotherapy, T cells and activity of NK cell of patients were determined before electroacupuncture treatment (before chemotherapy) and after 4-course electro-acupuncture treatments. RESULTS: Before chemotherapy, CD3 was low within the normal range, CD4 was much lower than the normal range, and CD8, CD4/CD8 and activity of NK cell were within the normal range. After one month of chemotherapy combined with electro-acupuncture, no decline of all the indices was found (P > 0.05). CONCLUSION: Electro-acupuncture can really increase the immune function of patients of chemotherapy. 展开更多

目的观察电针足三里和关元穴辅助治疗类风湿关节炎(rheumatoid arthritis,RA)的临床疗效及对患者关节功能、关节压痛指数、红细胞沉降率、类风湿因子和炎症因子的影响。方法将112例RA患者用随机数字表法分为对照组和观察组,每组56例。... 目的观察电针足三里和关元穴辅助治疗类风湿关节炎(rheumatoid arthritis,RA)的临床疗效及对患者关节功能、关节压痛指数、红细胞沉降率、类风湿因子和炎症因子的影响。方法将112例RA患者用随机数字表法分为对照组和观察组,每组56例。对照组采用常规西医治疗,观察组在对照组治疗基础上予电针足三里和关元穴治疗。比较两组临床疗效和不良反应发生情况,观察两组治疗前后关节功能、关节肿胀指数、关节压痛指数、RA患者病情评价(disease activity score 28,DAS28)评分以及红细胞沉降率、类风湿因子、炎症因子[C-反应蛋白(C-reactive protein,CRP)、白介素-1β(interleukin-1β,IL-1β)和肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)]水平的变化。结果观察组总有效率高于对照组(P<0.05)。治疗后,两组关节功能(关节僵硬、疼痛和日常活动)评分、DAS28评分、关节肿胀指数和关节压痛指数以及红细胞沉降率、类风湿因子、CRP、TNF-α和IL-1β水平均低于治疗前(P<0.05),且观察组上述观察指标均低于对照组(P<0.05)。对照组1例出现咽部梗阻感,疑为甲氨蝶呤所致;有2例出现消化道不适症状。观察组2例出现针刺后皮下血肿,1例出现消化道不适症状。结论在常规西医治疗基础上,电针足三里和关元穴辅助治疗RA可提高临床疗效,有助于改善患者关节功能和关节压痛,降低红细胞沉降率、类风湿因子和炎症因子水平。 展开更多

目的观察电针刺激对慢性心力衰竭(CHF)大鼠模型及心肌纤维化细胞模型的影响,探讨电针对CHF的治疗作用机制。方法选取24只雄性Wistar大鼠随机分为4组,分别为假手术组、假手术+电针组、心肌纤维化组及心肌纤维化+电针组,每组6只。将H9c2... 目的观察电针刺激对慢性心力衰竭(CHF)大鼠模型及心肌纤维化细胞模型的影响,探讨电针对CHF的治疗作用机制。方法选取24只雄性Wistar大鼠随机分为4组,分别为假手术组、假手术+电针组、心肌纤维化组及心肌纤维化+电针组,每组6只。将H9c2大鼠心肌细胞随机分为4组,对照组、对照+电针组、血管紧张素Ⅱ处理组及血管紧张素Ⅱ处理+电针组。心肌纤维化组大鼠采用腹主动脉缩窄法建立CHF模型,假手术组大鼠仅分离腹主动脉不进行钳夹,心肌纤维化+电针组大鼠在心肌纤维化组的基础上联合电针刺激大鼠双侧“内关”穴干预,假手术+电针组大鼠在假手术组的基础上联合电针刺激大鼠双侧“内关”穴干预。血管紧张素Ⅱ处理组采用含血管紧张素Ⅱ的培养基对H9c2大鼠心肌细胞进行培养及传代,对照组H9c2大鼠心肌细胞在正常培养基内进行培养及传代,血管紧张素Ⅱ处理+电针组是血管紧张素Ⅱ处理组的基础上在培养皿中予以电刺激,对照+电针组在对照组的基础上在培养皿中予以电刺激。检测各组大鼠干预12、16、20周室间隔舒张末期厚度(IVSd)、左室舒张末期后壁厚度(LVPWd)、左室收缩末期内径(LVEDs)、左室舒张末期内径(LVEDd)及左室射血分数(LVEF),以及血清B型钠尿肽(BNP)、乳酸脱氢酶(LDH)、肌酸激酶(CK)水平,检测各组H9c2大鼠心肌细胞在干预12、16、20周重组蛋白基质金属蛋白酶9(MMP9)及基质金属蛋白酶抑制剂-1(TIMP-1)相对表达情况。结果与假手术组干预同期比较,心肌纤维化组干预12、16、20周时IVSd、LVPWd、LVEDs及LVEDd均升高(P<0.05),LVEF降低(P<0.05),但假手术+电针组干预12、16、20周时各项指标与假手术组干预同期比较差异均无统计学意义(P>0.05)。与心肌纤维化组干预同期比较,心肌纤维化+电针组干预12、16、20周时IVSd、LVPWd、LVEDs及LVEDd均降低(P<0.05),LVEF升高(P<0.05)。与假手术组干预同期比较,心肌纤维化组干预12、16、20周时BNP、LDH及CK水平均升高(P<0.05),但假手术+电针组干预12、16、20周时各项指标与假手术组干预同期比较差异均无统计学意义(P>0.05)。与心肌纤维化组干预同期比较,心肌纤维化+电针组干预12、16、20周时BNP、LDH及CK水平均降低(P<0.05)。与对照组干预同期比较,血管紧张素Ⅱ处理组干预12、16、20周时MMP-9及TIMI-1相对表达水平均升高(P<0.05),但对照+电针组干预12、16、20周时各项指标与对照组干预同期比较差异均无统计学意义(P>0.05)。与血管紧张素Ⅱ处理组干预同期比较,血管紧张素Ⅱ处理+电针组干预12、16、20周时MMP-9及TIMI-1相对表达水平均降低(P<0.05)。结论电针对CHF的治疗作用可能是通过抑制心室重构,改善心功能,保护心肌细胞,减轻心肌纤维化而实现。 展开更多

