Objective To explore the efficacy of acupuncture at Sish4nc6ng (四神聪 EX- HN 1) and Baihui (百会 GV 20) as well as the possible mechanism. Methods Eighty cases of insomnia patients treated in our hospital were selected, and randomly divided into a control group and a treatment group according to the random number table method. Conventional treatment was carried out in the control group by giving cliazepam tablets, and acupuncture at EX-HN 1 and GV 20 was conducted in the treatment group. The sleep time and quality, PSQI score, and the level changes of blood glutamic acid (GLU), y-aminobutyric acid (GABA), norepinephrine (NE) and dopamine (DA) were compared in the patients in each group before and after treatment. Results Compared with the results before treatment, the time for falling asleep of the patients in the two groups shortened (both P〈0.05), the levels of GLU and GABA increased (all P〈0.05), the sleep time was longer (both P〈0.05), and the levels of NE and DA, as well as the PSQI score reduced (all P〈0.05). Compared with the control group, the levels of NE and DA, as well as the PSQI score of patients in the treatment group reduced after treatment (P〈0.05), the time for falling asleep shortened (P〈0.05), the levels of GLU and GABA increased (P〈0.05), and the sleep time was longer (P〈0.05). Conclusion Acupuncture at EX-HN 1 and GV 20 can prolong the sleep time, improve the sleep quality, and the clinical efficacy is superior to that of western medicine treatment. The mechanism may be related to the synthesis and release of amino acids neurotransmitters. 展开更多

目的:观察针刺四神聪穴对失眠大鼠下丘脑内5-HT和5-HIAA含量的变化,揭示5-HT和5-HIAA参与针刺疗效的中枢机制。方法:选取雄性Wistar大鼠50只,随机分成5组:空白对照组(简称空白组)、模型对照组(简称模型组)、安定组、手针组、电针组,采... 目的:观察针刺四神聪穴对失眠大鼠下丘脑内5-HT和5-HIAA含量的变化,揭示5-HT和5-HIAA参与针刺疗效的中枢机制。方法:选取雄性Wistar大鼠50只,随机分成5组:空白对照组(简称空白组)、模型对照组(简称模型组)、安定组、手针组、电针组,采用腹腔注射对氯苯丙胺酸(PCPA)制备失眠动物模型,通过7日针刺及药物组安定灌胃后,取大鼠下丘脑,通过荧光分光度法测定下丘脑内5-HT和5-HIAA含量。结果:PCPA腹腔注射后大鼠正常的睡眠—觉醒周期消失,且大鼠下丘脑内5-HT、5-HIAA的含量下降,与空白组比较有统计学意义(P<0.05)。模型组与空白组比较,下丘脑中5-HT和5-HIAA含量明显低于空白组,二者有显著性差异(P<0.01)。安定组与模型组相比,下丘脑中5-HT和5-HIAA含量无明显变化,二者无显著性差异(P>0.05)。手针组及电针组下丘脑中5-HT和5-HIAA含量均增加,与模型组相比,有显著性差异(P<0.01),且与安定组相比,有显著性差异(P<0.01)。电针组同手针组相比,下丘脑中5-HT和5-HIAA含量增加明显,二者有统计学意义(P<0.05)。结论:针刺四神聪穴可使PCPA化失眠大鼠下丘脑内降低的5-HT、5-HIAA含量升高,以电针作用为明显,这可能是实现其增加睡眠,提高睡眠质量的作用机制之一。 展开更多

Objective: To observe the clinical effect of Acupuncture at Baihui(GV 20), Sishencong(EX-HN 1) and Anmian [Extra, locates at the midpoint between Yiming(EX-HN 14) and Fengchi(GB 20)] for primary insomnia. Methods: A total of 112 patients with primary insomnia were randomized into an acupuncture group and a medication group according to the visiting sequence, 56 cases in each group. The acupuncture group was intervened by acupuncture at Baihui(GV 20), Sishencong(EX-HN 1) and Anmian [Extra, locates at the midpoint between Yiming(EX-HN 14) and Fengchi(GB 20)], and points selected based on syndrome differentiation, while the medication group was by oral intake of zopiclone tablet. The Pittsburgh sleep quality index(PSQI) was measured before and after treatment, and the clinical efficacy was compared after 1 month of treatment. Results: After treatment, subscales of PSQI and global score in the acupuncture group were improved, and intra-group comparisons showed statistical differences(all P〈0.05); subscales of sleep quality, sleep latency, sleep duration and global score of PSQI in the medication group were improved, showing statistical differences(all P〈0.05). Inter-group comparison of global and subscales scores of PSQI showed statistical differences(all P〈0.05). The total effective rate in the acupuncture group was 92.9%, higher than 67.9% in the medication group, and between-group comparison showed a statistical difference(P〈0.05). Conclusion: Acupuncture at Baihui(GV 20), Sishencong(EX-HN 1) and Anmian [Extra, locates at the midpoint between Yiming(EX-HN 14) and Fengchi(GB 20)] as the main treatment method can produce a better effect than oral intake of zopiclone tablet. 展开更多