目的观察坤泰胶囊结合电针周期疗法治疗卵巢储备功能低下的效果。方法将医院2019年2月—2021年6月收治的109例卵巢储备功能低下患者以随机数字表法分为观察组(55例)与对照组(54例),对照组患者均予以激素周期疗法治疗,观察组患者采取激... 目的观察坤泰胶囊结合电针周期疗法治疗卵巢储备功能低下的效果。方法将医院2019年2月—2021年6月收治的109例卵巢储备功能低下患者以随机数字表法分为观察组(55例)与对照组(54例),对照组患者均予以激素周期疗法治疗,观察组患者采取激素周期疗法及坤泰胶囊结合电针周期疗法治疗。各组数据观察:临床疗效、治疗前后中医证候(月经推后或月经量少、闭经、性欲减退、神疲乏力等)积分变化、雌二醇(Estradiol,E2)及卵泡雌激素(Follicle-Stimulating Hormone,FSH)、黄体生成素(luteinizing Hormone,LH)等血清激素水平变化、卵巢间质血流动力学指标变化、卵泡发育情况以及不良反应、治疗后1年受孕率。结果对照组与观察组患者治疗总有效率比较(81.48%vs94.55%)(P<0.05);治疗前,各组患者中医证候(月经推后或月经量少、闭经、性欲减退、神疲乏力等)积分、E2、FSH及LH等血清激素水平、阻力指数(Regular Insulin,RI)及搏动指数(Pulsatility Index,PI)、收缩期最大血流速度(Peak Systolic Veloci⁃ty,PSV)、动脉血流收缩期峰值流速/舒张末期流速(systolic phase diastolic phase,S/D)等卵巢间质血流动力学指标、排卵日子宫内膜厚度、非排卵日子宫内膜厚度及初级卵泡数、优势卵泡数等卵泡发育情况指标比较,P>0.05,经治疗后各组患者中医证候(月经推后或月经量少、闭经、性欲减退、神疲乏力等)积分、E2、FSH及LH等血清激素水平、RI及PI、PSV、S/D等卵巢间质血流动力学指标、排卵日子宫内膜厚度、非排卵日子宫内膜厚度及初级卵泡数、优势卵泡数等卵泡发育情况指标均改善,观察组患者治疗后中医证候(月经推后或月经量少、闭经、性欲减退、神疲乏力等)积分、E2、FSH及LH等血清激素水平、RI及PI、PSV、S/D等卵巢间质血流动力学指标、排卵日子宫内膜厚度、非排卵日子宫内膜厚度及初级卵泡数、优势卵泡数等卵泡发育情况指标均优于对照组(P<0.05);观察组治疗不良反应率与对照组不良反应率均较低(P>0.05);观察组治疗1年后受孕率(47.27%,26/55)高于对照组患者(25.93%,14/54)(P<0.05)。结论坤泰胶囊结合电针周期疗法治疗卵巢储备功能低下的效果较为显著,患者症状恢复较好,且卵巢功能恢复好,患者未来1年受孕率高,且不良反应较少,安全可靠,值得进行应用。 展开更多
